Mesially Directed Inclination of Posterior Teeth Contributing to Maxillary High Canine, and Changes during Orthodontic Treatment

Objective: The aim of study was to evaluate the nature of mesiodistal angulation of the posterior teeth in patients with maxillary high canine, and to identify possible associations of crowding with mesial axial angulation of the maxillary posterior teeth. Methods: The present study included seven Japanese male and 23 Japanese female patients. Study models were analyzed to evaluate the effectiveness of orthodontic treatment. A protractor was used to measure crown angulations, which were measured with reference to the posterior angle between the functional occlusal plane (FOP) and the long axis of the clinical crown of the lateral teeth and first molars. A sliding digital caliper was used to measure dental arch widths. Changes in the rotation of the maxillary molars were analyzed using “occlu-sograms”, which were printed from photographs of the occlusal aspect of the study model. Results: The maxillary teeth interacted with one another in the alveolar bone, resulting in progressive mesial tipping of the posterior teeth before orthodontic treatment. After orthodontic treatment, the angle relative to the FOP exhibited a significant increase, indicating that the maxillary teeth underwent distally directed uprighting (P P Conclusions: Based on the results, both uprighting of mesially tipped lateral teeth and expansion of narrow dental arches could prove to be keys to success in space regaining or correction of high canines and mild crowding.

Radiofrequent ablation(RFA)for treating hepatocellular carcinoma(HCC)has prevailed in Japan since it was assured legally to use in *** to algorithm of treating HCC,the application of RFA was 3 nodules whose diameter are less then 3cm or single nodule within 5cm in *** instruments are devided into expansive needle type and cool-tip *** tip needle system has been more accepted in our country because of small puncture *** RFA ablation,safety margin of more than 5mm is required to bring about local control. Incidence of local recurrence after RFA for HCC≦3cm and＞3cm were 6 to 21% and 8 to 40% respectively. Incidence of intrahepatic distant recurrence was 33 to 85% three years *** are also some reports decribing the complication and problems of *** presentation is focused on the complication of RFA.

Background Management of pacemaker(PM)infections among advanced aged patients possesses particular clinical challenges due to higher rates of concurrent cardiovascular disease and medical *** leadless cardiac pacemakers(LCPs)may provide new opportunities for better management options in this population,however,there is limited data especially in Asian populations to guide the decision *** We reviewed 11 octogenarians(median age:86[minimum 82–maximum 90]years;male:73%;median body mass index(BMI):20.1 kg/m^(2))who received Micra Transcatheter Pacing System(Medtronic Inc,Minneapolis,MN)implantations following transvenous lead extractions(TLEs)for PM *** All patients had more than two medical comorbidities(average 3.7 comorbidities).The indications for LCP implantations were atrioventricular block in four patients,atrial fibrillation bradycardia in five,and sinus node dysfunction in *** patients(73%)were bridged with temporary pacing using active fixation leads(median interval of 14.0 days),while one with severe dementia underwent a concomitant LCP implantation and TLE during the same *** TLEs and LCP implantations were successfully accomplished in all without any *** median time from the TLE procedure to discharge was 22 days(minimum 7–maximum 136).All patients remained free of infections during a mean follow-up period of 17.2±6.5 *** LCP implantations were safe and effective after removing the entire infectious PM system in all *** novel LCP technology may offer an alternative option for considering a re-implantation strategy after transvenous PM infections in elderly patients,particularly those with severe frailty and PM dependency.

Incorporating GSA-SPECT into CT-based dose-volume histograms for advanced hepatocellular carcinoma radiotherapy

In single photon emission computed tomography-based three-dimensional radiotherapy(SPECT-B-3DCRT), im-ages of Tc-99 m galactosyl human serum albumin(GSA), which bind to receptors on functional liver cells, are merged with the computed tomography simulation im-ages. Functional liver is defined as the area of normal liver where GSA accumulation exceeds that of hepato-cellular carcinoma(HCC). In cirrhotic patients with a gigantic, proton-beam-untreatable HCC of ≥ 14 cm in diameter, the use of SPECT-B-3DCRT in combination with transcatheter arterial chemoembolization achieved a 2-year local tumor control rate of 78.6% and a 2-year survival rate of 33.3%. SPECT-B-3DCRT was applied to HCC to preserve as much functional liver as possible. Sixty-four patients with HCC, including 30 with Child B liver cirrhosis, received SPECT-B-3DCRT and none ex-perienced fatal radiation-induced liver disease(RILD). The Child-Pugh score deteriorated by 1 or 2 in > 20% of functional liver volume that was irradiated with ≥ 20 Gy. The deterioration in the Child-Pugh score decreased when the radiation plan was designed to irradiate ≤ 20% of the functional liver volume in patients givendoses of ≥ 20 Gy(FLV20Gy). Therefore, FLV20 Gy ≤ 20% may represent a safety index to prevent RILD during 3DCRT for HCC. To supplement FLV20 Gy as a qualitative index, we propose a quantitative indicator, F 20 Gy, which was calculated as F 20 Gy = 100% ×(the GSA count in the area irradiated with ≥ 20 Gy)/(the GSA count in the whole liver).

AIM:To evaluate the effects of combined radiofrequen-cy ablation and transcatheter arterial embolization with iodized oil on ablation time,maximum output,coagula-tion diameter,and portal angiography in a porcine liver model. METHODS: Radiofrequency ablation (RFA) was applied to in vivo livers of 10 normal pigs using a 17-gauge 3.0 cm expandable LeVeen RF needle electrode with or with-out transcatheter arterial embolization (TAE) with iodized oil (n = 5). In each animal,2 areas in the liver were ab-lated. Direct portography was performed before and af-ter RFA. Ablation was initiated at an output of 30 W,and continued with an increase of 10 W per minute until roll-off occurred. Ablation time and maximum output until roll-off,and coagulated tissue diameter were compared between the 2 groups. Angiographic changes on portog-raphy before and after ablation were also reviewed. RESULTS: For groups with and without TAE with iodized oil,the ablation times until roll-off were 320.6 ± 30.9 seconds and 445.1 ± 35.9 seconds,respectively,maxi-mum outputs were 69.0 ± 7.38 W and 87.0 ± 4.83 W and maximal diameters of coagulation were 41.7 ± 3.85 mm and 33.2 ± 2.28 mm. Significant reductions of abla-tion time and maximum output,and significantly larger coagulation diameter were obtained with RFA following TAE with iodized oil compared to RFA alone. Portography after RFA following TAE with iodized oil revealed more occlusion of the larger portal branches than with RFA alone. CONCLUSION: RFA following TAE with iodized oil can increase the volume of coagulation necrosis with lower output and shorter ablation time than RFA alone in nor-mal pig liver tissue.

Successful coil embolization with a basic technique for ruptured posterior tibial artery in a patient with Ehlers-Danlos syndrome type IV: A case report

The surgical management of vascular complication with Ehlers-Danlos Syndrome type IV (EDS-IV) is extremely challenging because of its tissue or organ fragility including arteries. Less traumatic maneuvers and devices have been recommended to perform the endovascular treatment for the vascular lesion of EDS-IV, although the endovascular procedure is less invasive than surgical intervention. We report a 23-year-old man with EDS-IV suffered from left posterior tibial arterial rupture. We performed internal trapping of the ruptured segment using detachable coils with a standard technique via the contralateral femoral artery puncture. The patient was discharged without any complication associated with the endovascular procedure. Embolization by a basic technique, performed with greatest care, is a safe and useful treatment option which we radiologists should try for managing EDS-IV patients.

Ruptured high flow gastric varices with an intratumoral arterioportal shunt treated with balloon-occluded retrograde transvenous obliteration during temporary balloon occlusion of a hepatic artery

A patient presented with hematemesis due to gastric variceal bleeding with an intratumoral arterioportal shunt. Contrast-enhanced CT revealed gastric varices and hepatocellular carcinoma with tumor thrombi in the right portal vein. Angiography and angio-CT revealed a marked intratumoral arterioportal shunt accompanied with reflux into the main portal vein and gastric varices. Balloon-occluded retrograde venography from the gastro-renal shunt showed no visualization of gastric varices due to rapid blood flow through the intratumoral arterioportal shunt. The hepatic artery was temporarily occluded with a balloon catheter to reduce the blood flow through the arterioportal shunt, and then concurrent balloon-occluded retrograde transvenous obliteration （BRTO） was achieved. Vital signs stabilized immediately thereafter, and contrast-enhanced CT revealed thrombosed gastric varices. Worsening of hepatic function was not recognized. BRTO combined with temporary occlusion of the hepatic artery is a feasible interventional procedure for ruptured high flow gastric varices with an intratumoral arterioportal shunt.

Transhepatic catheter-directed thrombolysis for portal vein thrombosis after partial splenic embolization in combination with balloon-occluded retrograde transvenous obliteration of splenorenal shunt

A 66-year-old woman underwent partial splenic embolization （PSE） for hypersplenisrn with idiopathic portal hypertension （IPH）. One week later, contrast-enhanced CT revealed extensive portal vein thrombosis （PVT） and dilated portosystemic shunts. The PVT was not dissolved by the intravenous administration of urokinase. The right portal vein was canulated via the percutaneous transhepatic route under ultrasonic guidance and a 4 Fr. straight catheter was advanced into the portal vein through the thrombus. Transhepatic catheter-directed thrombolysis was performed to dissolve the PVT and a splenorenal shunt was concurrently occluded to increase portal blood flow, using balloon-occluded retrograde transvenous obliteration （BRTO） technique. Subsequent contrast-enhanced CT showed good patency of the portal vein and thrombosed splenorenal shunt. Transhepatic catheter-directed thrombolysis combined with BRTO is feasible and effective for PVT with portosystemic shunts.
