Juvenile Idiopathic Arthritis and Its HLA Association: A Study from Bangladesh

Background: HLA alleles may have association with overall JIA including specific sub-types. Determation of HLA DRB1, DPB1, DQA1, DQB1 and B27, may be helpful to diagnose JIA cases where diagnostic dilemma is present. The aim of the study was to find out the association of HLA alleles with JIA and its subtypes. Methods: This cross sectional study was conducted in the department of Pediatrics, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, from April 2020 to March 2021. A total of 50 cases and 25 controls attending pediatric rheumatology clinic and pediatric OPD were enrolled in the study. Data were collected by a pre-designed questionnaire containing socio-demographic, clinical and laboratory parameters. HLA DR-B1, DQ-A1, DQ-B1 and B27 typing were done for the cases and controls from blood samples by polymerase chain reaction sequence specific primer (PCR-SSP) method. Data were analyzed by SPSS version 26. Frequency, percentage, chi square test and Fisher exact tests were done for statistical analysis. Results: HLA DR-B1_10 had significant positive association whereas HLA DR-B1_03 and B1_16 had negative association with overall JIA cases. Poly-articular JIA, oligo-articular JIA and ERA had association with HLA DRB1_01, B1_08 and B1_12 respectively. HLA DQA1-01 and A1_02 had positive association. However, DQ-A1_06 was negatively associated with JIA cases. Oligo JIA was positively associated with HLA DQ-A1_01, whereas poly JIA and ERA had association with A1_02. Though, HLA DQ_B1 had no association with overall JIA cases, analysis showed association of poly JIA with DQ-B1_04. Frequency of HLA DQ-B1_02 and 03 were higher among ERA cases, though not significant, but they had strong association with HLA B27. Conclusion: HLA associations were present with overall JIA and each sub-groups had different patterns of HLA associations including some protective roles.

Background: Methotrexate (MTX) is the most effective and commonly used disease-modifying anti-rheumatic drug in the management of juvenile idiopathic arthritis. Several patients develop side effects, which may lead to low quality of life and non-compliance to MTX. To reduce MTX-induced side effects, folic acid supplementation is prescribed by most rheumatologists. Even after that, some patients have symptoms while receiving MTX. Objectives: To assess the efficacy of folinic acid in comparison to folic acid for reducing the side effects of MTX in JIA patients. Material and methods: In this prospective observational study, newly diagnosed cases of JIA who would be getting MTX were included by purposive sampling. Data were collected using a predesigned questionnaire. Among 40 patients, 20 received folinic acid (Group A), and 20 received folic acid (Group B). Disease activity levels were assessed by JADAS-27 (Juvenile Arthritis Disease Activity Score). Contents from the MISS (MTX Intolerance severity score) questionnaire were used to assess the side effects. All patients were evaluated at baseline, 6th, and 12th weeks. Results: There were significant differences in the frequency of MTX-related adverse events between folinic acid (Group A) and folic acid (Group B). Group A patients only had nausea (10% and 15% in the 6th & 12th week respectively) and vomiting (5% at both follow-ups). On the other hand, in addition to nausea (70% and 95% in the 6th & 12th week) and vomiting (20% and 90% in the 6th & 12th week), folic acid group patients had restlessness, crying, and irritability. Self-discontinuation of MTX was present in the folic acid group (5% & 10% in the 6th & 12th week). Improvement of disease activity was more in the folinic acid group. Conclusion: The folinic acid group had significantly fewer side effects. Improvement of disease activity was more and compliance was also better among them. Background:Growing pain (GP) is the most common form of nonspecific, recurrent leg pain in children aged 4 - 12 years. The exact etiology of GP is not known. However, some studies have found an association between vitamin D and Bone Mineral Status (BMD) status with GP in their study. Objectives: To assess the serum level of vitamin D, and BMD and to determine their association with growing pain in children. Methods: This cross-sectional analytical study was conducted in the Department of Paediatrics, Bangabandhu Sheikh Mujib Medical University (BSMMU). Sixty children between the age of 6 - 12 years were included in the study from March 2020 to August 2021. Children who fulfilled the Evans criteria of GP were enrolled as cases and thirty age and sex matched healthy children were recruited as the control in the study. Informed written consent was obtained from patients and parents. Serum 25-hydroxy-vitamin-D levels and BMD were performed among cases and controls and subsequently compared to see their association in growing pain. A preformed semi-structured questionnaire was completed for each participant which included socio-demographic, clinical and laboratory characteristics. Appropriate statistical tests were applied for data analysis and performed by SPSS version 22. A p-value less than 0.05 was considered as significant at a 95% confidence interval. Results: In this study, 96.7% of growing pain patients had hypovitaminosis D and among them, the majority (86.7%) was vitamin D deficient. There was a significant association between vitamin D with GP compared to healthy control. BMD was significantly lower in the lumbar vertebra (L1 - L4) and femoral neck region (both right and left) among GP children compared to the control group. Soil heavy metal pollution, influenced by both natural and anthropogenic factors, significantly reduces environmental quality. In this study, Cr, Ni, Cu, As, Cd, and Pb in eight different land-use soils from Patuakhali District in Bangladesh were assessed. Concentrations of Cr, Ni, Cu, As, Cd, and Pb in soils were 1-87, 5-271, 4-181, 0-80, 0.2-24.0, and 5-276 mg kg-1, respectively, measured using an inductively coupled plasma-mass spectrometer. The enrichment factor, pollution load index (PLI), and contamination factor (CJ) of metal i were used to assess the ecological risk posed by metals in soils. The PLI ranged from 0.78 to 2.66, indicating baseline levels to progressive deterioration of soil due to metal contamination. However, C;of Cd ranged from 1.8 to 12.0, which showed that the studied soils were strongly impacted by Cd. Considering the severity of the potential ecological risk of a single metal, the descending order was Cd > As > Pb > Cu > Ni > Cr. Soils under all land uses showed moderate to very high potential ecological risk.

·AIM:To identify various risk factors that may play a significant role in the development of congenital nasolacrimal duct obstruction(CNLDO).·METHODS:This observational case-control study included a case group of 122 children less than two years of age with CNLDO who underwent probing and irrigation treatment at the ophthalmology department of Imam Khomeini Hospital in Ahvaz,Iran,from June 2022 to June2024.A control group of 122 age-matched children without CNLDO was also included for *** was collected from the children's medical records.·RESULTS:The study found a significant correlation between the occurrence of CNLDO and several maternal factors,such as preeclampsia,the use of levothyroxine,hypothyroidism,having more than three pregnancies(gravidity>3),natural pregnancy,and gestational diabetes ***,in children,factors,such as oxygen therapy,anemia,reflux,jaundice,and a family history of CNLDO in first-degree relatives were associated with CNLDO,and maternal preeclampsia and hypothyroidism were found to significantly increase the risk of developing CNLDO in children.·CONCLUSION:Given that CNLDO affects both premature and full-term children,the present findings may potentially facilitate the early identification of children and infants at risk of nasolacrimal duct obstruction,thereby preventing the onset of chronic dacryocystitis.

Erosion and runoff reduction potential of vetiver grass for hill slopes:A physical model study

Severe erosion is caused by intense rainfall in tropical regions. The erodible soil of steep hill slopes,accompanied by destruction of vegetation due to human interventions results in accelerated erosion. A sustainable and cost-effective solution such as vetiver grass(Chrysopogon zizanioides) is, thus, required to control the erosion process. In the current study, 6 small-scale glass models: 1 bare and 5 with vetiver grass, having a slope angle of 37°have been constructed. One year after planting, artificial rainfall of extremely high intensity was applied to all 6 small models and the role of vetiver canopy and roots in erosion and runoff control was observed. To see the effect of soil texture, one among these 5 models was made with silty sand and others contained sandy silt. The results demonstrated that, for sandy silt, the inclusion of vetiver reduced the soil loss by 94%-97%, and soil detachment rates were lowered by 95%.The average runoff also was reduced by 21%. The canopy cover showed a positive impact on reducing both quantities. An increase in average root diameter from 1.6 to 2.5 mm increases the soil loss due to its negative impact on added cohesion. The added cohesion showed a linearly negative correlation with soil loss. A composite system of vetiver and jute geotextile was most effective in erosion reduction among 4vegetated models with sandy silt. Under same vetiver planting layout, the grass covered model of silty sand yielded 84% lower erosion and 62.5% lower runoff than the grass covered one with sandy silt. Thus,vetiver was more effective in erosion and runoff reduction for soil with a greater percentage of sand, and soil type dominated the erosion process.

Biomass estimation using allometric models is a nondestructive and popular *** of an allometric model can influence the accuracy of biomass *** Forest Department initiated a nationwide forest inventory to assess biomass and carbon stocks in trees and *** relationship between carbon storage and sequestration in a forest has implications for climate change mitigation in terms of the carbon sink in *** part of the national forest inventory,we aimed to derive multi-species biomass models for the hill zone of Bangladesh and to determine the carbon concentration in tree components(leaves,branches,bark and stem).In total,175 trees of 14 species were sampled and a semi-destructive method was used to develop a biomass model,which included development of smaller branch(base dia<7 cm)biomass allometry and volume estimation of bigger branches and *** best model of leaf,branches,and bark showed lower values for adjusted R2(0.3152-0.8043)and model efficiency(0.436-0.643),hence these models were not recommended to estimate *** best fit model of stem and total aboveground biomass(TAGB)showed higher model efficiency 0.948 and 0.837,respectively,and this model was recommended for estimation of tree biomass for the hill zone of *** best fit allometric biomass model for stem was Ln(Stem)=-10.7248+1.6094*Ln(D)+1.323*Ln(H)+1.1469*Ln(W);the best fit model for TAGB was Ln(TAGB)=-6.6937+0.809*Ln(D^2*H*W),where DBH=Diameter at Breast Height,H=Total Height,W=Wood *** two most frequently used pan-tropical biomass models showed lower model efficiency(0.667 to 0.697)compared to our derived TAGB *** best fit TAGB model proved applicable for accurate estimation of TAGB for the hill zone of *** concentration varied significantly(p<0.05)by species and tree *** concentration(48-49%)of carbon was recorded in the tree stem.
