Syndromic and sporadic inflammatory/hyperplastic small-bowel polyps:a comparative study

Background:Inflammatory/hyperplastic small-bowel polyps(SBPs)occur either sporadically or in patients with a polyposis syndrome;however,comparison between these two settings of the histological features of SBPs has not been reported and the etiology of sporadic inflammatory/hyperplastic SBPs remains ***:Twenty-eight cases of sporadic inflammatory/hyperplastic SBPs and nine cases of syndromic SBPs were retrieved from the Department of Anatomic Pathology at the Cleveland ***-demographics and histological features were compared between the two ***:Patients with syndromic inflammatory/hyperplastic SBPs were younger(48 vs.63 years;P=0.007)and had higher rates of hemorrhagic telangiectasia(55.6%vs.0%;P=0.000),gastric polyps(87.5%vs.21.4%;P=0.001),and family history of colon cancer(62.5%vs.11.1%;P=0.014).Sporadic cases were more frequently associated with gastro-esophageal reflux(35.7%vs.0%;P=0.079)and anti-reflux medication use(55.6%vs.11.1%;P=0.026).Histologically,the syndromic SBPs were more often of pure intestinal type(45.4%vs.3.8%;P=0.005)and had prominent vessels(81.8%vs.42.3%;P=0.036).Conclusions:Patients with syndromic SBPs are younger and have higher rates of hemorrhagic telangiectasia,gastric polyps,and family history of colon ***,syndromic inflammatory/hyperplastic SBPs are more likely to be of pure intestinal type and to have prominent vessels.

Background and Aim:Inflammatory bowel disease(IBD)is associated with an increased risk of colorectal cancer(CRC).Studies have shown tumorigenetic and histomorphological differences between IBD-associated CRC and non-IBD CRC,suggesting differences in tumor behavior and response to *** aimed to compare tumor recurrence and survival rates following postoperative chemotherapy in CRC patients with and without ***:Search of the Cleveland Clinic’s CRC database revealed 65 patients who had IBD-associated CRC and received postoperative adjuvant chemotherapy between 1994 and ***-one patients were excluded due to incomplete clinical *** score-matching based on age,surgery intent,CRC site,tumor grade,American Joint Committee on Cancer(AJCC)stage and T stage was used to match IBD and non-IBD patients(1:4).Competing risk and Cox regression models were used to analyze differences in disease-free survival and overall survival,***:Forty-four patients with IBD-associated CRC were matched to 176 patients with non-IBD *** IBD patients,29(66%)had ulcerative colitis,14(32%)had Crohn’s disease,and one(2%)had indeterminate *** IBD diagnosis age was 28.1±14.5 years,and mean IBD duration at time of CRC treatment was 21.5±12.6 ***(23%)IBD patients had tumor recurrence compared with 34(19%)non-IBD patients(P=.074).There was no significant difference in disease-free survival(hazard ratio[HR]=0.60;95%CI:0.35–1.05;P=0.074)or overall survival(HR=0.87;95%CI:0.54–1.4;P=0.58)between IBD and non-IBD ***:Patients with IBD-associated CRC have comparable rates of tumor recurrence and survival following postoperative chemotherapy as CRC patients without *** studies are needed to confirm these findings and guide therapeutic decisions.

Clinical outcomes of isolated renal failure compared to other forms of organ failure in patients with severe acute pancreatitis

To assess differences in clinical outcomes of isolated renal failure (RF) compared to other forms of organ failure (OF) in patients with severe acute pancreatitis (SAP). METHODSUsing a prospectively maintained database of patients with acute pancreatitis admitted to a tertiary medical center between 2003 and 2016, those with evidence of persistent OF were classified to renal, respiratory, cardiovascular, or multi-organ (2 or more organs). Data regarding demographics, comorbidities, etiology of acute pancreatitis, and clinical outcomes were prospectively recorded. Differences in clinical outcomes after development of isolated RF in comparison to other forms of OF were determined using independent t and Mann-Whitney U tests for continues variables, and χ2 test for discrete variables. RESULTSAmong 500 patients with acute pancreatitis, 111 patients developed persistent OF: mean age was 54 years, and 75 (67.6%) were male. Forty-three patients had isolated OF: 17 (15.3%) renal, 25 (21.6%) respiratory, and 1 (0.9%) patient with cardiovascular failure. No differences in demographics, etiology of acute pancreatitis, systemic inflammatory response syndrome scores, or development of pancreatic necrosis were seen between patients with isolated RF vs isolated respiratory failure. Patients with isolated RF were less likely to require nutritional support (76.5% vs 96%, P = 0.001), ICU admission (58.8% vs 100%, P = 0.001), and had shorter mean ICU stay (2.4 d vs 15.7 d, P < 0.001), compared to isolated respiratory failure. None of the patients with isolated RF or isolated respiratory failure died. CONCLUSIONAmong patients with SAP per the Revised Atlanta Classification, approximately 15% develop isolated RF. This subgroup seems to have a less protracted clinical course compared to other forms of OF. Isolated RF might be weighed less than isolated respiratory failure in risk predictive modeling of acute pancreatitis.

Portal hypertension secondary to myelofibrosis with myeloid metaplasia:A study of 13 cases

AIM:To describe the clinical presentation and complications of portal hypertension (PH) secondary to myelofibrosis with myeloid metaplasia (MMM).METHODS: Medical records for 123 patients with MMM were ***: Thirteen patients with PH secondary to MMM were identified. Median ages at time of MMM and PH diagnosis were 61 and 66 years, respectively. The interval from MMM diagnosis to presentation with one of the PH features ranged from 1 to 11 years. Variceal bleeding and ascites were the most common presentations. Of the eight patients who presented with variceal bleeding, six patients underwent endoscopic variceal ligation (EVL) with no variceal recurrence or hematological worsening during a 12-mo follow up ***: Patients with MMM might develop PH. Exact mechanisms leading to PH in MMM are still controversial. As in other etiologies, variceal bleeding and ascites are the most common presentations. Anemia may correlate with, and/or predict, the severity of the PH presentation in these patients. EVL can successfully control variceal bleeding in MMM. Further clinical studies are required.

Advanced control solutions for enhanced resilience of modern power-electronic-interfaced distribution systems

Modern power delivery systems are rapidly evolving with high proliferation of power-electronic(PE)-interfaced distributed energy resources(DERs).Compared to the conventional sources of generation,the PE-interfaced DERs,e.g.,solar and wind resources,are attributed to substantially different characteristics such as lower overload capability and limited frequency response *** paper focuses on effective management and control mechanisms for PE-interfaced DERs in power distribution systems with high penetration of renewables,particularly vailing conditions in the *** enhance the system performance resilience,we introduce an advanced model predictive control(MPC)based scheme to control the DER units,minimize the impact of transients and disruptions,speed up the response and recovery of particular metrics and parameters,and maintain an acceptable operation *** performance of the suggested control scheme is tested on a modified IEEE 34-bus test feeder,where the proposed solution demonstrates its effectiveness to minimize the system transient during faults,with an enhanced grid-edge and system-wide resilience characteristics in voltage profiles.

Use of testicular sperm in couples with SCSA-defined high sperm DNA fragmentation and failed intracytoplasmic sperm injection using ejaculated sperm

Sperm DNA fragmentation(SDF)has been linked with male infertility,and previous studies suggest that SDF can have negative influence on pregnancy outcomes with assisted *** performed a retrospective review of consecutive couples with a high SDF level that had intracytoplasmic sperm injection(ICSI)using testicular sperm(T-ICSI).We compared the T-ICSI outcomes to that of two control groups:87 couples with failed first ICSI cycle and who had a second ICSI cycle using ejaculated sperm(Ej-ICSI),and 48 consecutive couples with high sperm chromatin structure assay(SCSA)-defined SDF(>15%)that underwent an ICSI cycle using ejaculated sperm after one or more failed ICSI cycles(Ej-ICSI-high SDF).The mean number of oocytes that were retrieved and the total number of embryos were not different among the three *** mean number of transferred embryos in the T-ICSI group was higher than the Ej-ICSI group but not significantly different than the Ej-ICSI-high SDF group(1.4,1.2,and 1.3,respectively,P0.05).No significant difference was found in live birth rate when comparing T-ICSI to Ej-ICSI and Ej-ICSI-high SDF *** results suggest that pregnancy outcomes and live birth rates with T-ICSI are not significantly superior to Ej-ICSI in patients with an elevated SCSA-defined sperm DNA fragmentation and prior ICSI failure(s).

A comparative study of polymer nanocomposites containing multi-walled carbon nanotubes and graphene nanoplatelets

Featuring exceptional mechanical and functional performance, MWCNTs and graphene(nano)platelets(GNPs or Gn Ps;each platelet below 10 nm in thickness) have been increasingly used for the development of polymer nanocomposites. Since MWCNTs are now cost-effective at US$30 per kg for industrial applications, this work starts by briefly reviewing the disentanglement and surface modification of MWCNTs as well as the properties of the resulting polymer nanocomposites. GNPs can be made through the thermal treatment of graphite intercalation compounds followed by ultrasonication;GNPs would have lower cost yet higher electrical conductivity over 1,400 S cmthan MWCNTs. Through proper surface modification and compounding techniques, both types of fillers can reinforce or toughen polymers and simultaneously add anti-static performance. A high ratio of MWCNTs to GNPs would increase the synergy for polymers. Green, solvent-free systhesis methods are desired for polymer nanocomposites. Perspectives on the limitations, current challenges and future prospects are provided.

Evaluation of infertile women: Mini-review

Evaluation of infertility indicated for women failed to conceive after one year of unprotected intercourse and for women over 35 years failed to conceive after 6 mo of unprotected intercourse, because the fertility decline as women approach 40 years. Evaluation of infertile couple should begin with semen analysis of male partner. If the semen analysis is within normal range, evaluation then move on to female partner. Evaluation of the infertile woman should be carried in cost-effective manner to identify the causes of infertility using the least invasive methods. Methods of evaluation of infertile women include: 1) history and physical examination;2) evaluation of the male partner;3) documentation of ovulation;4) evaluation of the ovarian reserve;5) evaluation of cervical causes of infertility;6) evaluation of uterine causes of infertility;7) documentation of the tubal patency;and 8) exclusion of peritoneal causes of infertility. Evaluation of infertile couple should include evaluation of both partners. Semen analysis for the male partner, followed by documentation of ovulation and evaluation of genital tract patency. PCT is not used as routine evaluation of infertile women. Laparoscopy is not used as routine evaluation of infertile women unless there is suspected peritoneal factors of infertility or endometriosis or tubal occlusion. Ovarian reserve should only be done for infertile women with diminished response to external gonadotropins (not routine).
