Background: Cancer colon is one of the most common malignancies. After colon cancer surgery patients may experience severe pain. Several studies have reported that a significant decrease in postoperative pain with propofol while other studies have showed this effect was not significant. Aim: Our goal was to assess the effect of combined epidural anaesthesia either with propofol or sevoflurane on intraoperative fentanyl consumption and postoperative pain in patients undergoing open surgical resection of colon cancer. This paper presents an adaptive control system using model predictive control for a biogas-fueled power *** control scheme is derived from an anaerobic digestion model that includes the concentration of biodegradable volatile solid in the reactor,the concentration of volatile solid in influent,the concentration of acidogens,and the concentration of *** these concentrations are the state variables of model predictive *** whole biogas-fueled power system has been modeled,implemented,and tested in MATLAB/Simulink *** validate the performance of the proposed controller,different operation conditions are studied and *** simulation results prove the effectiveness and the applicability of the proposed control system under different operating conditions.

There is a well-known relationship between malignancy and impairment of kidney functions,either in the form of acute kidney injury or chronic kidney *** the former,however,bilateral malignant ureteral obstruction is a surgically correctable factor of this complex *** warrants urgent drainage of the kidneys in emergency ***,there are multiple controversies and debates about the optimal mode of drainage of the bilaterally obstructed kidneys in these *** review addressed most of the concerns and provided a comprehensive presentation of this topic from the recent ***,we provided different perspectives on the management of the bilateral obstructed kidneys due to *** the frequent trials for improving the success rates and functions of ureteral stents,placement of a percutaneous nephrostomy tube remains the most recommended tool of drainage due to bilateral ureteral obstruction,especially in patients with advanced ***,the disturbance of the quality of life of those patients remains a major unresolved *** the unfavorable prognostic potential of the underlying malignancy and the various risk stratification models that have been proposed,the response of the kidney to initial drainage can be anticipated and evaluated by multiple renal prognostic factors,including increased urine output,serum creatinine trajectory,and time-to-nadir serum creatinine after drainage.

Isolation and characterization of marine-derived actinomycetes with cytotoxic activity from the Red Sea coast

Objective: To isolate and evaluate the cytotoxic activity of different actinomycetes species isolated from the Red Sea coast in Sharm el-Sheikh, ***: Forty actinomycetes strains were isolated from different sediments and seawater samples collected from the Red Sea coast in Egypt. Actinomycetes were recognized by morphological and microscopic examinations. Cell viability and cytotoxicity induced by the crude extracts on breast cancer cell lines MDA-MB-231 were assessed using methylene blue assay. The strains with promising cytotoxic activity were identified by sequencing and amplifying the 16 S r RNA genes. The antibacterial activities of the crude extracts were performed using Kirby–Bauer disc diffusion ***: The results indicated that five ethyl acetate extracts exhibited cytotoxicity towards breast cancer cell lines MDA-MB-231. The highest cytotoxic activity was found for the ethyl acetate extracts of EGY2 and EGY39. The isolate EGY3 was identified as a new Streptomyces species, while the actinomycete EGY22 was found to be a member of the genus Nocardiopsis sp. The crude extract of the isolate EGY8 showed slightly high antimicrobial activity against different test ***: The results of the present study reveal that marine sediments of the Red Sea are a potent source of novel species of actinomycetes. The isolates may be useful in discovery of novel bioactive compounds and an important step in the development of microbial natural product research.

Background: Aspiration of foreign body is popular in pediatric age group and is considered as an important cause of respiratory distress and chocking in these children. Management strategies are different according to the mode of presentation. This study aims to assess the efficacy of rigid bronchoscopy and the role of bronchotomy as a safe alternative for failed removal of inhaled foreign body. Methods: We enrolled 254 patients with foreign body aspiration (FBA) up to 14 years old in a retrospective observational study. Our patients were reviewed from the Cardiothoracic Surgery Department and cases referred from Otorhinolaryngology Department, Menoufia University Hospital between June 2010 and July 2017. Using jet ventilation technique, foreign body was removed by the rigid bronchoscopy with either extracting forceps or postural drainage. Surgical interference such as bronchotomy needed in distally impacted foreign bodies (FBs). Results: Our study included 254 patients prepared for bronchoscopic FBs extraction. Most of cases (68.8%) presented early within first week (174 cases). No FBs detected in 44 cases by rigid bronchoscopy, although it was successful in 176 (69.5%) cases with non- impacted inhaled FBs. 14 cases only needed postural drainage due to inaccessible FBs, another 12 cases failed to extract FB with rigid bronchoscopy but they were managed with flexible bronchoscopy. Bronchotomy was needed for impacted FBs in eight cases (3%). Conclusion: This study’s findings support that rigid bronchoscopy is the gold standard in the diagnosis and removal of foreign body aspirations in pediatrics, but rigid bronchoscopy demonstrated less capability in the diagnosis and removal of small distally located foreign bodies. Bronchotomy is safe and effective alternative.

AIM: To study diagnostic laparoscopy as a tool for excluding donors on the day of surgery in living donor liver transplantation (LDLT).METHODS: This study analyzed prospectively collected data from all potential donors for LDLT. All of the donors were subjected to a three-step donor evaluation protocol at our institution. Step one consisted of a clinical and social evaluation, including a liver profile, hepatitis markers, a renal profile, a complete blood count, and an abdominal ultrasound with Doppler. Step two involved tests to exclude liver diseases and to evaluate the donor’s serological status. This step also included a radiological evaluation of the biliary anatomy and liver vascular anatomy using magnetic resonance cholan-giopanc reatography and a computed tomography (CT) angiogram, respectively. A CT volumetric study was used to calculate the volume of the liver parenchyma. Step three included an ultrasound-guided liver biopsy. Between November 2002 and May 2009, sixty-nine potential living donors were assessed by open exploration prior to harvesting the planned part of the liver. Between the end of May 2009 and October 2010, 30 potential living donors were assessed laparoscopically to determine whether to proceed with the abdominal incision to harvest part of the liver for donation. RESULTS: Ninety-nine living donor liver transplants were attempted at our center between November 2002 and October 2010. Twelve of these procedures were aborted on the day of surgery (12.1%) due to donor findings, and eighty-seven were completed (87.9%). These 87 liver transplants were divided into the following groups: Group A, which included 65 transplants that were performed between November 2002 and May 2009, and Group B, which included 22 transplants that were performed between the end of May 2009 and October 2010. Background: Diabetic peripheral neuropathy (DPN) is a common complication among children with TIDM and is related to poor glycemic control. The aim of the study is to determine prevalence and determinants of diabetic neuropathy in diabetic children. Material& Methods: This is a cross sectional study conducted on 50 children having Type I diabetes mellitus for more than year presented to Pediatric Neurology and Endocrinology Outpatient Clinics, Sohag University Hospital, Sohag, Egypt, during one year duration (2017). Children were subjected to detailed neurological examination and electrophysiologic studies in addition to recording of socio demographic and laboratory findings with special regard to level of HbA1c. Results: The mean age of the studied children was 10.5 ± 0.3 years ranging from 5 to 18 years. 22 cases (44%) were younger than 10 years, 27 children (54%) were females. DPN was diagnosed in 12% of the studied children. No significant relation was found as regard age, gender, family history, consanguinity and duration of the diseases. Significant association was found between the HbA1c level and development of DPN in the studied children. Conclusion: Based on the results of this study, 12% of the studied diabetic children had DPN of whom more than 16% had subclinical neuropathy indicating the need for annual screening of neuropathy using Nerve Conduction Study. Moreover strict glycaemic control is needed to prevent rapid development of diabetic among these children.

Preliminary molecular variability among haplotypes of Saudi Arabian house sparrow <i>Passer domesticus</i>

Genetic variability of Passer domesticus from different localities of Saudi Arabia was investigated. Six hundred and fifty nine nucleotides were sequenced from the mitochondrial cytochrome b gene. There was a slight difference among the studied haplotypes and most substitutions were synonymous. In some haplotypes at the west of Arabian Peninsula (mountainous habitat), two transitions only were non-synonymous. The data were used to construct the relationship of the Arabian house sparrow to its con specific taxa from Europe and America. Neigh-bor-joining (NJ), maximum-par-simony (MP) and maximum-likelihood (ML) analytical methods were used. The three methods showed clustering of the Arabian haplotypes in one group and their sister relationship with the haplotypes from Netherlands and America. A spanish Haplotypes came basal to both groups. Collecting more samples and more DNA data could be of necessary to clearly address the genetic variability of this rapidly propagated species in Arabia.
