Evaluation of Plasma Urokinase-Type Plasminogen Activator Receptor and Interleukin 34 in Patients with Chronic Hepatitis C as Serological Fibrosis markers

Background: Hepatitis C Virus (HCV) infection is a progressive disease that may result in chronic hepatitis, fibrosis and cirrhosis. Assessment of liver fibrosis is an essential factor in the management of chronic HCV. Objective: To evaluate plasma soluble Urokinase Plasminogen Activator Receptor (sUPAR) and interleukin-34 (IL-34) as serological markers of liver fibrosis in patients with chronic HCV. methods: This case-control study enrolled 60 chronic HCV patients who were subdivided into three groups of mild, moderate and severe hepatic fibrosis depending on Fibrosis-4 score (FIB-4). Patients were compared with 20 age and sex-matched controls. Plasma sUPAR and IL-34 levels were measured by Enzyme Linked Immunosorbent Assay (ELISA). Results: Plasma sUPAR and IL-34 were significantly increased in HCV patients when compared with controls, and their increase was positively correlated with the progression of hepatic fibrosis. Plasma sUPAR and IL-34 positively correlated with Aspartate Aminotransferase (AST) and Alanine Aminotransferase (ALT), and negatively correlated with hemoglobin concentration and platelet count. The output data of Receiver Operating Characteristic (ROC) curve to differentiate patients from controls revealed that sUPAR at cut-off > 186.2 ng/L and Area Under Curve (AUC) of 0.944 had (85%) sensitivity and (100%) specificity, and IL-34 at cut off > 16.4 ng/L and AUC of 0.942 had (75%) sensitivity and (100%) specificity. The output data of ROC curve to differentiate severe from mild to moderate hepatic fibrosis patients revealed that sUPAR at cut-off > 510 ng/L and AUC of 0.837 had (80%) sensitivity and (90%) specificity. While IL-34 at cut off > 55.3 ng/L and AUC of 0.844 had (85%) sensitivity and (80%) specificity. The effect of cigarette smoking on lipid profile and CBC (Complete Blood Count) of blood was investigated in this study. Spectrophotometric methods were used to estimate the amounts of nicotine, cholesterol, triglycerides, and HDL-cholesterol. The findings revealed that cigarette smokers and the duration of smoking had significantly higher levels of nicotine and cholesterol when compared to non-smokers. The data also revealed differences in the contents of hematological parameters between smokers’ and non-smokers’ blood samples, with high counts of WBC, mCHC, HGB, and PLT clearly visible in the smokers’ samples. However, there was no discernible effect on RBC, mCV, or mCH counts in the trial. Smoker samples had high PLT values (243.8 ± 84.26), while non-smoker samples had lower PLT values (229.3 ± 30.07 on average).

Synthesis of bicyclic systems containing chlorine atoms, and/or ether groups in aromatic rings can be con- sidered as an important method for building bicyclic system and production of new adducts. One of the most important types in the cycloaddition reaction is the Diels-Alder reaction (1,4 cycloaddition). In the present investigation a new ether of allylic type (dienophile) p-allyl bromo phenol was prepared and its structure was confirmed by molecular weight determination, refractive index, infrared spectra, and density. A new adduct was obtained by means of 1,4 cycloaddition reaction of hexachlorocyclopentadiene (HCP) and the new pre- pared dienophile. The reaction takes place without using solvent, catalysts, or elimination of any compound. The effect of variations in temperature, initial molar ratio and reaction duration were studied to determine the optimum conditions of the reaction. The optimum conditions reached were reaction temperature recorded 140?C, initial molar ratio diene: dienophile was 3:1 and the reaction duration time reached 6 h. Under these optimum conditions the maximum yield was 78%. The new adduct revealed very high biological effect as sulfate-reducing bacteria (SRB).

Soil salinity is one of the major yield-limiting factors for crop production in many agricultural regions all over the world. Besides following efficient management practices at the field scale to reduce accumulation of salts in the effective root-zone, the effective use of treatments to alleviate the effects of salinity stress and improve crop salt tolerance is a promising solution to ensure crop production in such adverse conditions. A field experiment was carried out to investigate the effect of foliar spray with plant-based biostimulant (i.e. with and/or without 3% yeast extract), three levels of proline (0, 25, and 50 mM), and combined with potassium fertilizers, as potassium sulfate, 48% K2O (0, 50, and 100 kg/fed.) on growth promotion, chemical composition of garlic leaves, bulb quality parameters as well as yield and its components of garlic plant grown under moderate saline soil. Results revealed that the interaction between foliar spray with yeast extract at 3% and proline at 50 mM combined with proper K level at 100 kg/fed., was the best interaction treatment for increasing vegetative growth parameters, i.e. plant height, number of leaves per plant, and mineral contents (N, P, K, S, Ca and Mg in leaves), and proline content of garlic leaves after 135 days from planting time, total yield/fed., and garlic yield quality parameters at harvesting time. In conclusion, the detrimental effects of salinity stress can be alleviated by stress tolerance-inducing compounds, such as yeast extract and proline with proper application rate of K fertilization during the growing season of garlic crop.

This paper studies the microfacies, fossil contents, and the depositional environment of Wadi As Sir Limestone Formation exposed in the Al-tayyar area Zarqa Governorate, Northeastern Jordan. A total number of 35 samples were collected from a 30 m thick quarry section and used to prepare 35 thin sections. Some samples were washed over a 63 μm sieve, oven-dried at 50°C, sieved, and picked for benthic foraminifera analysis. microscope analysis used to describe the microfacies and fossil contents. Four microfacies types and four lithological units are distinguished and described from the bottom to the top;the chalky unit (Unit-1) composed of bioclastic wackestone and biomicrite microfacies, and the dolomitic unit (Unit-2) immediately is existed above unit 1 composed dominantly of bioclastic mudstone and biomicrite microfacies. marly limestone (Unit-3) is the following upwardly unit composed of bioclastic packestone and biosparite microfacies, and the uppermost unit is limestone (Unit-4) consisted of bioclastic grainstone and biosparite microfacies. The fossil contents that were recognized in the studied thin sections and samples;bivalves, gastropods, pelecypods, cephalopods echinoderms, radiolarian, stromatoporoids, bone fragments, Saccaminopsis sp., Cibicidoides sp., Cibicides sp., Cyclammina sp., calcareous algae (Koninckopora and gymnocodiaceans), worm tubes, serpulids, and plentiful ostracods. The current study indicates that the Wadi As Sir Limestone Formation has deposited in a restricted circulation shallow shelf with low energy conditions most probably lagoonal environment.

目的:研究表皮通透屏障功能破坏后表皮CD44的表达。方法:外用胶带破坏裸鼠躯干部表皮通透屏障功能,分别于30min、1h、3h、6h、24h和48h后取皮肤标本,用免疫组化的方法观察CD44在表皮的表达。结果:在表皮通透屏障功能破坏后30min,表皮CD44的表达明显上调;1h和3h后,恢复至正常水平;6h后,表皮CD44的表达又明显增强,并持续到48h;人为地纠正屏障功能(封包)对表皮CD44的表达没有明显影响。结论:表皮通透屏障功能破坏后,表皮CD44的表达增强,其表达的改变与屏障功能本身无显著相关性。

Acute variceal bleeding in patients with liver cirrhosis and portal hypertension(PHT) is the most serious emergency complication among those patients and could have catastrophic outcomes if not timely managed. Early screening by esophago-gastro-duodenoscopy(EGD) for the presence of esophageal varices(EVs) is currently recommended by the practice guidelines for all cirrhotic patients. meanwhile, EGD is not readily accepted or preferred by many patients. The literature is rich in studies to investigate and validate non-invasive markers of EVs prediction aiming at reducing the unneeded endoscopic procedures. Gallbladder(GB) wall thickness(GBWT) measurement has been found promising in many published research articles. We aim to highlight the validity of sonographic GBWT measurement in the prediction of EVs based on the available evidence. We searched databases including Cochrane library, Pub med, Web of Science and many others for relevant articles. GBWT is associated with the presence of EVs in cirrhotic patients with PHT of different etiologies. The cut-off of GBWT that can predict the presence of EVs varied in the literature and ranges from 3.1 mm to 4.35 mm with variable sensitivities of 46%-90.9% and lower cutoffs in viral cirrhosis compared to non-viral, however GBWT > 4 mm in many studies is associated with acceptable sensitivity up to 90%. Furthermore, a relation was also noticed with the degree of varices and portal hypertensive *** cirrhotics, GBWT > 3.5 mm predicts the presence of advanced(grade Ⅲ-Ⅳ) EVs with a sensitivity of 45%, the sensitivity increased to 92% when a cut-off ≥ 3.95 mm was used in another cohort. Analysis of these results should carefully be revised in the context of ascites, hypoalbuminemia and other intrinsic GB diseases among cirrhotic patients. The sensitivity for prediction of EVs improved upon combining GBWT measurement with other non-invasive predictors, e.g., platelets/GBWT.

The present work introduces a mathematical model for ionic fluid that flows under the effect of both pulsating pressure and axial electromagnetic field. The fluid is treated as a Newtonian fluid applying Navier-Stokes equation. The fluid is considered as a neutral mixture of positive and negative ions. The effect of axial electric field is investigated to determine velocity profiles. Hydroelectric equation of the flow is deduced under dc and ac external electric field. Hence the effect of applied frequency （0-1 GHz） and amplitude （10-350 V/m） is illustrated. The ultimate goal is to approach the problem of EmF field interaction with blood flow. The applied pressure waveform is represented as such to simulate the systolic-diastolic behavior. Simulation was carried out using maple software using blood plasma parameters; hence velocity profiles under various conditions are reported.

Urinary Vascular Endothelial Growth Factor (VEGF) as a Novel marker for Early Prediction of Esophageal Varices in Patients with Chronic Liver Disease

Background/Aims: Vascular Endothelial Growth Factor (VEGF) has a crucial role in portal hypertension and collateral vessels formation. This study aims to assess urinary VEGF in cirrhotic patients as a predictor of presence of esophageal varices, and variceal bleeding. Settings and Design: 42 cirrhotic patients were randomly selected and classified into 2 groups according to the presence of variceal bleeding. methods and material: Urinary VEGF was measured and corrected against urinary creatinine. Platelet count, liver functions, abdominal ultrasonography and upper endoscopy were done. Statistical Analysis Used: Comparison was done by mann Whitney and Kruskal Wallis tests. Correlation was done using Spearman rank correlation. multivariable logistic regression was done to identify predictors of variceal bleeding and presence of large varices. Receiver operator characteristic curve (ROC) analysis was used to determine the optimum cut off value of predictors. Results and Conclusions: Urinary VEGF was lower in cirrhotic patients with esophageal varices than those without. Low VEGF, low platelet count and splenomegaly were found to be independent predictors of both the presence of large esophageal varices, and variceal bleeding. Cut-off values for platelet count ≤ 166.3 × 103/μL, and corrected VEGF ≤ 59.12 pg/mg were predictive of large esophageal varices with 93.1%, 86.2% sensitivity and 74.5%, 58.2% specificity respectively. While variceal bleeding could be predicted at a platelet count ≤ 153 × 103/μL, and corrected VEGF ≤ 45.08 pg/mg with 90.9%, 81.8% sensitivity and 72.6%, 59.7% specificity respectively. The study concludes that urinary VEGF can be used as an alternative to upper endoscopic screening.
