Variability of anti-human transglutaminase testing in celiac disease across Mediterranean countries

AIM To verify the precision and accuracy of transglutaminase antibodies(TGA)assays across Mediterranean countries.METHODS This study involved 8 referral centres for celiac disease(CD)in 7 Mediterranean countries.A central laboratory prepared 8 kits of 7 blinded and randomized serum samples,with a titrated amount of Human TGA Ig A.Each sample was analysed three times on three different days,with each centre running a total of 21tests.The results were included in a blindly coded report form,which was sent to the coordinator centre.The coordinator estimated the mean coefficient of Variation(Co Var=σ/μ),the mean accuracy(Accur=Vobserved-Vreal)and the mean percent variation(Var%=[(Vobserved-Vreal)/Vreal]×100).RESULTS The analysis showed that 79.17%of the mean variation fell between-25%and+25%of the expected value,with the accuracy and precision progressively increasing with higher titres of TGA.From values 1.25 times greater than the normal cut-off,the measurements were highly reliable.CONCLUSION TGA estimation is a crucial step for the diagnosis of CD;given its accuracy and precision,clinicians could be confident in establishing a diagnosis.

A new phosphate, Na2.5Y0.5Mg7(PO4)6 was synthesized as single crystals by the flux method and as a powdered sample by the Pechini technique, and investigated by single crystal X-ray diffraction, charge distribution (CD) and 31P NMR spectroscopy. Na_2BaMg(PO_4 )_2:synthesis, crystal structure and europium photoluminescence properties

Na2BaMg（PO4）2 was synthesized and characterized by X-ray diffraction, differential thermal analysis （DTA）, 31p NMR spectroscopy and photoluminescence measurements. This compound crystallized in the P3 ml space group of the trigonal system with the parameters of equivalent hexagonal cell a=0.5305（1） nm and c=0.6986（3） nm. The structure showed a perfect isotipysm with the mineral glaserite KaNa（SO4）2. Thus, it exhibited a lamellar framework which could be described as the stacking along the [001] direc- tion of [MgP2O84-]∞ layers of comer sharing MgO6 polyhedra and PO4 tetrahedra. The Ba2＋ cations were located within the interlayer space, while the Na＋ ones were found in large cavities bound to the layers. The DTA analysis showed that this compound melt con- gruently at 1385 K. The 31p NMR spectrum was consistent with the presence of a single phosphorus site in the structure. Optical properties were investigated for the 5% doped Eu3＋ and Eu2＋ phases. The trivalent europium was used as a local probe, replacing bar- ium while the divalent europium was used to examine whether this material could be used as an optical applications.

This work describes the scandium doping effect on the structural and magnetic properties of delafossite-type oxides CuCr1?- xScxO2. The lattice parameters were found to vary according to Vegard’s low. A reflection broadening is observed, that is ascribed to local lattice distortion due to the ionic radius difference between Cr3+ and the non-magnetic dopants. Magnetic susceptibility measurements show that the dominant interactions are antiferromagnetic (AFM) but that doping induces significant changes. The coupling between the local spins at the Cr sites and doped metal transition may enhance spin fluctuations at the Cr sites, which break the residual magnetic degeneracy as fluctuation-induced symmetry breaking in a highly magnetic degenerate ground state manifold of some frustrated systems.

Thermal Effects on Bond Properties of GFRP Rebars Embedded in Concrete

This paper presents the results of an experimental study on the thermal effects on glass fibre reinforced polymer (GFRP) bars embedded in concrete. The pullout test specimens were subjected to temperatures of 40°C, 60°C and 80°C during a continuous four months-period of time. The results were compared to the reference specimens (room temperature). It was found that up to 60°C, the loss in bond strength due to the temperature is not significant, whereas for the 80°C-temperature a reduction of 14% in the bond strength is observed. Also, the bond-slip relationship was modelled using the CMR-model and new coefficients are proposed for the bond-slip behaviour of GFRP bars.

Anterior and posterior urethral valves: a rare association

Association of anterior and posterior urethral valve is a rare congenital anomaly causing lower urinary tract obstruction in children. This paper highlights our experience with a two-month-old boy with double urethral valves. Antenatal ultrasonography failed to detect any abnormality. At the age of 1 month, he presented with urinary dribbling, straining at micturition, and febrile urinary tract infection. Clinically, the urethra was dilated at micturition. Diagnosis was confirmed by retrograde urethrogram and urethroscopy. Endoscopic ablation of both valves was done by electrocautery hook.

AIM:To assess the role of the major risk factors for hepatocellular carcinoma(HCC) development in the western part of North Africa.METHODS:A multicenter case control study was conducted in Tunisia,Morocco and Algeria in collaboration with Pasteur Institutes in these countries.A total of 164 patients with HCC and 250 control subjects without hepatic diseases were included.Prevalences of HBsAg,anti-hepatitis C virus(HCV)and diabetes were assessed.HCV and HBV genotyping were performed for anti-HCV and HBsAg positive patients.RESULTS:The mean age of patients was 62±10 years old for a 1.5 M:F sex ratio.Sixty percent of HCC patients were positive for anti-HCV and 17.9% for HBsAg.Diabetes was detected in 18% of cases.Odd ratio(OR)and 95% confidence intervals(CI) were 32.0(15.8-65.0),7.2(3.2-16.1) and 8.0(3.1 -20.0)for anti-HCV,HBsAg and diabetes respectively.Multivariate analysis indicated that the three studied factors were independent.1b HCV genotype and D HBV genotype were predominant in HCC patients.HCV was the only risk factor significantly associated with an excess of cirrhosis(90% vs 68% for all other risk factors collectively,P=0.00168).Excessive alcohol consumption was reliably established for 19(17.6%) cases among the 108 HCC patients for whom data is available.CONCLUSION:HCV and HBV infections and diabetes are the main determinants of HCC development in North Africa.An active surveillance and secondary prevention programs for patients with chronic hepatitis and nutrition-associated metabolic liver diseases are the most important steps to reduce the risk of HCC in the region.Salah Berkane,Department of Gastroenterology BologhineUniversity Hospital,Bologhine 16090,Algiers。

Glucose Metabolism Disorders in Cirrhosis: Frequency and Risk Factors in Tunisian Population. Results of a Cross-Sectional Study

Background and aims: Alterations in carbohydrate metabolism are frequently observed in cirrhosis;to determine the frequency of diabetes mellitus and impaired glucose tolerance in Tunisian cirrhotic patients and identify risk factors. Patients and methods: Cross-sectional study;fasting plasma glucose levels were measured in consecutive patients with cirrhosis. Oral glucose tolerance test was performed if fasting plasma glucose level was normal. Glucose metabolism disorders were then classified as: impaired glucose tolerance and diabetes mellitus. Cirrhotics with glucose metabolism disorder were compared to those without. Results: Seventy-seven patients with cirrhosis were included: 68.8% were diagnosed as having glucose metabolism disorder;diabetes in 42.8% and impaired glucose tolerance in 26%. The tests were able to identify 60.4% of glucose metabolism disorders. Univariate analysis disclosed a higher proportion of female gender (p = 0.04) and more frequent familial history of diabetes mellitus (p = 0.005) in the group with glucose metabolism disorder. There were no statistically differences regarding age, etiology and severity of the cirrhosis, and dry body mass index. Multivariate analysis showed that familial history of diabetes was the only independent risk factor (OR = 5.1, p = 0.005). Conclusion: In our study, the frequency of glucose metabolism disorders was 68.8%. Oral glucose tolerance test allowed disclosing nearly half of them, pointing a high incidence of latent glucose metabolism disorders. In this way, it should be routinely evaluated in all patients with cirrhosis. Familial history of diabetes was the only independent risk factor, suggesting that other factors in addition to liver disease may play a role.

Bone Mineral Density in Patients with Newly Diagnosed Inflammatory Bowel Disease: Frequency and Risk Factors in Tunisian Population. Results of a Prospective Study

Background and Aims: Osteopenia and osteoporosis are frequently encountered with Inflammatory Bowel Disease (IBD). Our aims were to determine the prevalence of low bone mineral density (BMD) in patients with recently diagnosed IBD, and to assess predictive factors of reduced BMD. Patients and Methods: Prospective study conducted from January 2008 to December 2012 and involved patients with IBD treated in the Department of Gastroenterology of the Internal Security Forces Hospital. The data collected included: age, gender, body mass index (BMI), diagnostic delay, disease activity, and disease localization. Laboratory findings included serum calcium, phosphate, albumin, hemoglobin, and C-reactive protein. BMD was assessed by dual energy X-ray absorptiometry (DEXA) of the lumber spine and femoral neck. According to WHO criteria, osteopenia was defined as a T-score between -1 and -2 SD, and osteoporosis as a T-score less than -2 SD. Results: A total of 34 patients (17 men, 17 women) were enrolled. Mean age was 37.1 ± 13.8 years (range 16 - 62). Twenty-two patients (65%) had Crohn’s disease (CD) and 12 patients (35%) had ulcerative colitis (UC). Mean BMI was 20.5 ± 4 kg/m2. Low BMD occurred in 50% of patients (12 CD, 5 UC). Thirteen patients (38.2%) exhibited osteopenia and 4 patients (10.8%) showed osteoporosis. Mean vertebral T-score was -0.933 ± 1.41 (range -4.1 to 1.7) and BMD in this site was 1.079 ± 0.17 g/cm2 (range 0.674 to 1.380). Mean femoral T-score was -0.398 ± 1.2 (range -3.1 to 2.4) and BMD in this site was 0.990 ± 0.173 g/cm2 (range 0.633 to 1.600). There was a positive correlation between T-score and albuminemia. Low BMI was found to be predictive factor of reduced BMD at the moment of IBD diagnosis. However, no correlation was found between BMD and the other studied variables (age, gender, smoking, history of fracture, disease location, duration of disease, activity, small bowel resection, serum calcium level, phosphate, C-reactive protein and hemoglobin). A Case Report of Osteosarcoma of the Skull

Background: Osteosarcoma is the most common primary malignancy and aggressive neoplasm of bone composed of spindle cells producing osteoid. The incidence of primary osteogenic sarcomas of the skull is about 1% to 2% of all skull tumors. Methods: We present an extraordinary case of osteosarcoma arising in the postero-temporal region of the skull. The clinical features, diagnosis and management are discussed. Results: A 34-year-old woman was operated in our department for mass, fixed to bone in the left postero-temporal area. Histopathology confirmed the diagnosis of osteogenic sarcoma. Conclusion: We review the literature of reported cases of primary osteogenic sarcomas of the skull to discuss the common clinical presentation, evaluation methods, and recommended treatment plans.
