Millions of cataract surgeries with intraocular lens(IOL) implantation are performed worldwide. Although cataract surgery brings many benefits to the patients, the risk of various complications is still a concern. One of the infrequent adverse events but potentially affecting on patients’ visual acuity and contrast sensitivity is losing the transparency of IOL. IOL opacification may lead to IOL removal or exchange, which is unpleasant to both the patient and the surgeon. Several reports of acute IOL clouding are available in the literature describing various etiologies of this phenomenon, however, the exact mechanism remained unclear in some cases. Herein, we aimed to review the causes and outcomes of intraoperative and early postoperative IOL opacification.

BACKGROUND Methanol is a highly toxic,non-potable alcohol.Outbreaks of methanol toxicity occur due to its fraudulent addition to alcoholic beverages as a cheaper substitute for ethanol.Recently,alongside the coronavirus disease 2019(COVID-19)pandemic,rumors circulated on social media that consuming alcohol can prevent or cure the virus,leading to a COVID-19 and methanol-induced optic neuropathy(MON)syndemic.AIM To investigate the impact of erythropoietin(EPO)on the outcomes of patients diagnosed with MON.METHODS In this prospective study,105 patients presenting with acute bilateral visual loss secondary to methanol intoxication were enrolled from March to May 2020 at Farabi Eye Hospital.A comprehensive ocular examination was conducted for all participants.Recombinant human EPO and methylprednisolone were administered intravenously to all patients for three consecutive days.RESULTS The mean age of the participants was 39.9 years(±12.6).Ninety-four patients were male and eleven were female.The mean pre-treatment best corrected visual acuity(BCVA)improved from 2.0±0.86 to 1.39±0.69 logarithm of the minimum angle of resolution post-treatment(P<0.001),with significant improvement observed in all age categories and genders(P<0.001).Visual acuity improvement was also significant regardless of whether the patient presented before or after 72 h(P<0.001),and the post-treatment BCVA remained significant at all monthly follow-up visits(P<0.001).CONCLUSION EPO and methylprednisolone therapy have been shown to be effective in improving visual outcomes in patients with MON when administrated within the first month of exposure.Public awareness efforts are necessary to prevent further outbreaks of methanol toxicity in the current COVID-19 era.

Coronavirus disease 2019(COVID-19) pandemic has caused significant changes in ophthalmic practice. The initial strategy of strict restriction of elective activities has been replaced with various guidelines to revitalize ophthalmic procedures considering the new safety concerns. In this manuscript, we reviewed recent recommendations for ophthalmic practice in different fields of ophthalmology during the COVID-19 pandemic.

Cell-based therapies for limbal stem cell deficiency: a literature review

Background and Objective:Limbal stem cell deficiency(LSCD)is characterized by the insufficiency of limbal stem cells to maintain the corneal epithelium.Severe cases of LSCD may be treated with limbal transplantation from healthy autologous or allogeneic limbal tissue.Multiple cell-based therapies have been studied as alternative treatments to improve success rates and minimize immunosuppressive regimens after allogeneic transplants.In this review,we describe the success rates,and complications of different cell-based therapies for LSCD.We also discuss each therapy’s relative strengths and weaknesses,their history in animal and human studies,and their effectiveness compared to traditional transplants.Methods:PubMed was searched for publications using the terms LSCD,cell-based therapy,cultivated limbal epithelial transplantation(CLET),cultivated oral mucosal epithelial transplantation(COMET),and mesenchymal stem cells from 1989 to August 2022.Inclusion criteria were English language articles.Exclusion criteria were non-English language articles.Key Content and Findings:current cell-based therapies for LSCD are CLET and non-limbal epithelial cells.Non-limbal epithelial cell methods include COMET,conjunctival epithelial autografts,and mesenchymal stem/stromal cells(MSCs).Moreover,several alternative potential sources of non-limbal cells have described,including induced pluripotent stem cells(iPSCs),human embryonic stem cells(hESCs),human dental pulp stem cells,hair follicle bulge-derived epithelial stem cells,amniotic membrane epithelial cells,and human umbilical cord lining epithelial cells.Conclusions:Cell-based therapies are a promising treatment modality for LSCD.While CLET is currently the only approved cell-based therapy and is only approved in the European Union,more novel methods have also been shown to be effective in human or animal studies thus far.Non-limbal epithelial cells such as COMET are also an alternative treatment to allogeneic transplants especially as a surface stabilizing procedure.iPSCs are cu

Comparison of anterior segment measurements using rotating Scheimpflug imaging and partial coherence interferometry

AIM:To compare central corneal thickness(CCT) and anterior chamber depth(ACD) measurements using rotating Scheimpflug imaging and partial coherence interferometry.·METHODS:As part of the first phase of Shahroud Eye Cohort Study with 5 190 subjects of 40 to 64 years of age,CCT and ACD were measured using Scheimpflug imaging with the Pentacam(Oculus,Inc.,Lynnwood,WA,USA) and partial coherence interferometry with the Allegro BioGraph(Wavelight,Erlangen,Germany).·RESULTS:After applying exclusion criteria,we had data of 4 387 subjects with a mean age of 50.7±6.2 years.Mean CCT with Pentacam and BioGraph were 528.6 ±33.2μm and 525.6 ±32μm respectively;the difference was statistically significant(P <0.001),but the correlation was high(R=0.920).Mean ACD measurements using Pentacam and BioGraph were 2.68 ±0.35mm and 2.62 ±0.33mm respectively;the inter-device difference was significant(P <0.001) with high correlation(R=0.944).The 95% limits of agreements between devices were-22.65μm to28.61μm and-0.16mm to 0.29mm for CCT and ACD measurements,respectively.·CONCLUSION:For both CCT and ACD,the BioGraph gave significantly lower values than the Pentacam(P <0.05).Despite the high inter-device correlation,the 95%limits of agreements were wide,and this may limit their interchangeability in measuring the CCT and ACD.

AIM: To study the distribution of ocular higher-order aberrations(HOAs) and mesopic pupil size in individuals screened for refractive surgery. · METHODS: Ocular HOAs and mesopic pupil size were studied in 2 458 eyes of 1 240 patients with myopia, myopic astigmatism and compound myopic astigmatism and 215 eyes of 110 patients with hyperopia, hyperopic astigmatism and compound hyperopic astigmatism using the Zywave aberrometer (Busch& Lomb). All patients had correctable refractive errors without a history of refractive surgery or underlying diseases. Root-mean-square values of HOAs, total spherical aberration, total coma and mesopic pupil size were analyzed. Ocular HOAs were measured across a ≥ 6.0 mm pupil, and pupil size measurements were performed under the mesopic condition. · RESULTS: The mean values of HOAs, total spherical aberration and total coma in the myopic group were 0.369 μm, ±0.233, 0.133±0.112μm and 0.330±0.188μm, respectively. In the hyperopic group the mean values of HOAs, total spherical aberration and total coma were 0.418μm±0.214, 0.202±0.209μm and 0.343±0.201μm, respectively. Hyperopes showed greater total HOAs (P <0.01) and total spherical aberration (P <0.01) compared to myopes. In age-matched analysis, only the amount of total spherical aberration was higher in the hyperopic group (P =0.05). Mesopic pupil size in the myopic group was larger (P ≤0.05). · CONCLUSION: The results suggested that significant levels of HOAs were found in both groups which are important for planning refractive surgeries on Iranians. There were significantly higher levels of total spherical aberration in hyperopes compared to myopes. Mesopic pupil size was larger in myopic group.

AIM：To determine characteristic features of ocular trauma resulted from self-trauma by writing instruments among pediatric population.METHODS：Thirty-six children who suffered from selfinflicted ocular trauma with a writing instrument were included in this prospective cross-sectional study.RESULTS：The mean age was 5.6±2.7y with male：female ratio of 1.77.The right eye was involved two times more than the left eye.The superomedial（55.5%）and inferomedial（30.6%）quadrants were the most common sites of injury.The leading culprit was colored pencils（44.4%）.During surgical exploration,no foreign body（FB）was found in 25（69.4%）patients while an FB was found in 11（30.5%）patients.Brain injury was present in two patients（5.6%）and only in superomedial quadrant injuries.Zone 1 was the most common site for ocular trauma associated with penetrating injury.The mean ocular trauma score（OTS）in penetrating injuries was 3.8±1.2.The best corrected visual acuity（BCVA）was 0.3±0.6 upon admittance and 0.08±0.21 after one year.The final BCVA was significantly correlated with the entrance site,better final BCVA was found in nasal entrance site（P〈0.05）.CONCLUSION：The ophthalmologists should keep a high index of suspicion to rule out penetrating eye injuries related to writing instruments in a young uncooperative child.Brain injury is a life-threatening event that should be ruled out by appropriate imaging.Medial canthal area as the most common site needs an especial attention in writing instrument injuries.

Dear Editor,We would like to report complications of intrastromal bevacizumab injection for corneal neovascularization(CNV)after lamellar keratoplasty(LK)in two patients.CNV is a significant risk factor for graft rejection.CNV leads to graft failure by disturbing the immune function of the cornea after penetrating keratoplasty(PKP).Studies reported that more than fifty percent of graft failure occurs in this condition[1].

INTRODUCTIONUrrets-Zavalia was first described as a syndrome consisting of a fixed,dilated pupil with iris atrophy following penetrating keratoplasty（PKP）in 1963 and back then it was thought that this syndrome was only related to keratoconus patients.Other findings that were not essential for the diagnosis were posterior synechiae,ectropion uvea,pigment dispersion,anterior subcapsular lens opacities and secondary glaucoma syndrome.
