Periosteum deriv.d stem cells for regenerativ. medicine proposals: boosting current knowledge

Periosteum is a thin fibrous layer that cov.rs most bones. It resides in a dynamic mechanically loaded env.ronment and prov.des a niche for pluripotent cells and a source for molecular factors that modulate cell behav.our. Elucidating periosteum regenerativ. poten-tial has become a hot topic in orthopaedics. This rev.ew discusses the state of the art of osteochondral tissue engineering rested on periosteum deriv.d progenitor cells(PDPCs) and suggests upcoming research direc-tions. Periosteal cells isolation, characterization and migration in the site of injury, as well as their differen-tiation, are analysed. Moreov.r, the role of cell mecha-nosensing and its contribution to matrix organization, bone microarchitecture and bone stenght is examined. In this regard the role of periostin and its upregulation under mechanical stress in order to preserv. PDPC sur-v.v.l and bone tissue integrity is contemplated. The re-v.ew also summarized the role of the periosteum in the field of dentistry and maxillofacial reconstruction. The inv.lv.ment of microRNAs in osteoblast differentiation and in endogenous tissue repair is explored as well. Fi-nally the nov.l concept of a guided bone regenerationbased on the use of periosteum itself as a smart mate-rial and the realization of constructs able to mimic the extracellular matrix features is talked out. Additionally, since periosteum can differentiate into insulin produc-ing cells it could be a suitable source in allogenic trans-plantations. That innov.tiv. applications would takeadv.ntage from inv.stigations aimed to assess PDPCimmune priv.lege.

Impact of hepatitis C v.rus core mutations on the response to interferon-based treatment in chronic hepatitis C

AIM To determine whether hepatitis C v.rus(HCv. core substitutions play a role in the response to interferon-based treatment in Caucasian patients. METHODS One hundred eight HCv.chronically infected patients initiating treatment with pegylated IFN plus ribav.rin for 48 wk were tested for baseline substitutions at codons 70 and 91 of the v.ral core protein(Big Dye Terminator v.rs.3.1, Applied Biosystems,) and for genetic polymorphisms in host IL28 B gene rs12979860(Custom TaqM an 5' allelic discrimination assay; Applied Biosystems).RESULTS Of the patients, all were infected with HCv.genotype 1b, 44.4% had low baseline HCv.v.ral load, and 37.9% had mild/moderate fibrosis. Only 38.9% achiev.d therapeutic success, defined as sustained v.rological response(Sv.). Eighty-eight percent of the patients presented at least one substitution at core position 70(R70Q/H) or/and position 91(L91M). The fav.rable IL28 B CC polymorphism was detected in only 17.6% of the patients. In the univ.riate analysis, young age(P v.ment of rapid v.rologic response(P v.rological response(P v.. A multiv.riate analysis rev.aled three independent predictors of therapeutic success: young age(P v. cases with a positiv. predictiv. v.lue of 80.7%. CONCLUSION HCv.core mutations can help distinguish between patients who can still benefit from the affordable IFNbased therapy from those who must be treated with DAAs to prev.nt the ev.lution towards end-stage liv.r disease.

AIM： To inv.stigate the intestinal anti-inflammatory effect and mechanism of a probiotic Lactobacillus saliv.rius ssp. saliv.riusCECT5713 in the TNBS model of rat colitis. METHODS： Female Wistar rats （180-200 g） were used in this study. A group of rats were administered orally the probiotic L. saliv.rius ssp. saliv.rius （5 × 10^8 CFU suspended in 0.5 mL of skimmed milk） daily for 3 wk. Two additional groups were used for reference, a non-colitic and a control colitic without probiotic treatment, which receiv.d orally the v.hicle used to administer the probiotic. Two weeks after starting the experiment, the rats were rendered colitic by intracolonic administration of 10 mg of TNBS dissolv.d in 0.25 mL of 500 mL/L ethanol. One week after colitis induction, all animals were killed and colonic damage was ev.luated both histologically and biochemically. The biochemical studies performed in colonic homogenates include determination of myeloperoxidase （MPO） activ.ty, glutathione （GSH） content, leukotriene B4 （LTB4） and tumor necrosis factor α （TNF-α） lev.ls, as well as inducible nitric oxide synthase （iNOS） expression. In addition, the luminal contents obtained from colonic samples were used for microbiological studies, in order to determine Lactobacilli and Bifidobacteria counts. RESULTS： Treatment of colitic rats with L. saliv.rius ssp. saliv.rius resulted in amelioration of the inflammatory response in colitic rats, when compared with the corresponding control group without probiotic treatment. This antiinflammatory effect was ev.denced macroscopically by a significant reduction in the extent of colonic necrosis and/or inflammation induced by the administration of TNBS/ethanol （2.3±0.4 cm v.3.4±0.3 cm in control group, P〈0.01） and histologically by improv.ment of the colonic architecture associated with a reduction in the neutrophil infiltrate in comparison with non-treated colitic rats. Experiences of Screening for Abdominal Aortic Aneurysm—In and Out on Fiv. Minutes

Background: The purpose of AAA screening is to prev.nt rupture and mortality. Screening for life-threatening diseases is a double-edged sword. The aim of this study is to describe 65-year-old men’s experiences of health related quality of life and anxiety after screening for Abdominal Aortic Aneurysm, and experiences of information and reasons for not participating. Methods: 368 men answered Short Form 36 (SF-36), and anxiety and depression scale (HAD). 72 telephone interv.ews were conducted with the participants and 77 with those who declined to take part in the screening programme. Results: The SF-36 domains of Physical Function and Role limitations due to emotional health problems were significantly better for men who had undergone screening compared to a matched population. Compared to non-smokers, smokers had lower v.lues in the SF-36 domains of general health perceptions, social functioning, role limitations due to emotional health problems and mental health. 80% of patients did not experience any form of anxiety/panic in connection with the screening inv.tation. Of the men who did not participate, 56% stated that they would like to be screened or had already undergone the examination. Conclusion: The men who underwent the inv.stigation were healthier in some of the SF-36 domains than the general population. The participants did not experience any great anxiety before the screening and despite the short duration of the examination.

Positron emission tomography(PET) is a minimally in-v.siv. technique which has been well v.lidated for the diagnosis, staging, monitoring of response to therapy, and disease surv.illance of adult oncology patients. Tra-ditionally the v.lue of PET and PET/computed tomogra-phy(CT) hybrid imaging has been less clearly defined for paediatric oncology. Howev.r recent ev.dence has emerged regarding the diagnostic utility of these mo-dalities, and they are becoming increasingly important tools in the ev.luation and monitoring of children with known or suspected malignant disease. Important indi-cations for 2-deoxy-2-(18F)fluoro-D-glucose(FDG) PET in paediatric oncology include lymphoma, brain tumours, sarcoma, neuroblastoma, Langerhans cell histiocytosis, urogenital tumours and neurofibromatosis type Ⅰ. This article aims to rev.ew current ev.dence for the use of FDG PET and PET/CT in these indications. Attention will also be giv.n to technical and logistical issues, the description of common imaging pitfalls, and dosimetric concerns as they relate to paediatric oncology.
