Evaluation of the versatility of perforators-enhanced rhomboid flap technique for ischial sores reconstruction

Background: Prolonged pressure on the skin known as pressure sores, can cause frequent injuries to the skin and underlying tissues. However, several prevention approaches, including non-surgical and surgical management,are available. In flap surgery for pressure sores, a variety of flap types may be used;each offers certain benefits and some incidences of complications and recurrence. This study evaluated the versatility of the perforatorenhanced rhomboid flap technique for ischial sore ***: This prospective study was conducted in the Plastic Surgery Department of Fayoum University *** perforator-enhanced rhomboid flap technique was employed in 20 patients with ischial pressure sores who failed to respond to conservative treatment. A proper history was obtained and a preoperative arterial duplex was performed to determine the site of the prominent perforators for their preservation at the base of the *** follow-ups and evaluations were ***: The mean age of the study group was 43.7 ± 13.1, ranging from 19 to 65 years. Moreover, 80% of the participants were males while 20% were female. The main etiology of the sores was paraplegia in 45% of *** mean surface area of sores was 24.8 ± 6.3 cm2. In 75% of cases, the color match of the flap was rated as excellent. Similarly, 70% of the patients rated flap thickness as excellent. In 55% of the cases, the scar appearance was deemed excellent. Overall satisfaction was reported as excellent by 40% of the patients, while only 5% reported poor satisfaction. Numerical Study of Flow and Gas Diffusion in the Near-Wake behind an Isolated Building

To assist validation of the experimental data of urban pollution dispersion, the effect of an isolated building on the flow and gaseous diffusion in the wake region have been investigated numerically in the neutrally stratified rough-walled turbulent boundary layer. Numerical studies were carried out using Computational Fluid Dynamics （CFD） models. The CFD models used for the simulation were based on the steady-state Reynolds-Average Navier-Stoke equations （RANS） with κ-ε turbulence models; standard κ-ε and RNG κ-ε models. Inlet conditions and boundary conditions were specified numerically to the best information available for each fluid modeling simulation. A gas pollutant was emitted from a point source within the recirculation cavity behind the building model. The accuracy of these simulations was examined by comparing the predicted results with wind tunnel experimental data. It was confirmed that simulation using the model accurately reproduces the velocity and concentration diffusion fields with a fine-mish resolution in the near wake region. Results indicated that there is a good agreement between the numerical simulation and the wind tunnel experiment for both wind flow and concentration diffusion. The results of this work can help to improve the understanding of mechanisms of and simulation of pollutant transport in an urban environment.

Assessment of plant diversity of endemic species of the Saharo-Arabian region in Egypt

Savanna, semi-deserts, and hot deserts characterize the Saharo-Arabian region, which includes Morocco, Mauretania, Algeria, Tunisia, Libya, Egypt, Palestine, Kuwait, Saudi Arabia, Qatar, Bahrain, the United Arab Emirates, Oman, Yemen, southern Jordan, Syria, Iraq, Iran, Afghanistan, Pakistan, and northern India. Its neighboring regions, the Sudano-Zambezian region belonging to the Paleotropical Kingdom and the Mediterranean and Irano-Turanian regions included in the Holarctic Kingdom, share a large portion of their flora with the Saharo-Arabian region. Despite the widespread acknowledgment of the region's global importance for plant diversity, an up to date list of the Saharo-Arabian endemics is still unavailable. The available data are frequently insufficient or out of date at both the whole global and the national scales. Therefore, the present study aims at screening and verifying the Saharo-Arabian endemic plants and determining the phytogeographical distribution of these taxa in the Egyptian flora. Hence, a preliminary list of 429 Saharo-Arabian endemic plants in Egypt was compiled from the available literature. Indeed, by excluding the species that were recorded in any countries or regions outside the Saharo-Arabian region based on different literature, database reviews, and websites, the present study has reduced this number to 126 taxa belonging to 87 genera and 37 families. Regarding the national geographic distribution, South Sinai is the richest region with 83 endemic species compared with other eight phytogeographic regions in Egypt, followed by the Isthmic Desert(the middle of Sinai Peninsula, 53 taxa). Sahara regional subzone(SS1) distributes all the 126 endemic species, Arabian regional subzone(SS2) owns 79 taxa, and Nubo-Sindian subzone(SS3) distributes only 14 endemics. Parathyroid Hormone as a Marker for the Red Cell Fragility in Different Stages of Chronic Kidney Disease

Objective: The aim of the work is to study the relationship between Red blood cell osmotic fragility and level of parathyroid hormone in patients with different stages of Chronic Kidney Disease including End Stage Renal Disease. Background: Anaemia is one of the common complications associated with Chronic Kidney Disease (CKD) responsible for the increase in the morbidity and mortality in such patients. Several factors have been attributed to causing renal anaemia, amongst which hyperparathyroidism is one of the less recognised reasons. The level of PTH in early stages of chronic kidney disease has not been much studied. The excess amount of Parathyroid Hormone (PTH) secondary to CKD has been suggested to be a causative factor for anaemia. Method: A number of chronic kidney disease patients were studied for the relationship between Red cell osmotic fragility and level of parathyroid hormone. Results: This study was conducted on a number of 111 patients with chronic kidney disease classified into three groups. The study revealed a significant fall in Hb%, along with a rise in Median Osmotic Fragility (MOF) and PTH in the CKD patients. iPTH and MOF were significantly lower in group 3 as compared with cases in group 1. Also, iPTH and MOF were significantly lower in cases in group 2 as compared with cases in group 1. Conclusions

Thalidomide is widely used in the treatment of multiple myeloma (MM). In recent years, several cases of pulmonary hypertension have been reported following treatment with thalidomide. The aim of this review was to evaluate the published literature on multiple myeloma patients with pulmonary hypertension following thalidomide treatment. A literature search was performed between 2000 and 2016. A total of 7 eligible studies were identified and deemed eligible, including 11 cases—approximately 37% (4 cases) with IgA (k), 27% (3 cases) with IgG (λ) MM, 27% (3 cases) with IgG (k) MM, and one case (9%) with primary plasma cell leukemia (PPCL). The vast majority of cases—82% (9 cases)—are associated with thalidomide, while only 18% (2 cases) are related to thalidomide derivatives (lenalidomide and pomalidomide). In conclusion, pulmonary hypertension induced by thalidomide or derivatives in multiple myeloma (MM) patients is related to a multifactorial etiology including the pathophysiology of the disease, thromboembolic events, preexisted cardiovascular conditions, comorbidities, and combination with other chemo- or bio-therapeutic agents. MM patients should be evaluated for signs and symptoms underlying cardiopulmonary disease before initiating, and during treatment with thalidomide.

Impact of dietary oils and fats on lipid peroxidation in liver and blood of albino rats

Objective:To investigate the effects of different dietary fat and oils(differing in their degree of saturation and unsaturation)on lipid peroxidation in liver and blood of ***:The study was conducted on SO albino rats that were randomly divided into 5 groups of 10 *** groups were fed on dietary butter(Group I),margarine(Croup II),olive oil(Group III),sunflower oil(Group IV)and com oil(Group V)for 7 *** 12 h of diet removal,livers were excised and blood was collected to measure malondialdehyde(MDA)levels in the supernatant of liver homogenate and in *** superoxide dismutase activity(SOD),glutathione peroxidase activity(GPx),serum vitamin E and total antioxidant capacity(TAC)levels were also measured to determine the effects of fats and oils on lipid ***:The results indicated that no significant differences were observed in SOD activity,vitamin E and TAC levels between the five ***,there was significant decrease of GPx activity in groups IV and V when compared with otlier *** results indicated that feeding corn oil caused significant increases in liver and blood MDA levels as compared with other oils and *** were positive correlations between SOD and GPx,vitamin E and TAC as well as between GPx and TAC(r:0.743;P<0.001)and between blood MDA and liver MDA(r:0.897;P<0.00l).The results showed also negative correlations between blood MDA on one hand and SOD,GPx,vitamin E and TAC on the other ***:The results demonstrated that feeding oils rich in polyunsaturated fatly acids(PUFA)increases lipid peroxidation significantly and may raise the susceptibility of tissues to free radical oxidative damage.

Nuclear fuel performance modeling and simulation are critical tasks for nuclear fuel design optimization and safety analysis under normal and transient *** performance is a complicated phenomenon that involves thermal,mechanical,and irradiation mechanisms and requires special multiphysics *** this study,a fuel performance model was developed using the COMSOL Multiphysics *** modeling was performed for a 2D axis-symmetric geometry of a UO2fuel pellet in the E110 clad for VVER-1200 *** modeling considers all relevant phenomena,including heat generation and conduction,gap heat transfer,elastic strain,mechanical contact,thermal expansion,grain growth,densification,fission gas generation and release,fission product swelling,gap/plenum pressure,and cladding thermal and irradiation *** model was validated using a code-to-code evaluation of the fuel pellet centerline and surface temperatures in the case of constant power,in addition to validation of fission gas release(FGR)*** prediction proved that the model could perform according to previously published VVER nuclear fuel performance parameters.A sensitivity study was also conducted to assess the effects of uncertainty on some of the model *** model was then used to predict the VVER-1200 fuel performance parameters as a function of burnup,including the temperature profiles,gap width,fission gas release,and plenum pressure.A compilation of related material and thermomechanical models was conducted and included in the modeling to allow the user to investigate different material/performance *** the model was developed for normal operating conditions,it can be modified to include off-normal operating conditions.
