5-year follow-up of combined non-topography guided photorefractive keratectomy and corneal collagen cross linking for keratoconus

AIM： To evaluate the visual outcomes of simultaneous non-topography guided photorefractive keratectomy（PRK） and corneal collagen cross-linking（CXL） in eyes with keratoconus 5 y after the ***： Prospective, interventional, non-randomized, and non-controlled case series design was used. Sixty eyes of 30 patients（16 males and 14 females; age： 21-41 y） with mild, non-progressive（stages 1-2） keratoconus were enrolled. Refraction, uncorrected distance visual acuity（UDVA） and corrected distance visual acuity（CDVA）, flat and steep keratometry readings, and adverse events were evaluated preoperatively and postoperatively. Data were collected preoperatively and postoperatively at 3 mo, 1, 2, 3, 4, and 5 y follow-up visits after combined non-topography-guided PRK with CXL was performed. All patients had at least 5 y of ***： All study parameters showed a statistically significant improvement at 5 y over baseline values. The mean follow-up time was 68.20±4.71 mo（range： 60-106 mo）. Patients showed a significant improvement in UDVA from 1.24±0.79 log MAR prior to combined non-TG-PRK＋CXL to 0.06±0.15 log MAR postoperatively at the time of their last follow-up visit. CDVA significantly increased from 0.06±0.19 log MAR preoperatively to 0.03±0.12 log MAR postoperatively. A significant decrease in the mean spherical equivalent（SE） refraction was observed from-2.28±1.8 to-0.79±0.93 diopters（D）（P〈0.05）, and the manifest sphere decreased from-1.62±1.23 to-0.27±0.21 D（P=0.001）. The manifest cylinder significantly decreased from-1.73±0.86 to-0.29±0.34 D postoperatively（P=0.001）. The mean steep keratometry was 45.13±1.32 vs 47.28±2.12 D preoperatively（P〈0.05）, and the preoperative mean steepest keratometry（Kmax） 48.6±3.1 was reduced significantly to 46.8±2.9 postoperatively（P〈0.05）. CONCLUSION： Combined non-TG-PRK with 15 min CXL is an effective and safe option for correcting mild refractive error and improving visual acuity in patients with mild stable keratoconu

本文介绍了基于RS-485通信协议的远程智能办公控制系统的基本原理。该系统主要包括个人计算机、接口工具和收发装置。利用PC串行端口和ART电缆在PC遥控器和微控制器之间建立通信,当数据信号转换为RS-485信号后,可以远距离(高达4000英尺)发送和接收数据,实现信号长距离传输。本文借助Visual Basic平台,利用C语言对微控制器(pic16f960)进行程,利用RS-232实现PC远程控制器和微控制器(pic16f960)之间实现串行连接。基于上述方法,用户可以轻松读取驱动程序在LCD模块上发送的数据。此外,本文利用RS-485的操作原理将RS-232信号转换成RS-485信号,以便其长距离传输。接收器则将RS-485信号转换回原始信号,供单片机使用,进而实现用户远程控制办公设备的开启与关闭。本文研究了RS-485使用的双绞线电缆的电气性能,与其他电缆相比,双绞线电缆抗干扰能力强,价格相对便宜。通过对该系统样机的测试,验证了该系统理论的可行性。

Array signal processing concentrates on sensor arrays that receive signals in spatial *** application of array signal processing includes source number recognition and direction of arrival estimation（DOA）and so *** applications often use the uniform linear array（ULA）,that are often costly and difficult to provide large aperture,since the maximum number of resolvable sources for a ULA ofsensors ***,increasing the degree of freedom（DOF）based on the concept of Khatri-Rao（KR）product has attracted much attention in array signal *** typical array is nested array,which can achieve（~2）degrees-of-freedom（DOFs）by using only（）*** nested array is obtained from a combination of two ULAs,one ULA is a dense sub-array,that utilizes the classical inter-sensor spacing,while the other ULA is a sparse array,as it employs a sparser inter-inter-element spacing.A preferable sparse array usually has a closed-form expression for sensors locations,and a long central uniform linear array segment in difference co-array（DCA）.All those lead to more resolvable sources and high estimation *** this thesis,a novel nested array configuration is *** proposed array has a closed-form expression for its sensor locations,it extends the concept of the two-level nested array and exploits the difference co-array to increase the achievable *** spacing between the dense and sparse array,and the spacing between the sensors of sparse array change according to the total number of physical *** holes in the difference co-array are interpolated by using nuclear norm to exploit a full difference *** proposed array structure can improve the accuracy and achieve a higher resolution in direction-of-arrival（DOA）*** numerical simulation results eventually validate the efficacy of the proposed array.

目的:本研究采用连续3天的日七次毛细血管血糖(Capillary Blood Glucose,CBG)来评估短期血糖波动指标与视网膜病变(Diabetic Retinopathy,DR)之间的关系,进一步研究短期血糖波动指数在实际应用中的理论基础以及糖尿病患者可接受的短期血糖波动指数控制范围。其中短期血糖波动指标包括:范围内的时间(Time in Range,TIR)、血糖水平标准差(Standard Deviation of Blood Glucose,SDBG)、血糖变异系数(Coefficient of Variation of Blood Glucose,CVBG)及平均血糖(Mean Blood Glucose,MBG)。方法:选取2019年1月至2020年6月在中国医科大学附属盛京医院内分泌科住院并连续检测了3天血糖数值(3天内测血糖的总次数不小于21次)的180例2型糖尿病患者(Diabetes Mellitus Type 2,T2DM)作为研究对象,其中DR患者80例,非DR患者100例。首先,采用描述性分析方法描述研究人群的一般特征,连续变量采用均数±标准差或中位数(四分位数间距)表示,服从正态分布的数据采用t检验进行比较,对于不服从正态分布的数据,采用Mann-Whitney U检验进行比较;分类变量采用频数(百分比)表示,使用卡方检验进行比较。其次,通过相关性分析确定短期血糖波动指标与糖化血红蛋白(Glycated Hemoglobin,Hb A1C)及DR之间是否有相关性。然后,使用二元多因素Logistic回归模型分析短期血糖波动指数与DR之间的相关影响。最后通过ROC曲线下面积(AUC)来评估短期血糖波动指标对DR的预测情况。结果:(1)通过糖尿病视网膜病变患者和非糖尿病视网膜病变患者的比较,发现2组患者在TIR、SDBG、MBG,甘油三酯(Triglyceride,TG),Hb A1c和糖尿病持续时间上存在显著差异(P0.05)。(3)通过相关性分析还发现TIR、SDBG、MBG与DR之间也存在明显相关性(P0.05)。其中通过Chi-squared法检验发现TIR值越低,DR风险越大(χ2=15.82,P<0.01),而SDBG和MBG值越高,DR的风险反而越高(χ=17.30,χ=13.13,P<0.01)。(4)Logistic回归分析结果提示:在调整了Hb A1c,糖尿病的病程及甘油三酯等变量后,TIR,SDBG及MBG仍然是DR的相关危险因素。(5)ROC曲线分析结果表明,与SDBG(AUC=0.669,95%CI 0.588-0.749,P<0.01)和MBG(AUC=0.688,95%CI 0.609-0.766,P<0.01)相比,TIR显示出更大的曲线下面积(AUC=0.717,95%CI 0.642-0.792 P<0.01),TIR对T2DM患者DR诊断的敏感性和特异性最高(敏感性0.675,特异性0.660)。结论:短期血糖波动指标中的SDBG,MGB和TIR与糖尿病视网膜病变密切相关,其中TIR可能是一个潜在的糖尿病视网膜病变的预测因素。

Thymus serpyllum L.,often known as wild thyme,has been used since ancient times due to its multifaceted culinary and medicinal *** is usually utilized in folk medicine to manage different health *** work aimed to investigate the chemical composition and biological characteristics of *** essential oil(EO),including its antimicrobial,antioxidant,and anti-inflammatory ***,we have prompted an in-silico simulation to reveal the underlying mode of action of these *** chemical characterization of ***(EO)by Gas Chromatography-Mass Spectrometry(GC-MS)indicated sabinene(17.33%),terpinen-4-ol(11.73%),phellandral(13.18%),and thymol(10.54%)as main *** antimicrobial screening utilized the disc-diffusion technique,MIC,and MBC *** disc-diffusion test’s results revealed significant anti-Candida activity and notable antibacterial *** MIC and MBC tests showed that *** EO effectively stops bacterial growth,including Gram-positive and Gram-negative strains and Candida *** tolerance level ratio demonstrated that this EO exhibits bactericidal and fungicidal effects on all tested bacteria and Candida ***,*** EO presented effective inhibitory activity against the 5-lipoxygenase(5-LOX)enzyme(IC50=744.19±0.1µg/mL)(p<0.05).It also effectively affected FRAP,β-carotene,DPPH,and ABTS *** light of these findings,*** holds promise for diverse applications across pharmaceuticals,nutraceuticals,and the food ***,further research and collaboration between traditional knowledge and modern medicine are crucial to fully realizing its potential benefits in these fields.
