Assessment of Biochemical and Antioxidative Status in Patients Suffering from Dengue Fever

Summary：A multi-centred study was designed to collect dengue epidemiologic data from government and registered private hospitals/clinics and maintained archive of frozen specimens in bio-bank to be used for future dengue epidemic control program, and assess the epidemiology of dengue fever（DF） by evaluating biochemical and oxidative status of patients. ELISA Ig M antibodies test was done to confirm DF. From August 2010 to December 2011, 101 confirmed blood samples of DF patients referred to pathology lab of Jinnah Hospital Lahore were subjected to the epidemiologic assessment by evaluating the biochemical and physiological indices and alterations of circulating antioxidants. Clinical features of DF patients and effect of fever on blood components and serum proteins of liver were recorded. The hospital stay in DF, dengue hemorrhagic fever（DHF） and dengue shock syndrome（DSS） showed significant difference. Significant increases in serum alanine amino transferase（alT）（P=0.000）, aspartate amino transferase（AST）（P=0.000）, alkaline phosphatase（alP）（P=0.000）, malondialdehyde（MDA） along with significant decreases in total protein（TP）（P=0.000）, reduced glutathione（GSH）（P=0.000）, superoxide dismutase（SOD）, catalase（CAT）（P=0.000）, and sialic acid contents（P=0.016） were observed. A positive correlation existed between bound sialic acid levels, liver enzymes and circulating antioxidants（r=0.656, P=0.016）. In the present study, alterations of circulating antioxidants in DF suggest that DF might be a metabolic response to an acute, self-limiting tropical viral infection, and a consequence of the viral inflammatory process.

Injury Pattern among Road Traffic Accidents’ Victims in Najran City, Saudi Arabia

Traffic accidents constitute the substantial cause of disability and mortality in the young Saudi individuals reaching horrible figures. The aim of this study was to review and identify the pattern of injury among road traffic accident (RTA) victims as well as the peak time of accidents and their implications. This retrospective hospital-based cross-sectional study was carried out in the period between April 2016 to March 2017 in King Khalid Hospital, Maternity and Children Hospital and Najran General Hospital, Najran, Saudi Arabia. all RTA victims admitted to the emergency departments of the pre-mentioned hospitals. Data collected from 435 Saudis and non-Saudis’ files were statistically analyzed with regard to age, gender, the timing of road traffic accidents, injury pattern, and neurological deficits incidence. Most of the RTAs’ cases (92%) were male victims with the highest peak in the age group 20 - 29 years. 58% of the RTAs happened in the evening (6:00 PM-9:00 PM) while 20% occurred in the morning. Head injury represented the most frequent pattern (36%) followed by the spinal injury (23%), lower limb injury (23%), upper limb injury (20%), thoracic injury (17%), pelvic injury and abdominal injury (8%). 4.4% of RTAs resulted in death whilst 9% of cases experienced neurological deficits. In conclusion, young male drivers in Najran recorded the highest frequency of RTAs. Initiation of road safety education and expansion of speed detectors (Saher) system deployment is highly recommended.

JAK2 Mutations in Chronic Myeloproliferative Neoplasm;Towards the Application of Personalized Treatments for Saudi Patients

The chronic myeloproliferative neoplasms (CMPN) are a group of clonal hematopoietic stem cell disorders in which large numbers of red blood cells, white blood cells, or platelets grow and spread excess in the bone marrow and the pe- ripheral blood. Cytogenetic analysis of the t (9:22) and molecular detection of BCR/ABL is the main diagnostic criteria in Philadelphia positive CMPN (CML). The identification of non-receptor tyrosine kinase JAK2 mutations (exon 14 JAK2 V617F and exon 12) have significantly contributed to our understanding of the molecular mechanisms in the pathogenesis of Philadelphia negative CMPN such as polycythemia vera (PV), essential thrombocythemia (ET) and primary myelofibrosis (PMF) patients. According to the revised WHO classification, JAK2 mutation is considered as a major diagnostic and clonal marker in Philadelphia negative CMPN which will play a major role in designing personal- ized treatments for the disease. JAK2 V617F mutation frequency is unknown in Saudi Arabia. Therefore, investigation of the JAK2 V617F mutation was carried out in DNA samples from 78 peripheral blood specimens corresponding to patients with polycythemia vera (PV) (n = 11), Chronic myeloid leukemia (CML) (n = 45), essential thrombocythemia (ET) (n = 10), idiopathic myelofibrosis (MF) (n = 12). We used polymerase chain reaction and direct DNA sequencing to detect the JAK2 mutation. Overall, the incidence of the JAK2 V617F mutation was 91% in PV, 40% in ET, and 25% in MF. This approach proved to be reliable and more sensitive in detecting the mutation. Two essential findings arose from our study. First, this technique could be carried out with DNA samples, even partially degraded, from routinely processed BM or peripheral blood specimens. Second, after correlation with morphological features, it turned out that the characteristics of the megakaryocytes were more specific than the mutational status of JAK2 in characterizing ET and PMF. Epidemiological Pattern of Breast Diseases among Females in the South-Western Region, Saudi Arabia

Background: Breast diseases cover several conditions. The majority of breast diseases are noncancerous. Some of these lesions are clinically unremarkable, which needed minimal intervention. However, some symptoms may be of clinical value and attract the attention of both the patient and the attending physician, especially when they become persistent. The study aimed to assess the prevalence, pattern, types, and clinical profile of breast diseases among females in the South-western region, during the period from 2018-2020. Methods: A retrospective record-based descriptive approach was used through reviewing medical records of all cases with breast disease attended King Khalid Hospital during the period from January 2018 to January 2020. Data extracted throng pre-structured questionnaire. Results: The study included 211 cases whose ages ranged from 18 to 58 years old with a mean age of 28.9 ± 12.8 years. Breast mass was the most recorded complaint (95.1%) followed with breast pain (32%), skin changes. Benign findings based on the final pathology report were recorded for more than two thirds of the cases, with the most common finding were fibroadenoma. Excisional biopsy and modified radical mastectomy were the most reported surgical interventions. Conclusions: In conclusion, the study revealed that the majority of the cases had benign breast disease (BBD), where fibroadenoma was the most frequent.

Choanal atresia (CA) is a rare occlusion of the posterior choanae. Unilateral cases have been reported more than bilaterally, and it’s more often right-sided in those patients. According to the literature, mixed bony-membranous atresia is the most common type. There is a high incidence of craniofacial and visceral anomalies associated with congenital choanal atresia. Therefore, investigation for associated congenital anomalies is an important step before the surgery. We report 2 cases of incidental finding of unilateral choanal atresia in a 21- and 17-year-old with nasal discharge being the only complaint in the former and nasal obstruction with headache in the latter. The patients were then scheduled for day-surgery as a case of choanal atresia for transnasal, endoscopic repair and posterior septectomy. The patients were discharged home on the same day with the absence of restenosis or other complications.

The Unusual Improvement of Intermittent Claudication Distance in Patient with Peripheral Arterial Disease Utilizing “2/3 Claudication Distance” Protocol of Exercise Training

Peripheral arterial disease (PAD) affects 12% - 20% of the population over 65 years. PAD is a component of systemic atherosclerosis and is associated with increased rate of all-cause mortality and cardiovascular events, compared with those without PAD. The strongest risk factors for PAD are older age and cigarette smoking. PAD accelerates functional decline leading to physical disability. Many randomized clinical trials demonstrated that treadmill walking training increases pain-free and maximal walking distances in patients with PAD. Exercise intervention is strongly recommended for patients with PAD and symptoms of intermittent claudication (IC). The need to provide an exercise training program to patients with peripheral arterial disease is essential, as almost half of these patients will eventually develop coronary artery disease. There are many existing treadmill walking protocols for patients with PAD. Most of these are based on claudication severity. We present a case of significant improvement of pain-free distance and maximum treadmill walking distance in a patient with PAD after 12 weeks of walking training program utilizing the “2/3 claudication distance” formula.

The Impact of Age and Gender on Severity and Types of Periodontal Diseases among Patients from Two Regions in Saudi Arabia

Background: Periodontitis is the predominant disease in the oral cavity and there are alterations in the periodontal tissues associated with the aging and gender of the patient. Objective: The present study was designed to assess the impact of age and gender of the patient on severity and types of periodontal diseases among patients from two regions in Saudi Arabia. Materials and Methods: The current study was done on 600 Saudi patients from Aseer and Tabuk regions in Saudi Arabia (50% males and 50% females) and divided into three equal groups (n = 200) according to the patients age: children and young patients (1 - 24 years old), adults patients (25 - 64 years old) and seniors patients (more than 65 years old). Plaque index (PLI), gingival index (GI), periodontal pocket depth (PPD) and clinical attachment loss (Cal) were recorded from all participants. The data were collected and analyzed with SPSS to determine the mean and standard deviation (±SD) and the values of significance (P ≤ 0.005). Results: The results of the present study revealed that 340 patients (56.7%) were affected with gingivitis dental biofilm induced and 260 patients were affected by periodontitis. Gingivitis dental biofilm-induced is especially detected in children and young patients (33.3%), adult patients (16.7%) and seniors patients (6.7%) whereas periodontitis cases were seen among the adult and seniors patients (16.7%) and (26.6%) respectively. Furthermore, the present study displayed the higher severity and prevalence of gingivitis dental biofilm-induced and periodontitis among females more than males in group II and group III maybe due to hormonal changes. The present study saw that there are statistically significant differences in clinical findings in the comparison between groups of this study (P ≤ 0.005). Risk Assessment Framework of mHealth System Vulnerabilities: A Multilayer Analysis of the Patient Hub

although there have been remarkable technological developments in healthcare, the privacy and security of mobile health systems (mHealth) still raise many concerns with considerable consequences for patients using these technologies. For instance, potential security and privacy threats in wireless devices, such as Wi-Fi and Bluetooth connected to a patient hub at the application, middleware and sensory layers, may result in the disclosure of private and sensitive data. This paper explores the security and privacy of the patient hub, including patient applications and their connections to sensors and cloud technology. Addressing the privacy and security concerns of the patient hub called for a comprehensive risk assessment by using the OCTAVE risk assessment framework. Findings reveal that the highest risk concerned data exposure at the sensory layer. In spite of the countermeasures presented in this paper, most served as a means to identify risk early as opposed to mitigating them. The findings can serve to inform users of the potential vulnerabilities in the patient hub before they arise.
