Type-2 Diabetes Mellitus and Glucagon-Like Peptide-1 Receptor toward Predicting Possible Association

Aim: This study aimed to investigate the effect of non-synonymous SNPs (nsSNPs) of the Glucagon-like peptide-1 Receptor (GLP-1R) gene in protein function and structure using different computational software. Introduction: The GLP1R gene provides the necessary instruction for the synthesis of the insulin hormones which is needed for glucose catabolism. Polymorphisms in this gene are associated with diabetes. The protein is an important drug target for the treatment of type-2 diabetes and stroke. Material and Methods: Different nsSNPs and protein-related sequences were obtained from NCBI and ExPASY database. Gene associations and interactions were predicted using GeneMANIA software. Deleterious and damaging effects of nsSNPs were analyzed using SIFT, Provean, and Polyphen-2. The association of the nsSNPs with the disease was predicted using SNPs & GO software. Protein stability was investigated using I-Mutant and MUpro software. The structural and functional impact of point mutations was predicted using Project Hope software. Project Hope analyzes the mutations according to their size, charge, hydrophobicity, and conservancy. Results: The GLP1R gene was found to have an association with 20 other different genes. Among the most important ones is the GCG (glucagon) gene which is also a trans membrane protein. Overall 7229 variants were seen, and the missense variants or nsSNPs (146) were selected for further analysis. The total number of nsSNPs obtained in this study was 146. After being subjected to SIFT software (27 Deleterious and 119 Tolerated) were predicted. Analysis with Provean showed that (20 deleterious and 7 neutral). Analysis using Polyphen-2 revealed 17 probably damaging, 2 possibly damaging and 1 benign nsSNPs. Using two additional software SNPs & GO and PHD-SNPs showed that 14 and 17 nsSNPs had a disease effect, respectively. Acute variceal bleeding in patients with liver cirrhosis and portal hypertension(PHT) is the most serious emergency complication among those patients and could have catastrophic outcomes if not timely managed. Early screening by esophago-gastro-duodenoscopy(EGD) for the presence of esophageal varices(EVs) is currently recommended by the practice guidelines for all cirrhotic patients. Meanwhile, EGD is not readily accepted or preferred by many patients. The literature is rich in studies to investigate and validate non-invasive markers of EVs prediction aiming at reducing the unneeded endoscopic procedures. Gallbladder(GB) wall thickness(GBWT) measurement has been found promising in many published research articles. We aim to highlight the validity of sonographic GBWT measurement in the prediction of EVs based on the available evidence. We searched databases including Cochrane library, Pub Med, Web of Science and many others for relevant articles. GBWT is associated with the presence of EVs in cirrhotic patients with PHT of different etiologies. The cut-off of GBWT that can predict the presence of EVs varied in the literature and ranges from 3.1 mm to 4.35 mm with variable sensitivities of 46%-90.9% and lower cutoffs in viral cirrhosis compared to non-viral, however GBWT > 4 mm in many studies is associated with acceptable sensitivity up to 90%. Furthermore, a relation was also noticed with the degree of varices and portal hypertensive *** cirrhotics, GBWT > 3.5 mm predicts the presence of advanced(grade Ⅲ-Ⅳ) EVs with a sensitivity of 45%, the sensitivity increased to 92% when a cut-off ≥ 3.95 mm was used in another cohort. Analysis of these results should carefully be revised in the context of ascites, hypoalbuminemia and other intrinsic GB diseases among cirrhotic patients. The sensitivity for prediction of EVs improved upon combining GBWT measurement with other non-invasive predictors, e.g., platelets/GBWT.

Asthma is the most common chronic pulmonary disease in the world,affecting more than three hundred million people from different races and age *** asthma is considered one of the main causes of increased healthcare expenditures,particularly in developing *** is the most essential and commonly used lung function *** is used mainly for the evaluation of lung function to obtain reliable data used for the detection of lung diseases,such as asthma,as well as for monitoring lung *** two spirometry parameters,peak expiratory flow rate(PEFR)and forced expiratory volume in one second(FEV1),are effective in the diagnosis and management of patients with ***,FEV1 is preferred as it is a more accurate parameter compared to PEFR for the evaluation and recognition of *** children with asthma,the most commonly used lung function parameter for asthma monitoring is *** was discovered that a decrease in FEV1 is associated with an increase in asthma severity.

COVID-19 is a respiratory infection that has lately begun to affect other vital organs,including the heart,kidney,and *** purpose of this study was to investigate the hepatic complications in COVID-19 patients and the risk of being admitted to ICU or facing ***:Comorbidities(hypertension and diabetes),COVID-19 symptoms,laboratory findings(ALT level,AST level,and albumin),complications during hospitalization,treatment protocol used,and survival outcomes were all studied in 200 COVID-19 infected Egyptian patients who had virological symptoms and were followed up until they recovered or *** was found that older people and those with higher blood glucose levels have a higher risk of developing liver-associated COVID-19 ***,the majority of patients who developed liver complications in the course of the infection had high mortality *** with diabetes,hypertension,or hepatic disease are at higher risk of ICU admission or ***,it is important to pay attention to these problems in the diagnosis and treatment of COVID-19 to develop a suitable individualized treatment *** was also a correlation between the mortality in COVID-19 patients and both,high blood glucose and liver enzyme *** can be attributed to the correlation between diabetes and liver disease as every disease may be a complication to the other;moreover,COVID-19 may lead to increased blood sugar levels in addition to ALT and AST *** theory is that COVID-19 may afiect the liver and hence people with chronic liver disease.

An Observational Study on the Management of COVID-19 Patients in Limited-Resource Hospitals

Background:The purpose of this study was to evaluate the effectiveness of limited-resource hospitals in managing mild and moderate hospitalized cases of COVID-19 with comorbidities and in preventing their progression to severe ***:Data were obtained from 88 moderate COVID-19 patients with comorbidities who were admitted to limitedresource *** data were classified into several parts:comorbidities,chronic medication before hospital admission,symptoms of COVID-19 before and during hospitalization,clinical features,laboratory findings on hospital admission,complications during hospitalization,as well as worst laboratory values during hospitalization,hospital stay,and *** clinical features,laboratory results,type of oxygen therapy used,and the final treatment outcome were all evaluated to assess for any potential ***:All patients were alive upon *** admission,the majority of patients(60.2%)received COVID-19 treatment,and the average hospital stay was 12 *** most common symptoms were fever(88.7%),cough(95.5%),shortness of breath(90.9%),myalgia(84.1%),confusion(63.6%),headache(62.5%),sore throat(88.7%),rhinorrhea(17%),chest pain(58%),diarrhea(19.3%),nausea and vomiting(38.6%),anosmia(62.5%),as well as dysgeusia(64.8%).Based on chest radiograph or computed tomography(CT)scan,9.1%of the patients had unilateral pneumonia,90.9% had bilateral pneumonia,and 96.6% had multiple mottling and ground-glass *** was found associated with a significant increase in headache(p=0.005),rhinorrhea(p=0.013),chest pain(p=0.007),and the need for positive airway pressure(p=0.008).Between pre-and post-hospital admissions,there was a significant increase in lactate dehydrogenase and ferritin but a decrease in platelet,D-dimer,hemoglobin,lymphocytes,neutrophils,and total leucocyte count(p<0.001).There was a significant association between hospital stay and D-dimer level(p=0.05).Conclusion:Limited-resource hospitals in Egypt w
