BACKGROUND Metabolic dysfunction-associated steatotic liver disease(MASLD)and metabolic dysfunction-associated steatohepatitis(MASH)are a growing health burden across a significant portion of the global patient ***,these conditions seem to have disparate rates and outcomes between different ethnic *** combination of MASLD/MASH and type 2 diabetes increases the risk of hepatocellular carcinoma(HCC),and Hispanic patients experience the greatest burden,particularly those in South *** To compare outcomes between Hispanic and non-Hispanic patients in the United States,while further focusing on the Hispanic population within Southeast Texas to determine whether the documented disparity in outcomes is a function of geographical circumstance or if there is a more widespread reason that all clinicians must account for in prognostic *** This cohort analysis was conducted with data obtained from TriNetX,LLC(“TriNetX”),a global federated health research network that provides access to deidentified medical records from healthcare organizations *** cohort networks were used:University of Texas Medical Branch(UTMB)hospital and the United States national database collective to determine whether disparities were related to geographic regions,like Southeast *** This study findings revealed Hispanics/Latinos have a statistically significant higher occurrence of HCC,type 2 diabetes mellitus,and liver fibrosis/cirrhosis in both the United States and the UTMB Hispanic/Latino *** mortality in Hispanics/Latinos was lower within the United States group and not statistically elevated in the UTMB *** This would appear to support that Hispanic patients in Southeast Texas are not uniquely affected compared to the national Hispanic population.

Hepatitis E virus(HEV)infection can cause severe complications and high mortality,particularly in pregnant women,organ transplant recipients,individuals with pre-existing liver disease and immunosuppressed ***,there are still unmet needs for treating chronic HEV ***,we screened a best-in-class drug repurposing library consisting of 262 drugs/*** screening,we identified vidofludimus calcium and pyrazofurin as novel anti-HEV *** calcium is the next-generation dihydroorotate dehydrogenase(DHODH)inhibitor in the phase 3 pipeline to treat autoimmune diseases or SARS-CoV-2 *** selectively targets uridine monophosphate synthetase(UMPS).Their anti-HEV effects were further investigated in a range of cell culture models and human liver organoids models with wild type HEV strains and ribavirin treatment failure-associated HEV ***,both drugs exhibited a sizeable therapeutic window against *** instance,the IC50 value of vidofludimus calcium is 4.6–7.6-fold lower than the current therapeutic doses in ***,their anti-HEV mode of action depends on the blockage of pyrimidine ***,two drugs robustly inhibited ribavirin treatment failure-associated HEV mutants(Y1320H,G1634R).Their combination with IFN-αresulted in synergistic antiviral *** conclusion,we identified vidofludimus calcium and pyrazofurin as potent candidates for the treatment of HEV *** on their antiviral potency,and also the favorable safety profile identified in clinical studies,our study supports the initiation of clinical studies to repurpose these drugs for treating chronic hepatitis E.

植物的生态过程依赖于叶片和根系的功能运行,但少有研究同时关注这两类性状在群落中的分布格局(CTD)。本文测定了广东亚热带森林44种共生木本植物的16个叶片和根系性状,旨在检验类比的叶片和根系性状是否耦合,是否沿类似的功能性状谱变异,是否表现出类似的CTD,其CTD和群落加权平均性状值(CWM)是否沿环境梯度表现出类似的响应。研究结果表明,叶片和根系性状变异的第一轴均呈显出由资源获取向资源保守变异的经济学谱,但叶片变异的第二轴主要由叶片碳含量表征,而根系变异的第二轴主要由反映菌根协作程度的根系粗细表征。类比的叶片和根系化学性状整体正相关,但形态性状的耦合程度较低。在群落水平上,叶片和根系性状第一轴的CWM均随海拔升高而变得更保守,而第二轴的变化则不一致。随土壤磷含量的降低或钾含量的升高,根系变得更细和在养分性状上更保守,而叶片性状无显著变化。当整合多维性状轴时,叶片性状呈发散格局,且随海拔、土壤磷和钾含量的增加呈收敛趋势;而根系性状呈收敛格局,但沿上述环境梯度呈发散趋势。该研究强调,叶片和根系性状与环境互作的差异可能导致不同的性状分布格局;植物可通过多样的地上和地下功能设计应对环境异质性。

The Asian citrus psyllid,Diaphorina citri,is the vector of the pathogenic bacteria that causes Huanglongbing,the most devasting disease affecting citrus *** cultivation of citrus tends to expand northward,the tolerance of psyllids to freezing temperatures needs to be *** mortality of *** to cold temperature has been previously studied,much less is known regarding the acclimation potential of psyllids to cold *** first evaluated cold resistance of *** depending on color morph and ***,we compared mortality of *** at−4°C for 10 hours between unacclimated psyllids,psyllids that were gradually acclimated to cold over 1 or 2 weeks,and psyllids intermittently acclimated also for 1 or 2 *** did not find major differences in cold resistance between color morphs and *** found that after 1 week of gradual acclimation,the survival of psyllids at−4°C increased to 86%,compared to 20%in the control *** did not increase after the second week of gradual *** intermittent acclimation improved survival compared to the control group,it was less effective than gradual acclimation with a survival at 30%and 70%after 1 and 2 weeks of acclimation,respectively,although this difference with gradual acclimation was not significant at week *** data show that gradual cold acclimation allows *** to survive brief periods below *** may serve as a mechanism responsible for increasing northern establishment of this pest.

Understanding Factors of Bias in the Way Information Is Processed Through a Metamemetic and Multilogical Approach

Training in complex thinking is required in fields like computer science and discussing sensitive topics that can easily polarize internet users’ propensities. Multilogicality and Metamemetic reasoning are strongly suggested as an approach to identifying and analyzing factors related to AI Bias and human biases. This approach entails identifying problems and deducting invalid premises, distinguishing them from valid premises or those we are uncertain about. The theme of this paper focuses on four groups of people: curators, developers, businesses, and users (the fourth group being the main focus). This approach offers a new way to apply critical thinking strategies in the context of living in a digital age.

DYNAMIC RESOURCE ALLOCATION FOR EFFICIENT PATIENT SCHEDULING： A DATA-DRIVEN APPROACH

Efficient staff rostering and patient scheduling to meet outpatient demand is a very complex and dynamic task. Due to fluctuations in demand and specialist availability, specialist allocation must be very flexible and non-myopic. Medical specialists are typically restricted in sub-specialization, serve several patient groups and are the key resource in a chain of patient visits to the clinic and operating room （OR）. To overcome a myopic view of once-off appointment scheduling, we address the patient flow through a chain of patient appointments when allocating key resources to different patient groups. We present a new, data-driven algorithmic approach to automatic allocation of specialists to roster activities and patient groups. By their very nature, simplified mathematical models cannot capture the complexity that is characteristic to the system being modeled. In our approach, the allocation of specialists to their day-to-day activities is flexible and responsive to past and present key resource availability, as well as to past resource allocation. Variability in roster activities is actively minimized, in order to enhance the supply chain flow. With discrete-event simulation of the application case using empirical data, we illustrate how our approach improves patient Service Level （SL, percentage of patients served on-time） as well as Wait Time （days）, without change in resource capacity.

Microscopic colitis as a missed cause of chronic diarrhea

AIM: To determine the prevalence of increased intraepithelial lymphocytes, using immunohistochemistry in patients with normal colonoscopy and near normal biopsy. METHODS: We retrospectively reviewed all non-malignant colon mucosal biopsies between 2005 and 2007, reported as normal, chronic inflammation or melanosis coli in patients who were undergoing routine colonoscopy. Immunohistochemistry using CD3 was performed on all mucosal biopsies and an intraepithelial lymphocyte count (IEL) was determined. Cases with an IEL count of ≥ 20 IELs per 100 surface epithelial cells were correlated with demographic, clinical and follow-up data. A further subgroup was evaluated for lymphocytic ***: Twenty (8.3%) of 241 cases revealed an IEL count ≥ 20. Six (2.5%) patients were identified as having lymphocytic colitis (P < 0.001), of whom, five were missed on initial evaluation (P = 0.01). Four of these five patients were labeled with diarrhea-predominant irritable bowel syndrome (IBS). On follow-up, three of the remaining 20 cases were diagnosed with malignancy (renal cell carcinoma and myelodysplastic syndrome) and one had an unknown primary tumor with multiple liver metastases. Two cases of collagenous colitis with an IEL count < 10 were included in this study. Increased IELs were not confined to patients with diarrhea as a primary presenting symptom, but were also present in patients with abdominal pain (n = 7), constipation (n = 3) and loss of weight (n = 1). CONCLUSION: Immunohistochemistry using CD3 is of value in identifying and quantifying IELs for the presence of microscopic colitis in patients with diarrheapredominant IBS.
