Legionellosis is the generic term used to describe infections caused by different varieties of Legionella spp.,including Legionnaires'disease(LD),a severe and potentially fatal form of pneumonia,and Pontiac fever,a self-limited flu-like *** is usually acquired through inhalation or aspiration of aerosols containing Legionella *** bacteria can cause acute consolidating pneumonia in susceptible patients who are at an advanced age,have underlying debilitating diseases,or are *** main natural reservoir for Legionella is water and this pathogen colonizes many different natural and man-made freshwater environments such as water networks,cooling towers,and water systems in buildings and *** recent years,various laboratory diagnostic tests for Legionella infections have changed *** the sequencing method is nowadays considered the fastest and most reliable method for differentiation and detection of different Legionella species,the isolation of these bacteria from clinical specimens is the golden standard for diagnosis of Legionnaires'*** the urinary antigen test as the most rapid and inexpensive method is routinely used for diagnosis of LD caused by Legionella pneumophila serogroup *** macrolides and fluoroquinolones are still the mainstays for the treatment of Legionella *** the prevention of spreading the contaminated water aerosols and controlling Legionella infections,an effective water treatment procedure is *** review describs and summarizes the latest available information about all aspects of Legionella and Legionnaires'disease.

Effects of shallomin and podophyllin solution 25% for genital HPV warts in women: a randomized controlled trial

Objective: To compare the effect of shallomin (pure fraction of Allium hirtifolium) with podophyllin 25% solution on external genital human papillomavirus warts in women. Methods: This study was a randomized controlled trial which was performed on two groups of 25 Iranian women with external genital warts at Imam Khomeini Hospital in Ahvaz, Iran. In the first group, shallomin was used once a day for six weeks at home. In the second group, 25% podophyllin solution, was applied on the lesion once weekly for six weeks. Results: Shallomin and podophyllin resulted in wart clearance in 13/23 (56.5%), and 12/24 (50%) of patients, respectively. The clearance rate for shallomin was not significantly different from that of podophyllin (P=0.082). Six weeks after the treatment, the sizes of the lesions in the shallomin group and the podophyllin group decreased by (1.43±0.53) mm and (1.64±0.70) mm, respectively. Conclusion: Shallomin is an effective treatment for genital warts, with similar efficacy to that of podophyllin.

Integron frequency of Escherichia coli strains from patients with urinary tract infection in Southwest of Iran

Objective: To investigate the frequency of integrase genes intI1, intI2 and intI3 of Escherichia coli strains, and their association with resistance to routinely used antibiotics. Methods: A total of 120 Escherichia coli strains were collected from patients with urinary tract infection in Ahvaz, Southwest of Iran. Antibiotic susceptibility testing was performed. The presence of intI1, intI2, and intI3 genes was determined by polymerase chain reaction. Results: Antibiotic susceptibility testing disclosed the highest resistance rate to ampicillin (91.7%) followed by trimethoprim/sulfamethoxazole (65.8%), and ceftazidime (56.7%). The imipenem susceptibility rate was 91.7%. IntI1 and intI2 were identified in 74 (61.6%) and 8 (6.6%) of Escherichia coli strains, respectively, but intI3 was not found in any isolates. The presence of integrons was significantly associated with resistance to ampicillin, trimethoprim/sulfamethoxazole, ceftazidime, and ciprofloxacin antibiotics (P<0.05). Conclusions: The high resistant Escherichia coli isolates harboring class 1 integrons (intI1) were detected in patients with urinary tract infection in our region. Therefore, preventive strategies are necessary to restrict further dissemination of resistant strains.

Distribution of extended-spectrum β-lactamase genes in antibiotic-resistant strains of Pseudomonas aeruginosa obtained from burn patients in Ahvaz, Iran

Objective: To evaluate the drug susceptibility profiles and the frequency of beta-lactamase encoding genes in Pseudomonas aeruginosa (P. aeruginosa) obtained from burn patients. Methods: Totally 93 non-duplicate clinical isolates of P. aeruginosa were recovered from burn patients of Taleghani Burn Hospital of Ahvaz. Antibiotic susceptibility testing was conducted by disk diffusion method according to the CLSI 2017 recommendations. PCR assay was performed by to find beta-lactamase encoding genes. Results: In this study, most clinical specimen was obtained via wound swabs [65 (69.9%)], followed by blood [14 (15.1%)] and biopsy (7 (7.5%))Forty-two (45.16%) patients were male and 51(54.84%) were female. High resistance was observed for most of antibiotics especially for gentamicin and ciprofloxacin (Up to 85%), whereas the highest susceptibility was reported for colistin (100.0%), followed by ceftazidime (66.7%). According to PCR results, 16.1% (15), 9.7% (9) and 14.0% (13) of isolates carried blaDHA, blaVEB and blaGES genes, respectively. It also revealed that the blaVEB gene was found to coexist within 2 isolates (2.2%). Conclusions: Antibacterial resistance is high among P. aeruginosa isolates. Colistin is highly active against multi-drug resistant P. aeruginosa isolates. Antimicrobial susceptibility testing can confine indiscriminate uses of antibiotics and resistance increase, and can improve management of treatment.

SHRIMP zircon U-Pb ages， geochemical， and Sr-Nd isotopic constraints on the petrogenesis of the Middle Eocene calc-alkaline andesitic rocks: Implications for continental arc magmatism and slab break-off in NE Iran

The Torbat-e-Heydariyeh andesitic rocks (THA) are part of the Cenozoic continental arc magmatic system of the northern branch of the Neotethys Ocean (NE Iran). Columnar jointing is the most significant feature of these rocks and they also show porphyritic， vitrophyric， and vitroglomeroporphyric textures. Plagioclase， clinopyroxene， ± orthopyroxene are the major mineral phases. SHRIMP U-Pb zircon dating yielded an age of 41.00 ± 0.69 Ma for the rocks (Middle Eocene， Bartonian). Geochemically， they are medium- to high-K calc-alkaline affinity. Primitive mantle-normalized diagrams exhibit enrichment in large ion lithophile elements (LILE)， such as Cs and Rb， and also depleted in high field strength elements (HFSE) and heavy rare earth elements (HREE)， with prominent negative anomalies of Ti， Nb， Y， and Yb， suggesting a tectonic setting of an active continental margin. The Chondrite-normalized REE diagram displays enrichment of light rare earth elements (LREE; LaN/YbN= 5.37–6.66) and small negative Eu anomalies (Eu/Eu* of 0.69–0.78). Thorium enrichment implies the reaction between the mantle wedge and the melt of subducting oceanic slab， and/or subducting sediment. The role of subducted sediments along with subducted oceanic lithosphere is evident in these magmatic rocks using Ba/La versus Th/Nd and Ba/Th versus LaN/SmNdiagrams. εNd(t) and (87Sr/86Sr)ivalues vary between -0.1 to +0.2 and 0.704 89 to 0.705 01， respectively， and are compatible with parental melts from subduction of the lithospheric mantle. We suggest that the THA rocks were produced by the partial melting of the metasomatized lithospheric mantle， which corresponds to slab break-off of the northward subducted Neotethys Oceanic slab in an extensional setting. The hot asthenospheric mantle upwelling triggered by the Neotethys slab break-off would severely heat the physically mixed mantle wedge peridotite and therefore caused partial melting to produce the Middle Eocene volcanic rocks in NE Iran.

Molecular detection of oxacillinase genes and typing of clinical isolates of Acinetobacter baumannii in Tehran, Iran

Objective:To determine patterns of antimicrobial resistance,analyze the prevalence of oxacillinase and molecular typing of strains of Acinetobacter baumannii(***).Methods:A total of 121 strains of *** were obtained from patients admitted to Imam Hossein and Imam Khomeini Hospitals,Tehran,Iran,from January 2016 to November *** susceptibility testing was performed by Kirby-Bauer disc diffusion method according to the Clinical and Laboratory Standards Institute *** presence of oxacillinase genes was assessed by polymerase chain reaction(PCR).To determine clonal relatedness,all isolates were subjected to repetitive sequence-based PCR(REP-PCR).Results:The isolates were obtained from 56(46.3%)males and 65(53.7%)females with the mean age of 39.5 *** with 100.0%sensitivity rate had the highest effect,while ceftriaxone with 16.5%sensitivity rate had the least effect on *** *** addition,96(79.3%)and 99(81.8%)isolates were resistant to imipenem and meropenem,respectively.A total of 109 isolates(90.0%)exhibited multiple drug resistance with 10 different *** total,75(75.7%)of carbapenem resistant isolates were positive for blaOXA-23-like,and 14(14.1%)for blaOXA-24-like *** five main clones A,B,C,D,and E were detected in 25(25.2%),36(36.4%),10(10.1%),8(8.0%),and 6(6.1%)of isolates,***:Carbapenem-resistant *** strains are high in the current *** control the spread of carbapenem-resistant *** strains,regular monitoring programs are needed.
