Background: Duodenoscope-related multidrug-resistant organism(MDRO) infections raise concerns. Disposable duodenoscopes have been recently introduced in the market and approved by regulatory agencies with the aim to reduce the risk of endoscopic retrograde cholangiopancreatography(ERCP) associated infections. The aim of this study was to evaluate the outcome of procedures performed with single-use duodenoscopes in patients with clinical indications to single-operator cholangiopancreatoscopy. Methods: This is a multicenter international, retrospective study combining all patients who underwent complex biliopancreatic interventions using the combination of a single-use duodenoscope and a single-use cholangioscope. The primary outcome was technical success defined as ERCP completion for the intended clinical indication. Secondary outcomes were procedural duration, rate of cross-over to reusable duodenoscope, operator-reported satisfaction score(1 to 10) on performance rating of the single-use duodenoscope, and adverse event(AE) rate. Results: A total of 66 patients(26, 39.4% female) were included in the study. ERCP was categorized according to ASGE ERCP grading system as 47(71.2%) grade 3 and 19(28.8%) grade 4. The technical success rate was 98.5%(65/66). Procedural duration was 64(interquartile range 15-189) min, cross-over rate to reusable duodenoscope was 1/66(1.5%). The satisfaction score of the single-use duodenoscope classified by the operators was 8.6 ± 1.3 points. Four patients(6.1%) experienced AEs not directly related to the single-use duodenoscope, namely 2 post-ERCP pancreatitis(PEP), 1 cholangitis and 1 ***: Single-use duodenoscope is effective, reliable and safe even in technically challenging procedures with a non-inferiority to reusable duodenoscope, making these devices a viable alternative to standard reusable equipment.

AIM: To provide consensus statements on the use of per-oral cholangiopancreatoscopy(POCPS).METHODS: A workgroup of experts in endoscopic retrograde cholangiopancreatography(ERCP), endosonography, and POCPS generated consensus statements summarizing the utility of POCPS in pancreaticobiliary disease. Recommendation grades used validated evidence ratings of publications from an extensive literature ***: Six consensus statements were generated:(1) POCPS is now an important additional tool during ERCP;(2) in patients with indeterminate biliary strictures, POCS and POCS-guided targeted biopsy are useful for establishing a definitive diagnosis;(3) POCS and POCS-guided lithotripsy are recommended for treatment of difficult common bile duct stones when standard techniques fail;(4) in patients with main duct intraductal papillary mucinous neoplasms(IPMN) POPS may be used to assess extent of tumor to assist surgicalresection;(5) in difficult pancreatic ductal stones,POPS-guided lithotripsy may be useful in fragmentation and extraction of stones; and(6) additional indications for POCPS include selective guidewire placement,unexplained hemobilia, assessing intraductal biliary ablation therapy, and extracting migrated stents. CONCLUSION: POCPS is important in association with ERCP, particularly for diagnosis of indeterminate biliary strictures and for intra-ductal lithotripsy when other techniques failed, and may be useful for preoperative assessment of extent of main duct IPMN, for extraction of difficult pancreatic stones, and for unusua indications involving selective guidewire placement,assessing unexplained hemobilia or intraductal biliary ablation therapy, and extracting migrated stents.

Epidural anesthesia is effective for extracorporeal shock wave lithotripsy of pancreatic and biliary calculi

AIM: To evaluate the efficacy of thoracic epidural analgesia for extracorporeal shock wave lithotripsy (ESWL). METHODS: ESWL is an effective, non-invasive technique for the treatment of difficult pancreatic and large bile duct calculi. The procedure is often painful and requires large doses of analgesics. Many different anesthetic techniques have been used. Patients with either large bile duct calculi or pancreatic duct calculi which could not be extracted by routine endoscopic methods were selected. Thoracic epidural anesthesia (TEA) was routinely used in all the subjects unless contraindicated. Bupivacaine 0.25% with or without clonidine was used to block the segments D6 to D12. The dose was calculated depending on the age, height and weight of the patient. It was usually 1-2 mL per segment ***: Ninety eight percent of the 1509 patients underwent ESWL under TEA. The subjects selected were within American Society of Anesthesiologists grade Ⅰ to Ⅲ. ESWL using EA permitted successful elimination of bile duct or pancreatic calculi with minimal morbidity. The procedure time was shorter in patients with TEA than in those who underwent ESWL under total intravenous anesthesia. CONCLUSION: Almost all patients undergoing ESWL with EA had effective blocks with a single catheter insertion and local anesthetic injection.

A comprehensive review of DC fault protection methods in HVDC transmission systems

High voltage direct current (HVDC) transmission is an economical option for transmitting a large amount of power over long distances. Initially, HVDC was developed using thyristor-based current source converters (CSC). With the development of semiconductor devices, a voltage source converter (VSC)-based HVDC system was introduced, and has been widely applied to integrate large-scale renewables and network interconnection. However, the VSC-based HVDC system is vulnerable to DC faults and its protection becomes ever more important with the fast growth in number of installations. In this paper, detailed characteristics of DC faults in the VSC-HVDC system are presented. The DC fault current has a large peak and steady values within a few milliseconds and thus high-speed fault detection and isolation methods are required in an HVDC grid. Therefore, development of the protection scheme for a multi-terminal VSC-based HVDC system is challenging. Various methods have been developed and this paper presents a comprehensive review of the different techniques for DC fault detection, location and isolation in both CSC and VSC-based HVDC transmission systems in two-terminal and multi-terminal network configurations.

Determining Potential Passive Islanding Detection Indicators for Single-point Single Inverter,Single-point Multi-inverter and Multi-point Multi-inverter Scenarios

Distributed Generation(DG)sources,predominantly renewable energy-based,such as solar photovoltaic and wind energy sources,have significantly penetrated into power system *** high penetration of DG sources may lead to the formation of unintentional islands,which is hazardous to both the equipment and personnel,if *** this paper,46 passive parameters are considered to find out which candidate(s)are promising for detecting sustained unintentional islands and avoid false DG *** 46 parameters are derived from point of common coupling(PCC)voltage,DG output current,frequency,power factor angle,active and reactive *** these parameters are tested on standard IEEE 13 and 34 bus distribution networks,integrated with inverter-interfaced DGs at different locations for different penetration levels in the MATLAB/SIMULINK *** parameters are tested for different islanding events(comprising of various local loads,such as conventional parallel RLC resonant load,R,RC,and RL loads)and for different non-islanding *** parameters are then ranked accordingly by a suitable averaging approach based performance ranking *** this analysis,the best candidates are obtained for detecting inverter DG islands at single-point(or single PCC)locations and multi-point(or multiple PCCs)*** best passive candidates are obtained for different island scenarios from which a set of promising islanding detection indicators are proposed.

A low power, eco-friendly multipurpose thermoelectric refrigerator

There has been an immense endeavor to mitigate global warming in spite of which it has only been worse. This paper presents the design and imple- mentation of a low power and eco-friendly refrigeration system using the thermoelectric effect. The conventional refrigerators make use of complex mechanisms which involves synchronous operation of various units, namely the compressor, condensers, expansion valves, evaporator, refi＇igerant and so on. But a thermoelectric refrigerator exploits the principle of the Peltier effect, thus avoiding the utilization of these complex components. This even helps curb the release of harmful chlorofluorocarbons （CFCs） into the atmosphere which contributes to the increase in global temperature. Moreover, the temperature can be controlled and set to required values with the help of a microcontroller. Hence, this can be used both for domestic and commercial purposes. The unit does not eject any harmful gases. Therefore, the heat expelled from the unit can be tapped for heating utilities, making the use of this device versatile in its application. Thus this proposal aims not only at reducing the air pollutants by not contributing to it but also at reducing the power consumption.

Transforming Education with Photogrammetry: Creating Realistic 3D Objects for Augmented Reality Applications

Augmented reality (AR) is an emerging dynamic technology that effectively supports education across different levels. The increased use of mobile devices has an even greater impact. As the demand for AR applications in education continues to increase, educators actively seek innovative and immersive methods to engage students in learning. However, exploring these possibilities also entails identifying and overcoming existing barriers to optimal educational integration. Concurrently, this surge in demand has prompted the identification of specific barriers, one of which is three-dimensional (3D) modeling. Creating 3D objects for augmented reality education applications can be challenging and time-consuming for the educators. To address this, we have developed a pipeline that creates realistic 3D objects from the two-dimensional (2D) photograph. Applications for augmented and virtual reality can then utilize these created 3D objects. We evaluated the proposed pipeline based on the usability of the 3D object and performance metrics. Quantitatively, with 117 respondents, the co-creation team was surveyed with open-ended questions to evaluate the precision of the 3D object created by the proposed photogrammetry pipeline. We analyzed the survey data using descriptive-analytical methods and found that the proposed pipeline produces 3D models that are positively accurate when compared to real-world objects, with an average mean score above 8. This study adds new knowledge in creating 3D objects for augmented reality applications by using the photogrammetry technique;finally, it discusses potential problems and future research directions for 3D objects in the education sector.
