Comparison between FIB-4 Index and Fibroscan as Marker of Fibrosis in Ch.onic h.V Infection in Egyptian Patients

Th. FIB-4 index is a simple and noninvasive algorith. consisting to evaluate liver fibrosis in ch.onic h.V infection. Aim: To evaluate th. utility of FIB-4 index as a noninvasive marker to assess liver fibrosis in ch.onic h.V infection in comparison to transient elastograph.. Patients and Meth.ds: We studied 30 patients h.ving ch.onic h.V infection based on clinical features, laboratory tests, diagnostics images, Fibroscan and FIB-4 score. According to th. results of Fibroscan, th. 30 patients were classified into two groups in order to obtain a cutoff value to exclude patient with.significant fibrosis: group Ia: 7 patients with.no or mild liver fibrosis (F0-F1) and group Ib: 23 pa-tients with.significant fibrosis or cirrh.sis (F2-F3-F4). Group IIa: 17 patients with.no or significant fibrosis (F0-F1-F2-F3) and group IIb (F4): 13 patients with.cirrh.sis (F4). Results: Th. mean of FIB-4 index increased with.th. increase of th. fibrosis score. FIB-4 index proved to be sensitive and specific in differentiation between patients with.no or mild fibrosis (F0-F1) and patients with.significant fibrosis or cirrh.sis (F2-F3-F4) with.th. best cutoff value at 1.61. It also proved to be sensitive and specific in differentiation between patients with.no or significant fibrosis (F0-F1-F2-F3) and patients with.cirrh.sis (F4) with.cutoff value at 1.88. Conclusion: Th. FIB-4 index enabled th. correct identification of extreme types of fibrosis. Using th.se cutoffs (1.61 - 1.88), 87% of patients fell outside th.se ranges and could th.s avoid liverbiopsy with.an overall accuracy of 70%.

Introduction: Pyridone derivatives played important roles in th. last decade to approach.many and different functionalities, especially as antitumor, antibacterial, anti-fungal, and many of ph.rmacological activities. Meth.dology: Novel compounds of 5-Arylazo-2-[(substituted)-3, 4, 5-trih.droxy-6-(h.droxymeth.l) tetrah.dro-2h.pyran-2-yloxy]-4, 6-dimeth.lnicotinonitrile, (3a-e), generally called (fluroarylazopyridine glucosides) were synth.sised via green protocol, microwave. Results: Th. compounds were investigated by (IR, 1h.MR, 13CNMR and mass spectrometry). Wh.re some of ph.rmacological activities like antibacterial and antifungal studies h.d been investigated and ch.racterized. It was found th.t 3a-d h.d ch.racterized by h.gh.activities as antibacterial and antifungal. Wh.re microwave synth.tic meth.ds were more efficient, gave h.gh.r products quantity, and more saving for time requirement and for using of much.more solvents.

Background: Physical activity has a cardinal role in preventing and treating cardiovascular diseases. Understanding the changes that occur to the cardiac muscle in athletes is still doubtful whether it is only an adaptation to exercise or a concealed pathological condition. Most of these changes are well documented in apparently healthy heart and characterized by cardiac enlargement with Brady-arrhythmias specially individuals participating in long distance runners with exercise capacity without signs of cardiovascular disease. Methods: The study searched the subclinical myocardial necrosis by comparing two groups of young marathon runners, both groups were chosen from Al Gizera youth center in January 2018. First group included adults above the age of 18 years and ran for 12 km. and the second was under 18 years and ran for about 8 km. Both groups were volunteered for this study. They are monitored carefully with blood pressure, heart rate and respiratory rate and their blood samples were collected pre- and post-race immediately for assessment of cardiac markers NTproBNP, Galectin-3 and cTnI. Results: All cardiac markers were elevated post exercise compared to that achieved in pre-exercise regardless age of athletes. Also, pre-exercise adult results were higher in galectin-3 and cTnI but not in proBNP while there is no difference in the acute response in both groups. These results revealed micro cardiac necrosis due to long run exercise with possible bad prognosis. Conclusions: It is important to develop risk assessment and prediction methods for screening the young athletes and consider the term athletic heart as a subclinical disorder rather than physiological adaptation.

Th. Impact of Neutroph.l to Lymph.cytic Ratio (NLR) as a Predictor of Treatment Outcomes in Rectal Carcinomas: A Retrospective Coh.rt Study

Background and aim: Th. prognostic role of neutroph.l to lymph.cyte ratio (NLR) h.s been sh.wn in many solid tumors included in a recent meta-analysis of one h.ndred studies. We aimed to evaluate th. prognostic value of neutroph.l to lymph.cyte ratio in treatment outcomes;response and survival of patients with.different stages of rectal cancers. Patients and meth.ds: All patients with.path.logically confirmed cancer rectum presented to our department during th. period from January 2012 to th. end of 2014 were included in th.s retrospective study, th.se recruited patients were evaluated th.ough.th.ir files to determine different objectives of our study. Results: Th. median overall survival was 31 ± 4.676 month. wh.le disease free survival was 40 ± 2.346 for th. wh.le study group;neutroph.l to lymph.cyte ratio was negatively correlated with.overall survival with.r = –0.743, P disease free survival with.r = –0.717, P total number of lymph.nodes dissected ratio with.r = +0.254, P = 0.028. Roc curve was used to find th. accurate cut point of NLR for th.se patients and was found to be of 4.5. Conclusion: Elevated pre-treatment NLR is an independent predictor of sh.rter survival in patients with.rectal cancer. Th.s parameter is a simple, easily accessible laboratory test for identifying patients with.poorer prognosis.

Reconstruction of h.meral h.ad Defect in Locked Posterior Dislocation Sh.ulder. A Case Series of Nine Patients

Locked posterior dislocation sh.ulder is uncommon and frequently missed injury. It account for 2% - 4% of all sh.ulder dislocations. It is commonly associated with.osseous defects in h.meral h.ad articular surface known as reverse h.ll-Sach. lesion. Numerous surgical procedures invented to repair th.s defect with.variable outcomes but evidence based management strategies are lacking. Among th.se procedures are: transfer of lesser tuberosity or subscapularis tendon, rotational osteotomy of h.merus, osteoch.ndral grafts. Salvage procedure as h.miarth.oplasty or total sh.ulder arth.oplasty used in h.ge non-constructable defect or very old neglected dislocation. In our case series, we treated 9 cases (2 females) of locked posterior sh.ulder dislocation with.antero-medial h.meral h.ad defects ranging between 30% - 50% of h.ad size. Open reduction of dislocation followed by transfer of th. lesser tuberosity togeth.r with.subscapularis tendon for reconstruction of th. h.meral h.ad defect. Th. transfer was fixed with.Eth.bond suture size 5-0 (Eth.con, Inc. Somerville, New Jersy). Th. mean follow-up period was 14.5 month. (range, 12 - 25 month.). Seven cases h.d no pain or restriction of activities of daily living. No patient h.d symptoms of instability of th. sh.ulder. According to UCLA Sh.ulder rating scale, th.re were 3 cases rated excellent, 4 cases rated good, one case rated fair and one case rated poor. It is concluded th.t reconstruction of th. h.meral h.ad defect provides good pain relief, stability and function for patients with.a locked posterior dislocation wh.re th. defect involves between 30% - 50% of th. articular surface circumference. Our tech.ique is simple, ch.ap and th.re is no need for second operation for h.rdware removal.

Objective: Th. purpose of th.s trial was to determine wh.th.r calcium supplementation of pregnant women with.low calcium intake reduces preeclampsia and preterm delivery. Study design: Randomized placebo- controlled, double- blinded trial in nulliparous normotensive women from populations with.dietary calcium h. were recruited before gestational week 20 received supplements (1.5 g calcium/d or placebo) th.ough.out pregnancy. Primary outcomes were preeclampsia and preterm delivery; secondary outcomes focused on severe morbidity and maternal and neonatal mortality rates. Results: Th. groups comprised 8325 women wh. were assigned randomly. Both.groups h.d similar gestational ages, demograph.c ch.racteristics, and blood pressure levels at entry. Compliance were both.85% and follow- up losses (calcium, 3.4% ; placebo, 3.7% ). Calcium supplementation was associated with.a non- statistically significant small reduction in preeclampsia (4.1% vs 4.5% ) th.t was evident by 35 weeks of gestation (1.2% vs 2.8% ; P = .04). Eclampsia (risk ratio, 0.68: 95% CI, 0.48- 0.97) and severe gestational h.pertension (risk ratio, 0.71; 95% CI, 0.61- 0.82) were significantly lower in th. calcium group. Overall, th.re was a reduction in th. severe preeclamptic complications index (risk ratio, 0.76; 95% CI, 0.66- 0.89; life- table analysis, log rank test; P = .04). Th. severe maternal morbidity and mortality index was also reduced in th. supplementation group (risk ratio, 0.80; 95% CI, 0.70- 0.91). Preterm delivery (th. neonatal primary outcome) and early preterm delivery tended to be reduced among women wh. were ≤ 20 years of age (risk ratio, 0.82; 95% CI, 0.67- 1.01; risk ratio, 0.64; 95% CI, 0.42- 0.98, respectively). Th. neonatal mortality rate was lower (risk ratio, 0.70; 95% CI, 0.56- 0.88) in th. calcium group. Conclusion: A 1.5 g calcium/day supplement did not prevent preeclampsia but did reduce its severity, maternal morbidity, and neonatal mortality, albeit th.se

Objective: To evaluate th. role of th.rapeutic amnioinfusion using a pediatric feeding tube in cases of intrapartum fetal distress. Meth.ds: A randomized clinical trial including 438 women admitted in labor at Assiut University h.spital with.nonreassuring fetal h.art rate tracing. Using sealed opaque envelopes, th. women were randomized to 2 groups. In th. amnioinfusion group th.y underwent transcervical amnioifusion (1000 mL of warmed sterile saline solution) in addition to conventional treatment. In th. control group th.y received conventional treatment only. Th. primary outcome was cesarean section rate for fetal distress. Th. secondary outcomes were neonatal and maternal complications. Results: Th. amnioinfusion group sh.wed a significant reduction in th. rate of cesarean section for fetal distress (relative risk [RR], 0.7; 95% confidence interval [CI], 0.6- 0.83), and a 30% reduction in abnormal fetal h.art rate patterns (RR, 0.7; 95% CI, 0.6- 0.83). Significantly fewer newborns h.d Apgar scores less th.n 7 at 1 and 5 min in th. amnioinfusion group th.n in th. control group (RR, 0.38; 95% CI, 0.26- 0.55 and RR, 0.31; 95% CI, 0.15- 0.64, respectively). Significantly fewer newborns h.d meconium below th. vocal cords in th. amnioinfusion group th.n in th. control group (RR, 0.36; 95% CI, 0.13- 0.97). Moreover, 14 newborns in th. amnioinfusion group needed admission to th. intensive care unit vs. 31 newborns in th. control group. Th.re were no significant differences between th. 2 groups regarding th. incidence rates of uterine h.pertonus and maternal temperature h.gh.r th.n 38° C. Conclusion: Th.rapeutic amnioinfusion is a simple and effective intervention th.t reduces th. rates of cesarean section for intrapartum nonreassuring fetal h.art tracing. In under-resourced settings, it can be performed using inexpensive cath.ters.

JUNO is a multi-purpose neutrino observatory under construction in th. south.of *** publication presents new sensitivity estimates for th. measurement of th. △m_(31)^(2),△m_(21)^(2),sin^(2)θ_(12),and sin^(2)θ_(13) oscillation parameters using reactor antineutrinos,wh.ch.is one of th. primary ph.sics goals of th. *** sensitivities are obtained using th. best knowledge available to date on th. location and overburden of th. experimental site,th. nuclear reactors in th. surrounding area and beyond,th. detector response uncertainties,and th. reactor antineutrino spectral sh.pe constraints expected from th. TAO satellite *** is found th.t th. △m_(21)^(2) and sin^(2)θ_(12) oscillation parameters will be determined to 0.5%precision or better in six years of data *** th. same period,th. △m_(31)^(2) parameter will be determined to about 0.2%precision for each.mass ordering *** new precision represents approximately an order of magnitude improvement over existing constraints for th.se th.ee parameters.
