Thiazide-associated hyponatremia in the elderly： what the clinician needs to know

Thiazide-induced hyponatremia is one of the main causes of decreased sodium levels in elderly individuals. This review presents the current evidence regarding the thiazide-associated hyponatremia. Thiazide-associated hyponatremia is observed mainly in patients with certain risk factors such as those receiving large doses of thiazides, having much comorbidity, such as heart failure, liver disease or malignancy, and taking several medications, such as non-steroidal anti-inflammatory drugs, selective serotonin re-uptake inhibitors or tricyclic antide- pressants. Sodium concentration should be monitored in patients with risk factors for developing thiazide-associated hyponatremia and clini- cians should measure promptly serum sodium levels in patients with neurologic signs indicating reduced sodium levels. The clinical and biochemical profile of patients with thiazide-associated hyponatremia may be that of extracellular volume depletion or the syndrome of inap- propriate antidiuretic hormone secretion （SIADH）. The investigation of possible thiazide-associated hyponatremia includes the exclusion of other causes of decreased sodium levels and the identification of the characteristics of hyponatremia due to thiazides （extracellular volume depletion-related or SIADH-like）. Treatment should be carefully monitored to avoid serious neurologic complications due to overcorrection. Clinicians should discourage prescribing thiazides in patients with a history of diuretic-associated hyponatremia and should prefer low doses of thiazides in patients with risk factors for developing thiazide-associated hyponatremia.

Quality attributes of chicken nuggets extended with different legume flours

Chicken nugget is a comminuted meat product commonly prepared from spiced chicken meat and other ingredients. The tenderness of chicken meat lowers its firmness and mouth feel which may reduce acceptability of chicken nugget made from it. Thus, acceptability of chicken nugget could be harnessed when legume flour extenders are used along. Therefore, acceptability as well as quality of chicken nugget from different legume flours were evaluated. Soyabean, groundnut and cowpea flours were used as meat extenders for development of raw chicken nuggets and thereafter cooked for consumption. The quality of both raw and cooked legume flour extended chicken nuggets were assessed based on functional properties, sensory properties, proximate composition, amino acid content and shelf stability in terms of lipid peroxidation and microbial load. In their raw state, the legume flour extenders competed favourably with each other. Among all, soyabean flour extender maintained remarkable functional properties that transcend into significant (P < 0.05) yield of 86.93% of chicken nugget in comparison to the control (86.37%), groundnut (84.95%) and cowpea (84.50%). Upon cooking, all the legume flour extended chicken nuggets varied in their quality attributes. Apart from the high level of flavour and low microbial load, cowpea extended chicken nugget was of low quality based on the parameters evaluated in comparison with other legume flour extended chicken nuggets. Of interest, soyabean extended chicken nugget followed by groundnut extended chicken nugget were of good quality based on sensory properties, high crude protein and amino acid levels, low cholesterol content and lipid peroxidation value as well as low microbial load.

Late-onset Alzheimer＇s disease （LOAD） is an age-related neurodegenerative disorder characterized by gradual loss of synapses and neurons, but its pathogenesis remains to be clarified. Neurons live in an environment constituted by neurons themselves and glial cells. In this review, we propose that the neuronal degeneration in the AD brain is partially caused by diverse environmental factors. We first discuss various environmental stresses and the corresponding responses at different levels. Then we propose some mechanisms underlying the specific pathological changes, in particular, hypothalamic-pituitary adrenal axis dysfunction at the systemic level; cerebrovascular dysfunction, metal toxicity, glial activation, and Aβ toxicity at the intercellular level; and kinase-phosphatase imbalance and epigenetic modification at the intracellular level. Finally, we discuss the possibility of developing new strategies for the prevention and treatment of LOAD from the perspective of environmental stress. We conclude that environmental factors play a significant role in the development of LOAD through multiple pathological mechanisms.

Internet of Vehicles（IoV） is regarded as an emerging paradigm for connected vehicles to exchange their information with other vehicles using vehicle-to-vehicle（V2V） communications by forming a vehicular ad hoc networks（VANETs）, with roadside units using vehicle-to-roadside（V2R） communications. IoV offers several benefits such as road safety, traffic efficiency, and infotainment by forwarding up-to-date traffic information about upcoming traffic. For instance, IoV is regarded as a technology that could help reduce the number of deaths caused by road accidents, and reduce fuel costs and travel time on the road. Vehicles could rapidly learn about the road condition and promptly respond and notify drivers for making informed decisions. However, malicious users in IoV may mislead the whole communications and create chaos on the road. Data falsification attack is one of the main security issues in IoV where vehicles rely on information received from other peers/vehicles. In this paper,we present data falsification attack detection using hashes for enhancing network security and performance by adapting contention window size to forward accurate information to the neighboring vehicles in a timely manner（to improve throughput while reducing end-to-end delay）. We also present clustering approach to reduce travel time in case of traffic congestion. Performance of the proposed approach is evaluated using numerical results obtained from simulations. We found that the proposed adaptive approach prevents IoV from data falsification attacks and provides higher throughput with lower delay.

This study reformulates the concept and contents of disaster risk reduction(DRR) in Hong Kong through an explorative study on collaborative place audit(CPA) and social vulnerability index(SVI) for *** believe that DRR should be place-and ***,DRR needs to go beyond technical concerns and address vulnerability and risk encountered in the built environment where an individual is located.A place-centered DRR begins with an assessment of person–environment relations from an interdependent *** becomes a significant scale at which to address vulnerability and risks across a range of environmental,socioeconomic,and institutional factors.A CPA is a ground-level assessment tool that identifies vulnerability and risk in the built and social *** audit encourages collaboration in problem solving that uses social capital to effect decisionmaking change in hierarchies and policy *** DRR facilitates distinguishing living-alone elders from the general *** perspective addresses varying degrees of vulnerability due to social and communicational isolation,poverty,disability,being sent to hospital and/or receiving institutional care,as well as lack of access to primary ***,SVI,based oncompound indicators,is developed to assess the differentiation of vulnerability across the territory with particular reference to the *** two approaches,namely,CPA and SVI,build community capacity to develop a resilient city,as well as to provide evidence-based recommendations that improve government-led disaster preparedness and contingency plans.

Travel time reliability(TTR) modeling has gain attention among researchers’ due to its ability to represent road user satisfaction as well as providing a predictability of a trip travel *** this significant effort,its impact on the severity of a crash is not well *** study analyzes the effect of TTR and other variables on the probability of the crash severity occurring on arterial *** address the unobserved heterogeneity problem,two random-effect regressions were applied;the Dirichlet random-effect(DRE)and the traditional random-effect(TRE) logistic *** difference between the two models is that the random-effect in the DRE is non-parametrically specified while in the TRE model is parametrically *** Markov Chain Monte Carlo simulations were adopted to infer the parameters’ posterior distributions of the two developed *** four-year police-reported crash data and travel speeds from Northeast Florida,the analysis of goodness-of-fit found the DRE model to best fit the ***,it was used in studying the influence of TTR and other variables on crash *** DRE model findings suggest that TTR is statistically significant,at 95 percent credible intervals,influencing the severity level of a crash.A unit increases in TTR reduces the likelihood of a severe crash occurrence by 25 ***,among the significant variables,alcohol/drug impairment was found to have the highest impact in influencing the occurrence of severe *** significant factors included traffic volume,weekends,speed,work-zone,land use,visibility,seatbelt usage,segment length,undivided/divided highway,and age.

Photoluminescence properties of a novel orange-red emitting Ba_2CaZn_2Si_6O_(17):Sm^(3+)phosphor

Novel orange-red emitting Ba_2Ca_（1–x）Zn_2Si_6O_（17）：x Sm^（3＋）（0.02≤x≤0.08） phosphors were synthesized using conventional solid-state reaction method under air atmosphere. The phase formation of the samples was characterized by powder X-ray diffraction patterns. Scanning electron microscopy（SEM） and photoluminescence properties were also investigated. The narrow excitation and emission spectra indicated the typical 4f-4f transitions of Sm^（3＋）. The dominant excitation line was around 405 nm attributed to ~6H_（5/2）→ ~4F_（7/2） and the emission spectrum consisted of four emission peaks at 562, 600, 647, and 708 nm corresponding to the various transitions ~4G_（5/2） to ~6HJ（J=5/2, 7/2, 9/2 and 11/2） of the Sm^（3＋） ions in the same order. The strongest emission band located at 600 nm was attributed to ~4G_（5/2）→~6H_（7/2） transition of Sm^（3＋）, producing bright orange-red color emission. The optimal dopant concentration of Sm3＋ ion in Ba_2CaZn_2Si_6O_（17）：x Sm^（3＋） phosphor was around 4 mol.% and the critical transfer distance（Rc） of Sm^（3＋） was calculated to be 2.65 nm. Decay time varied with the Sm^（3＋） concentrations in Ba_2 CaZn_2Si_6O_（17） phosphors. In addition, the Commission International del＇Eclairagethe（CIE） chromaticity coordinates of Ba_2Ca_（0.96）Zn_2Si_6O_（17）：0.04Sm^（3＋） phosphor was located in the orange-red region（0.547, 0.450） and the correlated color temperature（CCT） was 2543 K. The present results indicated that Sm^（3＋） activated Ba_2CaZn_2Si_6O_（17） phosphors may be used as an orange-red emitting phosphor for near-ultraviolet（n-UV） based white light emitting diodes（WLEDs） applications.

Stemflow is a focused point source input of precipitation and nutrients at the base of a tree or plant and can have a significant impact on site hydrology. To date, no known studies have modelled stemflow production for juvenile lodgepole pine （Pinus contorta vat. latifolia）. Meteorological conditions, tree characteristics, and stemflow were sampled for two juvenile lodgepole pine stands over the course of the 2009 growing season. Step-wise multiple regression was used to assess which meteorological and tree architecture variables influenced stemflow production for each research plot. Once predictor variables were identified, models were produced for each stand and a generic model was produced that applied to both plots. A model employing precipitation depth and crown projection area successfully explained 71.3% of the variation in stemflow production from sampled trees. Stemflow was found to represent 1.8% of the study period rainfall and, although not a large component of the plot-scale canopy water balance, it is an order of magnitude greater than the fractioning of stemflow from mature lodgepole and lodgepole pine dominated forest. Additionally, stemflow funnelling ratios were found to average 22.2 and 24.3 from the two sample plots over the study period with a single tree, single event maximum of 111.7 recorded for a tree with a 3.3 cm bole diameter and a rain depth of 17.4 mm.
