Association between high deductible health plan coverage and age at pediatric umbilical hernia repair

Background High deductible health plans(HDHPs)are associated with the avoidance of both necessary and unnecessary *** hernia repair(UHR)is a procedure that is frequently unnecessarily performed in young children,contrary to best practice *** hypothesized that children with HDHPs,as compared with other types of commercial health plans,are less likely to undergo UHR before 4 years of age but are also more likely to have UHR delayed beyond 5 years of *** Children aged 0-18 years old residing in metropolitan statistical areas(MSAs)who underwent UHR in 2012-2019 were identified in the IBM Marketscan Commercial Claims and Encounters Database.A quasi-experimental study design using MSA/year-level HDHP prevalence among children as an instrumental variable was employed to account for selection bias in HDHP ***-stage least squares regression modeling was used to evaluate the association between HDHP coverage and age at *** A total of 8601 children were included(median age 5 years,IQR 3-7).Univariable analysis revealed no differences between the HDHP and non-HDHP groups in the likelihood of UHR being performed before 4 years of age(27.7%vs 28.7%,p=0.37)or after 5 years of age(39.8%vs 38.9%,p=0.52).Geographical region,metropolitan area size,and year were associated with HDHP *** variable analysis demonstrated no association between HDHP coverage and undergoing UHR at5 years of age(p=0.87).Conclusions HDHP coverage is not associated with age at pediatric *** studies should investigate other means by which UHRs in young children can be avoided.

Broad bean cultivars increase extrafloral nectary numbers, but not extrafloral nectar, in response to leaf damage

Phenotypic plasticity allows organisms to maximize fitness, by optimizing the expression of costly defensive traits. Broad bean, Vicia faba L. “Broad Windsor”, produces increased numbers of extrafloral nectaries (EFns) in response to leaf damage to attract mutualistic partners and reduce herbivory. It is currently unknown, however, whether EFn induction is cultivar-specific or is a more general phenomenon. It has also not been determined whether broad beans increase nectar secretion rates in conjunction with EFn induction. We hypothesized that: a) as all broad beans have conspicuous EFns, all cultivars should produce additional EFns in response to leaf damage, and b) overall nectar secretion rates should increase with EFn numbers, to attract additional mutualists. We tested our hypothesis by subjecting three broad bean cultivars, Vicia faba L. “Broad Windsor”, “Stereo”, and “Witkiem” to mechanical leaf damage. The degree of change in plant traits associated with growth, in addition to EFn induction, was assessed 1 week after leaf damage. Extrafloral nectar volumes were also assessed, every 24 hours, pre- and post-leaf damage. We confirmed our first, but rejected our second, hypothesis. All cultivars produced additional EFns, but none increased extrafloral nectar volumes, when experiencing leaf damage. Further experimentation is required to determine if energetic tradeoffs limit multiple forms of defense (i.e., EFn vs. nectar induction), or if this alternative strategy is adaptive for attracting and retaining mutualists. Understanding the costs and benefits of EFn vs. nectar induction will provide insight into the evolution of defensive mutualisms between plants and predatory arthropods.

Choledochal cysts: Similarities and differences between Asian and Western countries

Choledochal cysts(CCs)are rare bile duct dilatations,intra-and/or extrahepatic,and have higher prevalence in the Asian population compared to Western *** of the current literature on CC disease originates from Asia where these entities are most *** are thought to arise from an anomalous pancreaticobiliary junction,which are congenital anomalies between pancreatic and bile *** similarities in presentation between Eastern and Western patients exist such as female predominance,however,contemporary studies suggest that Asian patients may be more symptomatic on *** though CC disease presents with an increased malignant risk reported to be more than 10%after the second decade of life in Asian patients,this risk may be overstated in Western *** this difference in cancer risk,management guidelines for all patients with CC are based predominantly on observations reported from Asia where it is recommended that all CCs should be excised out of concern for the presence or development of biliary tract cancer.

The development of fully solution-processed,biodegradable piezoelectrics is a critical step in the development of green electronics towards the worldwide reduction of harmful electronic ***,recent printing processes for piezoelectrics are hindered by the high sintering temperatures required for conventional perovskite fabrication ***,a process was developed to manufacture lead-free printed piezoelectric devices at low temperatures to enable integration with eco-friendly substrates and electrodes.A printable ink was developed for screen printing potassium niobate(KnbO3)piezoelectric layers in microns of thickness at a maximum processing temperature of 120℃ with high *** parallel plate capacitor and cantilever devices were designed and manufactured to assess the quality of this ink and evaluate its physical,dielectric,and piezoelectric characteristics;including a comparison of behaviour between conventional silicon and biodegradable paper *** printed layers were 10.7–11.2μm thick,with acceptable surface roughness values in the range of 0.4-1.1μ*** relative permittivity of the piezoelectric layer was *** poling parameters were optimised for the piezoelectric response,with an average longitudinal piezoelectric coefficient for samples printed on paper substrates measured as d33,eff,paper=13.57±2.84 pC/n;the largest measured value was 18.37 pC/n on paper *** approach to printable biodegradable piezoelectrics opens the way forward for fully solution-processed green piezoelectric devices.

Improving sorghum digestion in broilers by targeting fermentation of xylan

This study was to examine if it is possible to accelerate sorghum digestion in broiler chickens by targeting fermentation of the *** 500 broilers(n=960,80 birds per treatment)were fed 12 sorghumsoybean meal-based dietary treatments fed as 3 phases(starter d 0 to 12,grower d 13 to 23,finisher d 24 to 35),with 8 replicate pens of 10 birds per *** half of the treatments(n=6),10%of the sorghum in the diet was directly replaced with 10%wheat bran,as a source of fermentable *** diets were supplemented with either 0,50 or 2,000 mg/kg xylo-oligosaccharides(XOS),with or without xylanase *** weight gain(BWG),feed intake(FI)and feed conversion corrected for mortality(cFCR)was determined at d 0 to 35,and male and female body weight were measured on d *** d 35,ileum and caeca samples were collected from 2 birds per pen,for determination of caecal cellulase and xylanase activity,microbiota composition and short chain fatty acid(SCFA)concentration,and ileal XOS *** with 2,000 mg/kg XOS caused increased BWG at d 0 to 35(P=0.007)and enhanced caecal propionic,valeric and succinic acid concentration(Pn increased FI(P=0.018)and BWG(P=0.016),as well as caecal Bifidobacteria concentration(Pncentration was greatest when feeding combined wheat bran,2,000 mg/kg XOS,and xylanase,resulting in increased caecal total SCFA,acetic acid and butyric acid concentration,and xylanase and cellulase activity(Pnt that feed efficiency in birds fed sorghum-based diets is improved as a consequence of supplementing with fermentable fibre,xylanase and XOS.

The Effects of High-Intensity Acute Exercise on Face-name Memory in Healthy Young Adults

The majority of previous research evaluating the effects of acute exercise on episodic memory function have focused on explicit memory tasks involving word-list *** a more real-world application,the present experiment evaluates whether high-intensity acute exercise can improve episodic memory function of incidentally processed information as well as increase one’s ability to remember names associated with faces(face-name paradigm).A two-arm,parallel-group,randomized controlled intervention was ***(n=91;M_(age)=20 years)were randomized into one of two groups,including an experimental group and a control *** experimental group exercised for 20 min on a treadmill at a high-intensity(75%of heart rate reserve),while the control group engaged in a seated,time-matched *** memory was assessed from an intentional processing face-name paradigm in which participants encoded and subsequently recalled names that were paired with *** memory for incidentally processed information was evaluated with a computerized program involving spatial-temporal *** mean(SD)number of correctly recalled face-name pairs between the exercise and control groups,respectively,for immediate memory were 11.16(4.4)and 9.79(4.3),P>*** were similar for incidentally processed information,in that there were no differences between the exercise[7.30(1.7)]and control[7.52(1.9)]groups,P>***,we did not observe evidence that high-intensity acute exercise improves episodic memory for incidentally or intentionally processed information.
