Comparing corneal outcome between femtosecond laser-assisted cataract surgery and conventional phaco surgery in Fuchs’endothelial dystrophy patients:a randomized pilot study with 6mo follow up

AIM:To compare the corneal outcome in Fuchs’endothelial dystrophy(FED)patients between femtosecond laser-assisted cataract surgery(FlaCS)and conventional phaco surgery(CPS).METHODS:This was a randomized controlled study comparing one eye surgery by FlaCS and the contralateral eye operated by CPS(stop and chop technique)in FED *** corneal thickness,corneal light backscatter,corneal densitometry,and central corneal endothelial cell count and hexagonality(noncontact endothelial cell microscope),and corrected distance visual acuity(CDVA)were assessed preoperatively and at day 1,40,and 180 ***:Totally 31 patients(16 women)were *** day 40 postoperatively,the mean endothelial cell loss(ECL)was 23.67%by FlaCS and 17.30%by CPS(P=0.53).At day 180 postoperatively,ECL was 25.58%in FlaCS and 21.32%in CPS(P=0.69).Densitometry data in all layers and all annuli from anterior layer to posterior layer in annuli 0-2,2-6,6-10 and 10-12,total densitometry with all layers and all annuli was performed.A significant difference was found in 6-10(posterior layer)at day 1 with-1.42 grayscale units(GSU;95%CI:-2.66 to-0.19,P=0.02).In 10-12(anterior layer,central layer and all layers)at day 40 were significant different with 7.7(95%CI:1.89 to 13.50,P=0.009),3.97(95%CI:0.23 to 7.71,P=0.03),4.73 GSU(95%CI:0.71 to 8.75,P=0.02),*** the remaining parameters we found no difference between the two groups(P>0.05).Three CPS eyes suffered from corneal ***:There is no significant difference in corneal outcome between FlaCS and *** cell density and pentacam corneal outcome may be inadequate as outcome parameters in FED patients.

The human brain is a complex system, whose activity exhibits flexible and continuous reorganization across space and time. The decomposition of whole-brain recordings into harmonic modes has revealed a repertoire of gradient-like activity patterns associated with distinct brain functions. However, the way these activity patterns are expressed over time with their changes in various brain states remains unclear. Here, we investigate healthy participants taking the serotonergic psychedelic N,N-dimethyltryptamine(DMT) with the Harmonic Decomposition of Spacetime(HADES) framework that can characterize how different harmonic modes defined in space are expressed over time. HADES demonstrates significant decreases in contributions across most low-frequency harmonic modes in the DMT-induced brain state. When normalizing the contributions by condition(DMT and non-DMT), we detect a decrease specifically in the second functional harmonic, which represents the uni-to transmodal functional hierarchy of the brain,supporting the leading hypothesis that functional hierarchy is changed in psychedelics. Moreover, HADES'dynamic spacetime measures of fractional occupancy, life time and latent space provide a precise description of the significant changes of the spacetime hierarchical organization of brain activity in the psychedelic state.

Purpose: To examine patient survival in age- related maculopathy in a 14- year follow- up study. Design: Population- based 14- year cohort study. Participants: Nine hundred forty- six residents, aged 60 to 80 years, living in the sterbro district of Copenhagen, Denmark, participated in the first examination conducted from 1986 to 1988. These participants were followed until death or until May 1, 2002, whichever came first. Methods: Participants underwent an extensive ophthalmologic examination at Rigshospitalet, the National University Hospital of Copenhagen. Standardized protocols for physical examination, blood samples, and data from the National Central Person Register, the National Death Register, and the National Patient Register were used. Main Outcome Measures: Mortality and age- related maculopathy. Results: By May 1, 2002, 60.9% (577 of 946) of the participants of the baseline study cohort had died. The adjusted 14- year cumulative mortality hazard ratio for subjects with early and late age- related maculopathy at baseline was 1.26 (95% confidence interval [CI], 1.06- 1.51). We identified a strong correlation between mortality and agerelated maculopathy among women (relative risk, 1.59; 95% CI, 1.23- 2.07) but not among men. Conclusions: When adjusting for survival- related factors, age- related maculopathy is a significant risk indicator for poorer survival in women and may be amarker of underlying serious systemic factors or aging processes specific to women.

Objective: To estimate the occurrence of residual neuropathy and its self- reported health consequences in a population- based group of patients with Guillain- Barr syndrome (GBS) and to characterize quantitatively the concomitant motor, sensory, and autonomic impairments. Methods: Forty patients (mean age 46 years) with a confirmed diagnosis of GBS were studied a mean of 7 years (range 1 to 13 years) after the acute attack together with 40 healthy control subjects. The Dyck minimal criteria of neuropathy, the Neuropathy Symptom Score, the Neuropathy Disability Score, the Short Form- 36 (SF- 36)- generic health questionnaire, isokinetic dynamometry at ankle and wrist, quantitative sensory testing of thresholds for vibration, cold, and pain, autonomic function tests, nerve conduction studies, and a summed Neuropathy Rank Sum Score (NRSS)- were applied. Results: Nineteen patients (48% ) had residual neuropathy, which was independent of follow- up time. The patients with GBS reported lower health status than control subjects on the SF- 36 Physical Component Summary Scale (PCS; p = 0.01), and the PCS scores correlated with the NRSS (r = - 0.41, p = 0.009). In patients with GBS, muscle strength at ankle dorsal flexion was reduced by 13.9% (p = 0.001), sensory thresholds for vibration were increased in the foot (p large- and medium- sized myelinated fibers endures long after the acute attack of Guillain- Barr syndrome in approximately half of all patients, leads to motor and sensory dysfunction, and shows a trend toward impairing self- reported physical health status.

Analysis of Precipitation and Temperature Variability over Central Africa (1901-2015)

Africa is already experiencing the impact of climate change. Some of the manifestations of climate change in Africa are, changing weather patterns resulting in, flooding and drought. Temperature change has impacted health, livelihoods, productivity of food, availability of water, and state of security. This study examines the long-term climate variations in Central African Countries (Gabon, Cameroon, Republic of Congo, Central Africa Republic, Chad and Democratic Republic of Congo) for the period 1901 to 2015, and then investigates the possible influence of increases in greenhouse gas concentrations. To investigate climate patterns and trends in the Central African Countries, precipitation and temperature were analyzed on annual time scales using data collected from the World Bank Group Climate Change Knowledge Portal. Data was further aggregated using annual average blocks of 10 years. Linear and polynomial regression was performed. Also, linear time series slopes were analyzed to investigate the spatial and temporal trends of climate variability in Central African countries. Results of the analyses indicated that the mean annual temperature and precipitation records in some of the Central African Countries had both warming and cooling trends over the study period from 1901 to 2015. For example, differences between the maximum and the minimum rainfall data for Democratic Republic of Congo, Cameroon and Gabon were 13 mm, 13 mm and 11.1 mm, which corresponded to 11.04%, 10.03% and 10.44% respectively. The study also found the temperature of Chad to have significantly risen from 1901 to 2015 by almost 20%, while its rainfall’s variation was limited. Although the variation in rainfall in Chad was not dramatic, the temperature per 10 year rose by almost 20%. Chad’s temperature rose according to a cubic model from about 24.5°C to just below 27°C during the period 1901-1940. This was followed by a brief drop between 1940 and 1960. Use of GIS and Remote Sensing Technology as a Decision Support Tool in Flood Disaster Management: The Case of Southeast Louisiana, USA

The primary objective of this paper was to identify flood-prone areas in Southeast of Louisiana to help decision-makers to develop appropriate adaptation strategies and flood prediction, and mitigation of the effects on the community. In doing so, the paper uses satellite remote sensing and Geographic Information System (GIS) data for this purpose. Elevation data was obtained from the National Elevation Dataset (NED) produced by the United States Geological Survey (USGS) seamless data warehouse. Satellite data was also acquired from USGS Earth explorer website. Topographical information on runoff characteristics such as slope, aspect and the digital elevation model was generated. Grid interpolation TIN (triangulated irregular network) was carried from the digital elevation model (DEM) to create slope map. Image Drape was performed using ERDAS IMAGINE Virtual GIS. The output image was then draped over the NED elevation data for visualization purposes with vertical exaggeration of 16 feet. Results of the study revealed that majority of the study area lies in low-lying and very low-lying terrain below sea level. Policy recommendation in the form of the need to design and build a comprehensive Regional Information Systems (RIS) in the form of periodic inventorying, monitoring and evaluation with full support of the governments was made for the study area.

The neurophysiological mechanisms for persisting impairment of motor function after Guillain-Barr syndrome (GBS) were assessed in 37 unselected patients 1- 13 years after diagnosis. For evaluation of reinnervation and axonal loss, macroelectromyography (macro-EMG) including measurement of fiber density (FD) was performed. Data from neuropathy symptom score, neuropathy disability score, nerve conduction studies, and quantitative sensory examination were ranked and s ummed to a neuropathy rank sum score (NRSS). The isokinetic muscle strength at t he ankle was measured. Signs of axonal loss with increase of either macro motor unit potential (macro-MUP) amplitude or FD occurred in 76% of patients. The macro-MUP amplitude correlated with muscle strength and with NRSS. Patients wi th evidence of residual neuropathy had increased macro-MUP amplitude and FD as well as decreased muscle strength compared to patients without evidence of res idual neuropathy. We conclude that axonal loss takes place in a substantial numb er of GBS patients and is associated with permanent muscle weakness caused by in sufficient reinnervation. Possible patterns of pathology are discussed in relati on to the macro-EMG findings.

Purpose: To examine the association between potential risk factors and the 14 -year incidence of age-related maculopathy (ARM). Design: Population-based co hort study. Participants: At baseline, 946 volunteers participated in the study during 1986-88. These subjects were between 60 and 80 years of age and lived in the terbro district of Copenhagen. Excluding participants who had died since baseline, 359 subjects (97.3%of survivors) were reexamined 14 years later, duri ng 2000-2002. A total of 31.8%(301/946) of the original material was included in the risk factor analyses. Methods: Participants underwent an ophthalmological examination at Rigshospitalet, the National University Hospital of Copenhagen. Similar standardized protocols for physical examination were used at the baselin e and follow-up examinations. Age-relatedm- aculopa-thy lesionswere determined by the same grader grading colour fundus p hotographs from both examinations using a modification of the Wisconsin Age-rel ated Maculopathy Grading System protocol. Results: Of the 359 participants, 94 h ad incident early ARM and 52 had incident late ARM at follow-up in either eye. In logistic regression, the risk factors for early ARM or worse were as follows: cataract (odds ratio [OR] 2.8, 95%confidence interval [CI] 1.2-6.2); fami ly h istory of ARM (OR 4.5, 95%CI 1.3-15.5), and alcohol consumption >250 g/week (O R 4.6, 95%CI 1.1-19.2). High levels of apolipoprotein B ( > 100 mg/1) decrease d the risk of development of early ARM or worse (OR 0.4, 95%CI 0.2-0.8), while high levels of apolipoprotein A1 (≥150 mg/1) increased the risk of late ARM (O R 2.5, 95%CI 1.2-5.3). Advanced age at baseline was also associated with the i ncidence of late ARM(OR2.0, 95%CI 1.4-2.9). Conclusions: These findings indica te a direct correlation between age, cataract, family history, alcohol consumpti on, the apolipoproteins A1 and B and the 14-year incidence of ARM.

Purpose: To evaluate the long-term efficacy and safety of infliximab as treatment for noninfectious posterior uveitis. Methods: An open-label clinical trial including seven patients (12 eyes)-with posterior uveitis refractory to conventional treatment regimens with corticosteroids and at least one immunosuppressive agent. Three intravenous doses of 5 mg/kg of infliximab were administered at weeks 0, 2, and 6. Infliximab infusion was repeated in patients undergoing a relapse of uveitis after initial remission. Improvement was defined as amelioration of visual acuity or disappearance of retinal exudates and/or haemorrhages, decreased macular oedema and/or vitreous opacities. All patients were followed up for at least 36 months. Results: Six of the seven patients (five diagnosed with Beh et’s disease and one diagnosed with sarcoidosis) showed a significant improvement after the first infliximab dose. Only in one patient diagnosed with chronic idiopathic multifocal choroiditis did the drug have no effect, and this patient was withdrawn from the study. At the end of follow-up, one eye had lost one line of vision and three eyes showed improved vision. All eyes had improved in terms of signs of infla, mmation. No adverse effects of treatment were observed. Conclusion: Infliximab is efficient and safe for the long-term management of refractory posterior uveitis, especially in patients with predominant retinal vasculitis and vitritis.
