Pattern, Precipitants and Short Term Outcome of Heart Failure Patients Managed at Federal Medical Centre Nguru, a Tertiary Health Centre in Yobe State Northeastern Nigeria

Heart Failure (HF) is a syndrome characterized by typical symptoms (e.g. breathlessness, ankle swelling and fatigue) that may be accompanied by signs (e.g. elevated jugular venous pressure, pulmonary crackles and peripheral oedema) caused by a structural and/or functional cardiac abnormality, resulting in a reduced cardiac output and/or elevated intracardiac pressures at rest or during stress. Clinical studies investigating the aetiological patterns of heart failure in Sub-Saharan Africa (SSA) revealed that hypertension, cardiomyopathy and rheumatic heart disease account for more than two-thirds of cardiac disease cases. The objective of this study therefore is to assess the pattern, precipitating factors and short term outcome of heart failure among patients admitted into our hospital. Method: The study was a prospective cross-sectional type conducted among in patients with heart failure in the medical ward of the hospital. Results: A total of three and fifty four (354) subjects were recruited into the study, comprising one hundred and twenty nine males (36.4%) and two hundred and twenty five (63.4%) females. Majority of the patients were admitted in NYHA functional class IV. This however improved to class II at discharge though few were discharged in NYHA class III. Heart failure secondary to progressive hypertensive heart disease was the most common (39.0%) followed by peripartum cardiomyopathy (22.6%) while idiopathic dilated cardiomyopathy (11.0%) and rheumatic heart disease (7.3%) were the third and fourth causes of heart failure respectively. The most common precipitating factor for heart failure in this study was chest infection (44.9%). In conclusion, the study revealed that progressive hypertensive heart disease was the leading cause of heart failure, followed by peripartum cardiomyopathy while idiopathic dilated cardiomyopathy and rheumatic heart disease were ranked third and fourth causes of heart failure respectively. Evaluation of Temporal Changes in Deep Well Water Quality in Igabi Local Government Area of Kaduna State

The research work was carried out in Igabi Local Government of Kaduna State. Thirty water samples from boreholes were randomly collected within the major towns in November to April (Dry), and May to October (Rainy season). The samples were analyzed for physical, chemical, and bacteriological parameters and to observe any changes in the groundwater quality of the area. Analysis of variance (ANOVA), Pearson Correlation were performed on the data obtained using SPSS 10.0 for a window for significant variations and inter-element relationship. High mean turbidity was recorded in boreholes in Danfili (12.2 NTU), Kwarau (6.2 NTU). Mean turbidity of 4.3 NTU was recorded in Dry season for wells with Rigachikun having 5.1 NTU. It was observed that a negative correlation exists between well depth and heavy metal concentration. There was also a linear relationship of 0.686 between well distance and nitrate (faecal contaminant). Mean value for Fluoride (0.22 mg/l), Sodium (20.78 mg/l), Calcium (15.7 mg/l), Iron (0.013 mg/l), Nitrate (3.69 mg/l), Cadmium (0.00102 mg/l) Chloride (22.107 mg/l) recorded for sampled wells were all within WHO standard. A high value of 24 mg/l and 25 mg/l for Nitrate was recorded in Saminaka Road and Danfili. An indication of contamination during the rainy season indicates surface-groundwater intrusion (influx). The results of the bacteriological analysis indicated that most wells sampled have a range of 1 cfu/100 ml to 7 cfu/100 ml with Danfili Mani having the highest value (7 cfu/100 ml) which make it unsaved (not Potable).

非能动余热排出换热器(PRHR-HX)是AP1000反应器中PRHRs的重要安全部件之一,它在沸腾的条件下,通过对流换热的自然循环,将堆芯的热量输送到内置换料水箱(IRWST)中冷却。对PRHR-HX和IRWST的研究很多,但仍有许多问题需要研究,特别是两相流现象。本文选取单管模拟PRHR-HXC型管,管型几何形状与原型尺寸具有典型的对应关系,采用容积-功率比来确定单管加热体积,在按比例缩小的矩形IRWST中的水位与原型相同。采用ANSYSCFD-ICEM画网格,CFX求解器进行数值分析,瞬态单相流动,采用流体对流体界面模型来描述一次侧和二次侧之间的传热耦合。对单相流动的一次侧和二次侧进行了研究。分析了沿PRHR HX管的温度,换料水箱内温度、速度分布以及沿PRHR HX不同部位的传热系数。将CFD计算结果与ROSA-V大型试验装置(LSTF)的实验数据进行了对比,对模型进行了验证,并对PRHR-HX内侧单相流动换热关联关系进行了理论推导;并特别考虑了沿PRHR-HX二次侧的单相流动传热关系。HX内侧面温度从管口到管口急剧下降,IRWST中存在自然循环和分层现象,PRHR-HX 垂直管段的传热速率远大于水平管段。单管全尺寸随体积功率比缩放的IRWST能够很好地描述管束的热工水力特性。

Left Atrial Myxoma in a Woman with Postpartum Heart Failure: The Culprit or an Incidental Bystander

Myxoma is a benign and the most frequent tumor of the heart, accounting for approximately 30% of all primary cardiac tumors. Seventy five percent of atrial myxomas are in the left atrium. We present a case of a 20-year-old house wife presenting with biventricular heart failure and chest infection five months after her last child birth. Transthoracic echocardiography showed a mobile left atrial echogenic mass measuring 4.6 × 4.0 cm attached to the interatrial septum. Left ventricular systolic function was mildly reduced (EF 48%) with mitral and tricuspid incompetence. A diagnosis of left atrial myxoma with biventricular heart failure with chest infection was made. She improved on antibiotics and treatment of heart failure, and referred to Aminu Kano Teaching Hospital for further management.

Participatory Evaluation of the Relative Livestock Populations and the Assessment of the Status and Impacts of Newcastle Disease in Rural Communities of Two Northeastern States, Nigeria

Livestock is an important component of food security in many developing nations. However, animal diseases continue to undermine animal production and public health efforts thus widening poverty gap. Unfortunately, the conventional and what seems to be inefficient “top to bottom” disease surveillance and control policies are heavily relied upon such that negative impacts of diseases are underestimated. Participatory disease surveillance (PDS) techniques using semi-structured questioning assisted by key informants targeting focus groups were employed in 60 randomly selected villages of two Nigerian States during the years 2012 to 2014. Haemagglutination and ELISA tests to detect antibodies to Newcastle disease (ND) were conducted on 950 poultry sera. The status and economic burden of Newcastle disease and the relative livestock populations and some ethno-veterinary practices of these livestock farming communities were brought to the fore. Poultry, goats, cattle, sheep and pigs in descending order were the major livestock species kept in the study areas to which Kendalls Coefficient of Concordance (W = 0.9) agreed strongly. Accordingly, ND, lousiness, fowl pox and coccidiosis with percentage scores of ND-52%, Fowl Pox-31%, lousiness-17% and Salmonellosis-10% were important causes of poultry morbidity and mortality. ND sero-prevalence was 39%, relative morbidity;mortality and case fatality rates of 95%, 78% and 82% respectively were appraised. Again ND, coccidiosis, ectoparasitism and fowl pox were reported as seasonal poultry diseases (W = 0.6). Solanum nodiflorum and Momordica balsalmina were used to treat ND. Major livestock kept, and the ND status and effects in poultry in these livestock farming communities are here reported. Institutionalization of PDS would better inform strategic livestock policy reforms, and improve national food security and diseases surveillance and reporting system in Nigeria.

Penetrating Abdominal Injuries: Pattern and Outcome of Management in Khartoum

Background: The pattern and presentation of penetrating abdominal trauma vary according to places and the structure of the present health system. As well controversies in management exist ranging from mandatory exploration to selective non-operative management. Objectives: To determine the pattern of penetrating abdominal trauma, current management practiced and outcome in Khartoum. Patients and Methods: The study was prospective, descriptive and hospital-based. It was carried out at the main three hospitals in Khartoum State. It was conducted over a period of one year from 2012 Mar to 2013 Mar. All patients who presented penetrating injury to their abdomen were included. Results: The study included 85 patients with a mean age of 28 years (SD ± 10). The male to female ratio was 11:1. Most of the patients (89.4%) were in the first four decades of their life. Twenty-three patients (27.1%) presented shock. Stab wound is the commonest mode of trauma seen in 83.5% of our patients. The majority of our patients were managed by exploratory laparotomy (81.2%), however 16 (18.8%) underwent conservative measures. Of the operated group, solid organ injuries were found in 22.9%, yet hollow viscous injuries were reported in 86.9% of the patients. Registrars operated on 78.26% of the patients. The rate of negative laparotomy of this study was 8.7%. Complications and mortality were encountered in 25.9% and 4.7% respectively. The mean hospital stay was 8.47 days (SD ± 10.6). Conclusion: This study demonstrates no difference in the pattern of intra-abdominal injuries. The rate of operative treatment is acceptable, but more laparotomies can be avoided if the haemodynamic stable patients without features of peritonitis were given a period of observation. The overall outcome was satisfactory.

Conditional Heteroscedasticity in Streamflow Process: Paradox or Reality?

The various physical mechanisms governing the dynamics of streamflow processes act on a seemingly wide range of temporal and spatial scales;almost all the mechanisms involved present some degree of nonlinearity. Against the backdrop of these issues, in this paper, attempt was made to critically look at the subject of Autoregressive Conditional Heteroscedasticity (ARCH) or volatility of streamflow processes, a form of nonlinear phenomena. Towards this end, streamflow data (both daily and monthly) of the River Benue, Nigeria were used for the study. Results obtained from the analyses indicate that the existence of conditional heteroscedasticity in streamflow processes is no paradox. Too, ARCH effect is caused by seasonal variation in the variance for monthly flows and could partly explain same in the daily streamflow. It was also evident that the traditional seasonal Autoregressive Moving Average (ARMA) models are inadequate in describing ARCH effect in daily streamflow process though, robust for monthly streamflow;and can be removed if proper deseasonalisation pre-processing was done. Considering the findings, the potential for a hybrid Autoregressive Moving Average (ARMA) and Generalised Autoregressive Conditional Heteroscedasticity (GARCH)type models should be further explored and probably embraced for modelling daily streamflow regime in view of the relevance of statistical modelling in hydrology.

The present pandemic has posed a crisis to the economy of the world and the health ***,the race to expand research to understand some good molecular targets for vaccine and therapeutic development for SARS-CoV-2 is *** newly discovered coronavirus 2019(COVID-19)is a positive sense,single-stranded RNA,and enveloped virus,assigned to the beta CoV *** virus(SARS-CoV-2)is more infectious than the previously detected coronaviruses(MERS and SARS).Findings from many studies have revealed that S protein and RdRp are good targets for drug repositioning,novel therapeutic development(antibodies and small molecule drugs),and vaccine *** such as chloroquine,convalescent plasma,monoclonal antibodies,spike binding peptides,and small molecules could alter the ability of S protein to bind to the ACE-2 receptor,and drugs such as remdesivir(targeting SARS-CoV-2 RdRp),favipir,and emetine could prevent SASR-CoV-2 RNA *** novel vaccines such as mRNA1273(Moderna),3LNP-mRNAs(Pfizer/BioNTech),and ChAdOx1-S(University of Oxford/Astra Zeneca)targeting S protein have proven to be effective in combating the present *** exploration of the potential of S protein and RdRp is crucial in fighting the present pandemic.

The Effect of Gas Production, Utilization, and Flaring on the Economic Growth of Nigeria

The extent to which a country develops is anchored around its resources;and this is evidenced in how the revenue derived from natural resources (especially the oil and gas industries) has influenced national development strategies. This notwithstanding, the existence of natural resources does not always translate to development, as these often times lead to complacency and mismanagement;Nigeria may have suffered from this. This paper establishes the impact of gas production, utilization, and flaring on the estimated monetary value of the goods and services produced in Nigeria (GDP), using multiple linear regression analysis. The result shows that while gas utilization has a positive impact on the nation’s GDP, gas production and flaring are negatively associated with GDP. The paper concludes that for these to positively stimulate economic growth, there is need to invest more in infrastructure in the industry, and review the regulatory framework guiding operations of the oil and gas industry.
