Crohn's disease in adults: Observations in a multiracial Asian population

AIM： To determine the demography and clinical presentation of CD and secondly to determine any differences in the prevalence between the different ethnic groups in a multiracial Asian population. METHODS： Patients with CD who were seen in 2001- 2003 in the University of Malaya Medical Centre （UMMC） were enrolled in this study. Prevalence of disease was calculated for the group as a whole and by race with hospital admissions per ethnic group as the denominator. RESULTS： Thirty-four patients were diagnosed to have CD. Basic demographic data of patients; male：female 17：17; mean age 29.1 years （±13.5 years）; ethnic group： Malays 5 （14.7%）, Chinese 12 （35.3%） and Indians 17 （50%）. Twenty-six （76.5%） were diagnosed under the age of 40 and 8 （23.5%） were diagnosed over the age of 40. Location of the disease was as follows： ileocolonic 13 （38.2%）, terminal ileum only 9 （26.5%）, colon only 8 （23.5%）, and upper gastrointestinal 4 （11.8%）. Sixteen （47.1%） had penetrating disease, 9 （26.5%） had stricturing disease and 9 （26.5%） had nonpenetrating and non-stricturing disease. The hospital admission prevalence of CD was 26.0 overall, Indians 52.6, Chinese 6.9, and Malays 9.3 per 10^5 admissions per ethnic group. The difference between Indians and Malays： [OR 5.67 （1.97, 17.53）P〈 0.001] was statistically significant but not between the Indians and the Chinese [OR 1.95 （0.89, 4.35） P= 0.700]. The difference between the Chinese and the Malays was also not statistically significant. [OR 2.90 （0.95, 9.42）P= 0.063]. CONCLUSION： The clinical presentation of CD is similar to the Western experience. Although the overall prevalence is low, there appears to be a clear racial predominance among the Indians.

Background and Aim: Through accurate assessment of the risk factors and structural characteristics of children presenting with acute patella dislocation, this study aimed to evaluate the probability of recurrence and the effects on treatment. Materials and Methods: Demographic and radiological evaluation was made of each case. Contributory factors to patellofemoral instability were investigated. Surgical treatment was applied to 4 (15.39%) patients and conservative treatment to 22 (84.61%). Functional status before and after treatment was evaluated using the Kujala patellofemoral scoring system. Results: The patients comprised 8 (30.77%) males and 18 (69.23%) females with a mean age of 13.46 years. In 15 (57.69%) cases, there were patellofemoral complaints before the dislocation. The Kujala score was determined as mean 54.27 pre-treatment and 84.81 post-treatment in the conservative treatment group and mean 48.00 preoperatively and 75.25 postoperatively in the surgically treated group. No recurrence of patella dislocation was observed in any patient during the follow-up of both groups. Conclusions: Although it seems to be simple surgical intervention, as patella dislocation surgery can involve several complications, the surgical program must be very well planned and only applied to cases where necessary. With case-by-case assessment, the probability of recurrence of patella dislocation will be removed.

Primary sclerosing cholangitis in Turkish patients:characteristic features and prognosis

BACKGROUND:Primary sclerosing cholangitis(PSC)is a chronic cholestatic liver disease characterized by destruction and fibrosis of the bile *** study aimed to demonstrate the hepatic and extrahepatic characteristic findings and prognostic outcomes of Turkish patients with PSC. METHODS:The medical records of 35 consecutive patients with PSC from January 1988 to June 2007 were recorded *** the time of diagnosis,clinical features and laboratory data were collected. RESULTS:The mean age of the 35 patients was 41.69 years (range 15-80 years)at the time of diagnosis;14(40%)were female,and 21(60%)were *** mean duration of follow-up was 58.86 months(1-180 months).Twenty(57.1%) of the patients with PSC were asymptomatic and 22(62.9%) had inflammatory bowel *** the time of diagnosis, 20(57.1%)of the patients had both intra-and extra-hepatic ***-one(60%)of the patients,who had undergone ERCP for stent placement,had dominant bile duct stenosis. Cholangiocarcinoma was found in 2(5.7%)of the patients and cirrhosis was detected in 7(20%);5(14.3%)underwent liver *** median follow-up time after liver transplantation was 23 months and all are still *** (17.1%)patients died. CONCLUSIONS:PSC has a clinical course varied from advanced liver disease requiring liver transplantation within a short time to being asymptomatic for decades. The prognosis of Turkish patients with PSC is also disappointing as described in other studies.

Sweet plasmonics： Sucrose macrocrystals of metal nanoparticles

The realization of plasmonic structures generally necessitates expensive fabrication techniques, such as electron beam and focused ion beam lithography, allowing for the top-down fabrication of low-dimensional structures. Another approach to make plasmonic structures in a bottom-up fashion is colloidal synthesis, which is convenient for liquid-state applications or very thin solid films where aggregation problems are an important challenge. The architectures prepared using these methods are typically not robust enough for easy handling and convenient integration. Therefore, developing a new plasmonic robust platform having large-scale dimensions without adversely affecting the plasmonic features is in high demand. As a solution, here we present a new plasmonic composite structure consisting of gold nanoparticles （Au NPs） incorporated into sucrose macrocrystals on a large scale, while preserving the plasmonic nature of the Au NPs and providing robustness in handling at the same time. As a proof of concept demonstration, we present the fluorescence enhancement of green CdTe quantum dots （QDs） via plasmonic coupling with these Au NPs in the sucrose crystals. The obtained composite material exhibits centimeter scale dimensions and the resulting quantum efficiency （QE） is enhanced via the interplay between the Au NPs and CdTe QDs by 58% （from 24% to 38%）. Moreover, a shortening in the photoluminescence lifetime from 11.0 to 7.40 ns, which corresponds to a field enhancement factor of 2.4, is observed upon the introduction of Au NPs into the QD incorporated macrocrystals. These results suggest that such ＂sweet＂ plasmonic crystals are promising for large-scale robust platforms to embed plasmonic nanoparticles.

Kombucha and Health

Kombucha is a fermented drink produced by fermentation of sweetened black and/or green tea with kombucha culture, it is known to have health benefits. The microflora of this fermented beverage mostly consists of AAB （acetic acid bacteria） and yeasts. Depending on its origin, LAB （lactic acid bacteria） sometimes will also be able to be found in the microflora. The production of kombucha and formation of its beneficial components are the result of the metabolic activities of this microflora. This fermented tea has been produced and consumed in the Far East for hundreds of years as a tradition; it is used as an adjunctive treatment for certain diseases. The interest in this tea has increased recently, and kombucha has spread worldwide. Its microflora diversity and metabolites are becoming important subjects of scientific studies. Although there are a few companies in Turkey that manufacture and market kombucha, there is no comprehensive scientific study on this subject. The liquid portion of kombucha is claimed to have various medicinal effects on human health. Recent studies have suggested that kombucha tea prevents paracetamol induced hepatotoxieity and chromate induced oxidative stress in albino rats. As kombucha tea is rich in compounds known to be strong antioxidants, it is expected to ameliorate liver damage. This beneficial effects of kombucha tea are attributed to the presence of tea polyphenols, gluconic acid, glucuronic acid, lactic acid, vitamins, amino acids, antibiotics and a variety of micronutrients produced during fermentation. The functional character of the kombucha should be investigated by a larger number of scientific studies. Co-use or self-use with various foods of this tea should be widespread.

Human immunodeficiency virus(HIV) may result in devastating multi-organ complications, including cirrhosis. Consequently, liver transplantation is often required for these patients. We report a case of a 43-year-old female with cryptogenic cirrhosis and HIV on highly active antiretroviral therapy, presenting for non-related living donor liver transplantation. The intraoperative course was complicated by hepatic artery and portal vein thrombosis, requiring thrombectomy. On postoperative day-3, the patient required retransplantation with a cadaveric donor organ due to primary graft failure.

Internet and social media usage of orthopaedic patients:A questionnaire-based survey

AIM To evaluate social media usage of orthopaedic patients to search for solutions to their health problems. METHODS The study data were collected using face-to-face questionnaire with randomly selected 1890 patients aged over 18 years who had been admitted to the orthopaedic clinics in different cities and provinces across Turkey. The questionnaire consists of a total of 16 questions pertaining to internet and social media usage and demographics of patients, patients' choice of institution for treatment, patient complaints on admission, online hospital and physician ratings, communication between the patient and the physician and its *** It was found that 34.2%(n = 647) of the participants consulted with an orthopaedist using the internet and 48.7%(n = 315) of them preferred websites that allow users to ask questions to a physician. Of all questionaskers, 48.5%(n = 314) reported having found the answers helpful. Based on the educational level of the participants, there was a highly significant difference between the rates of asking questions to an orthopaedist using the internet(P = 0.001). The rate of questionasking was significantly lower in patients with an elementary education than that in those with secondary, high school and undergraduate education(P = 0.001) The rate of reporting that the answers given was helpful was significantly higher in participants with an undergraduate degree compared to those who were illiterate, those with primary, elementary or high school education(P = 0.001). It was also found that the usage of the internet for health problems was higher among managers-qualified participants than unemployed-housewives, officers, workers-intermediate staff(P < 0.05).CONCLUSION We concluded that patients have been increasingly using the internet and social media to select a specific physician or to seek solution to their health problems in an effective way. Van der Waals heterojunctions are fast-emerging quantum structures fabricated by the controlled stacking of two-dimensional(2D)*** to the atomically thin thickness,their carrier properties are not only determined by the host material itself,but also defined by the interlayer interactions,including dielectric environment,charge trapping centers,and stacking *** abundant constituents without the limitation of lattice constant matching enable fascinating electrical,optical,and magnetic properties in van der Waals heterojunctions toward next-generation devices in photonics,optoelectronics,and information *** review focuses on the charge and energy transfer processes and their dynamics in transition metal dichalcogenides(TMDCs),a family of quantum materials with strong excitonic effects and unique valley properties,and other related 2D materials such as graphene and hexagonalboron *** the first part,we summarize the ultrafast charge transfer processes in van der Waals heterojunctions,including its experimental evidence and theoretical understanding,the interlayer excitons at the TMDC interfaces,and the hot carrier injection at the graphene/TMDCs *** the second part,the energy transfer,including both Förster and Dexter types,are reviewed from both experimental and theoretical ***,we highlight the typical charge and energy transfer applications in photodetectors and summarize the challenges and opportunities for future development in this field.
