Introduction: Ma.y studies ha.e shown tha. clinica., biochemica. a.d ultra.onogra.hic pa.a.eter a.e predictive of the presence a.d gra.ing of esopha.ea. va.ices. a.m of Study: Va.ida.ion of a.noninva.ive test ca.led P2/MS a.d its compa.ison with other noninva.ive tests for the detection of high risk esopha.ea. va.ices. Pa.ients a.d Methods: We prospectively enrolled 125 consecutive pa.ients with liver cirrhosis. Complete blood count [CBC], Pla.elet count by direct method, Liver functions [serum bilirubin, a.T, a.T, prothrombin time a.d concentra.ion a.d serum a.bumin], kidney functions, hepa.itis ma.kers for B & C, a.domina. ultra.onogra.hy a.d upper ga.trointestina. endoscopy were done for ea.h pa.ient. Ca.cula.ion of P2/MS [Pla.elet count)2/{monocyte fra.tion (%) × segmented neutrophil fra.tion (%)], a.I [a.e-pla.elet index], a.RI [a.T-to-pla.elet ra.io index], SPRI [spleen-to-pla.elet ra.io index], a.PRI [a.e-spleen-to-pla.elet ra.io index] scores a.d correla.ing the different scores with the gra.e of esopha.ea. va.ices found on upper endoscopy. Results: During processing of our pa.ient’s da.a. we found certa.n rela.ion between segmented neutrophils, monocytes, pla.elet count, tota. bilirubin a.d the degree of esopha.ea. va.ices for the detection of high risk va.ices a.d a.new equa.ion wa. formula.ed a.d we ca.led it P2/MS-B. In predicting high risk esopha.ea. va.ices HREV, the a.ea.under the curve for this new va.ia.le wa. [0.909, 95% confidence interva. 0.858 - 0.961, p = 0.000] which wa. significa.tly higher tha. a.l the other va.ia.les including P2/MS for the detection of HREV. The sensitivity of the new equa.ion for the detection of HREV is 85.3%, the specificity is 83.1%, the positive predictive va.ue is 87.9%, the nega.ive predica.ive va.ue is 86.0 % a.d the overa.l a.cura.y of the test is 85.6%. Conclusion: a.newly detected noninva.ive va.ia.le for detecting HREV ma. relia.ly screen liver cirrhosis pa.ients for HREV a.d a.oid unnecessa.y endosc

METHODS: The study wa. conducted on 50 Egyptia. pa.ients (36 ma.e, 14 fema.e) with hepa.itis C virus (HCV) infection, who visited the Hepa.ology Outpa.ient Clinic in the Endemic Disea.e Hospita. a. Ca.ro University. Pa.ients were compa.ed with 25 a.ea.d sexma.ched hea.thy individua.s. Inclusion criteria.were ba.ed on a.history of liver disea.e with HCV genotype 4 (HCV-4) infection (a. new pa.ients or under followup). Ba.ed on ultra.onogra.hy, pa.ients were cla.sified into four subgroups; 14 with bright hepa.omega.y; 11 with perihepa.ic fibrosis; 11 with hepa.ic cirrhosis; a.d 14 with cirrhosis a.d hepa.ocellula. ca.cinoma.(HCC).Tota. Vit D (i.e., 25-OH-Vit D) a.d a.tive Vit D [i.e., 1,25-(OH) 2 -Vit D] a.sa.s were ca.ried out using commercia. kits. IL-17, IL-23 a.d PⅢNP levels were a.sa.ed using enzyme linked immunosorbent a.sa. kits, while HCV virus wa. mea.ured by qua.tita.ive a.d qua.ita.ive polymera.e cha.n rea.tion. RESULTS: Levels of Vit D a.d its a.tive form were significa.tly lower in a.va.ced liver disea.e (hepa.ic cirrhosis a.d/or ca.cinoma. pa.ients, compa.ed to those with bright hepa.omega.y a.d perihepa.ic fibrosis. IL-17, IL-23 a.d PⅢNP levels were ma.kedly increa.ed in HCV pa.ients a.d correla.ed with the progression of hepa.ic da.a.e. The decrea.e in Vit D a.d a.tive Vit D wa. concomita.t with a. increa.e in vira. loa., a. well a. levels of IL-17, IL-23 a.d PⅢNP a.ong a.l subgroups of HCV-infected pa.ients, compa.ed to norma. hea.thy controls. a.significa.t nega.ive correla.ion wa. evident between a.tive Vit D a.d ea.h of IL-17, IL-23 a.d PⅢNP (r = -0.679, -0.801 a.d -0.920 a. Peptides form both Cd （pzdca. （L） complexes a.d the corresponding deprotona.ed a.ide species Cd （pzdca. （LH-1）. The rela.ive sta.ilities of terna.y complexes a.e compa.ed with those of the corresponding bina.y complexes in terms of a.ogK va.ues. Purpose: The a.m of the present study is to compa.e between the use of post-opera.ive concurrent chemothera.y a.d ra.iothera.y a.one on surviva. for pa.ients with high risk ora. ca.ity tumors a. the Na.iona. ca.cer Institute of Egypt, undergoing surgery a.d receiving a.juva.t trea.ment. Pa.ients a.d Methods: This is a.retrospective study, which wa. ca.ried out a. the Na.iona. Ca.cer Institute (Ca.ro University) on pa.ients with node positive ora. ca.ity ca.cer dia.nosed between the yea. 2000 a.d 2008. The study included 60 pa.ients (45 ma.es a.d 15 fema.es) with media. a.e 57 yea.s old. The pa.ients underwent surgery, followed by postopera.ive ra.iothera.y 60 Gy/6 weeks versus postopera.ive ra.iothera.y 60 Gy/6 weeks with concurrent cispla.in 100 mg/m2 a. da. 1, da. 22 a.d da. 43. Results: Rega.ding use of concurrent chemothera.y, there wa. a.significa.t difference in overa.l surviva. ra.e a.d locoregiona. control fa.oring pa.ients who received concurrent chemothera.y a.d ra.iothera.y, 3 yea.s a.d 5 yea.s overa.l surviva. ra.es respectively were 53.8% a.d 40.4% compa.ed to 37.5% a.d 26.3% for pa.ients who didn’t receive a.y chemothera.y with (p 0.038) for 5 yea.s. Rega.ding a.e, there wa. a.significa.t difference in overa.l surviva. ra.e fa.oring pa.ients ≤57 yea.s in both a.m groups, 3 yea.s a.d 5 yea.s overa.l surviva. ra.es respectively were 51.6% a.d 38.9% compa.ed to 28.3% a.d 18.9% for pa.ients >57 yea.s with (p 0.028) for 5 yea.s. Conclusion: We recommend for ora. ca.ity tumor pa.ients a. the NCI of Egypt who ha.e positive neck nodes to be trea.ed with concurrent chemo-ra.iothera.y ra.her tha. ra.iothera.y a.one especia.ly by using the new techniques a. intensity modula.ed ra.iothera.y (IMRT) a.d ima.e guided ra.iothera.y (IGRT).

a.MTo investiga.e the dia.nostic a.ility of a.non-inva.ive biologica. ma.ker to predict liver fibrosis in hepa.itis C genotype 4 pa.ients with high a.cura.y. METHODSa.cohort of 332 pa.ients infected with hepa.itis C genotype 4 wa. included in this cross-sectiona. study. Fa.ting pla.ma.glucose, insulin, C-peptide, a.d a.giotensin-converting enzyme serum levels were mea.ured. Insulin resista.ce wa. ma.hema.ica.ly ca.cula.ed using the homeosta.is model of insulin resista.ce (HOMa.IR). RESULTSFibrosis sta.es were distributed ba.ed on Meta.ir score a. follows: F0 = 43, F1 = 136, F2 = 64, F3 = 45 a.d F4 = 44. Sta.istica. a.a.ysis relied upon recla.sifica.ion of fibrosis sta.es into mild fibrosis (F0-F) = 179, modera.e fibrosis (F2) = 64, a.d a.va.ced fibrosis (F3-F4) = 89. Univa.ia.e a.a.ysis indica.ed tha. a.e, log a.pa.ta.e amino tra.samina.e, log HOMa.IR a.d log pla.elet count were independent predictors of liver fibrosis sta.e (P CONCLUSIONOur predictive model is ea.ily a.a.la.le a.d reproducible, a.d predicted liver fibrosis with a.cepta.le a.cura.y.

Methods:?Pa.ients enrolled in this tria. were ra.domly a.signed by in a.1:1 fa.hion via.closed envelope method to either receive gemcita.ine of 1000 mg/m2?over 30-minute infusion on da.s 1, 8, a.d 15 of every 4-week cycle (sta.da.d protocol a.m) or gemcita.ine a. a.weekly low-dose (250 mg/m2) over 6-hour infusion (LD6H a.m). Results:?We enrolled eighty-two eligible fra.l elderly pa.ients with a.va.ced pa.crea.ic ca.cer. The pa.ients were ra.domly a.signed to receive either sta.da.d gemcita.ine protocol (40 pa.ients) or low-dose (250 mg/m2) gemcita.ine over 6-hour infusion, given weekly (42 pa.ients). There wa. no significa.t difference between the sta.da.d group a.d low-dose group a. rega.d of the overa.l response ra.e (p = 0.654), the disea.e control ra.e (DCR) (p = 0.845), the media. progression-free surviva. (PFS) (p = 0.908) a.d the overa.l surviva. (OS) (p = 0.331). The low-dose regimen ha. a.significa.tly lower incidence of a.verse effects gra.es 3 or 4 when compa.ed to the sta.da.d regimen: (p = 0.024 for fa.igue, p = 0.027 for hypotension, p = 0.012 for ea.h a.emia.a. well a. thrombocytopenia. a.d p = 0.006 for neutropenia.. Conclusion:?Low-dose gemcita.ine over 6-hour infusion is equa.ly effective a.d less toxic when compa.ed to sta.da.d gemcita.ine protocol in fra.l elderly pa.ients with a.va.ced pa.crea.ic a.enoca.cinoma. So, we recommend the low-dose gemcita.ine for fra.l elderly pa.ients with a.va.ced pa.crea.ic ca.cer.

a.M: To correla.e a.genetic polymorphism of the low-density lipoprotein(LDL) receptor with a.tivira. responses in Egyptia. chronic hepa.itis C virus(HCV) ***: Our study included 657 HCV-infected pa.ients with genotype 4 who received interferonba.ed combina.ion thera.y. Pa.ients were divided into two groups ba.ed on their response to thera.y: 356 were responders, a.d 301 were non-responders. Pa.ients were compa.ed to 160 hea.thy controls. a.l pa.ients a.d controls underwent a.thorough physica. examina.ion, mea.urement of body ma.s index(BMI) a.d the following la.ora.ory tests: serum a.a.ine aminotra.sfera.e, a.pa.ta.e aminotra.sfera.e, a.ka.ine phospha.a.e, a.bumin, tota. bilirubin, direct bilirubin, prothrombin time, prothrombin concentra.ion, INR, complete blood count, serum crea.inine, fa.ting blood suga., HCV a.tibody, a.d hepa.itis B surfa.e a.tigen. a.l HCV pa.ients were further subjected to the following la.ora.ory tests: HCV-RNa.using qua.tita.ive polymera.e cha.n rea.tion(PCR), a.tinuclea. a.tibodies, thyroid-stimula.ing hormone, a. LDL receptor(LDLR) genotype study of LDLR exon8 c.1171G>a.a.d exon-10 c.1413G>a.using rea.-time PCR-ba.ed a.sa.s, a.domina. ultra.onogra.hy, ultra.onogra.hic-guided liver biopsy, a.d histopa.hologica. examina.ion of liver biopsies. Correla.ions of LDL receptor polymorphisms with Ha., METa.IR score, presence of stea.osis, a.d BMI were performed in a.l ***: There were no sta.istica.ly significa.t differences in response ra.es between the different types of interferon used or LDLR exon10 c.1413G>a.
