Objective:To address the lack of research on invasive group B Streptococcus(GBS)infections in Malaysia and Southeast Asia through a comprehensive analysis of GBS isolates obtained from ***:Medical records from patients with GBS infection isolated from the sterile site,such as blood and cerebrospinal fluid from 14 July 2019 to 15 December 2020,were reviewed from six major hospitals in Peninsular *** criteria were invasive GBS,sterile sites and non-repeated GBS isolated from the same patients in the same *** isolates were re-identified for GBS and ***:A total of 118 patients were eligible,with a majority of non-pregnant adults(76.3%).Over half of the patients(62.7%)had underlying medical conditions,with diabetes as the most common disease,followed by respiratory disease,renal disease,cardiovascular disease and skin and soft tissue *** most common manifestations were sepsis,followed by soft tissue abscess,diabetic foot ulcer,wet gangrene and *** overall mortality was 7.6%.The most common serotype was serotype桋.Conclusions:Invasive GBS infection among non-pregnant adults showed a rising trend,particularly among diabetic *** study underscores the importance of reducing risk factors and highlights the necessity of developing GBS vaccination as a preventive strategy for both infants and adults.

Isolation,characterization and identification of plant growth-promoting rhizobacteria from the rhizosphere of Acacia mangium at sandy BRIS soil

This study has isolated,characterized,and identified potential plant growthpromoting rhizobacteria(PGPR)with multiple PGP characteristics(N_(2)-fixation,P-and K-solubilization,IAA,and siderophores production)from the rhizosphere BRIS soil of Acacia mangium.A total of 24 pure colonies were isolated and only 8 colonies were selected for further evaluation of the growth rate in 5%organic molasses medium supplemented with 2%KNO_(3).Based on the biochemical,potential PGP characteristics and growth performance,3 superior PGPR strains were selected and identified as Paraburkholderia unamae(UA1),Bacillus amyloliquefaciens(UA6),and Enterobacter asburiae(UAA2)by partial sequencing of the 16S rRNA *** selected bacterial strains either in single or mixed(UA1+UA6+􀀁UAA2)cultures have shown a significant biochemical estimation of the PGP *** strain has its own PGPR traits superiority with UA1 showing the best PGP characteristic followed by UA6 and *** use of mixed bacterial strains was beneficial as it showed the best performance in N_(2)-fixation,siderophores production,and significant effect on corn phenology,growth and yield compared to using a single *** types of microbes showed potential to be used as biofertilizer and should be exploited more.

Acute Transverse Myelitis in Pregnancy—A Case Report and a Literature Review

Purpose: This case report highlights a rare instance of a 26-week pregnant woman presenting with limb numbness, diagnosed with acute transverse myelitis (ATM) following COVID-19 vaccination. Notably, she tested positive for neuromyelitis optica (NMO) but did not exhibit the typical symptoms of blurred vision. Methods: Data were collected from I Seha in governmental hospitals in Bahrain, providing a comprehensive overview of the patient’s clinical journey. Results: The patient was treated with carbamazepine, steroids, and clexane, leading to the successful delivery of a healthy baby at 34 weeks. However, two years postpartum, she reported new episodes of blurred vision and numbness. Subsequent treatment with Rituximab resulted in significant improvement, with a diagnosis of Devic’s disease established. Conclusion: Acute transverse myelitis is a rare condition, particularly during pregnancy, and in this case, symptoms of Devic’s disease were masked by the absence of blurred vision. This condition predominantly affects females in their 20s during childbearing years and can lead to complications such as premature delivery. Effective management includes steroids and antiepileptic drugs, with clexane playing a crucial role in preventing embolisms. A delivery mode may vary based on obstetric factors, and MRI remains the gold standard for diagnosis. Close follow-up is essential for optimal patient outcomes.

Emerging Novel Therapeutic Approaches for the Treatment of Alzheimer’s Disease

Alzheimer’s disease (AD) is caused by synaptic failure and the excessive accumulation of misfolded proteins especially Aβ and tau, and associated with memory loss and cognitive impairment. Treatment of AD mainly consists of symptomatic therapy and disease-modifying therapy (DMT). Several monotherapies including small molecules or antibodies have been evaluated through multiple clinical trials, but a very few have been approved by the USFDA to intervene the disease’s pathogenesis. Past research has shown multifactorial nature of AD, therefore, multi-target drugs were proposed to target different pathways at the same time, however, currently no rationally designed multi-target directed ligand (MTDL) has been clinically approved. Different combinations and bispecific antibodies are also under development. Novel approaches like stem cell-based therapies, microRNAs, peptides, ADCs and vaccines cast a new hope for AD treatment, however, a number of questions remained to be answered prior to their safe and effective clinical translation. This review explores the small molecules, MTDL, and antibodies (monospecific and bispecific) for the treatment of AD. Finally, future perspectives (stem cell therapy, PROTAC approaches, microRNAs, ADC, peptides and vaccines) are also discussed with regard to their clinical applications and feasibility.

Evaluation of Functional and Radiological Outcome of Arthroscopic-Assisted Anatomical Coracoclavicular (CC) and Acromioclavicular (AC) Ligament Reconstruction in Chronic AC Joint Dislocation

Introduction: Acromioclavicular (AC) joint dislocation is a common shoulder injury, comprising 9% - 12% of shoulder girdle injuries. Optimal management remains challenging, with treatment decisions guided by the Rockwood classification system. Controversies surround grade III injuries, necessitating further classification. Non-operative treatment has shown favorable outcomes, while surgical interventions vary. Anatomical coracoclavicular reconstruction (ACCR) has demonstrated biomechanical advantages over traditional methods. Arthroscopic techniques offer advantages, minimizing deltoid detachment and allowing concurrent pathology identification. This study evaluates the outcomes of arthroscopic-assisted ACCR in chronic AC joint dislocation. Surgical Technique: Arthroscopic-assisted ACCR involves meticulous portal placement, tendon graft harvesting, diagnostic arthroscopy, and coracoid exposure. The clavicle tunnels were made to mimic the conoid and trapezoid ligament positions, using FibreTape#2 loop and Dog Bone Button for correct placement against the coracoid base, and passing the semitendinosus graft through to reconstruct the conoid ligament, reduction done and graft follow through for anatomical reconstruction. Methods: A retrospective cohort study at Hospital Kuala Lumpur analyzed 35 patients undergoing arthroscopic-assisted ACCR for Rockwood grade III - V AC joint dislocations. Inclusion criteria encompassed trauma ≥ 3 weeks prior, no prior shoulder injuries, and ≥12-month follow-up. Functional and radiological assessments utilized ASES scores and coracoclavicular distances, respectively. Statistical analysis employed descriptive statistics and logistic regression. Results: The mean age was 38.9 years (SD 11.26), and 34 of 35 patients were male. Grade IV injuries were predominant (37.1%). Waiting time for surgery averaged 234.9 days. Objective:To elucidate the characteristics,management strategies,risk factors,and clinical impacts associated with adverse drug reactions(ADRs)induced by first-line antitubercular drugs to enhance tuberculosis(TB)***:A retrospective cohort study was conducted by retrieving drug-susceptible TB records among adult patients who received TB treatment from 2018 to 2021 at 10 public health clinics in Sarawak,*** the initial TB treatment and occurrence of specific ADRs within the study period were *** analysis was performed to identify the risk factors associated with both overall ADRs and individual types of ***:Among 2953 cases,705(23.9%)developed *** reactions were the most prevalent(47.1%),followed by hepatotoxicity(32.8%)and gastrointestinal disturbances(29.8%).Six out of seven types of ADRs investigated occurred within the intensive phase,mostly manifesting at approximately 2 weeks of *** resulted in the majority(85.3%)of treatment discontinuations,while vision problems led to treatment modifications in half of the *** factors for all ADRs included age≥60 years,females,illicit drug use,and comorbidities such as HIV-positive,diabetes,and chronic liver *** consumption was independently associated with *** caused around one-third of interruptions exceeding 2 weeks(33.0%)and subsequently necessitated treatment restarts(34.5%).Conclusions:Understanding these various aspects contributes to improving the overall management of ADRs in TB *** ADR monitoring and reporting are essential to strengthen ADR management.

The structural,morphological and optical properties of single-phase polycrystalline La2-xSrxNiMnO6(x=0,0.3 and 0.5),synthesized by solid state reaction were *** samples were characterized by X-ray diffraction(XRD),scanning electron microscopy(SEM)/energy dispersive analysis of X-rays(EDAX),Raman spectroscopy and diffuse reflectance spectroscopy(DRS)to elucidate the role of A-site Sr-doping in double perovskite La2 *** analysis of XRD patterns revealed that all the samples have monoclinic structure with space group P21/*** gradient in the Williamson Hall plots revealed the presence of tensile strain in all the *** morphological studies revealed that average grain size increases along with appreciable decrease in porosity with Sr *** Ni/Mn antisite disorder was introduced in the La2 NiMnO6 by Sr-doping confirmed by an increase in the full width at half maximum(FWHM)and decrease in intensity of the Raman modes at around 540 and 665 cm-1 which correspond to the antisymmetric stretching and symmetric stretching modes,*** results reveal that the band gap in La2 NiMnO6 can be tuned down by Sr-doping to a value of1.37 eV(very close to 1.40 eV,considered as optimum value for better efficiency of a solar cell).Thus,Sr-doped La2 NiMnO6 may be of prime importance for applications in solar cells.
