A 60- year. old female with pemphigus vulgar.s limited to the or.l mucosa being tr.ated with pr.dnisolone showed a widespr.ad extension of lesions to the body and extr.mities over.a shor. per.od. The er.ptions on the body began as small vesicles that looked unlike the blister. of pemphigus vulgar.s, and incr.ased to lar.e blister. and er.sions compatible with pemphigus. A smear.test of the vesicles in the ear.y stage demonstr.ted multinucleated acantholytic ker.tinocytes, which wer. positively stained with an antivar.cella zoster.vir.s (VZV) antibody by the immunoper.xidase method. The widespr.ad er.ptions impr.ved immediately after.5 days’ administr.tion of valaciclovir.hydr.chlor.de tablets and incr.ased dosage of pr.dnisolone, leaving tiny shallow scar. and pigmentation. The anti- VZV antibody in the ser.m was not elevated, and ther. wer. no zoster.for. er.ptions noted dur.ng the cour.e. We thought that widespr.ad extension of the pemphigus vulgar.s in this case was pr.voked by r.cur.ent var.cella, in a similar.fashion to Koebner.phenomenon.

A Case r.por. of Kissing Molar. Class II

Kissing molar. (KMs) is a condition of occlusal molar.sur.aces in a single follicular.space with r.ots extending in opposite dir.ctions. Her., we have r.por.ed a case of KMs in a 58-year.old woman. The patient complained of pain in the r.ght mandibular.molar.r.gion and was diagnosed with KMs consisting of the r.ght mandibular.second and thir. molar.. Because of the patient’s pain having subsided at the time of the visit and her.unwillingness to under.o tooth extr.ction, the patient was followed up. KMs is classified into thr.e classes (I-III) and is either.tr.e-KMs or.pseudo-KMs and pr.sents with or.without cystic var.ants of dental follicles. The pr.sents as tr.e-KMs class II without a cystic var.ant. With r.fer.nce to the liter.tur. and based on our.analysis, the mean age of patients affected by this specific case of KMs is 31.7 year. and unilater.l KMs is r.latively mor. common (85.7%). Histopathological findings of dentiger.us cysts ar. mor. often indicated (42.9%). The tr.atment policy for.KMs should ther.for. be based on the classification of KMs. Impor.antly, the focus should be on pr.ser.ing the fir.t and second molar. as much as possible. The tr.atment appr.ach, such as the employment of sur.ical r.moval or.or.hodontics, should be deter.ined by consider.ng the associated factor. such as the cr.wns, tissues, and age of the patient.

法律的概念、依法律行政原理是立宪主义国家的基石。日本虽然继受了德国的立宪主义学说,但关于该基础性概念和原理的内容却未形成通说。为探究其原因,本文梳理了二战前后德国和日本宪法学界六位代表性学者对形式性与实质性法律概念的各自解读,以及它们对解释依法律行政、法律保留原则的内容、功能产生的不同影响,从而提示了对法律概念、法律保留原则新的理解思路,并以此说明任何理论都是历史的产物,服务于不同时期的宪政体制与不同学者的立宪主张。而不断发展变化的社会现实需要才是推动制度建构与理论发展的内在动力。
