Factors associated with recurrence of clubfoot treated by the Ponseti method

AIM To assess several associated factors on the recurrence of clubfoot after successful correction by the Ponseti method. METHODS A total of 115 children with 196 clubfeet deformities, treated by the Ponseti method, were evaluated. Demographic data, family history of clubfoot in firstdegree relatives, maternal educational level and brace compliance were enquired. Based on their medical files, the characteristics of the patients at the time of presentation such as age, possible associated neuromuscular disease or especial syndrome, severity of the deformity according to the Dimeglio grade and Pirani score, residual deformity after previous Ponseti method and number of casts needed for the correction were *** There were 83 boys(72.2%) and 32 girls(27.8%) with a male to female ratio of 2.6. The mean age at the initiation of treatment was 5.4 d(range: 1 to 60 d). The average number of casts applied to achieve complete correction of all clubfoot deformities was 4.2. Follow-up range was 11 to 60 mo. In total, 39 feet had recurrence with a minimum Dimeglio grade of 1 or Pirani score of 0.5 at the follow-up visit. More recurrence was observed in non-idiopathic clubfoot deformities(P = 0.001), noncompliance to wear braces(P < 0.001), low educational level of mother(P = 0.033), increased number of casts(P < 0.001), and more follow-up periods(P < 0.001). No increase in the possibility of recurrence was observed when the previous unsuccessful casting was further treated using the Ponseti method(P = 0.091). Also, no significant correlation was found for variables of age(P = 0.763), Dimeglio grade(P = 0.875), and Pirani score(P = 0.624) obtaining at the beginning of the serial casting. CONCLUSION Using the Ponseti method, non-idiopathic clubfoot, noncompliance to wear braces, low educational level of mother, increased number of casts and more followup periods had more association to possible increase in recurrence rate after correction of clubfoot deformity.

Objective:Germline mutations in mismatch repair(MMR)genes cause Lynch syndrome(LS).LS is an inherited disease,and an important consequence of MMR deficiency is microsatellite instability(MSI)*** phenotype influences the efficacy of5 fluorouracil(5-FU)***,cost effective,and easy to perform laboratory tests are required to include MSI detection in routine laboratory *** of CAT25 as monomorphic short tandem repeat sequence enables CAT25 to be an efficient screening tool among hereditary nonpolyposis colorectal cancer(HNPCC)patients compared with other methods used ***:Based on Amsterdam II criteria,31 patients in 31 families were shortlisted from a total number of 1,659 colorectal cancer *** status was examined in these patients using CAT25 and a commercially available Promega MSI five-markerbased detection system as well as immunohistochemical(IHC)staining of four important MMR *** were scored as high microsatellite instable(MSI-H),low(MSI-L),or stable(MSS).MSI status determined by CAT25 single mononucleotide marker was compared with that of five mononucleotide markers,Promega commercial kit,and IHC ***:MMR protein deficiency was observed on 7/31 probands using IHC methodology and 6/31 categorized as MSI-H using commercial kit or CAT25 single *** sensitivity and specificity of the CAT25 single marker were the same as those detected by five-marker Promega commercial kit in our ***:Based on our results,the performance of the CAT25 single mononucleotide marker for MSI status determination in our HNPCC patients is the same as that of the five-marker-based commercial kit.

A numerical study of propagation of cohesive fluid mud gravity currents in the form of lock-exchange was done using the OpenFOAM open source *** Eulerian approach solution for three separate phases was developed by incorporating a rheological model to predict the front position of cohesive fluid mud gravity *** model also simulates features in the complete movement phases including slumping,self-similar,and viscous in which the dynamics of propagation are affected by the balance of viscous and buoyancy forces,and the inertia force is *** influence of using different turbulence models containing sub-grid scale(SGS),modified SGS,detached eddy simulation(DES),delayeddetached eddy simulation(DDES),Launder-R eece-Rodi(LRR),and k-ε models on the accuracy of simulation results was evaluated by comparing with available experimental *** results show that the selection of the proper turbulence model is one of the most important issues for this type of the numerical *** more efficient turbulence model was suggested and tabulated for each stage of propagation and different selected concentrations of 1,045,1,140,and 1,214 g/*** different turbulence models(except k-ε) lead to front propagation dynamic simulation results that are in good agreement with the experimental measurements in the early stage of propagation for low concentrations,only DES,SGS,and modified SGS are able to capture the Kelvin-Helmholtz instability vortex shapes at the dense fluid interface,which is the main characteristic of the gravity current through the slumping *** calculated accuracies of SGS and modified SGS in predicting gravity current propagation for the both self-similar and viscous phases also are slightly better than DES,DDES,and LRR model *** results of this study confirmed the performance and efficiency of the modified SGS model in which the interaction coefficients between phases are calibrated for the numerical modeling of fluid mud

AIM: To evaluate the gastric emptying inhibitory effects of sugar and levodopa on H pylori eradication period. METHODS: A total of 139 consecutive patients were randomized into 6 groups. The participants with peptic ulcer disease or non-ulcer dyspepsia non-responding to other medications who were also H pylori-positive patients either with positive rapid urease test (RUT) or positive histology were included. All groups were pretreated with omeprazole for 2 d and then treated with quadruple therapy regimen (omeprazole, bismuth, tetracycline and metronidazole); all drugs were given twice daily. Groups 1 and 2 were treated for 3 d, groups 3, 4 and 5 for 7 d, and group 6 for 14 d. Groups 1 to 4 received sugar in the form of 10% sucrose syrup. Levodopa was prescribed for groups 1 and 3. Patients in groups 2 and 4 were given placebo for levodopa and groups 5 and 6 received placebos for both sugar and levodopa. Upper endoscopy and biopsies were carried out before treatment and two months after treatment. Eradication of H pylori was assessed by RUT and histology 8 wk ***: Thirty patients were excluded. Per-protocol analysis showed successful eradication in 53% in group 1, 56% in group 2, 58% in group 3, 33.3% in group 4, 28% in group 5, and 53% in group 6. Eradication rate, patient compliance and satisfaction were not significantly different between the groups. CONCLUSION: It seems that adding sugar or levodopa or both to anti H pylori eradication regimens may lead to shorter duration of treatment.

The major and trace elements and Sr-Nd-Pb isotopes of Miocene host granitoid rocks and their mafic microgranular enclaves（MMEs） were studied to understand the petrogenesis of MMEs in the Kashan complex, which is part of the Urumieh-Dokhtar magmatic belt（Iran）. The host rocks consist of quartz-diorite and tonalite associated with a dioritic intrusion. The enclaves show microgranular texture and the same mineralogy as their respective host with plagioclase, quartz and biotite. MMEs have a diorite to quartz-diorite composition and show geochemical characteristics mostly between their granitoid host and the diorite intrusion. Chondrite-normalized REE patterns of all samples are moderately fractionated [（La/Yb）N=2.1 to 12.9]. The MMEs display in part small negative Eu anomalies（Eu/Eu＊=0.54 to 0.99）, with enrichment of LILE and depletion of HFSE. The enclaves show emplacement depth of -4 to 6 km which is comparable with the host rocks. Moreover, the Hornblende-plagioclase equilibrium temprature of MMEs yields average temperatures of 795℃ which is slightly higher than the host ones. Identical mineral compositions and Nd-Sr-Pb isotopic features of MME-host granitoid pairs indicate interactions and parallel evolution of MME and enclosing granitoid in the Kashan plutons. Additionally, the geochemical and isotopic investigations of host and dioritic intrusions suggest a common source for their genesis. A thermal anomaly induced by underplated basic magma into a hot crust would have caused partial melting in the lower crust to generate Kashan granitoid rocks.

A surface Ti-WC composite was fabricated on CP-Ti by surface friction stirring(SFS)using a pinless WC-Cotool at a processing window of 800−2500 r/min and 8−50 mm/*** 1600 r/min-50 mm/min,a defect-free compositelayer with an average hardness of~HV 1170 is *** hardness was increased by WC and TiN reinforcingparticles,dissolved Co atoms in Ti,and the formation of ultrafine *** particles were incorporated into the Tisubstrate owing to the intense frictional interaction/heating at the tool-plate interface(~1000℃),which led to strengthloss and wear of the *** Williamson-Hall analysis of the XRD peaks of the SFSed sample confirmed a significantlysmall crystallite size(~100 nm).Wear tests showed that the wear resistance of the composite structure was about 4.5times higher than that of the *** analysis revealed a significant reduction in average value and fluctuations ofthe friction coefficient.

Jiroft region is one of the most historical cities, placed in the southeastern of Iranian plateau. Its history dates back to the second millennium B.C. The ancient history of Jiroft and its proximity to the River ofHalil Roud have attracted archeologists during the recent decades. So far various historical works have been unearthed from the region of which marble ones are especially noteworthy. Basically, marble works were used more commonly in central plateau of Iran than other ones. The marble works discovered in Jirofl region are only comparable with those from the recently excavation at Shahr-I Sokhta in Zabol. The scientific examination of such works may pave way to identify their internal structures and to compare them in terms of their provenance. The present study will focus on the characterization of marbles discovered in Konar Sandal, Jiroft. In addition to petro graphical studies, XRD and XRF investigations were used to identify the crystalline structures as well as balk chemical composition of the discovered marbles. The obtained results revealed that the major constituents of the discovered marbles in Jiroft were Ca and CaO. The structure of discovered marbles had orderly compact patterns resulted mainly from metamorphism conditions. This structure is actually Unsimilar to those from Shahr-I Sokhta.

Dikes in the north of Saveh are located in a region with an area of approximately 200 square kilometers, in a distance of approximately 100 kilometers south of Tehran. Dikes mentioned in terms of petrological composition, are divided into two categories: alkaline and intermediate to acidic. Alkaline dikes include: andesitic basalt and andesite and intermediate to acidic dikes include: trachyte and trachyandesite. In terms of geochemical, dikes in the north of Saveh have a dual nature of alkaline and calc-alkaline. Both groups are derived from more enrichment source than primitive mantle. Despite similarity of pattern of both groups, varieties of alkaline having less silica, in the elements Sr, Ti, Nb and Ta show more enrichment and in the elements Hf, Rb, Th, K show less enrichment than varieties of calc-alkaline.

Given special tectonic and climatic conditions, Iran has the most diverse land-forms and beautiful geological phenomena, which could attract many scientific, adventurous tourists and geological researchers. The present study aims at studying and introducing wonderful geomorphological attractions and numerous geotourism potentials of very hot paradise of the earth, e.g. Dasht-e Lut of Iran, a desert which contains a collection of exclusive desert relieves and geological and geomorphological records of the world. Such unique perspectives as the biggest Nebkhas of the world, the largest Yardangs or Kaluts of the world, the hottest point of the earth, as well as rare species of desert structures such as salt marshes and salt polygons, Gandom Beryan Hill, massive sand dunes, Barchans, clay zones in pretty shapes, saline river , etc., present a vast and diverse extension of geographical beauties and phenomena of the desert. Using library studies, interpretation of satellite images and field studies, this study has been conducted. The results of the studies show that Lut Desert has considerable capability for carrying out complementary studies as well as effect on economic development process in national and international levels. In addition, geotourism development obstacles in Lut Desert are identified, and such solutions as development of ways for access to attractions, erection of geotourism development centers, advertisement and introduction of the desert's attractions, dispatch of guides with tourists, promotion of the region security, extension of accommodations and… for better use of the potentials of the desert, are provided. Furthermore, towards developing the region geotourism, some other fun and exciting activities such as sand skiing, racings and desert rallies, endurance cycling, sun bath, star observation, camel riding and so on, are suggested.
