A clinical trial of radiotherapy with modified simultaneous integrated boost(SIB)technique against huge tumors was conducted.A 58-year-old male patient who had a huge pelvic tumor diagnosed as a rectal adenocarcinoma due to familial adenomatous polyposis was enrolled in this *** total dose of 77 Gy(equivalent dose in 2Gy/fraction)and 64.5 Gy was delivered to the center of the tumor and the surrounding area respectively,andapproximately 20%dose escalation was achieved with the modified SIB *** tumor with an initial maximum size of 15 cm disappeared 120 d after the start of the *** status of the patient improved from 4 to *** with modified SIB may be effective for patients with a huge tumor in terms of tumor shrinkage/disappearance,improvement of QOL,and prolongation of survival.

We report a rare case of a 68-year-old male with metachronous pancreatic metastasis that was resected2 years after salvage esophagectomy for local recurrence of esophageal squamous cell carcinoma(ESCC).Two years and 8 mo ago,he had undergone definitive chemoradiotherapy for the lower thoracic ESCC and achieved a complete *** used the protocol of the Japan Clinical Oncology Group trial *** 8 mo later,he developed a local recurrence of the ESCC and underwent thoracoscopic salvage esophagectomy followed by reconstruction with a conduit colon graft via a subcutaneous ***,a tumor of the pancreatic body was found on routine follow-up computed tomography(CT).The tumor diameter was 15 mm on CT,and the maximum standardized uptake value of the lesion was 5.49at 18F-2-fluoro-2-deoxy-D-glucose positron-emission tomography,strongly suggesting pancreatic *** addition,all tumor markers were within the reference ***,distal pancreatectomy was performed with the resultant histological diagnosis being confirmed as pancreatic metastasis of the *** was treated with adjuvant chemotherapy,and there has been no evidence of recurrence 9 mo after the *** of pancreatic metastasis offers a good prognosis and should be considered for solitary ESCC metastasis.

Regular assumption of finite element meshes is a basic condition of most analysis of finite element approximations both for conventional conforming elements and nonconforming elements. The aim of this paper is to present a novel approach of dealing with the approximation of a four-degree nonconforming finite element for the second order elliptic problems on the anisotropic meshes. The optimal error estimates of energy norm and L^2-norm without the regular assumption or quasi-uniform assumption are obtained based on some new special features of this element discovered herein. Numerical results are given to demonstrate validity of our theoretical analysis.

Biomimetic Drag Reduction Study on Herringbone Riblets of Bird Feather

Birds have gradually formed various excellent structures such as streamlined shape and hollow shaft of feather to improve their flying performance by millions of years of natural selection. As typical property of bird feather, herringbone riblets align along the shaft of each feather, which is caused by perfect link of barbs, especially for the primary and secondary feathers of wings. Such herringbone riblets of feather are assumed to have great impact on drag reduction. In this paper, microstructures of secondary feathers of adult pigeons are investigated by SEM, and their structural parameters are statistically obtained. Based on quantitative analysis of feather structure, novel biomimetic herringbone riblets with narrow smooth edge are proposed to reduce surface drag. In comparison with traditional microgroove riblets and other drag reduction structures, the drag reduction rate of the proposed biomimetic herringbone riblets is experimentally clarified up to 16%, much higher than others. Moreover, the drag reduction mechanism of herringbone riblets are also confirmed and exploited by CFD.

In order to estimate the motion errors of 5-axis machine center, the double ball bar （DBB） method is adopted to realize the diagnosis procedure. The motion error sources of rotary axes in 5-axis machining center comprise of the alignment error of rotary axes and the angular error due to various factors, e.g. the inclination of rotary axes. From sensitive viewpoints, each motion error is possible to have a particular sensitive direction in which deviation of DBB error trace arises from only some specific error sources. The model of the DBB error trace is established according to the spatial geometry theory. Accordingly, the sensitive direction of each motion error source is made clear through numerical simulation, which is used as the reference patterns for rotational error estimation. The estimation method is proposed to easily estimate the motion error sources of rotary axes in quantitative manner. To verify the proposed DBB method for rotational error estimation, the experimental tests are carried out on a 5-axis machining center M-400 （MORISEIKI）. The effect of the mismatch of the DBB is also studied to guarantee the estimation accuracy. From the experimental data, it is noted that the proposed estimation method for 5-axis machining center is feasible and effective.

Several applications of Origami have been tested in industrial area. However, Origami based design is found only in lamp shade and wrapping in design area, and has not been practically applied especially in clothing. In this paper, the conical truss model is applied in the clothing skirt, and the simulation software from 2D to 3D is developed by parametric Origami module （POM）. Finally, the pattern is copied on felt clothing, and the scaled model of skirt on the basis of conical reverse truss model design is made.

Long-term storage and quality maintenance of fresh fish are tremendously *** study aimed to develop a novel technology for long-term storage and quality improvement of fresh fish by utilizing deep sea and to clarify its detailed mechanisms focusing on protein ***,deep-sea aging of greater amberjack muscle was performed at depths of 4000 and 6000 m for approximately 4 *** total viable count of bacteria in the deep-sea-aged samples was significantly suppressed,especially at 6000 *** dodecyl sulfate-polyacrylamide gel electrophoresis(SDS-PAGE)analysis showed that the degradation of myosin heavy chain was accelerated in the deep-sea-aged samples.A significant increase in the content of free amino acids such as aspartic acid and serine,which relates to the enhancement of umami and sweet taste,was also observed in the deep-sea-aged *** differential scanning calorimetry(DSC)analysis indicated that protein conformational changes induced by deep-sea high pressure presumably increase the reactivity between protease and substrate in the fish sample,leading to enhanced ***,deep-sea aging contributes to improving the safety and quality of greater amberjack muscle during long-term ***,this novel technology can be used to achieve the sustainable utilization of marine resources.

Inhibitory effect of fluvastatin on ileal ulcer formation in rats induced by nonsteroidal antiinflammatory drug

AIM: Nonsteroidal anti-inflammatory drugs (NSAIDs)cause gastrointestinal damage as one of their side effects in humans and experimental animals. Lipid peroxidation plays an important role in NSAID-induced ulceration. The aim of this study was to investigate the inhibitory effect of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA)reductase inhibitors on the ulceration in small intestines of ***: The effects of three HMG-CoA reductase inhibitors, fluvastatin, pravastatin and atorvastatin on ileal ulcer formation in 5-bromo-2-(4-fluorophenyl)-3-(4-methylsulfonylphenyl) thiophene (BFMeT)-treated rats were examined. Antioxidative activity of the inhibitors was measured by a redox-linked colorimetric ***: Fluvastatin, which was reported to have antioxidative activity, repressed the ileal ulcer formation in rats treated with BFMeT an NSAIDs. However, the other HMG-CoA reductase inhibitors (pravastatin and atorvastatin)did not repress the ileal ulcer formation. Among these HMG-CoA reductase inhibitors, fluvastatin showed a significantly stronger reducing power than the others(pravastatin, atorvastatin).CONCLUSION: Fluvastatin having the antioxidaitive activity suppresses ulcer formation in rats induced by NSAIDs.

The ideal goal of chronic hepatitis B(CHB) treatment should be suppression of emergence of hepatocellular carcinoma through the disappearance of hepatitis B s antigen(HBs Ag) rather than the control of serum hepatitis B virus-DNA level. For this purpose, various types of combination therapies using nucleoside analogs(NAs) and interferon(IFN) have been conducted. The therapeutic effects of combination of two different kinds of agents are better than those of the monotherapy using NAs or IFN alone, probably because different pharmaceutical properties might act in a coordinated manner. Recently, combination therapies with NAs and IFN and sequential therapies with NAs administration followed by IFN therapy have been routinely employed. We previously reported that combination therapy using entecavir(ETV) and pegylated(PEG)-IFN showed antiviral effects in 71% of CHB patients; the effect of this combination was better than that using lamivudine(LAM) and PEG-IFN. This is partially explained by the better antiviral effects of ETV than those of LAM. In our analysis, the cohort of CHB consisted of the patients who showed a flare-up of hepatitis before antiviral therapy, and their baseline HBs Ag levels were relatively low. Therefore, in addition to the combination of the agents, the appropriate selection of patients is critical to achieve a good viral response.

AIM:To investigate whether endoscopic submucosal dissection(ESD) can be safely performed at small clinics,such as the Shirakawa ***:One thousand forty-seven ESDs to treat gastrointestinal tumors were performed at the Shirakawa Clinic from April 2006 to March *** efficacy,technical feasibility and associated complications of the procedures were *** ESD procedures were performed by five *** was induced with propofol for esophagogastorduodenal ***:One thousand forty-seven ESDs were performed to treat 64 patients with esophageal cancer(E),850 patients with gastric tumors(G:764 patients with cancer,82 patients with adenomas and four others),four patients with duodenal cancer(D) and 129 patients with colorectal tumors(C:94 patients with cancer,21 patients with adenomas and 14 others).The en bloc resection rate was 94.3%(E:96.9%,G:95.8%,D:100%,C:79.8%).The median operation time was 46 min(range:4-360 min) and the mean size of the resected specimens was 18 mm(range:2-150 mm).No mortal complications were observed in association with the ESD *** occurred in 12 cases(1.1%,E:1 case,G:9 cases,D:1 case,C:1 case) and postoperative bleeding occurred in 53 cases(5.1%,G:51 cases,D:1 case,C:1 case);however,no case required either emergency surgery or blood *** of the perforations and postperative bleedings were resolved by endoscopic clipping or *** other problematic complication observed was pneumonia,which was treated with conservative ***:ESD can be safely performed in a clinic with established therapeutic methods and medical services to address potential complications.
