A Study on the Feasibility of Total Quality Management Model and the Awareness and Attitude of the Managers of Health Care

Total Quality Management is not just an admirable phenomenon but it is any client’s right and can be done through contribution of the staff and the managers as well as the client’s of an organization. The purpose of the present study is to investigate the feasibility of TQM model and the attitude of the managers of health field of Yazd medical sciences university towards it. The present study was a descriptive and analytical one which was cross-section ally conducted from 2010 to 2011. The population under study consisted of the managers of the unit of staff vice-chancellor for health and treatment, the managers of health system and centers of cities and also the managers of Yazd’s public hospitals. 64 people were chosen using the formula for calculating the number of samples. A questionnaire with 48 questions was used. The questionnaire’s validity was approved by the faculty members whose major is management and its reliability by using α-cronbach 87%. To analyze the data, the software SPSS V.17 was used. Scoring method was based on likert. The findings of the study show that, considering feasibility of TQM, there is a significant relation between the awareness and attitude of the managers of health and treatment field on one side and the variables, age and University degree, on the other side p = 0.001. Considering binary logistic regression and the results obtained in the form of inter and backward stepwise, the most important factors affecting the feasibility of TQM among the managers of health and treatment field are the level of awareness and attitude with the predicted accuracy 79.3% and 87.9% respectively.

Helping Meet Reproductive Health Needs in Pakistan through a Client-centred Approach

Objective To test the feasibility of training clinic and facility based providers from the Health and Population Welfare Departments. Methods District Sargodha in Punjab province in Pakistan was selected for carrying out the project as it fulfilled the criteria of being a median district with generally similar characteristics as the rest of the country. A quasi-experimental design was used to test the effectiveness of the training programs. Two rounds of situation analyses were carried out both pre and post intervention. Results There was a positive attitudinal change in providers in the intervention areas as compared with the control sites. From a list of indicators it was seen that clinic based static providers considered it important to enquire who were the main decision makers were in the family, 60% in intervention versus 49 % in control, and if the client can persuade other members of the family to assist in solving her reproductive health needs, 19% in intervention and 3% in control. In case of community based providers, as compared with 16% in the control site, 76% in the intervention area considered seeking permission for examination to be important. They also felt that enquiring about the welfare of other family members was also necessary, 50% in intervention and 34% in control. Conclusion Training of providers could lead to attitudinal and behavioural changes that can help clients meet their reproductive health needs, and the intervention is feasible and replicable.

Is High Grade Prostatic Intraepithelial Neoplasia a Risk Factor for Prostate Cancer?—A Local Study

Objective: To determine the consequence of recognizing high grade prostatic intraepithelial neoplasia (HGPIN) & its extent on initial sextant prostatic biopsy then identifying its associated risk of finding prostate cancer on subsequent biopsy. Patients and methods: Seventy-one men were subjected to transrectal ultrasound guided sextant prostate biopsy due to elevated serum prostate specific antigen (***) > 4 ng /ml, an abnormal digital rectal examination (DRE) and/or transrectal ultrasound (TRUS) findings. The number, percentage, as well as bilateral and multifocal involvement of specimens positive for HGPIN were recorded in every patient. The percentage of cancer detected in these patients on repeat biopsy within 1 year of the initial biopsy was also recorded. Results: The mean age and mean *** level of our patients was 59.9 years and 7.9 ng/ml respectively. Of the 71 patients studied, initial biopsy revealed that (32.4%) had benign prostatic hyperplasia (BPH), (36.62%) had carcinoma, (25.35%) had HGPIN and (5.63%) had chronic prostatitis. On repeat biopsy within 1 year of initial biopsy cancer of the prostate was detected in 33.3% of our patients who were diagnosed with HGPIN on 1st biopsy. All of them had multifocal involvement on the initial biopsy. Conclusion: Recognizing HGPIN on 1st biopsy (particularly multifocal involvement) is associated with great risk of prostate cancer development on subsequent biopsy, thus comprehensive follow-up of these patients is necessary.

Some Important Features of Relativistic Charged Particles Produced in <sup>32</sup>S-Emulsion Interactions at 200 AGeV/c

An attempt has been made to study the multiplicity, angular and pseudo rapidity distributions of relativistic charged particles emerging from the interactions between sulphur and nuclear emulsion nuclei at 200 GeV/nucleon. The distributions from 200 AGeV are compared to the corresponding distributions from the predictions of Monte Carlo code FRITIOF samples. The pseudo rapidity distributions in different Nh-intervals translate to the target fragmentation region with increasing target mass. Finally, the scaling of multiplicity distributions of shower particles successfully describes the consequences of KNO scaling.

Carbon Dioxide Storage and Sequestration in Unconventional Shale Reservoirs

Carbon Dioxide (CO2) storage and sequestration in unconventional shale resources has been attracting interest since last couple of years due to the very unique characteristics of such formations have made them a feasible option for this object. Shale formations are found all around the world and the conventional assets are easily accessible, and also the huge move of operators toward developing unconventional reservoirs during past years leaves many of such formations ready for sequestering CO2. Today, the use of long horizontal wells that are drilled on a pad has the lowest amount of environmental footprint in which for storage and sequestration purpose also provides much more underground pore spaces available for CO2. In this paper we study the state of the art of the technology of CO2 storage and sequestration and provide different and fresh look for its complex phenomena from a mathematical modeling point of view. Moreover, we hope this study provides valuable insights into the use of depleted shale gas reservoirs for carbon sequestration, which as a result, a cleaner atmosphere will be achieved for the life of our next generations. Also, we present that the depleted shale gas reservoirs are very adequate for this purpose as they already have much of the infrastructure required to perform CO2 injection available in sites.

TNF-α release capacity is suppressed immediately after hemorrhage andresuscitation

Purpose： It has been suggested that patients with traumatic insults are resuscitated into a state of an early systemic inflammatory response. We aimed to evaluate the influence of hemorrhagic shock and resuscitation （HSR） upon the inflammatory response capacity assessed by overall TNF-α secretion capacity of the host compared to its release from circulating leukocytes in peripheral circulation. Methods： Rats （8/group） subjected to HS （MAP of 30e35 mmHg for 90 min followed by resuscitation over 50 min） were challenged with Lipopolysaccharide （LPS）, 1 mg/kg intravenously at the end of resuscitation （HSR-LPS group） or 24 h later （HSR-LPS24 group）. Control animals were injected with LPS without bleeding （LPS group）. Plasma TNF-α was measured at 90 min after the LPS challenge. In addition, whole blood （WB） was obtained either from healthy controls （CON） immediately after resuscitation （HSR）, or at 24 h post-shock （HSR 24）. WB was incubated with LPS （100 ng/mL） for 2 h at 37 C. TNF-α concentration and LPS binding capacity （LBC） was determined. Results： Compared to LPS group, HSR followed by LPS challenge resulted in suppression of plasma TNF-a in HSR-LPS and HSR-LPS24 groups （1835 ± 478, 273 ± 77, 498 ± 200 pg/mL, respectively）. Compared to CON the LPS-induced TNF-a release capacity of circulating leukocytes ex vivo was strongly declined both at the end of resuscitation （HSR） and 24 h later （HSR24）（1012 ± 259, 313 ± 154, 177 ± 63 ng TNF/mL, respectively）. The LBC in WB was similar between CON and HSR and only moderately enhanced in HSR24 （57 ± 6, 56 ± 6, 71 ± 5 %, respectively）. Conclusion： Our data suggest that the overall inflammatory response capacity is decreased immediately after HSR, persisting up to 24 h, and is independent of LBC.

Enhancing Biogas Production: Influence of Mixing Cow and Chicken Manures

The objectives of this study were to enhance the production of biogas through using a mixture of cow and chicken manure and to investigate the effect of digested manure on plant growth. Biogas digester consisted of a plastic polyethylene tank with a volume of 0.25 cubic meters. Cow and chicken manures were collected from certified animal and chicken farms. Mixture of manure was prepared by mixing 5 kg of chicken manure and 5 kg of cow manure together in 100 L plastic tank containing 90 L water free from chlorine. Percentage of manure in all cases was 10% (w/v). The manures were transferred to the digesters and kept in greenhouse to insure warm conditions (30°C - 39°C). Produced biogas was measured each two days and digestate samples were taken every two days for pH, EC, BOD, COD determination. Results showed increased biogas production and reached the highest quantity after 28 days and the amount of biogas produced from the mixture of cow and chicken manures was higher than the amount produced from each manure individually. Decreases in pH values were observed during biogas production followed by BOD, COD reduction. On the other hand, increases on EC values were observed. Application of digested manure in agriculture increased yield of lettuce by 75% comparing with the control sample. The study recommended using cow manure mixed with chicken manure to increase the production of biogas and to use the digested manure in agricultural.

A 10-Year Saudi Experience of Using Adalimumab in Treating Juvenile Idiopathic Arthritis

Background: Traditionally, management of Juvenile Idiopathic Arthritis (JIA) involves use of non-steroidal anti-inflammatory drugs (NSAIDS) or disease-modifying anti-rheumatic drugs (DMARDs), such as methotrexate (MTX) or sulfasalazine;or steroids. However, in several cases, a low therapeutic response or important side effects is encountered. This study reports our experience in using adalimumab in JIA patients by assessing the efficacy and safety of this treatment in this category of patients. Methods: A retrospective study was conducted among 38 patients with JIA at the Pediatric Department, King Abdulaziz Univesrity Hospital, Jeddah, Saudi Arabia, in the period January 2005-March 2016. Patients’ records were reviewed and relevant demographic and clinical data were collected. Data were analyzed using SPSS version 21 and represented using tables. Results: The 38 patients were distributed as 11 (28.9%) males and 27 (71.1%) females;mean ± SD age was 11.91 ± 4.54 (range = 3 - 19) years. Mean ± SD (range) disease duration was 3.26 ± 2.52 (0 - 12) years and most frequent diagnoses included polyarticular rheumatoid factor (RF) negative form 12 (31.6%), followed by systemic and oligoarticular JIA with 9 (23.7%) cases each. Before adalimumab, fever was present in 13 (34.2%) cases, followed by rash in 8 (21.0%) cases;while 21 (55.3%) were asymptomatic. Thirty-one (81.6%) were in failure of MTX, 19 (50%) of steroids, 7 (18.4%) of NSAIDS and 3 (7.9%) had had intraarticular injections. Biologically, ANA, RF and anti-CCP were positive in 22 (57.9%), 8 (21.1%) and 4 (10.5%) of the cases, respectively. Uveitis was present in 11 (28.9%) of the patients. Analysis of adalimumab efficacy showed 10 (52.6%) cases of complete remission, 9 (23.7%) of partial remission and 9 (23.7%) other where treatment was discontinued. Phosphate Desorption Characteristics of Some Representative Soils of Bangladesh: Effect of Exchangeable Anions, Water Molecules and Solution to Soil Ratios

Establishment of phosphate (P) retention and release capacity of soils is essential for effective nutrient management and environmental protection. In this experiment, we studied the influence of soil properties on P desorption and the relationship between phosphate sorption and desorption. Among the soil series, the Ghior soil had the highest percent clay (59.32%) and free iron oxide (15241 mg·kg–1) content. Along the catena of the calcareous soils, percent clay contents increased. For sorption study, the soils were equilibrated with 0.01 M CaCl2 solution containing 0, 1, 2, 4, 8, 16, 25, 50, 100 and 150 mg·P·L–1 solution. For desorption, three extractants namely, SO42- (0.005 M) as Na2SO4, HCO3- (0.01 M) as NaHCO3 and distilled water were used at extractant to soil ratios of 30:1, 60:1 and 100:1 (v/w). Among the sorption equations, the Langmuir equation showed better fit to the sorption data at higher P concentrations. The amount of phosphate desorbed by all the three extractants increased significantly with the increasing extractant to soil ratios. Phosphate desorption by and water molecules was highly correlated with pH, percent clay and free iron oxide content of the soil. Significant positive correlation (r > 0.64, P L). Phosphate desorption by SO42- and water molecules was also positively correlated with Freundlich constant, N (r > 0.67, P 0 (r > 0.72, P –0.77, P L). The results suggest that freshly sorbed phosphate ions (inner-sphere complex forming species) can be readily desobed by outer-sphere complex forming species like sulphate and bicarbonate ions. Water molecules also desorbed significant amount of freshly sorbed phosphate from the soil colloids.
