Prevalence of Malnutrition among Hospitalized Pediatric Patients in a Tertiary Hospital in Jordan: Pilot Study

Introduction: Failure to thrive (FTT) occurs when a child’s growth falls below expected standards, typically due to inadequate nutrient intake. Factors include insufficient oral intake, poor nutrient absorption, and increased metabolic demands. Previous studies have shown high malnutrition rates in hospitalized children due to illness-related metabolic demands, decreased appetite, and inadequate intake. Our objective was to assess the growth and nutritional status of children admitted to King Abdullah University Hospital (KAUH) and identify the prevalence of undernutrition and associated factors. Methods: This prospective, cross-sectional cohort study was conducted at KAUH from July to December 2022. We included children aged 2 months to 16 years, excluding those with conditions altering growth parameters. Data were collected through pediatric data sheets, physical examinations, and laboratory tests. Anthropometric measurements were taken, and growth was assessed using CDC growth charts. Nutritional anemia was defined as hemoglobin 16. Results: A total of 111 patients were included (56.8% male, median age 65 months). Most patients had normal height (82%) and weight (86.5%) upon admission. However, 14.4% were stunted, 11.7% were underweight, 14.4% were overweight, and 3.6% were obese. Growth impairment correlated with higher anemia rates (p = 0.042). Nutritional anemia was present in 12% of patients. No significant relation was found between breastfeeding history and growth retardation, but maternal perception of malnutrition correlated well with actual malnutrition. Conclusion: Our study found a malnutrition prevalence of 19.8%, highlighting the need for systematic nutritional screening in hospitalized children. The study’s limitations include its small sample size and specific patient population, suggesting the need for larger, multicenter studies for more generalizable results.

Elevated alanine aminotransferase activity is not associated with dyslipidemias,but related to insulin resistance and higher disease grades in non-diabetic non-alcoholic fatty liver disease

Objective:To explore demographic and metabolic factors associated with increased alanine aminotransferase(ALT)activity in non-diabetic non-alcoholic fatty liver disease(NAFLD)***:Overall 372 patients who consecutively attended to Gastroenterology Clinic of Baqiyatallah University of Medical Sciences,Tehran,Iran awere diagnosed as NAFLD entered into *** criteria were having diabetes mellitus and fasting blood glucose over126 mg/dL,active hepatitis B virus infection,having hepatitis C virus positive serology,and to be under corticosteroid *** levels were considered pathologically high when it was over30 IU/L for men and over 19 IU/L for ***:Bivariate analyses using t test and chisquare test showed that patients with pathologically augmented ALT levels had significantly higher NAFLD grades in their ultrasonographic evaluations(P=0.003).Moreover,these patients represented significantly higher homeostatic model assessment levels(P=0.003),levels of serum insulin(P=0.002),fasting blood glucose(P<0.001),and uric acid(P=0.02).The prevalence of insulin resistance was also higher in patients with increased serum ALT *** logistic regression models showed that ultrasonographic grading of NAFLD(P=0.027)and insulin resistance(P=0.013)were the only variables significantly associated with abnormal ALT ***:This study shows that the associations of increased ALT serum levels in NAFLD patients are different from what are supposed *** excluding diabetic patients from our population,we find that increased ALT levels are not associated with dyslipidemias but are independently associated with insulin resistance and NAFLD grading on ultrasonographic *** studies are needed to confirm our results.

Simulation of Maximum Power Point Tracking for Photovoltaic Systems

A PV （photovoltaic） solar panels exhibit non-linear current--voltage characteristics, and according to the MPT （maximum power transform） theory, it can produce maximum power at only one particular OP （operating point）; namely, when the source impedance matches with the load impedance, a match which cannot be guaranteed spontaneously. Furthermore, the MPP （maximum power point） changes with temperature and light intensity variations. Therefore, different algorithms have been developed for finding MPPT （maximum power point tracking） based on offline and online methods. Evaluating the performance of these algorithms for various PV systems operating under highly dynamic environments are essentials to ensure producing reliable, efficient, cost-effective and high performance systems. One possible approach for system evaluation is to use computer simulation. This paper addresses the use of Matlab software as a simulation tool for evaluating the performance of PV solar systems and finding the MPPT.

Intelligent Fault Detection of Retainer Clutch Mechanism of Tractor by ANFIS and Vibration Analysis

In this study, ANFIS, as decision support system, is applied to detect the faults of MF 285 mechanism tractor clutch. Maintenance mechanisms include normal mode, rolling element failure, seal failure and attrition-based. Experiments were carried out in three speeds: 1000, 15,000, 2000 RPM and two conditions. The sensor was mounted vertically and horizontally. Vibrating spectrum of the time domain and the frequency of vibration data were obtained. Thirty-three statistical parameters of vibration signals in frequency domain and time were chosen as the sources attribute to detect errors. Finally, the top three features as input vectors to the ANFIS were evaluated. Using statistical parameters the performance of the system was calculated with the experimental data and training of ANFIS model. The system could not provide a seal to identify the fault. Regardless of the vibration data obtained from the classification of the seal, the overall classification accuracy of the ANFIS was 99.14% in the amount of 100% of the sensor installed vertically and horizontally. The results showed that this system can be used as an intelligent diagnosis system.

Developing an Intelligent Fault Diagnosis of MF285 Tractor Gearbox Using Genetic Algorithm and Vibration Signals

This article investigates a fault detection system of MF285 Tractor gearbox empirically. After designing and constructing the laboratory set up, the vibration signals obtained using a Piezoelectric accelerometer which has been installed on the Bearing housings are related to rotary gear number 1 in two directions perpendicular to the shaft and in line with the shaft. The vector data were conducted in three different speeds of shaft 1500, 1000 and 2000 rpm and 130 repetitions were performed for each data vector state to increase the precision of neural network by using more data. Data captured were transformed to frequency domain for analyzing and input to the neural network by Fourier transform. To do neural network analysis, significant features were selected using a genetic algorithm and compatible neural network was designed with data captured. According to the results of the best output mode for each position of the sensor network in 1000, 1500 and 2000 rpm, totally for the six output models, all function parameters for MATLAB Software quality content calculated to evaluate network performance. These experiments showed that the overall mean correlation coefficient of the network to adapt to the mechanism of defect detection and classification system is equal to 99.9%.

Milk is very susceptible for growth and development of many microorganisms because it is rich in nutrients and has suitable condition. Thus, it is very vital to respect sanitary conditions at all stages of production and transport of raw milk. The objective of the present investigation was to determine the total bacterial, and acidity in raw milk from milk collection centers in Malayer and Nahavand cities. 52 milk samples were collected from 13 centers in summer 2012 and were analyzed according to standard methods. None of the raw milk samples under investigation, had superior or grade 1 quality. Only 7.7% of the samples were classified as raw milk with grade 2 quality and 92.3% were non-standard. The mean of microbial total count in base was 3.8 × 107 CFU/ml and average acidity in the samples was 0.163. The city of Tochqhaz and Aliabade-Damagh has the lowest and the highest microbial total count. In general the result of this study showed unsatisfactory conditions of milk in the milk collection centers of the Malayer and Nahavand cities.

Plasma concentration of Natriuretic Peptide (NP) is a valuable diagnostic tool for heart failure (HF). It can help rule out or confirm a diagnosis of HF based on symptoms, but its use is not clearly defined. NPs should be used in conjunction with physical examination and other diagnostic tests. However, it is important to note that several conditions besides the diagnosis of HF may cause NPs levels to be elevated. Additionally, there are situations when NP concentrations may be below diagnostic thresholds in [1]. This consensus statement aims to provide a straightforward diagnostic flowchart for clinicians in both the emergency department and outpatient settings to aid in diagnosing both acute and chronic HF. The diagnosis of acute HF can be ruled out with a BNP level of 100 pg/mL or NTproBNP level of 300 pg/mL, regardless of the patient’s age. To identify HF, a 3-level cut-off point based on the patient’s age is recommended. Chronic heart failure can be ruled out with a BNP level of 35 pg/mL or NTproBNP level of 125 pg/mL, regardless of thepatient’s age [1].

Effect of mild head injury on intelligence in Zahedan, Iran

Objective： To investigate the effects of mild head injury （HI） on the victims＇ intelligence by measuring their intelligence quotient （IQ）.Methods： This cohort study was performed in Khatamol-Anbia Hospital, Zahedan, Iran and the IQs of 30 mild HI patients were measured right after the injury （IQ0） and six months later （IQ6）. The IQs of 90 close relatives of the patients were also measured at the same period of time as the non-exposure group. The IQs were measured with Wechsler adult intelligence scale-revised （WAIS-R）. The IQ0, IQ6 and their differences （IQ change） were compared in HI patients and their relatives using the Student＇s t ***： The mean IQ0 of the HI patients was similar to their relatives. The IQ6 of HI patients appeared to be less than those of their relatives. Moreover, the IQ6 of the HI patients appeared to be less than their initial scores. HI was associated with more decrease in IQ6 compared with IQ0and the female subjects showed more decrease in IQ6 compared with their ***： HI seems to be associated with decrease in IQ six months after the injury and it is more evident in female HI patients.
