AIm: To assess the presence of spinal accessory neuropathy in patients with chronic neck ***: Patients with pain either regional or focal in the neck or shoulders for at least 6 mo(chronic neck pain) were recruited randomly from the Rheumatology and Rehabilitation Outpatient Clinic at the Faculty of medicine-Suez Canal University. Two groups were compared: 30 patients with chronic neck pain with mean age(36.97 ± 12.45 years) and 10 apparently healthy controls. Trapezius muscle examination including inspection and range of motion both active and passive was performed. A full clinical neurological examination was carried out to exclude peripheral neuropathy and motor neuron disease. According to the subject's type of work, cases were categorized into labor-intensive and non-labor intensive tasks. A nerve conduction study(NCS) was performed on spinal accessory nerves at both sides for all patients and controls. Parameters including latencies and amplitudes of compound motor action potential(CmAP) were compared with the chronicity of neck pain using the neck disability score. This cross sectional study was carried in the Rheumatology and Rehabilitation Department, at Suez Canal University Hospital, Ismailia, ***: Physical examination revealed that 80% of cases had spinal trapezius muscle spasm. Restricted neck motion was present in 16.6% of cases. No one suffered from muscle wasting or weakness. Pain was bilateral in 18 patients(60%), localized to the right side in six patients(20%) and localized to the left side in six patients(20%). The causes of neck pain in the patients studied were nonspecific, due to physical stresses, cervical spondylosis and mild cervical disc herniation. mean disease duration in patients with labor-intensive tasks was(3.9 ± 2.1 years), which was longer than that in patients with non-labor intensive tasks(3.1 ± 1.9 years); however, this difference was statistically insignificant. The compound （1 z）-2-（hydroxymethyl）-dodec-1-ene-1,3-diol was previously isolated from the integument of the red palm weevil Rhynhophorus ferruginous and found to antagonize calcium chloride induced dysrrhythmia in mice. Further calcium-dependent pharmacological actions performed in this study revealed that it has the ability to inhibit platelets aggregation induced in guinea pigs with either ADP or archegonia acids, and the compound also decreased the arterial pressure in rats in a dose-dependent manner and exerted potential ant consultant activity in mice against pentylene tetrazole-induced seizures. However, no complete protection was observed. Analgesic studies in mice using the hot plate （58 ℃） revealed its potential analgesic activities. Its effective dose 50 （ED50） was 85.7 ± 6.3 mg/kg （N= 5）. The compound seamed to produce all of the above actions via a calcium antagonistic action.

Systemic infammation is a recognized feature in chronic kidney disease （CKD）. The role of systemic infammation in the pathogenesis of vascular calcifcation was recently settled. FGF23 was recently accused as a direct stimulus of systemic inflammation. This finding explains the strong association of FGF23 to vascular calcifcation and increased mortality among CKD.

Effect of Cyclooxygenase Inhibition on P-Glycoprotein Expression and Phenytoin Level in Brain Tissue of Pilocarpine Induced Epilepsy in Rats

Background: Increased brain P-glycoprotein (P-gp) expression may play important role in resistance to antiseizure drugs. The present work aimed to overcome the drug resistance that develop due to overexpression of P-gp with subsequent increase in brain phenytoin level in epileptic rats, using either non-selective (indomethacin) or selective (celecoxib) cyclooxygenase inhibitors. methods: Fifty-six adult male albino rats were randomly divided into seven groups. Epilepsy was induced using the lithium pilocarpine model. Rats received indomethacin (2.5 mg/kg) or celecoxib (20 mg/kg), either alone or combined with phenytoin (50 mg/kg). Seizures were evaluated using Racine score. motor coordination was assessed using open field and rotarod tests. Phenytoin brain level was measured using High Performance Liquid Chromatography (HPLC), glutamate expression was measured using Enzyme Linked Immunosorbent Assay (ELISA), ATP Binding Cassette Subfamily B member 1 (ABCB1) gene expression was assessed using Real Time-Polymerase Chain Reaction (RT-PCR), and immunohistochemical analysis was done for P-gp expression. Results: Phenytoin combination with either indomethacin or celecoxib had improved the Racine score, motor coordination on rotarod apparatus, and open field test results. Also, phenytoin combination with either indomethacin or celecoxib decreased brain glutamate level, ABCB1 gene and P-gp expression, and increased brain phenytoin level compared to treatment with phenytoin alone. This indicated that both P-gp inhibitors indomethacin and celecoxib, increased the level of phenytoin that reached the brain of rats. However, brain uptake of phenytoin was significantly enhanced using celecoxib rather than indomethacin (CI 95%, 17.092: 32.808, P-value Conclusion: Cyclooxygenase inhibition using either celecoxib or indomethacin resulted in downregulation of P-gp expression, with subsequent increase in brain phenytoin level in epileptic rats.

The epigenetic regulation of gene expression is critical for ensuring the proper deployment and stability of defined genome transcription programs at specific developmental stages. The cellular memory of stable gene expression states during animal and plant development is mediated by the opposing activities of Polycomb group （PcG） factors and trithorax group （trxG） factors. Yet, despite their importance, only a few trxG factors have been characterized in plants and their rotes in regulating plant development are poorly defined. In this work, we report that the closely related Arabidopsis trxG genes ULTRAPETALA1 （ULT1） and ULT2 have overlapping functions in regulating shoot and floral stem cell accumula- tion, with ULT1 playing a major role but ULT2 also making a minor contribution. The two genes also have a novel, redun- dant activity in establishing the apical-basal polarity axis of the gynoecium, indicating that they function in differentiating tissues. Like ULT1 proteins, ULT2 proteins have a dual nuclear and cytoplasmic localization, and the two proteins physically associate in planta. Finally, we demonstrate that ULT1 and ULT2 have very similar overexpression phenotypes and regulate a common set of key development target genes, including floral mADS-box genes and class I KNOX genes. Our results reveal that chromatin remodeling mediated by the ULT1 and ULT2 proteins is necessary to control the development of mer- istems and reproductive organs. They also suggest that, like their animal counterparts, plant trxG proteins may function in multi-protein complexes to up-regulate the expression of key stage- and tissue-specific developmental regulatory genes.

The Potential Effect of Special Formulas on Cirrhotic Rats

Liver protective effect of special formulas (1 and 2) was assessed against carbon tetra chloride (CCl4) which induced liver damage in Wister albino rats. The two prepared formulas reduced the changes in body weight and liver weight caused by CCl4 in rats. The toxicity of CCl4 is related to loss in body weight and increase in liver weight in rats. The weight ratios of liver to body weight (LW/BW) significantly increased in rats treated with CCl4 followed by other groups. Formulas 1 and 2 could play an important role in improvement of hematological indices in liver cirrhosis rats. Feeding treated rats on special formulas showed improvement in liver function compared to rats fed on basal diet, reflected by significant reduction of the activity of transaminases (ALT and AST), alkaline phosphatase (ALP) and total bilirubin. There was a significant increase in total protein, albumin and globulin in serum. Significant increase in liver weight in rats treated with CCl4. There are no histopathological changes in all groups under study except for group 4 (CCl4 treated rats fed on basal diet) which orally administrated with CCl4 and had congestion of central vein and hepatic sinusoids.
