AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma (HCC) with a panel of serum proteins in combination with α-fetoprotein (AFP).METHODS: Serum levels of interleukin (IL)-8, soluble intercellular adhesion molecule-1 (sICAM-1), soluble tumor necrosis factor receptor II (sTNF-RII), proteasome, and β-catenin were measured in 479 subjects categorized into four groups: (1) HCC concurrent with hepatitis C virus (HCV) infection (n = 192); (2) HCV related liver cirrhosis (LC) (n = 96); (3) Chronic hepatitis C (CHC) (n = 96); and (4) Healthy controls (n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different ***: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model (-11.3 + 7.38 × Prot + 0.00108 × sICAM + 0.2574 × β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × sICAM-1 + (4.251 × 10-4) × sTNF + 0.02567 × β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Background: The diagnosis of unexplained infertility may be very frustrating for infertile couples and they show higher depression levels when compared to fertile couples. Aim: To assess the psychiatric aspects of Egyptian women with unexplained infertility. Methods: 220 infertile women were included in this study (110 with unexplained infertility and 110 with explained infertility). The psychiatric aspect of these women was evaluated using the translated and validated Hamilton Anxiety Rating Scale and the Beck Depression Inventory-II. Findings: The number of women with mild depression was significantly higher in the unexplained infertility group (47/110 (42.7%)) versus (22/110 (20%)) in the explained infertility group (P = 0.007). Introduction: Patients undergoing major oncological surgery may suffer from severe bleeding. Sometimes, it is difficult to anesthesiologist to take decision about timing of administration blood products to such patients. The aim of this study is to evaluate the use of continuous noninvasive hemoglobin monitoring as a guide for blood transfusion practice. Methods: One hundred patients undergoing elective abdominal cancer surgeries were randomly allocated into two groups, Group I (n = 50): laboratory Hb was obtained at baseline (immediate preoperative), intraoperative (when to suggest transfusion triggering value) and immediate postoperative. Group II (n = 50): The probe of Masimo for SpHb monitoring was applied immediately after induction of anesthesia at the index finger. Laboratory Hb was obtained at baseline (immediate preoperative), intraoperative (when to suggest transfusion triggering value) and immediate postoperative. Results: A number of transfused units of RBC were significantly lower in SpHb group than in control group (p value 0.05). Conclusion: SpHb monitoring had clinically acceptable absolute and trend accuracy. SpHb monitoring altered transfusion decision making and resulted in decreased RBC utilization and decreased RBC costs while facilitating earlier transfusions when indicated.

Objective:To estimate the result of egg yolk replacement with alternative cryopreservatives such as plant-derived lecithin from soybean on sperm quality parameters pre and post freezing in buffalo ***: The control cryopreservation extender was tris-citric acid-fructose-egg yolk-glycerol (TCFYG) diluent. Semen samples were extended gradually 1:10 with TCFYG control extender and tris-citric acid-fructose-glycerol (TCFG) extender plus variable concentrations of soybean lecithin (0.5%, 1.0%, 1.5%, 2.0%, 2.5% and 3.0%) to ensure 60 million active spermatozoa/mL of the extended semen. The diluted semen samples were refrigerated slowly (roughly for 2 h) up to 5℃ and equilibrated for 2 h. Semen was filled into 0.25 mL polyvinyl French straws (IMV, France). After equilibration period, the straws were placed horizontally on a rack and frozen in a vapor 4 cm above liquid nitrogen for 10 min and were then dipped stored in liquid nitrogen at -196℃. Results:The respective overall percentages of forward motile spermatozoa, live spermatozoa, morphologically normal spermatozoa, acrosome integrity and hypo-osmotic swelling reactivity observed primarily in fresh semen, after equilibration (pre-freeze stage) and post freezing (post-thaw stage) in TCFYG (control) extended semen declined progressively and statically (P<0.01) during these periods of study. Pre-freezing stage: replacement of egg yolk into TCFG with soybean lecithin at concentrations of 1.0% and 1.5% significantly (P<0.01) ameliorated the maintenance of (motility, viability, acrosome and membrane integrity %), meanwhile it had significantly (P<0.01) reduced the abnormality % of spermatozoa to the lowest value compared to control TCFYG and to some other concentrations in use. Post-thaw stage: the replacement of egg yolk with 1.0% soybean lecithin (SL) showed significantly (P<0.01) higher percentage of sperm progressive motility compared to 1.5% SL and TCFYG control. AIM： To identify the trend, possible risk factors and any pattern change of hepatocellular carcinoma （HCC） in Egypt over a decade. METHODS： All HCC patients attending Cairo Liver Center between January 1993 and December 2002, were enrolled in the study. Diagnosis of HCC was based on histopathological examination and/or detection of hepatic focal lesions by two imaging techniques plus α-fetoprotein level above 200 ng/mL. The duration of the study was divided into two periods of 5 years each; period Ⅰ （1993-1997） and period Ⅱ （1998-2002）. Trend, demographic features of patients （age, gender, and residence）, risk factors （HBsAg, HCV-Ab, schistosomiasis and others） and pattern of the focal lesions were compared between the two periods. Logistic regression model was fitted to calculate the adjusted odds ratios for the potential risk factors. The population attributable risk percentage was calculated to estimate the proportion of HCC attributed to hepatitis B and C viral infections. RESULTS： Over a decade, 1 328 HCC patients out of 22 450 chronic liver disease （CLD） patients were diagnosed with an overall proportion of 5.9%. The annual proportion of HCC showed a significant rising trend from 4.0% in 1993 to 7.2% in 2002 （P = 0.000）. A significant increase in male proportion from 82.5% to 87.6% （P = 0.009）; M/F from 5：1 to 7：1 and a slight increase of the predominant age group （40-59 years） from 62.6% to 66.8% （P = 0.387）in periods Ⅰ and Ⅱ respectively, reflecting a shift to younger age group. In the bivariate analysis, HCC was significantly higher in rural residents, patients with history of schistosomiasis and/or blood transfusion. Yet, after adjustment, these variables did not have a significant risk for development of HCC. There was a significant decline of HBsAg from 38.6% to 20.5% （P = 0.000）, and a slight increase of HCV-Ab from 85.6% to 87.9% in periods I and II respectively. We isolated naturally occurring actinomycetes with an ability to produce metabolites having antifungal property against, Colletotrichum gloeosporioides, the causal agent of mango anthracnose. One promising strain was strong antifungal activity, was selected for further studies. Based on the physiological and biochemical characteristics, the bacterial strain was identical to Streptomyces aureofaciens. Culture filtrate collected from the exponential and stationary phases inhibited the growth of fungus tested, indicating that growth suppression was due to extracellular antifungal metabolites present in culture filtrate. Isolate highly produced extracellular chitinase and β-1,3-glucanase during the exponential and late exponential phases, respectively. In order to standardize the metabolite production some cultural conditions like different incubation time in hours, pH, carbon sources and concentrations and nitrogen source were determined. During fermentation, growth, pH and hydrolysis enzymes production were monitored .Treatment with bioactive components exhibited a significantly high protective activity against development of anthracnose disease on mango trees and increased fruit yield.

A Comprehensive Assessment of Water Quality at the Rosetta Branch of the Nile River, Egypt

The aim of the present study is to assess the water quality along the Rosetta branch of the Nile River, Egypt. The study area extends from upstream of the EI-Rahawy drain to the end of the branch. The correlation matrix was performed to help identify the nature of correlations between the different parameters. The WQI （water quality index） was calculated seasonally at different points along the Rosetta branch to provide a simple indicator of water quality at these points. The results of WQI calculations showed that the fecal coliform is the main cause of poor water quality along the Rosetta branch. A statistical analysis was also performed using a two-way ANOVA （analysis of variance） to identify the significant sources of water pollution and to determine the impact of the parameters on a mass loading. A significant difference was observed between the impacts of the pollution sources on the water quality. Also, a significant difference was observed between the impacts of each parameter in the mass loading. The results showed that the E1-Rahawy, Tala and Sabal drains are the major sources for water quality degradation along the Rosetta branch and that the effect of the EI-Tahrir and the Zawyet El-Baher drains on the water quality is not significant.
