Patient Feedback for Acupuncture Practice Improvement:A Survey from Mayo Clinic

Objective： To understand patient satisfaction with acupuncture appointment duration for the first and subsequent appointments in the plan of care, time spent with the acupuncture provider, willingness to pay for longer duration appointments, and lack of insurance coverage. Methods： Patients who were referred for an acupuncture consult to the Center for Integrative Medicine at Mayo Clinic were asked to participate in the survey in August 2014. Patients were given paper survey at time of appointment check-in and entered into Research Electronic Data Capture Survey database tool to reveal patient-reported outcomes. Results： In total, 104 of 329 patients receiving acupuncture treatment responded to the survey with a 32% response rate. Insight and recommended changes to the duration of the initial and subsequent appointments were obtained. Most patients reported that time spent with the provider（76 cases, 74%） and appointment length（74 cases, 68%） were ＂just right＂. Seventy cases（70%） of respondents indicated that they wanted longer treatment. Patients reported return appointments with duration of 30 min was not enough. Fifty-three patients（62%） were willing to pay for additional costs not covered by insurance. Conclusions： We used patient feedback to assess the acupuncture practice in a complementary and integrative medicine program. Most patients referred for the acupuncture consult appeared to be satisfied with the current level of practice. These findings were used to establish a quality improvement plan that may be implemented to improve patient satisfaction with the acupuncture practice.

Background:The milk fat globule membrane(MFGM)proteomes of colostrum and transition milk are rich sources of proteins that are likely important for neonatal calf *** addition,characterization of these proteomes could also yield valuable information regarding mammary gland physiology of the early postpartum lactating *** objectives of this research were to characterize the MFGM proteomes of colostrum and transition milk through sample collections at four timepoints postpartum,including the first milking(M1,colostrum),second milking(M2,transition milk),fourth milking(M4,transition milk),and fourteenth milking(M14,mature milk),and compare these proteomes between multiparous(MP;n=10)and primiparous(PP;n=10)Holstein dairy *** MFGM proteins were labeled using Tandem Mass tagging and analyzed using liquid chromatography-tandem mass spectrometry(LC-MS/MS).Protein identification was completed using MASCOT and Sequest in Proteome Discoverer *** scaled abundance values were analyzed using PROC MIXED in SAS to determine the effects of milking(MIL),parity(PAR),and MIL×*** adaptive false-discovery rate(FDR)-adjusted P values were determined using PROC *** characterization and bioinformatic analysis were completed using a combination of PANTHER,Blast,and ***:A total of 104 common proteins were identified in each of the MFGM *** analysis revealed that 70.2%of identified proteins were affected by *** these,78.1%were lower in M14 compared with M1,including immune-related proteins lactotransferrin,lactadherin and *** affected 44.2%of *** the proteins affected by PAR,84.8%were higher in MP cows compared with PP cows,including apolipoprotein E and histones 2A,2B,3,and 4 *** subfamily 1 member 1A and annexin 5 were higher in samples from PP ***×parity affected 32.7%of identified proteins,including lactotransferrin,gelsolin,vitamin D binding protein,and S100 ***

Differences in neuroplasticity after spinal cord injury in varying animal models and humans

Rats have been the primary model to study the process and underlying mechanisms of recovery after spinal cord injury. Two weeks after a severe spinal cord contusion, rats can regain weight-bearing abilities without therapeutic interventions, as assessed by the Basso, Beattie and Bresnahan locomotor scale. However, many human patients suffer from permanent loss of motor function following spinal cord injury. While rats are the most understood animal model, major differences in sensorimotor pathways between quadrupeds and bipeds need to be considered. Understanding the major differences between the sensorimotor pathways of rats, non-human primates, and humans is a start to improving targets for treatments of human spinal cord injury. This review will discuss the neuroplasticity of the brain and spinal cord after spinal cord injury in rats, non-human primates, and humans. A brief overview of emerging interventions to induce plasticity in humans with spinal cord injury will also be discussed.

Through ingestion and subsequent egestion,Arctic seabirds can bioaccumulate microplastics at and around their colony breeding *** microplastics in Arctic seabirds have been well documented,it is not yet understood to what extent these particles can act as transport vehicles for plastic-associated contaminants,including legacy persistent organic pollutants(POPs),trace metals,and organic *** investigated the occurrence and pattern of organic and inorganic co-contaminants of microplastics in two seabird species from the Canadian Arctic-northern fulmar(Fulmarus glacialis)and black-legged kittiwake(Rissa tridactyla).We found that fulmars had higher levels of plastic contamination and emerging organic compounds(known to be plastic additives)than kittiwakes,whereas higher concentrations of legacy POPs were found in kittiwakes than the ***,fulmars,the species with the much larger foraging range(~200 km),had higher plastic pollution and overall contaminant burdens,indicating that birds may be acting as long-range transport vectors for plasticassociated *** results suggest a potential connection between plastic additive contamination and plastic pollution burdens in the bird stomachs,highlighting the importance of treating plastic particles and plastic-associated organic additives as co-contaminants rather than separate pollution issues.

Advances in alcoholic liver disease:An update on alcoholic hepatitis

Alcoholic hepatitis is a pro-inflammatory chronic liver disease that is associated with high short-term morbidity and mortality(25%-35% in one month) in the setting of chronic alcohol use. Histopathology is notable for micro- and macrovesicular steatosis, acute inflammation with neutrophil infiltration, hepatocellular necrosis, perivenular and perisinusoidal fibrosis, and mallory hyaline bodies found in ballooned hepatocytes. Other findings include the characteristic eosinophilic fibrillar material(mallory's hyaline bodies) found in ballooned hepatocytes. The presence of focal intense lobular infiltration of neutrophils is what typically distinguishes alcoholic hepatitis from other forms of hepatitis, in which the inflammatory infiltrate is primarily composed of mononuclear cells. Management consists of a multidisciplinary approach including alcohol cessation, fluid and electrolyte correction, treatment of alcohol withdrawal, and pharmacological therapy based on the severity of the disease. Pharmacological treatment for severe alcoholic hepatitis, as defined by Maddrey's discriminant factor ≥ 32, consists of either prednisolone or pentoxifylline for a period of four weeks. The body of evidence for corticosteroids has been greater than pentoxifylline, although there are higher risks of complications. Recently head-to-head trials between corticosteroids and pentoxifylline have been performed, which again suggests that corticosteroids should strongly be considered over pentoxifylline.
