背景灌注指数是一种无创性的来自干血氧饱和度监测的反映外周循环灌注的指标。本研究评估了灌注指数做为探索成人丙泊酚麻醉过程中血管内注入包含15μg肾上腺素的硬膜外试验剂量药物后变化指标的有效性；同时将它与常规评估标准-心率（HR）（如果升高≥10bpm则为阳性）及收缩压（SBP）（如果升高≥15mmHg则为阳性）进行了可靠性比较。方法40例全静脉麻醉、择期行普外科手术的患者随机静脉注射3ml含5μg/ml肾上腺素浓度为15mg／ml的利多卡因或3ml生理盐水（每组n=20）。监测注射后5分钟的心率、收缩压和灌注指数（PI）。结果注射试验剂量在39±15秒后平均最大灌注指数降低65％±13％。心率和血压的最大增量分别为49±25秒后19±8bpm和102±34秒后17±7mmHg。用灌注指数作为血管内注射的评估指标（如果灌注指数较注射前降低≥10％则为阳性），其敏感性、特异性、阳性预期值和阴性预期值是100％（95％可信区间CI=83％-100％）。相对地，心率和血压指标的敏感性分别为95％（CI=76％-99％）和90％（CI=70％-97％）。结论灌注指数是评价成年丙泊酚静脉麻醉患者血管内注射硬膜外试验剂量药物后常规血流动力学变化的可靠的选用指标。

The use of terrestria. la.er sca.ning（TLS） in the ca.es ha. been growing dra.tica.ly over the la.t ***, TLS a.plica.ion to ca.e sta.ility a.sessment ha. not received much a.tention of *** study a.tempted to utilize rock surfa.e orienta.ions obta.ned from TLS point cloud collected a.ong ca.e pa.sa.es to（1） investiga.e the influence of rock geostructure on ca.e pa.sa.e development, a.d（2）a.sess ca.e sta.ility by determining a.ea. susceptible to different fa.lure types. The TLS point cloud wa. divided into six pa.ts（Entry ha.l, Cha.ber, Ma.n ha.l, shaft 1, shaft 2 a.d shaft 3）, ea.h representing different segments of the ca.e pa.sa.es. Furthermore, the surfa.e orienta.ion informa.ion wa. extra.ted a.d grouped into surfa.e discontinuity joint sets. The computed globa. mea. a.d best-fit pla.es of the entire ca.e show tha. the outcrop dips 290° with a.ma.or north-south strike. But a. individua. level, the pa.sa.es with dip a.gle between 26° a.d 80° a.e fea.ured with dip direction of 75°-322°. Kinema.ic tests revea. the potentia. for va.ious fa.lure modes of rock slope. Our findings show tha. toppling is the domina.t fa.lure type a.counting for high-risk rockfa.l in the ca.e, with proba.ilities of 75.26%, 43.07%a.d 24.82% in the Entry ha.l, Ma.n ha.l a.d shaft 2, respectively. Unlike shaft 2 cha.a.terized by high risk of the three fa.lure types（32.49%, 24.82% a.d 50%）, the cha.ber a.d shaft 3 pa.sa.es a.e not suffering from slope fa.lure. The results a.so show tha. the cha.a.teristics of rock geostructure considera.ly influence the development of the ca.e pa.sa.es, a.d four sections of the ca.e a.e susceptible to different slope fa.lure types, a. va.ying degrees of risk.

涡旋压缩机是一种新型的节能容积式压缩机,随着对它的研究越来越深入,其应用也越来越广泛。但是由于涡旋压缩机独特的结构和工作方式,使得准确分析涡旋压缩机零部件的受力变形与工作过程中压缩腔内工作介质特性变得困难,这不利于涡旋压缩机的生产和实际应用。（1）本文介绍了涡旋齿的啮合基本原理,阐述了啮合的基本准则;对基于渐开线几何形状的涡旋型线进行了分析;采用数学解析法对涡旋压缩腔的面积进行了计算;并用积分法求出了等截面涡旋压缩机工作腔容积随主轴转角的变化量;最后,利用SolidWorks软件对涡旋压缩机进行了三维建模,完成了涡旋压缩机各部件的设计、装配。（2）建立了基于能量守恒和质量守恒的数学模型;通过对数学模型的求解,得到了压缩机腔内的温度、压力和质量随主轴旋转角度的变化规律;根据对涡旋压缩机数学模型进行的定量分析,揭示了涡旋压缩机几何参数与性能的关系。（3）创建涡旋压缩机三维模型,基于热载荷和结构载荷对其边界条件进行约束,进行了有限元分析;模拟了涡旋压缩机工作时的变形和应力,研究了涡旋压缩机的静态与动态特性;考虑其实际工作状态对模型进行了简化;利用热载荷条件,得到了动、静涡旋的温度分布规律。结果表明,利用此种方法得到的涡旋压缩机的温度分布更符合实际环境。结果表明,采用本文方法得到的涡旋盘温度分布更符合实际情况;动涡旋和静涡旋应具有一定的刚度,以避免两涡旋相互干扰;进行了压缩机共振模态分析,确定了避免共振的压缩机转速范围。

Introduction: The ma.a.ement of fra.tures of the tibia.shaft is a. importa.t a.pect of orthopa.dic ca.e, a.d the selection of the surgica. method for fixa.ion ca. substa.tia.ly impa.t pa.ient outcomes. The current review a.ms to compa.e the outcomes of a.ult tibia.fra.tures trea.ed with solid na.ls to those trea.ed with hollow na.ls. Methods: a.sea.ch on Scopus, PubMed, a.d Cochra.e Libra.y, using three keywords (Outcome, Tibia.shaft fra.tures, Na.l) wa. conducted in a.ril 2023. Results were compiled a.d two independent reviewers screened a.d selected eligible a.ticles a.ter removing duplica.es, titles a.d a.stra.ts were rea. to exclude ineligible studies. Full-text a.ticles of the rema.ning pa.ers were rea. to select eligible studies which were further critica.ly a.pra.sed to a.certa.n their methodologica. qua.ity. The da.a.extra.ted from the selected pa.ers were synthesized using a.combina.ion of pooling of results, tests of sta.istica. difference (t-test a.d chi-squa.e) a.d na.ra.ive synthesis methods. Results: a.tota. of 2295 a.ticles were obta.ned from the da.a.a.es a.d cita.ion sea.ching. a.tota. of 9 pa.ers were identified a. eligible a.d included in the review. Findings revea.ed tha. there is no sta.istica. difference in the outcomes of tibia.fra.tures trea.ed with either solid or hollow na.l groups such a. dura.ion of surgery (p = 0.541), ra.e of dela.ed a.d non-union (p = 0.342), a.d ra.e of surgica. site infections (p = 0.395). Conclusion: Intra.edulla.y na.ling of tibia.shaft fra.tures with either solid or hollow na.ls ha.e simila. functiona. outcomes.
