Endoscopic ultrasound elastography for evaluation of lymph nodes and pancreatic masses:A multicenter study

AIM:To evaluate the ability of endoscopic ultrasound(EUS) elastography to distinguish benign from malignant pancreatic masses and lymph ***:A multicenter study was conducted and included 222 patients who underwent EUS examination with assessment of a pancreatic mass(n=121) or lymph node(n=101).The classification as benign or malignant,based on the real time elastography pattern,was compared with the classif ication based on the B-mode EUS images and with the fi nal diagnosis obtained by EUS-guided fi ne needle aspiration(EUS-FNA) and/or by surgical *** interobserver study was ***:The sensitivity and specificity of EUS elastography to differentiate benign from malignant pancreatic lesions are 92.3% and 80.0%,respectively,compared to 92.3% and 68.9%,respectively,for the conventional B-mode *** sensitivity and specificity of EUS elastography to differentiate benign from malignant lymph nodes was 91.8% and 82.5%,respectively,compared to 78.6% and 50.0%,respectively,for the B-mode *** kappa coefficient was 0.785 for the pancreatic masses and 0.657 for the lymph ***:EUS elastography is superior compared to conventional B-mode imaging and appears to be able to distinguish benign from malignant pancreatic masses and lymph nodes with a high sensitivity,specificity and *** might be reserved as a second line examination to help characterise pancreatic masses after negative EUS-FNA and might increase the yield of EUS-FNA for lymph nodes.

One case of intrahepatic cholangiocarcinoma amenable to resection after radioembolization

We report the case of a 57-year-old man who was diagnosed with a large unresectable cholangiocarcinoma associated with 2 satellite nodules and without clear margins with the right hepatic *** 4 cycles of GEMOX(stopped due to a hypertransaminasemia believed to be due to gemcitabine)and 4 cycles of FOLFIRINOX,the tumor remained stable and continued to be considered ***(resin microspheres,SIRS-spheres)targeting the left liver(474 MBq)and segment IV(440 MBq)was *** injection was very well tolerated,and 4 more cycles of FOLFIRINOX were given while waiting for radioembolization *** computed tomography scan,a partial response was observed;the tumor was far less hypervascularized,and a margin was observed between the tumor and the right hepatic vein.A left hepatectomy enlarged to segmentⅧwas *** pathological exam,most of the tumor was acellular,with dense fibrosis around visible *** cells were observed only at a distance from *** can be useful in the treatment of cholangiocarcinoma,allowing in some cases a secondary resection.

Background and Study Aims:The aim of this study was to evaluate the efficacy and outcomes of treatment by endoscopic mucosal resection(EMR)of patients with high-grade dysplasia(HGD)or *** and Methods:Between January 1995 and January 2002,50 patients(35 men,15 women)were treated by EMR for 52 sessile *** median size of the polyps was 27.5 mm(range 10-60).The “lift and cut”EMR technique was *** the lesion was poorly differentiated or infiltrated the muscularis mucosae to more than 1000 μm,the patient was referred for *** the other cases,follow-up was ***:Complications occurred in 9.6%of cases and were always treated *** rate of endoscopically complete resection was judged to be 98.1%.Argon plasma coagulation was applied to the margins of the lesion in 21.6%of *** examination showed 38 HGDs and 14 *** patients had a lesion reaching the deep or lateral margin;four were referred for surgery;two patients for whom surgery would have been high risk were followed up,and both developed local recurrence;and one patient was followed up,without recurrence,because infiltration was less than 1000 μm.A total of 43 patients were followed up after complete *** patients died during follow-up;neither death could be reliably attributed to colorectal *** patients were lost during the *** 34 patients,information from a mean follow-up of 17.3 months(6-57)was available and recurrence was observed in five cases(15%).Conclusions:EMR appears to be a safe and efficient treatment of HGD and early colorectal ***,correct analysis of submucosal infiltration is essential to assess the completeness of the resection.
