Drainage vs. non-drainage after cholecystectomy for acute cholecystitis:a retrospective study

many surgeons practice prophylactic drainage after cholecystectomy without reliable evidence,this study was conducted to answer the question whether to drain or not to drain after cholecystectomy for acute calculous cholecystitis.A retrospective review of all patients who had cholecystectomy for acute cholecystitis in Aseer Central Hospital,Abha,Saudi Arabia,was conducted from April 2010 to April *** were extracted from hospital case *** data included clinical presentation,routine investigations and liver function *** data included excessive adhesions,bleeding,bile leak,and drain *** cases such as pericholecystic collections,mucocele and empyema were also *** who needed therapeutic drainage were *** data included hospital stay,volume of drained fluid,time of drain removal,and drain site *** study included 103 patients allocated into two groups;group A（n = 38） for patients with operative drain insertion and group B（n = 65） for patients without drain *** number of patients with preoperative diagnosis of acute non-complicated cholecystitis was significantly greater in group B（80%） than group A（36.8%）（P 〈 0.001）.Operative time was significantly longer in group *** patients who were converted from laparoscopic to open cholecystectomy were in group *** analysis revealed that hospital stay was significantly（P 〈 0.001） longer in patients with preoperative *** was no added benefit for prophylactic drain insertion after cholecystectomy for acute calculous cholecystitis in non-complicated or in complicated cases.

Damming rivers causes two main problems affecting a river's geomorphic ecosystem and its water *** Tallowa Dam on the Shoalhaven River in southeastern NSW,Australia,provides a case study to find the best sustainable solutions to avoid these *** project uses a civil-infrastructure idea to design a grid of collector pipes from the reservoir bottom *** data analysis using ArcGIS 10.2 is used to determine the best grid location for the pipes in the *** and sediment samples have been analysed for grain size,heavy metal and organic matter *** dam has led to a significant decline in sediment transport and water discharge to the lower reaches of the river which has resulted in greater erosion of sediment and higher salinity rates within the lower river and coastal *** quality in the reservoir has been affected by increased sediment accumulation,particularly mud,which has increased the amount of heavy metal and nutrient pollution that could eventually affect the water users.A proposed solution is to use over-storage water to remove accumulated sediment from the base of the dam through a net of collector pipes controlled by automechanical gates,instead of flowing over the top of the *** would maximize the volume of upstream sediment and contained pollutants that can be released from the reservoir into the downstream river ecosystems as well as providing better water quality and a longer water storage time.

AIm: To assess the efficacy and safety of combinedpegylated interferon and ribavirin therapy in hepatitis C virus (HCV) infection in renal transplant recipients. mETHODS: This is a retrospective chart review of post renal transplant patients who were positive for anti-HCV and HCV-RNA, and who have received treatment with combination of pegylated interferon and ribavirin between October 2003 and December 2008. Only patients with stable graft function and absence of evidence of cirrhosis and who received the therapy for continuous 48 wk were included. Nineteen patients (13 male and 6 female) were identified and included. The patient's complete blood count, liver and kidney prof ile, and cal- culated glomerular f iltration rate (GFR) were monitored every 6-8 wk while on treatment. HCV-RNA was tested at 12 wk for early virological response, at 48 wk for end of treatment response (ETR), and then retested at 24, and 48 wk after completion of therapy for sustained virological response (SVR). Liver biopsies were obtained before treatment from all patients and graft kidney biopsies were performed as required. RESULTS: Of the entire cohort, 9 patients (47.4%) showed an ETR and 8 had SVR (42.1%). Of the 8 patients with abnormal alanine aminotransferase (alT) levels at baseline, 78.9% had their alT normalized (including the virological non responders). alT was normal in all responders at the end of therapy and at 24 wk post therapy (100%). Only one patient (5.3%) developed an increase in creatinine and decline in GFR from baseline towards the end of treatment. This patient's kidney biopsy revealed borderline rejection. There was no impact on response by HCV-genotype, initial HCV RNA load, age or sex of the patient or duration post transplant before commencement of therapy. all patients tolerated treatment in the same way as non-transplant with no unusual or increased occurrence of side effects. With technological advancements in 6G and Internet of Things(IoT), the incorporation of Unmanned Aerial Vehicles (UAVs) and cellularnetworks has become a hot research topic. At present, the proficient evolution of 6G networks allows the UAVs to offer cost-effective and timelysolutions for real-time applications such as medicine, tracking, surveillance,etc. Energy efficiency, data collection, and route planning are crucial processesto improve the network communication. These processes are highly difficultowing to high mobility, presence of non-stationary links, dynamic topology,and energy-restricted UAVs. With this motivation, the current research paperpresents a novel Energy Aware Data Collection with Routing Planning for6G-enabled UAV communication (EADCRP-6G) technique. The goal of theproposed EADCRP-6G technique is to conduct energy-efficient cluster-baseddata collection and optimal route planning for 6G-enabled UAV ***-6G technique deploys Improved Red Deer algorithm-based Clustering (IRDAC) technique to elect an optimal set of Cluster Heads (CH) andorganize these clusters. Besides, Artificial Fish Swarm-based Route Planning(AFSRP) technique is applied to choose an optimum set of routes for UAVcommunication in 6G networks. In order to validated whether the proposedEADCRP-6G technique enhances the performance, a series of simulationswas performed and the outcomes were investigated under different *** experimental results showcase that the proposed model outperformed allother existing models under different evaluation parameters.

The number of mobile devices accessing wireless networks isskyrocketing due to the rapid advancement of sensors and wireless communicationtechnology. In the upcoming years, it is anticipated that mobile datatraffic would rise even more. The development of a new cellular networkparadigm is being driven by the Internet of Things, smart homes, and moresophisticated applications with greater data rates and latency *** are being used up quickly due to the steady growth of smartphonedevices andmultimedia apps. Computation offloading to either several distantclouds or close mobile devices has consistently improved the performance ofmobile devices. The computation latency can also be decreased by offloadingcomputing duties to edge servers with a specific level of computing ***-to-device (D2D) collaboration can assist in processing small-scaleactivities that are time-sensitive in order to further reduce task delays. The taskoffloading performance is drastically reduced due to the variation of differentperformance capabilities of edge nodes. Therefore, this paper addressed thisproblem and proposed a new method for D2D communication. In thismethod, the time delay is reduced by enabling the edge nodes to exchangedata samples. Simulation results show that the proposed algorithm has betterperformance than traditional algorithm.

A Novel Lightweight Image Encryption Scheme

Encryption algorithms are one of the methods to protect dataduring its transmission through an unsafe transmission medium. But encryptionmethods need a lot of time during encryption and decryption, so itis necessary to find encryption algorithms that consume little time whilepreserving the security of the data. In this paper, more than one algorithmwas combined to obtain high security with a short implementation time. Achaotic system, DNA computing, and Salsa20 were combined. A proposed5D chaos system was used to generate more robust keys in a Salsa algorithmand DNA computing. also, the confusion is performed using a new *** proposed chaos system achieves three positive Lyapunov *** results demonstrate of the proposed scheme has a sufficient peak signalto-noise ratio, a low correlation, and a large key space. These factors makeit more efficient than its classical counterpart and can resist statistical anddifferential attacks. The number of changing pixel rates (NPCR) and theunified averaged changed intensity (UACI) values were 0.99710 and UACI33.68. The entropy oscillates from 7.9965 to 7.9982 for the tested encryptedimages. The suggested approach is resistant to heavy attacks and takes lesstime to execute than previously discussed methods, making it an efficient,lightweight image encryption scheme. The method provides lower correlationcoefficients than other methods, another indicator of an efficient imageencryption system. Even though the proposed scheme has useful applicationsin image transmission, it still requires profound improvement in implementingthe high-intelligence scheme and verifying its feasibility on devices with theInternet of Things (IoT) enabled.
