AIM: To assess the prevalence of nutritional disorders in children with inflammatory bowel disease(IBD) in saudi ***: The data from a national cohort of children newly diagnosed with IBD between 2003 and 2012 were analyzed. The diagnosis of IBD and the differentiation between Crohn's disease(CD) and ulcerative colitis(UC) were confirmed by gastroenterologists according to the standard criteria. The body mass index(BMI) of each child [weight(kg)/height^2(m)] was calculated at the time of diagnosis. The World Health Organization standards and references were used and the BMI for age > +1 and standard deviation score were used to define overweight and thinness, respectively. Age stratification analysis was performed to investigate any age-related variation in the prevalence of nutritional status between children s of age and ***: There were 374 children from 0.33 to 17 years of age, including 119(32%) children with UC and 255(68%) with CD. All of the children were saudi nationals, and 68(57%) of the UC and 150(59%) of the CD children were males. A positive history of anorexia at the time of diagnosis was found in 30(25%) patients with UC and 99(39%) patients with CD. The prevalence of thinness was 31%, 35% and 24% in children with IBD, CD and UC, respectively, with a significantly higher prevalence of thinness in children with CD than in children with UC(P = 0.037) only in the age group of 10-17 years(P = 0.030). The prevalence of overweight was 16 %, 15% and 20 % in the children with IBD, CD and UC, respectively, indicating a higher prevalence in UC that was statistically significant only in the age group of 10-17 years(P = 0.020). CONCLUsION: A high proportion of children with IBD presented with overweight instead of the classical underweight. Awareness of this finding is important for patient care.

Molecular Characterization of Extended spectrum <i>β</i>-Lactamase Genes in Clinical <i>E. coli</i>Isolates

Extended spectrum Beta-Lactamases (EsBLs) encoding genes (TEM, sHV and OXA) were amplified from multidrug resistance E. coli. The multidrug resistance E. coli isolates from different clinical sources were documented to be plasmid encoded and resistance against β-lactam and cephalosporin. Conventional laboratory analysis showed that seventy percent (70%) of the selected multidrug resistant clinical isolates were EsBLs positive, showing a ≥5 mm increase in zone diameter for either antibiotics compared to its zone when tested alone. The antibiotic susceptibility result showed that 100% of the isolates were resistant to amoxicillin-clavulanic acid, amoxicillin, cefuroxime and ampicillin-sulbactam while 90% of the isolates were resistant to ceftazidine and tetracycline, 80% to ofloxacin, 70% to ceftriazon, nalidixic acid, cefalexin, 60% to ciprofloxacin, 50% to nitrofurantoin, 40% to chloramphenicol and 20% to gentamicine. The multiplex PCR with primers TEM (931bp), sHV (868), OXA-2 (478), aac(3)-IIa (900) and rmtA (634), which are genes responsible for extended spectrum β-lactamase and aminoglycoside resistance in E. coli shows that: isolate W15 comprises of three (3) resistant gene, which corresponds with TEM resolving as a 931 base pair, sHV 868 base pair, and a 478 bp indicating OXA-2 that is faint probably indicating a low concentration of the gene. Isolate B2 comprises single resistant gene, which is interpreted as OXA-2 with 478 base pair while isolate URO2, U64 and s45 comprises of two resistance genes which resolve as 868 and 478 base pair indicating sHV and OXA-2 respectively. However, isolates s57, U58 and B7 showed no gene amplification despite the various degree of resistance in MIC and antibiotic susceptibility profile test obtained with conventional detection analysis. Fecal microbiota transplantation(FMT)has been used in a wide variety of *** researchers hypothesize that the dysbiosis of intestinal microbiota plays an important role in the development of gut-derived infections;thus,FMT is a potential therapeutic target against multiple organ dysfunction syndrome(MODs).A 106-year-old male patient was initially diagnosed with cerebral infarction and pulmonary *** the course of hospitalization,the patient developed *** patient received a single nasogastric infusion of sterile-filtered,pathogen-free feces from a healthy *** samples were collected every two days post-infusion to monitor changes in the microbiota composition in response to *** FMT,MODs and severe diarrhea were alleviated;the patient’s fecal microbiome diversity resembled that of the healthy donor’s fecal microbiome;moreover,his clinical symptoms improved remarkably with the changes in fecal ***,no observable side effects were noted during FMT *** findings warrant further investigation of FMT as a putative new therapy for treating microbiota-related diseases,such as MODs.

The prediction of formation pore pressure and fracture pressure gradients is a significant step towards the drilling *** this study,the formation pressures of twelve wells from NC98 field-sirte Basin(Waha Oil Company)were calculated by employing empirical methods,Eaton's equations,that depend on the real drilling and well-logging *** the results,the normal pore pressure in the NC98 field in sirte basin is 9.89 kPa/m(0.437 Psi/ft),and it is extending from the top of the wells in the investigated area to 2134 m(7,000 ft).A subnormal to normal pore pressure zone is noticed in the interval of 2134 m to 2743 m(7,000 ft.-9,000 ft).Then,slightly subnormal to somewhat abnormal(overpressure)region is seen from 2743–3414(9,000 ft.-11,200 ft).Beyond to that depth and down to the top of the reservoir,the overpressure zone was clearly *** on the results,the casing seating depth and the equivalent mud weight were simply determined for the area of study.

Deployment of Pre-Industrial Autonomous Microbe sensor in saudi Arabia’s Injection seawater system

Microbial growth in water injection systems can lead to many problems, including biofouling, water quality deterioration, injectivity loss, microbial corrosion, and reservoir formation damage. Monitoring of microbial activities is required in any mitigation strategy, enabling operators to apply and adjust countermeasures properly and in due time. In this study, the pre-industrial autonomous microbe sensor (AMs) was constructed with technical improvements from the prototype for increased sensitivity, durability, robustness, and maintainability. The pre-industrial AMs was lab validated, field proven, and deployed at critical locations of seawater injection network for automated detection of microorganisms under the saudi Arabia’s harsh environment. An excellent correlation between AMs measurement data (fluorescence count) and actual count of microbial cell number under microscope was established (coefficient of determination, R2 > 0.99) for converting AMs fluorescence count to cell numbers (cell mL-1) in the injection seawater. The pre-industrial AMs only required monthly maintenance with solutions refill, and was able to cope with hot summer months even without protection in an air-conditioned shelter. The study team recommended wider deployment of the online AMs for real-time monitoring of bacteria numbers in the various strategic locations in saudi Aramco’s complex seawater injection network, as an integral component of pipeline corrosion and leak mitigation program.
