Role of Right Ventricular-Pulmonary Arterial Coupling Measured by Echocardiography in Predicting Right Ventricular Dysfunction after Cardiac Surgery

Background: The tricuspid annular plane systolic excursion (TAPSE) and pulmonary artery systolic pressure (PASP) is an indirect estimate of right ventricular-pulmonary arterial (RV-PA) coupling that has been shown to correlate with invasive measures. We aimed to assess the ability of the tricuspid annular plane systolic excursion/pulmonary systolic pressure ratio (TAPSE/PASP) as a measure for RV-PA coupling to predict the development of RV dysfunction after cardiac surgery. Methods: This prospective study was conducted on 100 patients with ischemic heart disease and undergoing cardiac surgery (coronary artery bypass graft (CABG)) with normal preoperative right ventricular function, classified according to RV function outcomes into 2 groups: Normal RV group (65 patients) and RV dysfunction group (35 patients). All cases underwent per and postoperative transthoracic echocardiography. Results: By using receiver operating characteristic curve analysis, pre-operative TAPSE/PASP ratio could significantly predict the RV dysfunction (P 0.58, with AUC of 94%, 88.6% sensitivity, and 89.2% specificity. Post-operative TAPSE/PASP Ratio could significantly predict the RV dysfunction (P 0.39, with AUC of 84%, 100% sensitivity, and 76.9% specificity. Pre-operative TAPSE/PASP ratio could significantly predict mortality (P Conclusion: The TAPSE/PASP ratio is an excellent tool for CABG patients for its ability to detect and predict the development of RV dysfunction after cardiac surgery, along with the prediction of mortality in post-operative CABG patients.

An Investigation on Safety Performance Assessment of Close-Following Behavior of Heavy Vehicle Using Empirical-Simulation Technique

One of the main causes of rear-end crashes is attributed to close-following and hazardous driving behavior. A study was conducted to investigate the close-following behavior of heavy vehicle under various heavy vehicle categories, travel speeds and gross vehicle weights (GVW). Investigation is based on data obtained from simulation and empirical observations. A safety performance assessment of close-following behavior of heavy vehicles by using empirical-simulation technique is proposed. The simulation, which incorporates vehicle dynamics, is to generate the minimum safe time gap (MSTG) for truck-following-car situations. MSTG is defined as the minimum time required by the following vehicle to decelerate and stop without hitting the leading vehicle when both leading and following vehicles apply the emergency brakes. Based on comparison between the actual time gap data and the MSTG, a safety performance assessment technique that considers vehicle type, vehicle braking characteristics, truck GVW and speed is proposed for truck-following-car situation.

Heuristic Algorithm for Minimizing the Electricity Cost of Smart House

This framework proposes a heuristic algorithm based on LP （linear programming） for optimizing the electricity cost in large residential buildings, in a smart grid environment. Our heuristic tackles large multi-objective energy allocation problem （large number of appliances and high time resolution）. The primary goal is to reduce the electricity bills, and discomfort factor. Also, increase the utilization of domestic renewable energy, and reduce the running time of the optimization algorithm. Our heuristic algorithm uses linear programming relaxation, and two rounding strategies. The first technique, called CR （cumulative rounding）, is designed for thermostatic appliances such as air conditioners and electric heaters, and the second approach, called MCR （minimum cost rounding）, is designed for other interruptible appliances. The results show that the proposed heuristic algorithm can be used to solve large MILP （mixed integer linear programming） problems and gives a decent suboptimal solution in polynomial time.

Preparation and Characterization of Conductive Cellulosic Fabric by Polymerization of Pyrrole

Preparation of conductive cotton fabrics, by polymerization of pyrrole in presence of FeCl3 as an oxidizing agent and tetraethylammonium p-toluene sulfonat (TEAp-TS) as a doping agent which in turn caused a reduction in the moisture regain of the substrate, resulted in production conductive cotton fabrics in laboratory scale. Factors affecting the properties of the fabrics were studied, such as ratio between FeCl3 and TEAp-TS and pyrrole concentration. Polypyrrole coated cotton fabrics were characterized by scanning electron microscopy (SEM) and Fourier transform-infrared (FT-IR) spectroscopy. Resistivity, weight gain, color strength, tensile strength and elongation of the coated fabric with polypyrrol were monitored according to ASTM procedures. The results had shown that, the conductivity, weight gain, color strength, tensile strength and elongation increase by increasing FeCl3, TEAp-TS and pyrrole concentration. This is due to the increase of amount of the conductive polymer on the fabric surface. FeCl3 (0.25 M/L), TEAp-TS (0.125 M/L), pyrrole (0.3 M/L), temperature (25°C) and time (4 h) represented the most appropriate formulation for effecting polymerizing of pyrrole and anchoring on cotton fabrics.

The Role of Electrocardiogram DETERMINE Score in Prediction of Coronary Artery Disease Severity

Background: A major cause of mortality and disability on a global scale is myocardial infarction (MI). These days, the most reliable way to detect and measure MI is via cardiovascular magnetic resonance imaging (CMR). Aims and Objectives: To evaluate the effectiveness of the Electrocardiogram DETERMINE Score in predicting the severity of coronary artery disease (CAD) in patients who have experienced an Acute Myocardial Infarction (AMI) & to assess improvements in left ventricular function at 6 months following coronary artery bypass grafting (CABG). Subjects and Methods: This Observational cohort study was done at the Cardiology and Radiology department and cardiac surgery department, Al-Azhar university hospitals and Helwan University hospital. The study involved 700 cases who patients diagnosed with Acute Myocardial Infarction and fulfilled specific criteria for selection. Result: There was highly statistically significant relation between Myocardial infarction size and ECG Marker Score as mean infarct size elevated When the number of ECG markers increased. There was a highly statistically significant relation between myocardial infarct segments, myocardial infarction size and improvement of cardiac function 6 months post-CABG. Conclusion: The study found that larger myocardial infarctions corresponded with higher DETERMINE Scores. It concluded that an ECG-based score better estimates infarct size than LVEF alone. Additionally, there was a significant statistical correlation between the size and segmentation of myocardial infarction and better cardiac function six months after CABG.
