Clinical Pharmacokinetic and Bioequivalence Studies of Two Brands of Cephradine in Healthy Korean Using HPLC Method

The goal of our research was to compare the pharmacokinetics and evaluate the bioequivalence of two brands of cephradine 500 mg capsules in 24 normal Korean volunteers. The plasma samples were acquired at 13 time points for 8 h after administration. The concentrations of cephradine in human plasma were measured by a high-performance liquid chromatography (HPLC). Isocratic mobile phase which consisted of acetonitrile, methanol, and 20 mM potassium phosphate (15/5/80, v/v/v, pH 3.48) was used to separate the analytical column cosmosil cholester (250 × 4.6 mm, 3 μm). Analytes were detected in ultraviolet (260 nm). The novel analytical method was described as simple sample preparation, a short retention time (less than 6 min) and making it suitable for use in clinical trials. Pharmacokinetic parameters, such as AUC0-t (20.54 vs 18.42 μg·h/mL), AUC0-infinity (21.22 vs 19.14 μg·h/mL), Cmax (12.69 vs 12.81 μg/mL), Tmax (1.22 vs 0.92 h), half-life (1.02 vs 1.13 h), extrapolation (3.22% vs 3.75%), and Ke (0.73 vs 0.69 h–1) were determined for the reference and test drugs in plasma. Pharmacokinetic parameters with a 90% confidence interval were 87% - 95% for AUC0-t and 91% - 115% for Cmax. They were satisfied within the bioequivalence range 80% - 125% of the KFDA guidelines. Therefore, our HPLC method was well applied in a bioequivalence and pharmacokinetic study of two formulations in normal subjects.

AIM: To evaluate the relationship between hepatic fat infiltration and abdominal fat volume by using computed tomography (CT).METHODS: Three hundred and six patients who visited our obesity clinic between November 2007 and April 2008 underwent fat protocol CT *** age range of the patients was 19 to 79 years and the mean age was 49 *** male to female ratio was 116:*** and spleen attenuation measurements were taken with three regions of interests (ROIs) from the liver and two ROIs from the *** attenuation indices (HAIs) were measured as follows: (1) hepatic parenchymal attenuation (CTLP);(2) liver to spleen attenuation ratio (LS ratio);and (3) difference between hepatic and splenic attenuation (LSdif).Abdominal fat volume was measured using a 3 mm slice CT scan starting at the level of the umbilicus and was automatically calculated by a *** fat was classified into total fat (TF),visceral fat (VF),and subcutaneous fat (SF).We used a bivariate correlation method to assess the relationship between the three HAIs and TF,VF,and ***: There were significant negative correlations between CTLP,LS ratio,and LSdif with TF,VF,and SF,*** CTLP showed a strong negative correlation with TF and VF (r = -0.415 and -0.434,respectively,P < 0.001).The correlation between CTLP and SF was less significant (r = -0.313,P < 0.001).CONCLUSION: Fatty infiltration of the liver was correlated with amount of abdominal fat and VF was more strongly associated with fatty liver than SF.

为了满足学习者的需求,越南本土出版了不少汉语词典,包括纸质版词典、网络词典和手机App。这些词典存在良莠不齐的问题,不能完全满足学习者的需求。对其中4部权威和典型的汉语词典进行学习特征分析后发现,越南的纸质版汉语词典虽然有用户意识,但学习特征不明显;网络词典的学习特征比较突出,但文本内容存在诸多不准确之处。因此,开发出以《国际中文教育中文水平等级标准》为指导的面向越南汉语学习者的国别化工具书,需要中越两国学者及出版机构的共同努力。

Co-Occurring Polysubstance Use and Physical Disease of Persons with Mental Disorders in Residential Treatment Program

The purpose of this study was to describe the prevalence of co-occurring polysubstance use and to examine the differences in physical disease between polysubstance users and single/no substance users with mental disorders. This retrospective cross-sectional study included 1949 clinical records of psychiatric patients from 11 residential treatment programs between 2007 and 2011. Demographic variables, psychiatric diagnoses, and data on substance use and physical disease were obtained from the clinical records. Chi-square analyses were used to examine substance use difference in the prevalence of each physical disease category. This study found that the prevalence of co-occurring polysubstance use was 53.5%. Chi-square analyses identified that co-occurring polysubstance users reported more respiratory, digestive, musculoskeletal, and HIV/AIDS diseases but less endocrine diseases than single/no substance users. Therefore, integrated treatment programs for treating patients with co-occurring substance use and physical disease should be developed and expanded for this high-risk group.

BACKGROUND:Adequate airway management plays an important role in high-quality cardiopulmonary resuscitation(CPR).Airway management is usually performed using an endotracheal tube(ETT)during ***,no study has assessed the effect of ETT size on the fl ow rate and airway pressure during ***:We measured changes in peak inspiratory fl ow rate(PIFR),peak airway pressure(Ppeak),and mean airway pressure(Pmean)according to changes in ETT size(internal diameter 6.0,7.0,and 8.0 mm)and with or without CPR.A tidal volume of 500 mL was supplied at a rate of 10 times per minute using a mechanical *** compressions were maintained at a constant compression depth and speed using a mechanical chest compression device(LUCAS2,mode:active continuous,chest compression rate:102±2/minute,chest compression depth 2–2.5 inches).RESULTS:The median of several respiratory physiological parameters during CPR was significantly different according to the diameter of each ETT(6.0 vs.8.0 mm):PIFR(32.1 L/min[30.5–35.3]vs.28.9 L/min[27.5–30.8],P<0.001),Ppeak(48.84 cmH2O[27.46–52.11]vs.27.45 cmH2O[22.53–52.57],P<0.001),and Pmean(18.34 cmH2O[14.61–21.66]vs.13.66 cmH2O[8.41–19.24],P<0.001).CONCLUSION:The changes in PIFR,Ppeak,and Pmean were related to the internal diameter of ETT,and these values tended to decrease with an increase in ETT *** airway pressures were measured in the CPR group than in the no CPR group.

Understanding the folding processes of a protein into its three-dimensional native structure only with its amino-acid sequence information is a long-standing challenge in modern science. Two- hundred independent folding simulations (starting from non-native conformations) and two- hundred independent unfolding simulations (starting from the folded native structure) are performed using the united-residue force field and Metropolis Monte Carlo algorithm for betanova (three-stranded antiparallel beta-sheet protein). From these extensive computer simulations, two representative folding pathways and two representative unfolding pathways are obtained in the reaction coordinates such as the fraction of native contacts, the radius of gyration, and the root- mean-square deviation. The folding pathways and the unfolding pathways are similar each other. The largest deviation between the folding pathways and the unfolding pathways results from the root-mean-square deviation near the folded native structure. In general, unfolding computer simulations could capture the essentials of folding simulations.

AIM： To identify the clinical features and outcomes of infrequently reported leptomeningeal carcinomatosis （LMC） of gastric ***： We analyzed 54 cases of cytologically confirmed gastric LMC at four institutions from 1994 to ***： The male-to-female ratio was 32：22, and the patients ranged in age from 28 to 78 years （median,48.5 years）. The majority of patients had advanced disease at initial diagnosis of gastric cancer. The clini-cal or pathologic tumor, node and metastasis stage ofthe primary gastric cancer wasin 38 patients （70%）.The median interval from diagnosis of the primarymalignancy to the diagnosis of LMC was 6.3 mo, rang-ing between 0 and 73.1 mo. Of the initial endoscopic f indings for the 45 available patients, 23 （51%） of the patients were Bormann typeand 15 （33%） patientswere Bormann type. Pathologically, 94% of cases proved to be poorly differentiated adenocarcinomas. Signet ring cell component was also observed in 40% of patients. Headache （85%） and nausea/vomiting （58%） were the most common presenting symptoms of LMC. A gadolinium-enhanced magnetic resonance imaging was conducted in 51 patients. Leptomeningeal enhancement was noted in 45 cases （82%）. Intrathecal （IT） chemotherapy was administered to 36 patients-primarily methotrexate alone （61%）, but also in combi-nation with hydrocortisone/± Ara-C （39%）. The median number of IT treatments was 7 （range, 1-18）. Concomitant radiotherapy was administered to 18 patients, and concomitant chemotherapy to seven patients. Sev-enteen patients （46%） achieved cytological negative conversion. Median overall survival duration from the diagnosis of LMC was 6.7 wk （95% CI： 4.3-9.1 wk）. In the univariate analysis of survival duration, hemoglobin, IT chemotherapy, and cytological negative conversion showed superior survival duration （P = 0.038, P = 0.010, and P = 0.002, respectively）.
