Early detection of hepatocellular carcinoma co-occurring with hepatitis C virus infection: A mathematical model

AIM: To develop a mathematical model for the early detection of hepatocellular carcinoma(HCC) with a panel of serum proteins in combination with α-fetoprotein(AFP).METHODS: Serum levels of interleukin(IL)-8, soluble intercellular adhesion molecule-1(s ICAM-1), soluble tumor necrosis factor receptor Ⅱ(s TNF-RⅡ), proteasome, and β-catenin were measured in 479 subjects categorized into four groups:(1) HCC concurrent with hepatitis C virus(HCV) infection(n = 192);(2) HCV related liver cirrhosis(LC)(n = 96);(3) Chronic hepatitis C(CHC)(n = 96); and(4) Healthy controls(n = 95). The R package and different modules for binary and multi-class classifiers based on generalized linear models were used to model the data. Predictive power was used to evaluate the performance of the model. Receiver operating characteristic curve analysis over pairs of groups was used to identify the best cutoffs differentiating the different groups. RESULTS: We revealed mathematical models, based on a binary classifier, made up of a unique panel of serum proteins that improved the individual performance of AFP in discriminating HCC patients from patients with chronic liver disease either with or without cirrhosis. We discriminated the HCC group from the cirrhotic liver group using a mathematical model(-11.3 + 7.38 × Prot + 0.00108 × s ICAM + 0.2574 ×β-catenin + 0.01597 × AFP) with a cutoff of 0.6552, which achieved 98.8% specificity and 89.1% sensitivity. For the discrimination of the HCC group from the CHC group, we used a mathematical model [-10.40 + 1.416 × proteasome + 0.002024 × IL + 0.004096 × s ICAM-1 +(4.251 × 10-4) × s TNF + 0.02567 ×β-catenin + 0.02442 × AFP] with a cutoff 0.744 and achieved 96.8% specificity and 89.7% sensitivity. Additionally, we derived an algorithm, based on a binary classifier, for resolving the multi-class classification problem by using three successive mathematical model predictions of liver disease status. A hydrogen liquefaction concept with an innovative configuration and a capacity of 4 kg·s^(-1)(345.6 t·d^(-1))is *** concept involves an ammonia absorption refrigeration system for the pre-cooling of hydrogen and MR streams from 25℃ to-30℃.The ammonia absorption refrigeration system is fed by exhaust gases of the Pa rand gas power plant that are normally dissipated to the environment with a temperature of 546℃.The simulation is performed by Aspen HYSYS V9.0,using two separate equations of state for simulating hydrogen and MR streams to gain more accurate results especially for ortho-para *** show that conversion enthalpy estimated by Aspen HYSYS,fits very well to the experimental *** the important independent variables and composition of MRs are done using trial and error procedure,a functional and straightforward method for complicated *** minimum temperature limit in the cooling section is lowered,and an ortho-para converter is implemented in this *** proposed concept performs well from energy aspects and leads to COP and SEC equal to 0.271 and 4.54 kW·h·kg^(-1),*** main advantage of this study is in the low SEC,eliminating the losses of the distribution network,and improving the ability of the hydrogen liquefaction for energy storage in off-peak times.

Correlation between microstructural features and tensile strength for friction welded joints of AA-7005 aluminum alloy

Similar friction welded joints of AA-7005 aluminum rods were fabricated using different combinations of process parameters such as friction pressure(1.0, 1.5 and 2.0 MPa) and friction time(10, 15 and 20 s). Interfacial microstructure and formation of intermetallic compounds at the joint interface were evaluated via scanning electron microscopy(SEM) equipped with energy dispersive spectrum(EDS), and optical microscopy(OM). Microstructural observations reveal the formation of intermetallic phases during the welding process which cannot be extruded from the interface. Theses phases influence the tensile strength of the resultant joints. From the tensile characteristics viewpoint, the greatest tensile strength value of 365 MPa is obtained at 1.5 MPa and 15 s. Finally, the role of microstructural features on tensile strength of resultant joints is discussed.

Simple and sensitive method for the determination of trace amounts of thorium using a benzoquinone derivative

A novel simple, sensitive, rapid, direct, and spectrophotometry-based procedure was investigated to determine Th(IV) at trace amounts. The new method is based on Th(IV) chelation with 3,6-dichloro-2,5-dihydroxy-l,4-benzoquinone(DDBQ). The reagent reacts with Th(IV) in 0.1 M HCl to form an orange 1:2 complex. The stability constant value is 6.62×10~4 for the Th(IV)complex. The Th(IV)-DDBQ obtained shows one peak with a maximum at about 346 nm. The chelate forms immediately and the absorbance remains stable for over24 h. Beer's law was obeyed in the concentration range0–10 μg mL^(-1). The molar absorptivity and Sandell's sensitivity were 4.4×10~4 L mol^(-1)cm^(-1) and 0.0053 μg cm^(-2), respectively. Different analytical parameters were tested in detail. Interfering ion(cations and anions) effects were tested. Methods for Th(IV) determination by second and third-derivative spectrophotometry were also introduced at about 344 and 341 nm, respectively. These two derivative orders offer the feature of sensitivity without the necessity for solvent extraction,heating, or pre-concentration steps. Finally, the methods were successfully utilized for Th(IV) determination in monazite, environmental water, and wastewater samples.

Assessment of sediment yield using RS and GIS at two sub-basins of Dez Watershed,Iran

Soil erosion is a serious threat to soil and water resources in semi-arid *** Pacific Southwest Inter Agency Committee(MPSIAC)and Erosion Potential Method(EPM),as two well-known models,have shown their performance in many case *** goal of present study is to assess the efficiency of these methods for estimating the sediments yield and erosion intensity within short-term and longterm timeframes over two sub-basins of Dez watershed,west of *** results showed that the study area can be categorized into slight,moderate,high and very high erosion *** half of the study area is highly susceptible to erosion due to the geological formations and land ***,the longterm(i.e.30 years)sediment yield of 387 and 615(kton)y^(-1) estimated by MPSIAC and EPM models demonstrated the superiority of *** to the measured value of 612(kton)y^(-1),the performance of EPM was *** splitting the dataset into six periods of five years,the sediment yield was predicted in short-term periods by both aforementioned *** segmentation provides the opportunity to evaluate the impact of extreme flooding events on the models *** results showed that both models failed in estimation of sediment load during flood ***,the correlation coefficients for estimating the sediment yield were found to be R=0.93 and R=0.85 for EPM and MPSIAC models respectively,for short-term simulations.

Heart Failure Prediction in Athletic Heart Remodeling among Long Distance Runners

Background: Physical activity has a cardinal role in preventing and treating cardiovascular diseases. Understanding the changes that occur to the cardiac muscle in athletes is still doubtful whether it is only an adaptation to exercise or a concealed pathological condition. Most of these changes are well documented in apparently healthy heart and characterized by cardiac enlargement with Brady-arrhythmias specially individuals participating in long distance runners with exercise capacity without signs of cardiovascular disease. Methods: The study searched the subclinical myocardial necrosis by comparing two groups of young marathon runners, both groups were chosen from Al Gizera youth center in January 2018. First group included adults above the age of 18 years and ran for 12 km. and the second was under 18 years and ran for about 8 km. Both groups were volunteered for this study. They are monitored carefully with blood pressure, heart rate and respiratory rate and their blood samples were collected pre- and post-race immediately for assessment of cardiac markers NTproBNP, Galectin-3 and cTnI. Results: All cardiac markers were elevated post exercise compared to that achieved in pre-exercise regardless age of athletes. Also, pre-exercise adult results were higher in galectin-3 and cTnI but not in proBNP while there is no difference in the acute response in both groups. These results revealed micro cardiac necrosis due to long run exercise with possible bad prognosis. Conclusions: It is important to develop risk assessment and prediction methods for screening the young athletes and consider the term athletic heart as a subclinical disorder rather than physiological adaptation.

Achalasia and thyroid disease

AIM: To investigate some possible etiologies of achalasia by screening patients with achalasia for some autoimmune diseases such as thyroid disease. METHODS: We examined 30 known cases of achalasia (20 females, 10 males). Their age ranged 15-70 years. All of them were referred to our institute for treatment. Their sera were evaluated to detect some possible associations with rheumatoid disease, thyroid disease, inflammatory process, anemia, etc. RESULTS: Seven out of 30 patients (23%) had thyroid disease including four patients with hypothyroidism (13.3%), two patients with hyperthyroidism (6.6%), and one had only thyroid nodule but was in euthyroid state (3.3%). Two of these hypothyroid patients had no related clinical symptoms (subclinical) and two had clinical manifestations of hypothyroidism. There were no correlations between the intensity of thyroid diseases and the severity of achalasia symptoms. CONCLUSION: The etiology of achalasia is unknown although autoimmunity has been implicated and is supported by several studies. Thyroid disease presents concomitantly with achalasia in about one fourth of our patients who may have a common etiology.

Risks of suicidality in adult patients with epilepsy

AIM: To determine the prevalence and risks of suicidality in a group of patients with epilepsy. METHODS: Included were 200 adult patients and 100 matched healthy subjects. The clinical interview using The Diagnostic and Statistical Manual of Mental Disorders(4th edition), Beck Depression Inventory(2nd edition)(BDI-Ⅱ), Hamilton Anxiety Rating Scale(HAM-A), Yale-Brown Obsessive Compulsive Scale(Y-BOCS) and Eysenck Personality Questionnaire-Revised Rating Scale testings were used for diagnosis and assessment of severity of psychiatric symptoms. Blood concentrations of serotonin, catecholamines and dopamine were also ***: Suicidality was reported in 35%(compared to 9% for controls), of them 80%, 72.86%, 55.71% and 52.9% had depression, anxiety, obsession and aggression respectively. Patients with suicidality had higher scores of BDI-Ⅱ(P = 0.0001), HAM-A(P = 0.0001), and Y-BOCS(P = 0.037) and lower scores of psychotic(P = 0.0001) and extroversion(P = 0.025) personality traits. Regardless the presence or absence of suicidality, patients with epilepsy had low serotonin(P = 0.006), noradrenaline(P = 0.019) and adrenaline(P = 0.0001) levels. With suicidality, significant correlations were identified between:(1) age and scores of BDI-Ⅱ(r = 0.235, P = 0.0001) and HAM-A(r = 0.241, P = 0.046);(2) age at onset and concentrations of noradrenaline(r =-0.502, P = 0.024);(3) duration of illness and scores of BDI-Ⅱ(r = 0.247, P = 0.041), Y-BOCS(r = 0.270, P = 0.025) and neurotic personality trait(r =-0.284, P = 0.018); and(4) doses of antiepileptic drugs and scores of psychotic personality traits(r =-0.495, P = 0.006 for carbamazepine; r =-0.508, P = 0.0001 for valproate).CONCLUSION: This is the first study which systematically estimated the prevalence and risks of suicidality in a homogenous group of patients with epilepsy. This study emphasizes the importance of epilepsy itself as a risk for suicidality and not its treatment.
