Long term outcomes of Cohen’s cross trigonal reimplantation for primary vesicoureteral reflux in poorly functioning kidney

BACKGROUND Open ureteric reimplantation by cross trigonal technique described by Cohen is considered a common surgical option for correction of vesicoureteral reflux(VUR). There is a lack of evidence in literature though for what happens to such kidneys, in the long run, particularly those which are poorly *** To assess the long-term outcomes of ureteric reimplantation in poorly functioning kidneys in children with unilateral primary *** Children with unilateral primary VUR and a relative renal function of less than 35% who underwent open or laparoscopic ureteric reimplantation between January 2005 and January 2017 were included in the study. Patients who had a follow up of less than five years were excluded. Preoperative evaluation consisted of a voiding cystourethrogram and Dimercaptosuccinic acid(DMSA) scan. In the follow-up period, patients underwent a diuretic scan at 6 weeks and 6 *** up ultrasound was done for change in grade of hydronephrosis and retrovesical ureteric diameter. Subsequent follow up was done at 6 monthly intervals with evaluation for proteinuria and hypertension and any recurrent urinary tract infection(UTI). For assessment of cortical function, DMSA was repeated annually for 5 years after surgery. A paired-samples t-test was used to test the mean difference of DMSA between pre-post *** During this period, 36 children underwent ureteric reimplantation for unilateral primary VUR. After excluding those with insufficient follow-up, 31 were included in the analysis. Most of the patients were males(n = 26/31, 83.8%). Patient’s age(mean ± SD, range) was 5.21 ± 3.71, 1-18 years. The grades of VUR were grade Ⅱ(1patient), grade Ⅲ(8 patients), grade Ⅳ(10 patients), and grade Ⅴ(12 patients). The pre and postoperative DMSA was 24.064 ± 12.02 and 24.06 ± 10.93, which was almost the same(statistically equal, paired-samples t-test: P = 0.873). Palaeodrainages of the Sunda Shelf detailed in new maps

The Sunda Shelf is a prime biodiversity hotspot where some of the planet's most endemic species can be found. Much of the diversity in this important bioregion has been shaped by sea-level fluctuations that took place during the Pleistocene. Using depth contours obtained from the General Bathymetric Chart of the Oceans Grid 2023, we provide high-resolution reconstruction of land areas and palaeo-drainages on the Sunda Shelf at the 50, 75, 100, and 120 m isobaths, as well as palaeo-catchments at the 120 m isobath, in order to elucidate the history of this region. The maps presented here aim to reconstruct the connection between palaeo-rivers in the Sunda Shelf and contemporary rivers and identify possible dispersal routes and barriers for the floras and faunas/biotas associated with riverine systems found here.

Innovative dispersion techniques of graphene nanoplatelets(GNPs)through mechanical stirring and ultrasonication:Impact on morphological,mechanical,and thermal properties of epoxy nanocomposites

Graphene nanoplatelets(GNPs)have attracted tremendous interest due to their unique properties and bonding *** study focuses on the effect of GNP dispersion on the mechanical,thermal,and morphological behavior of GNP/epoxy *** study aims to understand how the dispersion of GNPs affects the properties of epoxy nanocomposite and to identify the best dispersion approach for improving mechanical performance.A solvent mixing technique that includes mechanical stirring and ultrasonication was used for producing the *** transform infrared spectroscopy was used to investigate the interaction between GNPs and the epoxy *** measurements of density and moisture content were used to confirm that GNPs were successfully incorporated into the *** findings showed that GNPs are successfully dispersed in the epoxy matrix by combining mechanical stirring and ultrasonication in a single step,producing well-dispersed nanocomposites with improved mechanical ***,the nanocomposites at a low GNP loading of 0.1 wt%,demonstrate superior mechanical strength,as shown by increased tensile properties,including improved Young's modulus(1.86 GPa),strength(57.31 MPa),and elongation at break(4.98).The nanocomposite with 0.25 wt%GNP loading performs better,according to the viscoelastic analysis and flexural properties(113.18 MPa).Except for the nanocomposite with a 0.5 wt%GNP loading,which has a higher thermal breakdown temperature,the thermal characteristics do not significantly *** effective dispersion of GNPs in the epoxy matrix and low agglomeration is confirmed by the morphological *** findings help with filler selection and identifying the best dispersion approach,which improves mechanical *** effective integration of GNPs and their interaction with the epoxy matrix provides the doorway for additional investigation and the development of sophisticated *** fiel

Modeling the boundary layer flow of ternary hybrid nanofluids is important for understanding and optimizing their thermal performance,particularly in applications where enhanced heat transfer and fluid dynamics are *** study numerically investigates the boundary layer flow of alumina-copper-silver/water nanofluid over a permeable stretching/shrinking sheet,incorporating both first and second-order velocity *** mathematical model is solved in MATLAB facilitated by the bvp4c function that employs the finite difference scheme and Lobatto IIIa *** solver successfully generates dual solutions for the model,and further analysis is conducted to assess their *** findings reported that only one of the solutions is *** the shrinking sheet case,increasing the first-order velocity slip delays boundary layer separation and enhances heat transfer,while,when the sheet is stretched,the second-order velocity slip accelerates separation and improves heat *** layer separation is most likely to occur when the sheet is shrinking;however,this can be controlled by adjusting the velocity slip with the inclusion of boundary layer suction.

Machine learning(ML)is increasingly applied for medical image processing with appropriate learning *** applications include analyzing images of various organs,such as the brain,lung,eye,etc.,to identify specific flaws/diseases for *** primary concern of ML applications is the precise selection of flexible image features for pattern detection and region *** of the extracted image features are irrelevant and lead to an increase in computation ***,this article uses an analytical learning paradigm to design a Congruent Feature Selection Method to select the most relevant image *** process trains the learning paradigm using similarity and correlation-based features over different textural intensities and pixel *** similarity between the pixels over the various distribution patterns with high indexes is recommended for disease ***,the correlation based on intensity and distribution is analyzed to improve the feature selection ***,the more congruent pixels are sorted in the descending order of the selection,which identifies better regions than the ***,the learning paradigm is trained using intensity and region-based similarity to maximize the chances of ***,the probability of feature selection,regardless of the textures and medical image patterns,is *** process enhances the performance of ML applications for different medical image *** proposed method improves the accuracy,precision,and training rate by 13.19%,10.69%,and 11.06%,respectively,compared to other models for the selected *** mean error and selection time is also reduced by 12.56%and 13.56%,respectively,compared to the same models and dataset.

A bibliometric analysis of railway safety research: Thematic evolution, current status, and future research directions

This study reports a bibliometric analysis of a sample of 787 articles from the Web of Science database. Railway safety is a broad research field with high research ***-assisted techniques such as bibliometric analysis could be used to study the thematic evolution and intellectual structure of such high-activity research fields. This study aims to identify recent advances in railway safety research and suggest future research directions. We first set out to identify the themes that define railway safety research through science mapping. Using citation count and Page Rank of articles, the most prestigious articles published within the last decade were identified for content *** on artificial intelligence applications to railway safety, particularly in hazard identification, infrastructure and equipment control, and condition monitoring, have gained traction in recent years. The security and safety of proposed cyber-physical systems for railway applications have been widely studied. Studies on next-level train control systems such as ETCS-3 and CTCS-4 have focused on satellite-based train localization and5G technology(and beyond) for railway communication systems. Research on human reliability in railway operations and management has also received considerable attention from researchers. However, little consideration is given to addressing issues related to the reliability of expert judgments-a key instrument in practical railway safety assessment. In the end, we suggest six avenues for future research in railway safety.

Output Voltage Stabilization of Non-Ideal DC-DC Zeta Converter with Output Voltage Error Elimination via Hybrid Control

In this paper, we proposed an output voltage stabilization of a DC-DC Zeta converter using hybrid control. We modeled the Zeta converter under continuous conduction mode operation. We derived a switching control law that brings the output voltage to the desired level. Due to infinite switching occurring at the desired level, we enhanced the switching control law by allowing a sizeable output voltage ripple. We derived mathematical models that allow one to choose the desired switching frequency. In practice, the existence of the non-ideal properties of the Zeta converter results in steady-state output voltage error. By analyzing the power loss in the zeta converter, we proposed an improved switching control law that eliminates the steady-state output voltage error. The effectiveness of the proposed method is illustrated with simulation results.
