Recent Techniques in Management of Lumbosacral Region Tumours: A Prospective Comparative Study

Background: The lumbosacral plexus consists of the ventral rami from the L2 to S3 nerve roots, with some additions from the L1 and S4 nerve roots as well. The standard treatment for spinal cord compression is urgent surgically decompressive laminectomy over the affected level with resection as much as possible of the exposed tumor to relieve the cord compression. We aim to correlate the impact of preoperative clinical picture, duration of symptoms, and the pathological nature of the lumbo-sacral spine tumors on the outcome of comparative study different modalities of management. Patients and Methods: During the period between 2015 and 2017, thirty cases were studied and surgically managed in Al-Azhar University Hospital, Damietta. Cases were selected for surgery if they were diagnosed as having a symptomatizing lumbosacral lesion. Results: Thirty patients were included in the current study;the commonest lesion in this study was ependymoma eight patients (27%), astrocytoma six patients (20%), and meningioma five patients (17%). Nerve sheath tumor comprised five cases (17%) with the hemangioblastomas three cases (10%), epidermoid one case (3%), and ependymoma of the filum terminale two cases (6%). The mean age of the included patients was 38.96 ± 15.42. The mean duration of lesions was 13.66 ± 10.93. They were 15 male patients (50%), and 15 female patients (50%), with male: female ratio 1:1. The mean duration of symptoms ranged for 1:60 months. The most common presenting manifestations were sensory followed by motor and sphinctric disturbance. Complications occurred in 8 patients (23%), and the most common was postoperative CSF leak. 19 out of 30 cases were surgically totally resected (63%), while 11 cases (37%) were grossly totally resected. Conclusions: Ependymoma and meningioma have more favorable outcome compared to other spinal tumors. Accurate and rapid diagnosis prior to surgery also associated with good outcome.

Effect of Changing the Effluent Path of Omar-Bek Drain on the Damietta Branch Water Quality

The Omar-Bek drain, which represents the main source of water pollution along the Damietta branch of the Nile River, receives about 600,000 m3 (158,503,200 gallons) daily of untreated domestic, agricultural, and industrial wastes. The main purpose of this research consisted of investigating alternatives of managing water quality at the Damietta branch;a comparison was conducted between the current situation and two proposed scenarios. The first scenario involved changing the effluent path of the Omar-Bek drain to another drain is called “Main Western drain”. The second scenario centered around improving water quality at the Omar-Bek drain by constructing a WWTP with a design capacity of more than 150,000 m3/day (39,625,800 gallons) and by improving water quality at this drain by increasing the efficiency of WWTPs that discharged daily about 60,000 m3 (15,850,300 gallons) of partially treated wastewater to the drain. The current situation and the two proposed scenarios were simulated by using river pollutant (RP) modeling. It was concluded that the Omar-Bek drain has no significant effect on the Damietta branch water quality and that, instead of changing the path of the drain, improving the efficiency of the existing WWTPs discharging to the Omar-Bek drain and preventing the direct discharge of domestic wastewater to the drain will provide the most effective ways of increasing the water quality of the Damietta branch.

Chiari Malformation Type 1 in Adults Managed by Surgical Decompression: New Prospective

Aim of the work: This study aims to assess the value of the surgical management in the improvement of the symptoms & signs of patients with Chiari malformation type 1 and radiological follow up in adults. Patients and methods: This study included 30 consecutive patients with Chiari malformation type I who were indicated for surgery at neurosurgery department. Data were collected prospectively from the involved patients who were evaluated preoperatively and underwent evaluation by CT scanning of the brain & skull and MRI imaging of the brain and spine. CT and MRI were done as the routine follow up investigations for all patients. We operated through midline suboccipital craniectomy, durotomy in y shaped manner, shrinkage of cerebellar tonsils by bipolar electrocautery, duroplasty by fascia lata graft, watertight closure. Results: The assessment from E. J. N. S. (Egyptian Journal of Neurosurgery) vol. 24 no. 2 June 2009 used for evaluating the patients clinically. 18 patients reported good outcome, 6 fair, 6 poor. We evaluated the size of the syrinx if present preoperative in follow up. 15 (50%) patients showed marked reduction (more than or equal to 60%) in size of syrinx, 3 (10%) mild reduction (less than or equal to 30%) in size and 12 (40%) with no change. Conclusion: The Chiari type 1 malformation constitutes a controllable malformation with good outcomes. With current microsurgical techniques, the results of the bony decompression and duroplasty became excellent. Before undergoing surgical treatment for CM-I, symptomatic patients and their families should be given clear information about the success of treatment and potential complications.

Structural Health Monitoring for Reinforced Concrete Containment Using Inner Electrical Resistivity Method

Nuclear power plants (NPPs) are considered as the main source for generating electricity nowadays in some countries. The effect of impact of heavy fully loaded aeroplane such as (Boeing 747-200c) causes leakage of the radiation through the cracks generated on the external RC containment of NPPs, and this leads to severe damage for humans and cities. In this research paper, external RC containment is modeled using ANSYS and hit by Boeing 747-200c which is the heavier aeroplane compared to other jets and causes severe damage for external RC containment. In addition, the impact location for Boeing 747-200c is considered at 30 m vertical height. RC containment response was studied after the impact of an aeroplane and a proposed structural health monitoring technique is applied using embedded sensors in order to detect and locate the embedded cracks that is generated due to the effect of impact of heavy aeroplane. It was concluded that RC containment is intact except for the impact region which is damaged. An experimental program was applied on a part of the element in ANSYS which is away from the impact region. Four specimens were cast using heavy weight concrete in laboratory. Three cracked specimens consist of different lengths of vertical cracks which represent different times of impact in order to replicate crack propagation as in ANSYS. The cracks are simulated inside laboratory specimens using failure criteria. The parameters used in detecting the cracks for specimens are the percentage change in electrical resistivity and Decimal Logarithm

Assessment of seminal plasma laminin in fertile and infertile men

Aim： To assess laminin levels in the seminal plasma of infertile and fertile men, and to analyze the correlation of laminin levels with sperm count, age, sperm motility and semen volume. Methods： One hundred and twenty-five recruited men were equally divided into five groups according to their sperm concentration and clinical examination： fertile normozoospermia, oligoasthenozoospermia, non-obstructive azoospermia （NOA）, obstructive azoospermia （OA） and congenital bilateral absent vas deferens （CBAVD）. The patients＇ medical history was investigated and patients underwent clinical examination, conventional semen analysis and estimation of seminal plasma laminin by radioimmunoassay. Results： Seminal plasma laminin levels of successive groups were： 2.82 ± 0.62, 2.49 ± 0.44, 1.77 ± 0.56, 1.72 ± 0.76, 1.35 ± 0.63 U/mL, respectively. The fertile normozoospermic group showed the highest concentration compared to all infertile groups with significant differences compared to azoospermic groups （P 〈 0.05）. Testicular contribution was estimated to be approximately one-third of the seminal laminin. Seminal plasma laminin demonstrated significant correlation with sperm concentration （r = 0.460, P 〈 0.001） and nonsignificant correlation with age （r = 0.021, P = 0.940）, sperm motility percentage （r = 0.142, P = 0.615） and semen volume （r = 0.035, P = 0.087）. Conelusion： Seminal plasma laminin is derived mostly from prostatic and testicular portions and minimally from the seminal vesicle and vas deferens. Estimating seminal laminin alone is not conclusive in diagnosing different cases of male infertility.

The Choice of Appropriate Scenario in Order to Reduce the Effect of Thermal Pollution at the Damietta Branch Caused by Cooling Water Discharged from Kafr-Al-Batek Power Station

The main purpose of this research effort was to investigate and reduce the volume of thermal polluted cooling water from returning to the Kafr-Al-Batek power station. Traditional cooling systems, such as cooling towers or ponds can be very challenging with regards to implementation in developing countries;mainly due to the lack of financial capacity. This research focused on low-cost simulation solutions that could improve thermal outcomes. Comparisons were performed between three different scenarios to decrease the elevated temperature of the discharged water (43 °C) released by the Kafr-Al-Batek power station on the Damietta branch. The different scenarios were simulated by using Star CCM+ software. The base scenario examined the discharge angle of an existing outlet. The second scenario examined a new outlet downstream from the existing outlet. The third scenario increased the width of the existing outlet in order to reduce flow velocity. A comparative analysis is provided between the aforementioned solutions to identify the most suitable and cost-effective alternative. Simulation results show that changing the discharge angle from 90° to 135° is the most effective solution. Applying this solution has the potential to decrease the water temperature at the inlet by 7 degrees Celsius (from 32 °C to 25 °C).

On the Computing of the Minimum Distance of Linear Block Codes by Heuristic Methods

The evaluation of the minimum distance of linear block codes remains an open problem in coding theory, and it is not easy to determine its true value by classical methods, for this reason the problem has been solved in the literature with heuristic techniques such as genetic algorithms and local search algorithms. In this paper we propose two approaches to attack the hardness of this problem. The first approach is based on genetic algorithms and it yield to good results comparing to another work based also on genetic algorithms. The second approach is based on a new randomized algorithm which we call 'Multiple Impulse Method (MIM)', where the principle is to search codewords locally around the all-zero codeword perturbed by a minimum level of noise, anticipating that the resultant nearest nonzero codewords will most likely contain the minimum Hamming-weight codeword whose Hamming weight is equal to the minimum distance of the linear code.

Hepatitis C virus(HCV)is still one of the main causes of liver disease *** disorders,including nonalcoholic fatty liver disease(NAFLD),induced by HCV have been shown to accelerate the progression of fibrosis to cirrhosis and to increase the risk of hepatocellular *** optimal peroxisome proliferator-activated receptor gamma coactivator 1-alpha(PPARGC1A)activity is crucial to prevent NAFLD *** present study aims to investigate the associations between two common PPARGC1A polymorphisms(rs8192678 and rs12640088)and the outcomes of HCV infection in a North African context.A series of 592 consecutive Moroccan subjects,including 292 patients with chronic hepatitis C(CHC),100 resolvers and 200 healthy controls were genotyped using a TaqMan allelic discrimination ***1A variations at rs8192678 and rs12640088 were not associated with spontaneous clearance of HCV infection(adjusted ORs=0.76 and 0.79 respectively,P[0.05,for both).Furthermore,multivariable logistic regression analysis showed that both SNPs were not associated with fibrosis progression(OR=0.71;95%CI 0.20–2.49;P=0.739;OR=1.28;95%CI 0.25–6.54;P=0.512,respectively).We conclude that,in the genetic context of South Mediterranean patients,rs8192678 and rs12640088 polymorphisms of PPARGC1 A are neither associated with spontaneous clearance nor with disease progression in individuals infected with HCV.
