Correlations between serum kidney injury molecule-1,cystatin C and immunosuppressants:A cross-sectional study of renal transplant patients in Bahrain

Renal transplant patients receive several immunosuppressive drug regimens that are potentially nephrotoxic for treatment.Serum creatinine is the standard for monitoring kidney function;however,cystatin C(Cys C)and kidney injury molecule-1(KIM-1)have been found to indicate kidney injury earlier than serum creatinine and provide a better reflection of kidney function.Here,we assessed Cys C and KIM-1 serum levels in renal transplant patients receiving mycophenolate mofetil,tacrolimus,sirolimus,everolimus,or cyclosporine to evaluate kidney function.We used both the Chronic Kidney Disease Epidemiology Collaboration(CKD-EPI)2021 equation,which is based on creatinine and combined creatinine with Cys C,and the CKD-EPI 2012 equation,which is based on Cys C alone,to estimate glomerular filtration rate(GFR).Then,we assessed the association between serum KIM-1 and GFR<90 mL per minute per 1.73 m2.We observed significantly higher serum Cys C levels in patients with the elevated serum creatinine,compared with those with normal serum creatinine.The estimated GFRs based on creatinine were significantly higher than those based on the other equations,while a significant positive correlation was observed among all equations.Serum KIM-1 levels were negatively correlated with the estimated GFRs by the CKD-EPI Cys C and the combined creatinine with Cys C equations.A serum KIM-1 level above 0.71 ng/mL is likely to indicate GFR<90 mL per minute per 1.73 m2.We observed a significant correlation between serum creatinine and Cys C in our renal transplant patients.Therefore,serum KIM-1 may be used to monitor renal function when using potentially nephrotoxic drugs in renal transplants.

Molecular design and applications of a nanostructure green Tripodal surface active ionic liquid in enhanced oil recovery: Interfacial tension reduction, wettability alteration, and emulsification

Surface active ionic liquids (SAILs) are considered as prominent materials in enhanced oil recovery thanks to their high interfacial activity. This study reports the preparation and applications of a nanostructure Tripodal imidazolium SAIL as an environmentally-friendly substitute to the conventional surfactants. The product has a star-like molecular structure centered by a triazine spacer, namely [(C_(4)im)_(3)TA][Cl_(3)], prepared by a one-step synthesis method and characterized with FT-IR, NMR, XRD, and SEM analysis methods. The interfacial tension of the system was decreased to about 78% at critical micelle concentration of less than 0.08 mol·dm^(−3). Increasing temperature, from 298.2 to 323.2 K, improved this capability. The solid surface wettability was changed from oil-wet to water-wet and 80% and 77% stable emulsions of crude oil–aqueous solutions were created after one day and one week, respectively. Compared to the Gemini kind homologous SAILs, the superior effects of the Tripodal SAIL were revealed and attributed to the strong hydrophobic branches in the molecule. The Frumkin adsorption isotherm precisely reproduced the generated IFT data, and accordingly, the adsorption and thermodynamic parameters were determined.

A study of efficacy of Nigella sativa in treatment of Meniere's disease: A randomized, placebo controlled clinical trial

Purpose:Meniere's disease(MD),first introduced by Prosper Meniere,is characterized mainly by vertigo,tinnitus,aural fullness and sensorineural hearing loss.Though the exact pathophysiology of MD is unknown,immunologic and inflammatory interactions are possible underlying mechanisms involved in MD.This study is aimed to investigate the immunomodulatory and anti-inflammatory effect of Nigella sativa on MD as a therapeutic agent.Methods:We divided 40 patients with definite MD into two groups of 20 cases.The study group received 1 g of Nigella sativa oil daily for three months and the control group received a placebo.Changes in hearing,tinnitus and vertigo were estimated by pure tone audiometry,tinnitus handicap inventory questionnaire and dizziness handicap inventory questionnaire,respectively.Results:At the end of the study we did not observe any significant improvement in study's group hearing threshold,tinnitus and vertigo compared to the control group.Conclusions:In this study,statistical analysis showed that Nigella sativa failed to improve signs and symptoms of MD.However,further investigations with a larger study population are needed to ascertain the current conclusion.

Objective:Over 50% of the world populations are infected with Helicobacter pylori (H. pylori). Most subjects are asymptomatic;however, in 1994, H. pylori has been categorized as group I carcinogen. The aim of the study was to investigate the relationship between H. pylori infection and gastric cancer. Methods:Thirty gastric cancer patients (GCs) and 30 gastritis patients were enrolled in the study. H. pylori was cultured on non-selective and selective medias, infection density was assessed by quantitative culture. Antibiotic sensitivity testing was performed. PCR was done for the H. pylori 16S rRNA gene in addition to cagA, vacA and iceA genes. Results:H. pylori could be cultured from 100% of specimens obtained from all patients. The density of H. pylori was higher in cancer cases than in gastritis patients. The 16S rRNA was detected in all GC patients (100%) while it was only detected in 70% of gastritis patients. The cagA gene was found in 53.3% vs 13.3% of GC and gastritis patients, respectively. The vacA gene was present in all GC patients (by at least one of its alleles) while it was only found in 33.3% of gastritis patients. The vacA s1m1 combination was the most predominant genotype in GC patients, while m2 was the commonest allele in gastritis patients (10%). The iceA gene was found in 86.7% vs 40% of GC and gastritis patients, respectively. Simultaneous presence of multiple H. pylori strains was proved, both phenotypically and genotypically. Conclusion:The development of GC is linked to infection with H. pylori harboring certain virulence genes. Higher infection density of H. pylori was found in GC patients. Co-existence of more than one strain of H. pylori in the same patient occurs in both malignant and benign lesions.

Dieback intensity but not functional and taxonomic diversity indices predict forest productivity in different management conditions: Evidence from a semi-arid oak forest ecosystem

The relationships between different aspects of diversity(taxonomic,structural and functional)and the aboveground biomass(AGB)as a major component of global carbon balance have been studied extensively but rarely under the simultaneous influence of forest dieback and management.In this study,we investigate the relationships between taxonomic,functional and structural diversity of woody species(trees and shrubs)and AGB along a gradient of dieback intensity(low,moderate,high and no dieback as control)under two contrasted management conditions(protection by central government vs.traditional management by natives)in a semi-arid oak(Quereus brantii Lindl.)forest ecosystem.AGB was estimated and taxonomic diversity,community weighted average(CWM)and functional divergence indices were produced.We found that the aerial biomass was significantly higher in the intensively used area(14.57(±1.60)t/hm^(2))than in the protected area(8.70(±1.05)t/hm^(2))due to persistence of some large trees but with decreasing values along the dieback intensity gradient in both areas.CWM of height(H),leaf nitrogen content(LNC)and leaf dry matter content(LDMC)were also higher in the traditional managed area than in the protected area.In contrast,in the protected area,the woody species diversity was higher and the inter-specific competition was more intense,explaining a reduced H,biomass and LDMC.Contrary to the results of CWM,none of the functional diversity traits(FDvar)was affected by dieback intensity and only FDvar values of LNC,leaf phosphorus content(LPC)and LDMC were influenced by management.We also found significantly positive linear relationships of AGB with CWM and FDvar indices in the protected area,and with taxonomic and structural diversity indices in the traditional managed area.These results emphasize that along a dieback intensity gradient,the leaf functional traits are efficient predictors in estimating the AGB in protected forests,while taxonomic and structural indices provide better results in forests under a high human pressur

Next-generation cellular networks are expected to provide users with innovative gigabits and terabits per second speeds and achieve ultra-high reliability,availability,and ultra-low latency.The requirements of such networks are the main challenges that can be handled using a range of recent technologies,including multi-access edge computing(MEC),artificial intelligence(AI),millimeterwave communications(mmWave),and software-defined networking.Many aspects and design challenges associated with the MEC-based 5G/6G networks should be solved to ensure the required quality of service(QoS).This article considers developing a complex MEC structure for fifth and sixth-generation(5G/6G)cellular networks.Furthermore,we propose a seamless migration technique for complex edge computing structures.The developed migration scheme enables services to adapt to the required load on the radio channels.The proposed algorithm is analyzed for various use cases,and a test bench has been developed to emulate the operator’s infrastructure.The obtained results are introduced and discussed.

In computer vision,emotion recognition using facial expression images is considered an important research issue.Deep learning advances in recent years have aided in attaining improved results in this issue.According to recent studies,multiple facial expressions may be included in facial photographs representing a particular type of emotion.It is feasible and useful to convert face photos into collections of visual words and carry out global expression recognition.The main contribution of this paper is to propose a facial expression recognitionmodel(FERM)depending on an optimized Support Vector Machine(SVM).To test the performance of the proposed model(FERM),AffectNet is *** uses 1250 emotion-related keywords in six different languages to search three major search engines and get over 1,000,000 facial photos online.The FERM is composed of three main phases:(i)the Data preparation phase,(ii)Applying grid search for optimization,and(iii)the categorization phase.Linear discriminant analysis(LDA)is used to categorize the data into eight labels(neutral,happy,sad,surprised,fear,disgust,angry,and contempt).Due to using LDA,the performance of categorization via SVM has been obviously enhanced.Grid search is used to find the optimal values for hyperparameters of SVM(C and gamma).The proposed optimized SVM algorithm has achieved an accuracy of 99%and a 98%F1 score.

Calcitonin and vitamin D3 have high therapeutic potential for improving diabetic mandibular growth

The goal of this study was to assess the effect of the intermittent combination of an antiresorptive agent （calcitonin） and an anabolic agent （vitamin D3） on treating the detrimental effects of Type 1 diabetes mellitus （DM） on mandibular bone formation and growth. Forty 3-week-old male Wistar rats were divided into four groups： the control group （normal rats）, the control C＋D group （normal rats injected with calcitonin and vitamin D3）, the diabetic C＋D group （diabetic rats injected with calcitonin and vitamin D3） and the diabetic group （uncontrolled diabetic rats）. An experimental DM condition was induced in the male Wistar rats in the diabetic and diabetic C＋ D groups using a single dose of 60 mg.kg-1 body weight of streptozotocin. Calcitonin and vitamin D3 were simultaneously injected in the rats of the control C＋D and diabetic C＋D groups. All rats were killed after 4 weeks, and the right mandibles were evaluated by micro-computed tomography and histomorphometric analysis. Diabetic rats showed a significant deterioration in bone quality and bone formation （diabetic group）. By contrast, with the injection of calcitonin and vitamin D3, both bone parameters and bone formation significantly improved （diabetic C＋ D group）（P 〈 0.05）. These findings suggest that these two hormones might potentially improve various bone properties.

Metaheuristic optimization algorithms present an effective method for solving several optimization problems from various types of applications and fields.Several metaheuristics and evolutionary optimization algorithms have been emerged recently in the literature and gained widespread attention,such as particle swarm optimization(PSO),whale optimization algorithm(WOA),grey wolf optimization algorithm(GWO),genetic algorithm(GA),and gravitational search algorithm(GSA).According to the literature,no one metaheuristic optimization algorithm can handle all present optimization problems.Hence novel optimization methodologies are still needed.The Al-Biruni earth radius(BER)search optimization algorithm is proposed in this paper.The proposed algorithm was motivated by the behavior of swarm members in achieving their global goals.The search space around local solutions to be explored is determined by Al-Biruni earth radius calculation method.A comparative analysis with existing state-of-the-art optimization algorithms corroborated the findings of BER’s validation and testing against seven mathematical optimization problems.The results show that BER can both explore and avoid local optima.BER has also been tested on an engineering design optimization problem.The results reveal that,in terms of performance and capability,BER outperforms the performance of state-of-the-art metaheuristic optimization algorithms.

Burden of Congenital Defects Diagnosed through Ultrasonography in Soba Fetomaternal Unit, Khartoum, Sudan

Background: Congenital anomalies are among the leading causes of fetal loss, despite it can be identified prior to birth through advanced technology in expert hands. Our research aimed at estimating the prevalence of congenital anomalies in Sudan. Methods: A facility-based retrospective cross-sectional study combined with a community-based survey through a telephone interview was implemented on a purposive convenient sample of 138 participants. The data were computerized in Epi Info 7. Google Earth Pro enabled to collect the geographical coordinates for the residence of the participants. Descriptive statistics were performed through SPSS 23 and ArcGIS 10.3 was used to generate the geographical distribution map of congenital defects to visualize the catchment areas of Soba Ultrasonography Unit. Results: Of the 138 participants, the estimated prevalence of congenital defects was 2.2/10,000 live births. The ultrasonography screening revealed that neural tube defects were the most prevalent anomalies with 13.0% (18/138), which represented 47.4% (18/38) of all defects. Concerning children, a mortality rate of 23.2% (32/138) was reported. Conclusions: The child mortality rate post ultrasound screening of 23.2%, and the neural tube defects being the most common anomalies appealed to Sudan health authorities for focusing on more preventive antenatal practices to strengthen and promote maternal and child health.
