A first search for the Ξ_(bc)^(+)J/ψΞ_(c)^(+) decay is performed by the LHCb experiment with a data sample of proton-proton collisions, corresponding to an integrated luminosity of 9 fb−1 recorded at centre-of-mass energies of 7, 8, and 13 TeV. Two peaking structures are seen with a local (global) significance of 4.3(2.8) and 4.1(2.4) standard deviations at masses of 6571 and 6694 MeV/c2, respectively. Upper limits are set on the Ξ+bc baryon production cross-section times the branching fraction relative to that of the B+c→J/ψD+s decay at centre-of-mass energies of 8 and 13 TeV, in the Ξ+bc and in the B+c rapidity and transverse-momentum ranges from 2.0 to 4.5 and 0 to 20GeV/c, respectively. Upper limits are presented as a function of the Ξ+bc mass and lifetime.

Cerebral Localization of Chronic Lymphocytic Leukemia simulating Progressive Multifocal Leukoencephalopathy: The Lessons from a Case

Background: Central neurological involvement is the most frequent extra hematological manifestation of chronic lymphocytic leukemia;it is multifactorial and rarely due to a cerebral localization of the disease. We report a case of cerebral localization of chronic lymphoid leukemia whose clinical and radiological aspects were very suggestive of progressive multifocal leukoencephalopathy. Case Presentation: A 65-year-old patient who was HIV-negative (human immunodeficiency virus), had consulted for bilateral axillary, cervical and inguinal lymphadenopathy associated with major asthenia and hyper lymphocytosis (lymphocyte count was 11 giga/l). Chronic lymphocyticleukemia with TP53 mutation was diagnosed and treatment with Ibrutinib 420 mg/day was initiated. After 2 months of treatment, the evolution was marked by the onset of neurological disorders whose clinical-radiological presentation and temporal evolution had led to the diagnosis of progressive multifocal leukoencephalopathy. In the absence of virological evidence in the cerebrospinal fluid analysis, a stereotactic biopsy of the brain lesions had been performed, making it possible to formally rule out this infectious hypothesis and to demonstrate cerebral invasion by tumour cells. Immuno-chemotherapy combining Rituximab-Cyclophosphamide-Doxorubicin-Vincristine-Prednisone-Ibrutinib (RCHOP-Ibrutinib) with intrathecal chemotherapy resulted in a very good clinical-radiological response. Conclusion: The appearance of neurological manifestations in the context of chronic lymphocytic leukemia must systematically lead to a search for a cerebral localization of the disease. In the absence of virological evidence in the cerebrospinal fluid, any suspicion of progressive multifocal leukoencephalopathy in this context should lead to the histological study of brain lesions.

Neurofibromatosis Type 1 in Four Children Cases

Neurofibromatosis Type 1 (NF-1 or Von Recklinghausen disease) is an autosomal dominant genetic disease, characterized by an extreme variability of its clinical expression which is also found in the same family. Our work focuses on the exploitation of four cases of patients with NF-1 who were enrolled in the paediatric neurology consultation at Rabat Children’s Hospital. They are two infants and two children. Otherwise the diagnosis was made in front of the existence of café au lait and lentiginous spots in two boys, also the existence of café au lait spots and abnormalities in brain imaging in two girls. Thus an evolution was marked by a favorable outcome for three patients and neurological sequelae in one patient.

The Relationship between serum 25-Hydroxyvitamin D and Resistin Levels in saudi Diabetic Patients

Objectives: The aim of this study was to investigate the relationship between the levels of 25-hydroxyvitamin D and the adipokine resistin in saudi diabetic patients and those at high risk to develop atherosclerotic cardiovascular diseases. Methodology: One hundred and sixty seven subjects were recruited for the cross-sectional study at King Fahad Medical City, saudi Arabia. Blood samples were analyzed for biochemical parameters. serum 25-hydroxyvitamin D and resistin levels were measured for all the participants. Result: Fifty six type-1, 55 type-2 diabetic patients and 56 healthy controls were recruited. serum 25-hydroxyvitamin D level in the control group was 44.21 ± 41.80 μg/ml compared to 50.67 ± 35.60 μg/ml in the diabetic group (P = 0.427) with no significant different between type-2 diabetes mellitus and type-1 diabetes mellitus (P = 0.628). Resistin level in the control group was 14.00 ± 7.39μg/ml compared to 20.21 ± 16.94 μg/ml in the diabetic group (P ≤ 0.01), no significant difference between type-2 diabetes mellitus and type-1 diabetes mellitus (P = 0.817). Resistin in those taking vitamin D supplementation was 21.34 ± 18.27 μg/ml;and in those not taking vitamin D supplementation, it was 17.4 ± 11.19 μg/ml;(P = 0.237). There was significant negative correlation between 25-hydroxyvitamin D and resistin in type-2 diabetes mellitus (P ≤ 0.043) and no significant correlation was found between vitamin 25 (OH) D and resistin level in type-1 diabetes mellitus (P = 0.538). Blood urea and creatinine showed significant positive correlation with vitamin 25 (OH) D in one hand and resistin level on the other hand. Discussion and Conclusions: several in vitro and in vivo studies have confirmed that the vitamin 25 (OH) D have numerous important functions in the body. The circulating vitamin 25 (OH) D and resistin levels were associated with general inflammation in renal diseases. Redo Cardiac surgery: Bleeding Control

Redo cardiac surgery increases?mortality and morbidity. The aim of this study was to determine if aprotinin was superior to tranexamic acid concerning control bleeding loss after redo valve surgery. A retrospective study was conducted from January 1994 until December 2014. 221 patients underwent redo cardiac valve surgery and separated into two groups: aprotinin group (n?=?85) and tranexamic acid group (n?=?136). Univariate tests were applied for data analysis. A total of 221 patients were enrolled in this study. This cohort was separated into two groups: aprotinin group (n?=?85) and tranexamic acid group (n?=?136). Euroscore in tranexamic acid group was higher: 5.96 ± 3.04 vs.?5.17 ± 2.83 in aprotinin group?(p?=?0.055). There was no statistical difference in postoperative mortality between the two groups (p?= 0.153). No statistical differences were reported concerning: total blood loss (p?= 0.51), red blood cells transfusion (p?= 0.215), reexploration for bleeding (p?= 0.537) and postoperative renal failure (p?= 0.79). There were statistical differences concerning mechanical ventilation time, which is longer in tranexamic acid group (p?= 0.008) and the use of inotropic drug support, which is more frequent in the tranexamic acid group (p?= 0.001). Our results demonstrated that tranexamic acid and aprotinin reduce transfusion requirement and blood loss. Due to financial reason, we chose tranexamic acid in preventing blood loss in redo valve surgery.

The first search for the doubly heavyΩ^(0)_(bc)baryon and a search for the E^(0)_(bc)baryon are performed using pp collision data collected via the LHCb experiment from 2016 to 2018 at a centre-of-mass energy of 13 TeV,corresponding to an integrated luminosity of 5.2 fb^(-1).The baryons are reconstructed via their decays to Λ^(+)_(c)π^(-)and E^(+)_(c)π^(-).No significant excess is fbund for invariant masses between 6700 and 7300 MeV/c^(2),in a rapidity range from 2.0 to 4.5 and a transverse momentum range from 2 to 20 MeV/*** limits are set on the ratio of the Ω^(0)_(bc)and E^(0)_(bc)production cross-section times the branching fraction to Λ^(+)_(c)π^(-)(E^(+)_(c)π^(-))relative to that of the Λ^(0)_(b)(E^(0)_(b))baryon,for different lifetime hypotheses,at 95%confidence *** upper limits range from 0.5 x 10^(-4)to 2.5 x 10^(-4)for theΩ^(0)_(bc)→Λ^(+)_(c)π^(-)(E^(0)_(bc)→Λ^(+)_(c)π^(-))decay,and from 1.4x 10^(-3)to 6.9 x 10^(-3)for theΩ^(0)_(bc)→E^(+)_(c)π^(-)(E^(0)_(bc)→E^(+)_(c)π^(-))decay,depending on the considered mass and lifetime of theΩ^(0)_(bc)(E^(0)_(bc))baryon.
