The Diabetic Foot Research in Arabs’ Countries

Objective: To review all the studies on diabetic foot disorders (DFDs) that were published on the Pubmed? site aiming to identify the contributions of the different Arabs’ countries in the world scientific literature on this topic. methods: The Pubmed? site was searched using different key words for searching all the abstracts on Diabetes mellitus (Dm) and DFDs published from Arabs’ League countries (n = 22). For this review, the 22 countries were classified into 3 groups: Group 1 (G1): Gulf Council Countries (GCC) countries (n = 6), Group 2 (G2): African Arabs’ countries (n = 10), Group 3 (G3): Asian and/or Eastern mediterranean Arabs’ countries (n = 6). All the abstracts on Dm coming from all of the 22 Arabs’ countries were initially reviewed to locate the ones related to DFDs’ management. All of the articles related to DFDs were reviewed by the senior author. A publication index was created to allow a comparison between the productivity of various countries and correlate that to the population number. Results: By April 2012, a total of 906 articles were published on Dm, out of them 115 (11.6%) were related to DFDs. The largest number of Dm/DFDs research came from G1 countries (n = 437/51) followed by G2 (n = 307/38) and finally G3 (n = 162/26). The percentages of the studies related to DFDs were therefore: 11.6%, 12.3% and 20.6% respectively. Saudi Arabia was the top on the list of all studied countries with 31 studies related to DFDs out of the 187 on Dm (16.5%). Conclusion: more research on DFDs is needed in most of the Arabs’ countries particularly those in the GCC region which reported very high prevalence rates and are expected to hold these rates for the coming decades. Also, special attention is needed for those low-income Arabs’ countries that had no contributions in DFDs’ research.

Left Ventricular measurements in Black Sub-Saharan Africans and White maghreb

Background: Evidence that blacks have greater left ventricular mass (LVm) than whites has been demonstrated by large population-based American studies. However, to our knowledge, there is no study to date comparing LVm in Black Sub-Saharan Africans (BSSA) and the maghreb white population. We compared LVm measured echocardiographically in asymptomatic BSSA and maghreb. methods and Results: A total of 100 asymptomatic BSSA and 189 maghreb, (18 to 55 years old), underwent resting two-dimensional transthoracic echocardiography. LVm and geometry were assessed according to the 2015 American Society of Echocardiography and the European Association of Cardiovascular Imaging updated guidelines for cardiac chamber quantification. Crude or indexed LVm to body surface area or height2.7 was similar in BSSA and in maghreb (132.7 ± 37.0 vs. 134.2 ± 35.7 g;73.1 ± 17.8 vs. 72.9 ± 16.2 g/m2;32.1 ± 9.8 vs. 33.6 ± 9.5 g/m2.7). However, the left ventricular posterior wall was thicker in BSSA. Patterns of left ventricular geometry (normal, concentric remodeling, or concentric or eccentric hypertrophy) were equally distributed among the two ethnic groups. Conclusions: Left ventricular posterior wall thickness but not LVm is greater in BSSA than in maghreb.

The Relationship between Serum 25-Hydroxyvitamin D and Resistin Levels in Saudi Diabetic Patients

Objectives: The aim of this study was to investigate the relationship between the levels of 25-hydroxyvitamin D and the adipokine resistin in Saudi diabetic patients and those at high risk to develop atherosclerotic cardiovascular diseases. methodology: One hundred and sixty seven subjects were recruited for the cross-sectional study at King Fahad medical City, Saudi Arabia. Blood samples were analyzed for biochemical parameters. Serum 25-hydroxyvitamin D and resistin levels were measured for all the participants. Result: Fifty six type-1, 55 type-2 diabetic patients and 56 healthy controls were recruited. Serum 25-hydroxyvitamin D level in the control group was 44.21 ± 41.80 μg/ml compared to 50.67 ± 35.60 μg/ml in the diabetic group (P = 0.427) with no significant different between type-2 diabetes mellitus and type-1 diabetes mellitus (P = 0.628). Resistin level in the control group was 14.00 ± 7.39μg/ml compared to 20.21 ± 16.94 μg/ml in the diabetic group (P ≤ 0.01), no significant difference between type-2 diabetes mellitus and type-1 diabetes mellitus (P = 0.817). Resistin in those taking vitamin D supplementation was 21.34 ± 18.27 μg/ml;and in those not taking vitamin D supplementation, it was 17.4 ± 11.19 μg/ml;(P = 0.237). There was significant negative correlation between 25-hydroxyvitamin D and resistin in type-2 diabetes mellitus (P ≤ 0.043) and no significant correlation was found between vitamin 25 (OH) D and resistin level in type-1 diabetes mellitus (P = 0.538). Blood urea and creatinine showed significant positive correlation with vitamin 25 (OH) D in one hand and resistin level on the other hand. Discussion and Conclusions: Several in vitro and in vivo studies have confirmed that the vitamin 25 (OH) D have numerous important functions in the body. The circulating vitamin 25 (OH) D and resistin levels were associated with general inflammation in renal diseases. Renal Pleomorphic Sarcoma in Polycystic Kidney Disease: Case Report

We report a case of pleomorphic sarcoma in a 42-year-old man with adult polycystic kidney disease [APKD]. Abdominal ultrasonography, computed tomography and mRI scans have revealed heterogenous bilateral renal cysts with a voluminous mass in the upper pole left kidney. Radical left nephrectomy with histopathologic and immunohistologic examination have confirmed the diagnosis of pleomorphic sarcoma. Sarcoma associated with adult polycystic kidney disease is extremely rare and does not have particular radiological or biological features when compared to primary renal sarcoma.

The first search for the doubly heavyΩ^(0)_(bc)baryon and a search for the E^(0)_(bc)baryon are performed using pp collision data collected via the LHCb experiment from 2016 to 2018 at a centre-of-mass energy of 13 TeV,corresponding to an integrated luminosity of 5.2 fb^(-1).The baryons are reconstructed via their decays to Λ^(+)_(c)π^(-)and E^(+)_(c)π^(-).No significant excess is fbund for invariant masses between 6700 and 7300 meV/c^(2),in a rapidity range from 2.0 to 4.5 and a transverse momentum range from 2 to 20 meV/*** limits are set on the ratio of the Ω^(0)_(bc)and E^(0)_(bc)production cross-section times the branching fraction to Λ^(+)_(c)π^(-)(E^(+)_(c)π^(-))relative to that of the Λ^(0)_(b)(E^(0)_(b))baryon,for different lifetime hypotheses,at 95%confidence *** upper limits range from 0.5 x 10^(-4)to 2.5 x 10^(-4)for theΩ^(0)_(bc)→Λ^(+)_(c)π^(-)(E^(0)_(bc)→Λ^(+)_(c)π^(-))decay,and from 1.4x 10^(-3)to 6.9 x 10^(-3)for theΩ^(0)_(bc)→E^(+)_(c)π^(-)(E^(0)_(bc)→E^(+)_(c)π^(-))decay,depending on the considered mass and lifetime of theΩ^(0)_(bc)(E^(0)_(bc))baryon.

A first search for the Ξ_(bc)^(+)J/ψΞ_(c)^(+) decay is performed by the LHCb experiment with a data sample of proton-proton collisions, corresponding to an integrated luminosity of 9 fb−1 recorded at centre-of-mass energies of 7, 8, and 13 TeV. Two peaking structures are seen with a local (global) significance of 4.3(2.8) and 4.1(2.4) standard deviations at masses of 6571 and 6694 meV/c2, respectively. Upper limits are set on the Ξ+bc baryon production cross-section times the branching fraction relative to that of the B+c→J/ψD+s decay at centre-of-mass energies of 8 and 13 TeV, in the Ξ+bc and in the B+c rapidity and transverse-momentum ranges from 2.0 to 4.5 and 0 to 20GeV/c, respectively. Objective:To study the chemical profile,antimicrobial properties,and synergistic effect with known antibiotics of essential oil extracted from the marine red macroalgae Centroceras clavulatum(***)montagne,collected in ***:The chemical composition of the oil was analyzed by gas chromatography-mass *** oil was evaluated for antibacterial(Pseudomonas aeruginosa,Escherichia coli,Bacillus subtilis,micrococcus luteus,Staphylococcus aureus,and Klebsiella pneumoniae),and antifungal activity(Candida albicans,Candida glabrata,Candida krusei,and Candida parapsilosis),by the disc diffusion *** minimum inhibitory and minimum microbicidal concentrations of the oil were determined,as well as the synergistic effects of its application combined with the antibiotics ciprofloxacin and fluconazole,by the checkerboard ***:Thirty molecules were identified in the essential oil,comprising 96.27%of the total oil *** such as carvacrol(36.06%)were the most abundant compounds,followed by caryophyllene(14.67%),endo-borneol(9.04%),pyroterebic acid(3.23%)and caryophyllene oxide(3.13%).The oil exhibited a moderate antimicrobial activity with inhibition zone diameters ranging from 9.0 to 15.0 *** minimum inhibitory concentration values varied between 0.9 and 14.7 mg/mL,and Bacillus subtilis and Escherichia coli were the more sensitive bacteria with 0.9 and 1.9 mg/mL,*** minimum microbicidal concentration values ranged from 0.4 to 14.7 mg/mL.A significant synergic action was observed when the oil was applied in combination with ciprofloxacin and fluconazole,with fractional inhibitory concentration index values ranging from 0.31 to *** was found in 80%of the combinations and a 2 to 16-fold reduction of antibiotics mIC was ***:Our findings suggest that the essential oil of Centroceras clavulatum should be further appraised for its potential use in the management of multi-drug resistant microo

Cerebral Localization of Chronic Lymphocytic Leukemia Simulating Progressive multifocal Leukoencephalopathy: The Lessons from a Case

Background: Central neurological involvement is the most frequent extra hematological manifestation of chronic lymphocytic leukemia;it is multifactorial and rarely due to a cerebral localization of the disease. We report a case of cerebral localization of chronic lymphoid leukemia whose clinical and radiological aspects were very suggestive of progressive multifocal leukoencephalopathy. Case Presentation: A 65-year-old patient who was HIV-negative (human immunodeficiency virus), had consulted for bilateral axillary, cervical and inguinal lymphadenopathy associated with major asthenia and hyper lymphocytosis (lymphocyte count was 11 giga/l). Chronic lymphocyticleukemia with TP53 mutation was diagnosed and treatment with Ibrutinib 420 mg/day was initiated. After 2 months of treatment, the evolution was marked by the onset of neurological disorders whose clinical-radiological presentation and temporal evolution had led to the diagnosis of progressive multifocal leukoencephalopathy. In the absence of virological evidence in the cerebrospinal fluid analysis, a stereotactic biopsy of the brain lesions had been performed, making it possible to formally rule out this infectious hypothesis and to demonstrate cerebral invasion by tumour cells. Immuno-chemotherapy combining Rituximab-Cyclophosphamide-Doxorubicin-Vincristine-Prednisone-Ibrutinib (RCHOP-Ibrutinib) with intrathecal chemotherapy resulted in a very good clinical-radiological response. Conclusion: The appearance of neurological manifestations in the context of chronic lymphocytic leukemia must systematically lead to a search for a cerebral localization of the disease. In the absence of virological evidence in the cerebrospinal fluid, any suspicion of progressive multifocal leukoencephalopathy in this context should lead to the histological study of brain lesions.
