Prostate Cancer in the Thies Region, Senegal: Epidemiological, Diagnostic and Therapeutic Aspects

Introduction: Prostate cancer is the leading urological cancer. It is the most common cancer in men over 50. Objective: To determine the epidemiological, diagnostic and therapeutic characteristics of prostate cancer in hospitals in the Thiès region. Patients and Methods: We conducted a descriptive study from January 1st, 2015 to December 31st, 2020. We included all cases of primary prostate cancer confirmed on histology. Results: We collected data on 318 cases of primary prostate cancer during the study period. Mean patient age was 72.7 years (Range: 49;94 years). Family history of prostate cancer was found in 22 patients (6.91%). The average consultation time was 18.6 months. The circumstances of discovery were dominated by obstructive voiding disorders (97.16%). Digital Rectal examination was suggestive in 55.40% of patients. PSA level was above 20 ng/ml in 76.7% of patients. Prostatic adenocarcinoma was the only histological type (100%). Localized cancer represented 7.2% and locally advanced cancer occurred in 36.5% of cases, while metastatic cancer accounted for 56.3%. Radical prostatectomy was performed in 3.18% of cases. Mortality rate was estimated at 8.50% after 1 year. Conclusion: Prostate cancer is the leading urological cancer in the Thies region. It is characterized by the predominance of locally advanced and metastatic forms.

Overview of Adrenals Tumors in Dakar Hospitals

Introduction: Adrenal tumours (AT) are commonly encountered in clinical practice. For any patient presenting an adrenal mass, there are two crucial points to consider: is the adrenal mass malignant and is it hormonally active? The objective of our study was to evaluate the epidemiological, diagnostic, therapeutic, and evolutionary aspects of AT in a developing country. Methodology: We conducted a retrospective study spanning 17 years (from January 2005 to October 2022) in four Dakar hospital services. We included all patient medical records with explored AT. Results: AT was diagnosed in 35 patients who had a mean age of 36.62 years (range: 12-79), and a female predominance (26 women to 9 men). Among these patients, 17.2 % had incidental AT. Hypertension was the commonest presenting symptom in 27 patients (77%), which was associated with Mnards triad in 14 patients (40%). Abdominal pain was reported by 20 patients (57%), and 14 presented with an abdominal mass. Among the patients, 29 had functional AT: 22 with pheochromocytoma, 5 with primary hyperaldosteronism, and 2 with hypercortisolism. Imaging evaluations revealed that 33 patients had unilateral AT and one had bilateral AT. Tumour sizes varied, 5 patients had tumours 10 cm. Two patients presented with metastases. The therapeutic approach involved adrenalectomy in 32 patients. Perioperative complications were observed in five patients, including haemorrhage (two patients), hypotension (two cases), and hypertensive crisis (one case). One patient with bilateral pheochromocytoma developed acute adrenal insufficiency postoperatively, followed by abdominal herniation. Histopathological examination confirmed pheochromocytoma in 21 patients, Conns adenoma in 5, adrenocortical carcinoma in 3, cortisol adenoma in 2, and adrenal cyst in 1 patient. Immunovirological Discordance and Associated Factors among People Living with HIV under Antiretroviral Treatment at Hôpital de Jour de Donka, Guinea

The antiretroviral treatment (ART) has significantly reduced the number of new HIV/AIDS infections and related deaths. However, cases of immunovirological discordance (IVD) are found in various locations. The objective of this study was to determine the profile of People living with HIV (PLHIV) with IVD and to identify associated factors. We conducted a cross-sectional study based on the records of PLHIV under ART for at least 6 months, followed at Hpital de Jour Donka from 2015 to 2017, and having both viral load (CV) and CD4 T-cell count. Prevalence of IVD was 34.57%, with 23.87% for immunological discordance (ID) and 10.7% for virological discordance (VD). Females were predominant (66.26%), and male gender influenced IVD with a statistically significant difference (p = 0.006) and was associated with VD (p = 0.007). The average age was 38.77 11.30 years. PLHIV were classified at WHO stages 3 and 4 (86.01%). The median initial haemoglobin level was 11.5 g/L [3.2 - 12]. The mean initial CD4 T-cell count was 272.84 cells/mm3 201.6. The median initial viral load (VL) was 147,337 copies/mL [1092 - 31,675,000]. The initial CD4 T-cell count 3 was associated with IVD with a statistically significant difference (p = 0.0009) and correlated with ID (p = 0.000). Prurigo was associated with IVD with a statistically significant difference (p = 0.003). Cerebral toxoplasmosis was not associated with IVD but was associated with ID (p = 0.04). This study allowed us to describe the profile of PLHIV with IVD. The main associated factors were male gender, initial CD4 T-cell count 3, toxoplasmosis, prurigo, and herpes zoster.

Connective tissue diseases (CTDs) are Autoimmune diseases (AIDs) characterized by the appearance of autoantibodies, which are diagnostic markers. Investigations of these autoantibodies play a major role in the management of several autoimmune diseases. The objective of this study was to describe the profile of anti-ENA antibodies according to the clinical symptoms of mixed CTDs in Conakry teaching Hospital. We performed a cross-sectional study during six months. A total of 20 patients was recruited and we measured antibodies using the ELISA technique. The mean age of our patients was 36.5 years, with a predominance of females. Cutaneous and rheumatological signs were the main clinical manifestations. SLP was the most frequent CTDs;the threshold of ENA antibodies positivity was higher in scleroderma with and SLP. Anti-ENA identification reveals the frequency of anti-SSA (83.33%), anti-U1RNP (66.66%) and anti-histone (50%) antibodies. Antinuclear antibodies (ANA) react with various components of the cell nucleus. Their detection is of major interest in the diagnosis of CTDs. Our results highlight the importance of determining the specificity of these antibodies to guide differential diagnosis.

Neonatal Hemoperitoneum: Diagnosis Always Delicate

The hemoperitoneum is a rare and severe disease in neonatal period that can cause a condition of hemorrhagic shock. We presented the case of an infant hospitalized for hemodynamic disorders which could to suspect maternofetal infection. The clinical deterioration with the appearance of shock signs, anemia, abdominal distension and Ultrasound allowed to correct diagnosis. Labour dystocia (apart from unnoticed minimal obstetrical trauma) does not seem to be responsible for hemoperitoneum as the majority of the reported cases. According to current recommendations, the treatment of this condition is primarily based on reanimation measures. Surgery is rarely needed in certain cases. Conservative treatment should be considered.

CT Aspects of Psychomotor Delays in Children Aged 0 to 5 Years at Bogodogo University Hospital (Ouagadougou)

Background: Psychomotor delays in children are mainly investigated by CT imaging, due to the low availability and accessibility of magnetic resonance imaging in our context. Our aim was to study the CT aspects of psychomotor retardation (PMR) in children under 5 years of age at the University Hospital of Ouagadougou. Materials and Methods: Descriptive study with retrospective collection carried out over a period of twelve months (from September 2020 to August 2021). Our study focused on the reports of cranioencephalic CT scans performed on children aged 0 to 5 years, in whom a clinical diagnosis of psychomotor retardation had been made. Variables studied included socio-demographic data, clinical data, technique of CT examination, description of lesions and normality or not of the conclusion. Results: Cranioencephalic CT in children aged 0 - 5 years represented 9.78% of all CT scans performed in this age group. The sex ratio was 1.39. The mean age of the subjects in these reports was 18.31 ± 13.97 months and the modal class was [13 - 24] months. The CT scan was considered pathological in 85.10% of cases (n = 57). The most frequently noted encephalic anomalies were cerebral atrophy isolated in 54.4% of the cases (n = 31) or associated with other lesions in 30.1% of the cases (n = 17). Hydrocephalus was noted in 19.5% of the cases (n = 11). Conclusion: The lesions were mostly sequelae of cerebral ischaemia or cerebromeningeal infections. There were few cerebral malformations. There was no tumour etiology noted.

Diagnosis of Male Hypogonadism: Experience of a Subsaharan African Endocrinology Department: Transversal Study from January 1^st, 2020 to July 31^st, 2022

Introduction: Hypogonadism should be suspected in a man who has symptoms and signs of testosterone deficiency. Clinical manifestations depend on the severity and duration of testosterone deficiency, whether the testicular deficit is concerning only androgen synthesis, spermatogenesis, or both. The objective of our study was to evaluate the clinical and aetiological characteristics of male hypogonadism in Dakar’s suburb. Patients and methods: We conducted a transversal study from January 1st, 2020 to July 31st, 2022. We included all male patients aged at least 14 years old with hypogonadism confirmed by a low level of early-morning free testosterone based on two different dosages. For all patients included, sociodemographic and diagnostic parameters were collected by using a pre-established registration form. Results: In total, 20 patients were selected. The average age was 36.3 years old [14 - 62 years old]. Half of the patients were overweight. Five patients had an abdominal circumference greater than 94 cm (37 inches). The other comorbidities found in our patients were type 2 diabetes (n = 1), hypertension (n = 1) and primary hypercholesterolemia in 2 patients. The functional signs reported by the patients were: couple’s infertility in 17 patients, decreased libido in 14 patients, erectile dysfunction in 13 patients, premature ejaculation in 2 patients and anejaculation in 4 patients. The physical examination revealed a bilateral testicular atrophy in 17 patients and a unilateral testicular atrophy in 2 patients;no patient had varicocele or urethral meatus abnormalities. Ten patients presented a micropenis. A eunuchoid morphotype was present in 6 patients and a short stature was noted in 2 patients. It was peripheral hypogonadism (HH) in 18 patients and hypogonadotropic hypogonadism (Hh) in 2 patients. The hypogonadotropic hypogonadism was isolated in both cases. Evaluation of CT Scans Reports in Pancreatic Tumors at Ouagadougou (Burkina Faso)

Background: There are no recommendations for the use of standardised CT reports in oncology in our country. The aim of this study was to evaluate CT reports of pancreatic tumors in the city of Ouagadougou. Materials and Methods: Descriptive, multicenter, cross-sectional study conducted from 1st January 2013 to 31 December 2021. It concerned CT scan reports from five public and private imaging centers in the city of Ouagadougou. During the study period, 41 reports of pancreatic tumors were collected. We evaluated the reports using the standardized model developed by the Society of Abdominal Radiology and the American Pancreatic Association as a reference. Results: CT scan reports were not standardised. The writing style was free. Concerning the lesion, the aspect of the tumor at the pancreatic time was the item with the least information (24.4%). The status of the superior mesenteric artery was mentioned in 17%, the celiac trunk and the portal trunk in 12.2% of cases and 9.7% for the superior mesenteric vein. No report noted the appearance of the common hepatic artery. The status of the lymph nodes, liver and peritoneal cavity was mentioned in all reports. The exact location of the lymph nodes was not specified. The analysis of the reports classified them into two groups: potentially resectable tumours in the absence of secondary locations in 31.7% of cases and locally advanced tumours or presence of metastases in 68.3% of cases. Conclusion: The CT reports were not standardized. The items allowing evaluating the loco-regional extension of the tumor were the least specified. This may suggest the high rate of potentially resectable tumours in our study.

Secondary Metabolites and Antioxidant Activity of Different Parts of the Baobab Fruit (<i>Adansonia digitata</i>L.)

The objective of this study was to determine the polyphenol, flavonoid and tannin content and the antioxidant power of methanolic extracts from the different parts of the Senegalese baobab fruit. Phytochemical screening revealed the presence of saponosides, tannins, alkaloids, sterols, flavonoids, coumarins and total sugars in all extracts from the fruit parts. The total polyphenol content was determined by the folin-ciocalteu method. This method is based on the quantification of the total concentration of hydroxyl groups present in the extract. In an alkaline medium, the reagent of folin-ciocalteu, oxidizes the phenols to ion phenolates and partially reduces its hetero-polyacids, hence the formation of a blue complex. The absorbance is read at 765 nm against a control. The Flavonoids vere determined using aluminum trichloride and sodium hydroxide. Aluminum trichloride forms a yellow complex with flavonoids and sodium hydroxyde forms a pink complex absorbing in the visible range at 510 nm. The alternative colorimetric method based on reactions with vanillin in an acidic medium made it possible to determine the tannin content. Absorbances were measured at 500 nm. The results show that the methanolic extracts of the fibers and shell have very high polyphenol, flavonoid and tannin contents. In fact, the polyphenol contents of the fiber (159.00 ± 0.93 μg EAG/mg extract) and shell (155.39 ± 0.89 μg EAG/mg extract) were much higher than those of the pulp (27.21 ± 0.26 μg EAG/mg extract) and seeds (18.36 ± 0.07 μg EAG/mg extract). Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease deadly. In Africa, little data exists on this condition and the form familial is the most common with a younger age of onset. Our observations relate to three brothers, born from a consanguineous marriage. The diagnosis of amyotrophic lateral sclerosis was certain placed in all patients according to the criteria of Awaji. The average age of patients was 16.66 years, and the average age of onset of symptoms was 10.33 years old. The female gender was the most represented with a sex ratio of 0.5. Our patients benefited from rilusole, vitamin D and physiotherapy. A only patient presented with dysphagia. However, no cases have been observed at this day.
