The a.socia.ion of a.group of prespecified a.herosclerotic risk genotypes with recurrent corona.y events(corona.y-rela.ed dea.h, nonfa.a. myoca.dia. infa.ction, or unsta.le a.gina. wa. investiga.ed in a.cohort of 1,008 pa.ients a.ter infa.ction during a. a.era.e follow-up of 28 months. We used a.ca.rier-ship a.proa.h with time-dependent survivorship a.a.ysis to eva.ua.e the a.era.e risk of ea.h ca.ried genotype. Contra.y to expecta.ion, the ha.a.d ra.io for recurrent corona.y events per ca.ried versus nonca.ried genotype wa. 0.89(95%confidence interva. 0.80 to 0.99, p=0.03) a.ter a.j.stment for releva.t genetic, clinica., a.d environmenta. cova.ia.es. This ha.a.d ra.io, derived from the 7 prespecified genotypes, indica.ed a. a.era.e 11%reduction in the risk of recurrent corona.y events per ca.ried versus nonca.ried genotype. a. 1 yea. a.ter hospita. discha.ge, the cumula.ive proba.ility of recurrent corona.y events wa. 26%in those who ca.ried ≤1, 20%for those with 2 to 4, a.d 13%for those with ≥5 of these genotypes(p=0.02). This unexpected risk reversa. is a.likely consequence of cha.ges in the mix of risk fa.tors in pre-a.d postinfa.ction popula.ions. In conclusion, this under a.precia.ed, popula.ion-ba.ed, risk-reversa. phenomenon ma. expla.n the inconsistent a.socia.ions of genetic risk fa.tors with outcome events in previous reports involving corona.y popula.ions with different risk a.tributes.

a.M: Prospective eva.ua.ion of repea. endoscopic retrogra.e chola.giopa.crea.ogra.hy (ERCP) for fa.led Schutz gra.e 1 bilia.y ca.nula.ion in a.high-volume center. METHODS: Prospective intention-to-trea. a.a.ysis of pa.ients referred for bilia.y ca.nula.ion following recent unsuccessful ERCP. RESULTS: Fifty-one pa.ients (35 fema.e; mea. a.e: 62.5 yea.s; a.e ra.ge: 40-87 yea.s) with previous fa.led bilia.y ca.nula.ion were referred for repea. ERCP. The indica.ion for ERCP wa. prima.ily choledocholithia.is (45%) or pa.crea.ic ma.igna.cy (18%). Successful bilia.y ca.- nula.ion wa. 100%. The precut needle knife sphincterotomy (NKS) ra.e wa. 27.4%. Complica.ions occurred in 3.9% (post-ERCP pa.crea.itis). a. identif ia.le rea.on for initia. unsuccessful bilia.y ca.nula.ion wa. present in 55% of ca.es. Compa.ed to a.cohort of 940 na.ve pa.pilla.pa.ients (fema.e 61%; mea. a.e: 59.9 yea.s; a.e ra.ge: 18-94 yea.s) who required sphincterotomy over the sa.e time period, there wa. no sta.istica. difference in the ca.nula.ion success ra.e (100% vs 98%) or postERCP pa.crea.itis (3.1% vs 3.9%). Precut NKS use wa. more frequent (27.4% vs 12.7%) (P = 0.017). CONCLUSION: Referra. to a.high-volume center following unsuccessful ERCP is a.socia.ed with high technica. success, with a.fa.ora.le complica.ion ra.e, compa.ed to routine ERCP procedures.
