Development and Optimization of Universal Bone Conduction hearing Spectacles

Background: Non-implantable bone anchored hearing devices (BChDs) are utilized for patients with conductive or mixed hearing loss who are unsuitable for conventional hearing aids or have unresolved middle ear issues. These devices can be surgically implanted or attached using adhesive plates, dental sticks, elastic headbands, or bone conduction spectacles. Optimal fitting of bone conduction spectacles requires appropriate frame selection and contact pressure in the temporal and mastoid areas. The ANSI S3.6 and DIN EN ISO 389-3 standards recommend a contact area of approximately 1.75 cm2 and a maximum force of 5.4 N for effective sound transmission and comfort. Methods: This study aimed to evaluate the technical fit and mechanical stability of universal bone conduction hearing spectacles compared to established systems. A Sen-Pressure 02 thin-film sensor connected to an Arduino Uno R3 board measured contact force in the temporal and mastoid areas. Several BChDs were tested, including the Bruckhoff la belle BC D50/70, Radioear B71 headset, Radioear B71 elastic headband, Cochlear Baha SoundArc M, and Cochlear Baha elastic headband, on a PVC artificial head, with data analyzed using ANOVA and LSD post hoc tests. Results: The la belle BC D50/70 spectacles showed comparable contact force to established BChDs, ensuring adequate sound transmission and comfort. Significant differences were observed between the systems, with the Radioear B71 headset exhibiting the highest forces. The la belle BC D50/70 had similar forces to the Radioear B71 elastic headband. Conclusion: The la belle BC D50/70 universal bone conduction hearing spectacles are a technically equivalent alternative to established BChDs, maintaining pressure below 5.4 N. Future research should explore the impact of different contact forces on performance and comfort, and the integration of force control in modified spectacles. Protective Effects of Estradiol on Myocardial Contractile Function Following hemorrhagic Shock and Resuscitation in Rats

Background： hemorrhagic shock （hS） results in myocardial contractile dysfunction. Studies showed that 17β-estradiol protects the myocardium against contractile dysfunction. The study investigated the cardioprotective effects of treatment with 17β-estradiol before resuscitation following 1 h of hS and resuscitation. Methods： Male Sprague-Dawley rats were assigned to 2 sets of experimental protocols： Ex vivo and in vivo treatment and resuscitation. Each set had three experimental groups （n = 6 per group）： Normotensive （N）, hS and resuscitation （hS-R） and hS rats treated with 17β-estradiol （E） and resuscitated （hS-E-R）. Rats were hemorrhaged over 60-rain to reach a mean arterial blood pressure of 40 mmhg. In the ex vivo group, hearts were resuscitated by perfusion in the Langendorff system. In the 17β-estradiol treated group, 17β-estradiol 280 μg/kg was added for the first 5 min. Cardiac function was measured. Left ventricular generated pressure （LVGP） and ＋dP/dt were calculated. In the in vivo group, rats were treated with 17β-estradiol 280 ±g/kg s.c. after 60-rain hS. Resuscitation was performed in vivo by the reinfusion of the shed blood for 30-min to restore normotension. Results： Treatment with 17β-estradiol before resuscitation in ex vivo treated and resuscitated isolated hearts and in the in vivo treated and resuscitated rats following hS improved myocardial contractile function. In the in vivo treated group, LVGP and ＋dP/dt max were significantly higher in 17β-estradiol treated rats compared to the untreated group （LVGP 136.40 ± 6.61 compared to 47.58 ± 17.55, and ＋dP/dt 661.85 ± 49.88 compared to 88.18 ± 0.85）. Treatment with 17β-estradiol improved LVGP following hS. Conclusions： The results indicate that treatment with 17β-estradiol before resuscitation following hS protects the myocardium against dysfunction.

Plant secondary metabolites play vital role in plant stress response. In this study we investigated whether root colonization of tomato （Solanum lycopersicum） infected by Trichoderma harzianum leads to alterations in the biosynthesis of secondary plant metabolites including phytohormones and osmolyte proline under drought stress. Exposure of tomato to drought caused a drastic decline in plant growth and physiological parameters. Tomato inoculated with T. harzianum showed increased root and shoot growth and chlorophyll pigments as compared to uninoculated controls as well as drought stressed plants. Proline and total soluble protein content was increased in plants inoculated with T. harzianum under both normal as well as drought conditions. An obvious increase in phenol and flavonoid content was observed due to T. haczianum. In addition, T. hat-zianum inoculated plants maintained higher levels of growth regulators indole acetic acid, indole butyric acid, and gibberellic acid under drought stress. Improved secondary metabolites which play an important role in plant stress tolerance by T. hat-zianum may have coordinately worked for bringing the growth regulation by protecting membranes from reactive oxygen species （ROS） and enhance plant growth through accessing more nutrients by root system.

Calcitonin and vitamin D3 have high therapeutic potential for improving diabetic mandibular growth

The goal of this study was to assess the effect of the intermittent combination of an antiresorptive agent （calcitonin） and an anabolic agent （vitamin D3） on treating the detrimental effects of Type 1 diabetes mellitus （DM） on mandibular bone formation and growth. Forty 3-week-old male Wistar rats were divided into four groups： the control group （normal rats）, the control C＋D group （normal rats injected with calcitonin and vitamin D3）, the diabetic C＋D group （diabetic rats injected with calcitonin and vitamin D3） and the diabetic group （uncontrolled diabetic rats）. An experimental DM condition was induced in the male Wistar rats in the diabetic and diabetic C＋ D groups using a single dose of 60 ***-1 body weight of streptozotocin. Calcitonin and vitamin D3 were simultaneously injected in the rats of the control C＋D and diabetic C＋D groups. All rats were killed after 4 weeks, and the right mandibles were evaluated by micro-computed tomography and histomorphometric analysis. Diabetic rats showed a significant deterioration in bone quality and bone formation （diabetic group）. By contrast, with the injection of calcitonin and vitamin D3, both bone parameters and bone formation significantly improved （diabetic C＋ D group）（P 〈 0.05）. These findings suggest that these two hormones might potentially improve various bone properties.

Liver Fat Scores for Noninvasive Diagnosis and Monitoring of Nonalcoholic Fatty Liver Disease in Epidemiological and Clinical Studies

Nonalcoholic fatty liver disease(NAFLD)is strongly associated with the metabolic syndrome and type 2 diabetes and independently contributes to long-term *** often asymptomatic but reversible,it would require population-wide screening,but direct diagnostics are either too invasive(liver biopsy),costly(MRI)or depending on the examiner’s expertise(ultrasonography).hepatosteatosis is usually accommodated by features of the metabolic syndrome(***,disturbances in triglyceride and glucose metabolism),and signs of hepatocellular damage,all of which are reflected by biomarkers,which poorly predict NAFLD as single item,but provide a cheap diagnostic alternative when integrated into composite liver fat *** liver index,NAFLD LFS,and hepatic steatosis index are common and accurate indices for NAFLD prediction,but show limited accuracy for liver fat *** indices are rarely *** fibrosis scores are commonly used in clinical practice,but their mandatory reflection of fibrotic reorganization,hepatic injury or systemic sequelae reduces sensitivity for the diagnosis of simple ***-induced liver fat changes are poorly reflected by liver fat indices,depending on the intervention and its specific impact of weight loss on *** limited validity in longitudinal settings stimulates research for new ***,hepatokines,markers of cellular integrity,genetic variants but also simple and inexpensive routine parameters might be potential ***,liver fat indices lack precision for NAFLD prediction or monitoring in individual patients,but in large cohorts they may substitute nonexistent imaging data and serve as a compound biomarker of metabolic syndrome and its cardiometabolic sequelae.

Frequency of diabetic retinopathy and associated risk factors in Khartoum,Sudan:population based study

AIM： To assess the frequency and associated risk fac- tors of diabetic retinopathy among Sudanese individuals with diabetes attending Makka Eye complex in Khartoum, Sudan. METhODS： The cross sectional hospital based study recruited 316 individuals with diabetes from Makkah Eye Complex Retina Clinic. Standard questionnaire was used to collect demographic data, medical history and life style characteristics. Blood samples were taken to measure hbAlc and lipid profile. Fundus and slit lamp examination were performed for screening of diabetic retinopathy. RESULTS： Among 316 participants, 187 （59.2%） were males and 129 （40.8%） were females. The mean age of participants was 58.7＋_10.5y. The overall frequency of retinopathy was 261 （82.6%）. The percentages of the total participants with proliferative diabetic retinopathy （PDR） were 126 （39.9%） and non-proliferative diabetic retinopathy （NPDR） were 135 （42.7%）. importantly, duration of diabetes mellitus （DM）（72.2% of more than 10y）, being on oral hypoglycaemic drugs （versus insulin）, and hypertension were all significant risk factors for diabetic retinopathy （P=0.00, 0.01 and 0.00 respectively）. Complications ofdiabetes like diabetic foot （17.7%）, history of amputation （6.7%） and clinically significant macular edema （CSME）（47.4%） of the eyes were all significant risk factors （P〈0.05）. Logistic regression analysis showed that duration of diabetes, hypertension and CSME were found to be absolute risk factors （P=-0.007, 0.003 and 0,000 respectively）. Duration of DM of more than 10y have more than double risk （OR--2.8）, while having hypertension triples the risk of retinopathy （OR=3.1）. CONCLUSION： high rates of diabetic retinopathy are noted among individuals with diabetes attending Makkah Eye hospital in capital Khartoum. Urgent strategies are needed to monitor and treat hypertension and optimize diabetes control in individuals with diabetes. More investment in diabetes services is urgently needed.

Effect of Vitex agnus-castus plant extract on polycystic ovary syndrome complications in experimental rat model

Objective: To investigate ameliorative effects of Vitex agnus-castus (VAC) and VAC containing pharmaceutical supplement (VPS) against polycystic ovary syndrome (PCOS). Methods: PCOS in the rats was induced by daily administration of letrozole at 1 mg/kg body weight concentration for 21 d. PCOS rats were then treated daily either with metformin, VAC plant extract or VPS at 70, 8 or 8 mg/kg body weight concentration for 15 d. Rats that received none of these treatments were considered as control. Blood and ovaries were collected from all the rats. Serum glucose, cholesterol, triglyceride and high density lipoprotein-cholesterol were measured spectrophotometrically. Serum insulin, estrogen, progesterone, testosterone, luteinizing hormone, follicle-stimulation hormone, catalase, superoxide dismutase, malondialdehide and reduced glutathione were measured using enzyme-linked immunosorbent assay. Results: Rats treated with letrozole demonstrated a significant increase in serum testosterone, estrogen, cholesterol, luteinizing hormone, triglycerides, glucose, insulin, and malondialdehide levels, and a significant decline in progesterone, follicle-stimulating hormone, high density lipoprotein-cholesterol, catalase and reduced glutathione levels compared to control. Contrarily, no significant change in superoxide dismutase was noted in response to letrozole treatment. Rats treated with metformin, VAC or VPS showed a remarkable reversal in the levels of parameters affected by letrozole treatment. Conclusions: Data indicate that VAC and VPS exert potential ameliorative effects against PCOS through the modulation of hormonal and lipid profile as well as oxidative stress. Moreover, the favorable effects of these compounds are comparable to that of metformin.
