BAcKGROUND: Transc.a.ial Doppler ultrasonography that is aimed at residual obstructive intrac.a.ial blood flow may help expose thrombi to tissue plasminogen activator (t- PA). Our objective was to determine whether ultrasonography can safely enhance the thrombolytic.a.tivity of t- PA. METHODS: We treated all patients who had acute ischemic stroke due to occlusion of the middle cerebral artery with intravenous t- PA within three hours after the onset of symptoms. The patients were randomly assigned to receive continuous 2- MHz transc.a.ial Doppler ultrasonography (the target group) or placebo (the control group). The primary combined end point was complete recanalization as assessed by transc.a.ial Doppler ultrasonography or dramatic clinical recovery. Secondary end points included recovery at 24 hours, a favorable outcome at three months, and death at three months. RESULTS: A total of 126 patients were randomly assigned to receive continuous ultrasonography (63 patients) or placebo (63 patients). Symptomatic intracerebral hemorrhage occurred in three patients in the target group and in three in the control group. complete recanalization or dramatic clinical recovery within two hours after the administration of a t- PA bolus occurred in 31 patients in the target group (49 percent), as compared with 19 patients in the control group (30 percent; P=0.03). Twenty- four hours after treatment of the patients eligible for follow- up, 24 in the target group (44 percent) and 21 in the control group (40 percent) had dramatic clinical recovery (P=0.7). At three months, 22 of 53 patients in the target group who were eligible for follow- up analysis (42 percent) and 14 of 49 in the control group (29 percent) had favorable outcomes (as indicated by a score of 0 to 1 on the modified Rankin scale) (P=0.20). cONcLUSIONS: In patients with acute ischemic stroke, continuous transc.a.ial Doppler augments t- PA- induced arterial recanalization, with a nonsignificant trend toward an increa

Background and Purpose - We evaluated the impact of admission hyperglycemia (HG) on stroke outcome in relation to the timing of reperfusion in patients treated with tissue plasminogen activator (tPA). Methods - We studied 138 consecutive stroke patients with a documented middle cerebral artery (McA) occlusion treated with intravenous tPA cose was determined at baseline before tPA administration. HG was defined as a glucose level >140 mg/dL. National Institutes of Health Stroke Scale (NIHSS) scores were obtained at baseline and 24 hour. Transc.a.ial Doppler monitoring of recanalization was conducted during the first 12 hour of stroke onset. mRS was used to assess outcome at 3 months. Results - Median baseline NIHSS score was 17 points. At baseline, 42 (37.3% ) patients were hyperglycemic.a.d 96 (62.7% ) *** was achieved cA remained occluded at 12 hours. A logistic regression model revealed that baseline NIHSS score >16 points (odds ratio [ORAbstract:, 3.32; 95% cI, 2.18 to 24.7; P=0.032) and admission glucose level >140 mg/dL (OR, 5.65; 95% cI, 1.97 to 16.18; P=0.002) independently predicted poor outcome (modified Rankin scale,3 to 6) at 3 months. After adjusting by age, stroke severity, site of McA occlusion, and degree of recanalization, the contribution of HG for poor outcome was higher as shorter the time to reperfusion. The highest odds for poor outcome related to HG corresponded to patients who recanalized cI, 1.8 to 14.3; P=0.002), as compared with those who recanalized between 3 and 6 hours (OR, 2.1; 95% cI, 1.1 to 16; P=0.034) and between 6 to 12 hours (OR, 1.1; 95% cI, 0.7 to 21; P=0.43). Wind turbine blades have been constantly increasing since wind energy becomes a popular renewable energy source to generate electricity. Therefore, the wind sector requires a more efficient and representative c.a.acterization of vertical wind speed profiles to assess the potential for a wind power plant site. This paper proposes an alternative c.a.acterization of vertical wind speed profiles based on Ward's agglomerative clustering algorithm, including both wind speed module and direction data. This approach gives a more accurate incoming wind speed variation around the rotor swept area, and subsequently, provides a more realistic.a.d complete wind speed vector c.a.acterization for vertical profiles. Real wind database collected for 2018 in the Forschungsplattformen in Nordund Ostsee(FINO) research platform is used to assess the methodology. A preliminary pre-processing stage is proposed to select the appropriated number of heights and remove missing or incomplete data. Finally, two locations and four heights are selected, and 561588 wind data are c.a.acterized. Results and discussion are also included in this paper. The methodology can be applied to other wind database and locations to c.a.acterize vertical wind speed profiles and identify the most likely wind data vector patterns.

We construct the maximal graded left quotient algebra of every graded algebra A without homogeneous total right zero divisors as the direct limit of graded homomorphisms （of left A-modules） from graded dense left ideals of A into a graded left quotient algebra of A. In the case of a superalgebra, and with some extra hypothesis, we prove that the component in the neutral element of the group of the maximal graded left quotient algebra coincides with the maximal left quotient algebra of the component in the neutral element of the group of the superalgebra.

On the chiral covariant approach to ρρ scattering

We examine in detail a recent work（D. Gülmez, U. G. Mei？ner and J. A. Oller, Eur. Phys. J. c,77： 460（2017））, where improvements to make ρρ scattering relativistically covariant are made. The paper has the remarkable conclusion that the J =2 state disappears with a potential which is much more attractive than for J =0,where a bound state is found. We trace this abnormal conclusion to the fact that an ＂on-shell＂ factorization of the potential is done in a region where this potential is singular and develops a large discontinuous and unphysical imaginary part. A method is developed, evaluating the loops with full ρ propagators, and we show that they do not develop singularities and do not have an imaginary part below threshold. With this result for the loops we define an effective potential, which when used with the Bethe-Salpeter equation provides a state with J =2 around the energy of the f2（1270）. In addition, the coupling of the state to ρρ is evaluated and we find that this coupling and the T matrix around the energy of the bound state are remarkably similar to those obtained with a drastic.a.proximation used previously, in which the q2 terms of the propagators of the exc.a.ged ρ mesons are dropped, once the cut-off in the ρρ loop function is tuned to reproduce the bound state at the same energy.

In this study,we investigate the two step sequential one pion production mec.a.ism,that is,np(I=0)→π^(-)pp followed by the fusion reaction pp→π^(+)d,to describe the np→π^(+)π^(-)d reaction withπ^(+)π^(-)in state I=*** this reaction,a narrow peak identified with a"d(2380)"dibaryon has been previously *** discover that the second reaction step pp→π^(+)d is driven by a triangle singularity that determines the position of the peak of the reaction and the high strength of the cross *** combined cross section of these two mec.a.isms produces a narrow peak with a position,width,and strength,that are compatible with experimental observations within the applied approximations *** novel interpretation of the peak accomplished without invoking a dibaryon explains why this peak has remained undetected in other reactions.
