Kawasaki disease, <i>Mycoplasma pneumoniae</i>infection and anaplastic large cell lymphoma: An uncommon association

Kawasaki disease (KD) is an acute febrile systemic vasculitis occurring predominantly in young children less than 5 years of age. Although imperfectly known, the aetiopathogenesis of KD would be secondary to immunological abnormalities that could constitute a favourable terrain for neoplasms. We report on a case in a 2-year-old girl who presented clinical manifestations compatible with Kawasaki disease complicated by coronary aneurysm. Aetiopathological investigations revealed M. pneumoniae infection as specific IgM were present in the serum (Elisa). The patient was initially treated by intravenous immunoglobulins (IVIG) and aspirin to anti-inflammatory dose. Following a few days of desquamation, resolution of the symptomatology occurred. Four weeks later she developed disseminated tumorous syndrome. Lymph node biopsy revealed massive infiltration by large cells lymphomatous proliferation. Histologic and immunophenotypic findings were characteristic of ALK-1+ anaplastic large cell lymphoma. Four weeks later, the patient died from a severe nosocomial infection complicated by septic shock. Our observation is the first cases describing the association between anaplastic large cell lymphoma, KD and M. pneumoniae. Immunologic disorder due to KD and M. pneumoniae infection may play probably a central role for malignancy.

Recently,the Institute for Radiological protection and Nuclear Safety(IRSN)has launched VSEAL(Vertical SEALing)project to investigate the impact of gas migration on the long-term performance of bentonite based vertical sealing systems(VSS).The first VSEAL in situ test was emplaced in IRSN’s Underground Research Laboratory(URL)in Tournemire(France)in 2019 and was equipped with 76 wired and wireless *** test is still in progress,but the collected set of data provides already valuable information of the hydro-mechanical behavior of VSS during *** swelling core consists of a mixture of highdensity pellets and powder of MX80 bentonite in a ratio of 80/20(in dry mass).An innovative method was adopted to drill a 1-m diameter and w10-m deep shaft in order to minimize the rock perturbation at the *** a specific protocol was adopted to install the bentonite mixture together with a careful characterization of the core during construction,VSEAL 1 constitutes the unique in situ sealing test with a well-known initial structural distribution of the pellets and the *** heterogeneities occurred within the experiment during the installation process:a damaged zone developed around the shaft walls due to the interruption of the installation operations caused by COVID19 lockdown in France;a technological gap with a variable thickness between the last pellets layer and the top confining lid and a heterogeneous distribution of the bentonite powder at some layers inducing large inter pellets voids close to the bentonite-rock *** injected water volume,relative humidity,water content and swelling pressure in both radial and axial directions were *** of the results showed that the presence of installation-induced heterogeneities led to the generation of preferential flow paths that influenced the swelling pressure evolution at radial and axial directions.

This study investigates the dynamic connectedness between stock indices and the effect of economic policy uncertainty(EPU)in eight countries where COVID-19 was most widespread(China,Italy,France,Germany,Spain,Russia,the US,and the UK)by implementing the time-varying VAR(TVP-VAR)model for daily data over the period spanning from 01/01/2015 to 05/18/*** showed that stock markets were highly connected during the entire period,but the dynamic spillovers reached unprecedented heights during the COVID-19 pandemic in the first quarter of ***,we found that the European stock markets(except Italy)transmitted more spillovers to all other stock markets than they received,primarily during the COVID-19 *** analysis using a nonlinear framework showed that the dynamic connectedness was more pronounced for negative than for positive ***,findings showed that the direction of the EPU effect on net connectedness changed during the pandemic onset,indicating that information spillovers from a given market may signal either good or bad news for other markets,depending on the prevailing economic *** results have important implications for individual investors,portfolio managers,policymakers,investment banks,and central banks.

Agricultural soils in semi-arid regions have frequently been degraded due to adverse climatic conditions,organic matter depletion,and poor farming *** enhance soil quality,this study examines the reuse of sewage sludge(SS)as an available source of organic matter in a typical Mediterranean sandy-loam ***,we studied the cumulative effect of two annual applications of 40,80 and 120 tons of sludge per ha on soil quality in absence of *** dose-dependent improvement of organic matter content was the most significant event that reflected sludge application rates,and consequently influenced other soil ***,soil structural stability increased by 13.3%,28.8%and 59.4%for treatments SS-40,SS-80 and SS-120 respectively as compared to unamended *** stability improvement was also confirmed by the dose-dependent variation of other edaphic factors including calcium content,the microbial quotient as well as Welt and C:N *** param-eters are involved in cementing soil aggregates by cation bridging,the formation of microbial mucilage,and clay-humic *** magnetic susceptibility(SMS)was measured in situ as a possible rapid tool to evaluate soil *** showed significant correlation with sludge dose and stability amelioration testifying to the aggregation role that can play Al2O3 and particularly Fe2O3 minerals added by the hematite-rich ***,analytical results and field observations revealed no trends of soil salinization or acidification by excessive sludge *** avoiding the rhizosphere effect,outcomes could reflect the resilience and intrinsic capacity of the soil to cope with excessive sludge loads.

Angiokeratoma corporis diffusum is a dermatological hallmark of several inherited lysosomal storage disorders including Anderson-Fabry disease and other enzyme deficiencies such as fucosidosis. We report a 4-year-old boy with neurodevelopment delay who was diagnosed as having fucosidosis following recognition of dermatological signs, angiokeratoma and telangiectasies. The diagnosis was confirmed by leukocyte oligosaccharide enzyme analysis.

Introduction. Pemphigoid gestationis is a bullous autoimmune sub-epidermal dermatosis, occurring during pregnancy and/or postpartum. The objective of our study was to define the epidemio-clinical profile, the histopathological and immunopathological features, the treatment and the course of pemphigoid gestationis in Tunisian patients. Patients and methods. This was a retrospective study concerning the cases of pemphigoid gestationis recorded between 1989 and 2003 in the dermatology department in La Rabta Hospital in Tunis. The patients were included according to clinical, histopathological and immunopathological criteria. Results. We retained 20 patients: 15 multiparae and 5 primiparae. The average age at onset was 29. The first clinical signs appeared in the 3 rd trimester in 60 p. 100 of all cases. Clinically, the urticarial patches were noticed in 90 p. 100 of all cases and blisters in 65 p. 100 of the cases. The eruption was located mainly on the trunk and the limbs. The face was affected in 7 cases, the mucous membranes in 3 cases, the palms in 2 cases and the soles in 1 case. Cutaneous histologic examination revealed a sub-epidermal blister in n cases and a lymphohistiocyte infiltrate in all cases. Direct immunofluorescence showed a linear deposition of the third component of the complement along the basement membrane zone in all cases. Fetal prognosis was assessed in 13 cases: 1 fetal death, 1 still-born, 3 miscarriages, 1 anencephaly and 2 cases of transit bullous affection in the new-born. Discussion. Our results are similar to those in the literature, but with some particular aspects: the late onset of the pemphigoid gestationis in the course of the 3rd trimester of pregnancy, the frequent involvement of the face and the mucous membranes and the absence of the two main fetal risks: prematurity and hypotrophy. Moreover, we underline the efficiency of topical class I corticosteroids in the treatment of pemphigoid gestationis.

Background: Risk factors for erysipelas (cellulitis) were rarely evaluated in controlled studies. Regional variations of these risk factors have never be assessed. Objective: To assess risk factors for erysipelas of the leg in Tunisia. Subjects and Methods: Case- control study in seven hospital centers in Tunisia. Cases were 114 consecutive patients with erysipelas of the leg [sudden onset (< 24 h) of a well- demarcated dermo- hypodermatitis with fever or chills]. Two controls were matched to each case for age, sex, and hospital (n=208). Main outcome measures are local and general suspected risk factors for erysipelas of the leg. Results: In multivariate analysis, disruption of the cutaneous barrier (i.e. traumatic wound, toe- web intertrigo, excoriated leg dermatosis or plantar squamous lesions) and leg edema were independently associated with erysipelas of the leg, with respective odds ratios of 13.6 (95% confidence interval: 6.0- 31) and 7.0 (1.3- 38). No association was observed with diabetes, alcoholism, or smoking. Conclusions: We confirmed the major role of local risk factors and the minor role of general risk factors for erysipelas of the leg, in a setting different than the one previously studied. Detecting and treating toe- web intertrigo and traumatic wounds should be considered in the prevention of erysipelas of the leg.
