The dynamic response of mechanical and civil structures subjected to high-amplitude vibration is often dangerous and undesirable. Sometimes controlled vibration is desirable as in the machinery used in the formation of rigid hard material as Ceramics and diamond. This process is done via the passive control methods. The main purpose of this paper is to how reduction of vibration of nonlinear system subjected to multi-excitation forces via a nonlinear absorber. The nonlinear differential equations describing the model, which describe ultrasonic cutting machine are solved by using perturbation method. The effects of different parameter on the response of the system are studied. The stability of the numerical solution is investigated by using frequency response equations and phase-plane method. The simulation results are achieved using Matlab and Maple programs. A comparison is made with the available published work.

Fabrication and Characterization of Tungsten Heavy Alloys Using Chemical Reduction and Mechanical Alloying Methods

A novel reduction technique has been developed to synthesize nano-sized tungsten heavy alloys powders and compared with the same powders processed by mechanical alloying technique. in the first method, nano-sized tungsten heavy alloys powders have been obtained by reduction of precursors obtained by spray drying of several appropriate aqueous solutions, which were made from salts containing tungsten, cobalt, and nickel. By adjusting the stoichiometry of the component of the solutions, it is possible to obtain the desired chemical composition of the tungsten heavy alloys powders. in the second method, highly pure elemental powders of tungsten heavy alloys have been mechanically alloyed in a tumbler ball mill for different milling time. The investigated tungsten heavy alloy powders with the composition (95%W-3.5%Ni-1.5%Fe), (93%W-4.5%Ni-1.0%Fe-1.5%Co), and (90%W-6%Ni-4%Cu) have been prepared using both methods. The prepared powders have been compacted at 70 bar (200 MPa) and sintered in vacuum furnace at 1400℃. Vacuum sintering was carried out to achieve full densification of the produced tungsten heavy alloys. The investigated materials were going to be evaluated the physical and mechanical properties of the sintered parts such as density;electrical conductivity, hardness, and transverse rupture strength. The results reveal that, the grain size of alloys fabricated by chemical reduction technique (53.1 - 63.8 nm) is smaller than that fabricated by mechanical alloying technique (56.4 - 71.4 nm).

Formaldehyde is a chemical commonly used in hospitals as a tissue preservative;histopathology laboratory personnel are therefore among the workers most heavily exposed to formaldehyde. This study measured the formaldehyde exposure through ambient and personal air sampling, assessed the symptoms of poor health as well as estimating the health risk among hospital workers. We conducted a comparative cross-sectional study of both histopathology laboratory (exposed) and administration (nonexposed) workers in four hospitals in the Klang Valley, Selangor, Malaysia. Ambient and personal exposure to formaldehyde was measured using the OSHA 52 and NiOSH 2541 methods, respectively. The 8-hr time-weighted-average formaldehyde concentration was higher in exposed areas (0.25 ± 0.11 ppm) than nonexposed areas (0.08 ± 0.02 ppm). Histopathology workers were exposed to between 140% and 480% higher concentrations of formaldehyde than administration workers. Personal exposure was highest during grossing tasks (0.797 ± 0.436 ppm). A total of 67% of the exposed workers exhibited the same ten health symptoms related to formaldehyde exposure, and 57% of the nonexposed workers reported similar symptoms at their current workplace. Notably, symptoms of eye irritation, headache, drowsiness, and chest tightness were significantly more prevalent (p i square and Fisher’s exact tests) among the exposed workers than the nonexposed workers. Among those with symptoms, 37% of the exposed workers, and 16% of the nonexposed workers believed that the symptoms were related to their current working environment. The noncancer effect of formaldehyde from air inhalation poses a potential risk of eye irritation among exposed workers. The cancer risk was not significant in both groups. Formaldehyde levels and symptoms of poor health were significantly higher among the exposed group. Exposure and risk could be minimised by strengthening control measures to improve indoor air quality in the workplace.

Clinical Outcome of Low Dose Contrast during Percutaneous Coronary intervention in Patients with Moderate to Severe Kidney impairment

Background: Chronic kidney disease patients are at a greater risk for nephropathy requiring dialysis after percutaneous coronary intervention. Such patients are usually deferred due to fear of “Renalism”.?Objectives This study assesses the outcome of Low dose contrast protocol during PCi in CKD patients whose e-GFR 60 ml/min/1.72 m and investigates a safety margin for contrast use in these high-risk categories.?Methods: Patients were into three groups according to CV/e-GFR ratio: Group (A) low-dose: CV/e-GFR ratio 2.0 Group (B) medium-dose: CV/e-GFR ratio > 2.0 and × bodyweight\***). Group (C) high-dose: CV/e-GFR ratio > MACD. Results: A total of 73 patients were enrolled. Average age was 54 ± 8 years,81.4% were male and 18.6% were females and 52% were diabetic. Mean baseline e-GFR was 40 ± 8.0 ml/min/1.73m2. Contrast Volume used in group A was (58.26 ± 15.05) (n = 24), in group B (109.42 ± 17.11) (n = 26) and in group C (304.5 ± 60.30) (n = 23), respectively. The incidences of Ci-AKi in the 3 groups were 0%, 11.5% and 35%, respectively (p = 0.02). All-cause death 0%, 17% and introduction of maintenance hemo dialysis was 0%, 11.5% and 26%, respectively (p Conclusion: Low dose contrast protocol is safe, effective and easily applicable technique without Ci-AKi or death.

First report on molecular characterization of Leishmania species from cutaneous leishmaniasis patients in southern Khyber Pakhtunkhwa province of Pakistan

Objective: To report presence of Leishmania major in Khyber Pakhtunkhwa of Pakistan, where cutaneous leishmaniasis(CL) is endemic and was thought to be caused by Leishmania tropica only. Methods: Biopsy samples from 432 CL suspected patients were collected from 3 southern districts of Khyber Pakhtunkhwa during years 2011–2016. Microscopy on Giemsa stained slides were done followed by amplification of the ribosomal internal transcribed spacer 1 gene. Results: Leishmania amastigotes were detected by microscopy in 308 of 432 samples(71.3%) while 374 out of 432 samples(86.6%) were positive by ribosomal internal transcribed spacer 1 PCR. Subsequent restriction fragment length polymorphism confirmed Leishmania tropica in 351 and Leishmania major in 6 biopsy samples. Conclusions: This study is the first molecular characterization of Leishmania species in southern Khyber Pakhtunkhwa. it confirmed the previous assumptions that anthroponotic CL is the major CL form present in Khyber Pakhtunkhwa province. Furthermore, this is the first report of Leishmania major from a classical anthroponotic CL endemic focus identified in rural areas of Kohat district in southern Khyber Pakhtunkhwa.

Purpose: Spleen is enlarged in a variety of clinical conditions including infectious, infiltrative, immunologic and malignant states. Evaluation of splenic size is important in every physical examination of the abdomen by a physician. Determination of its size by palpation can be extremely inaccurate because spleen is never palpable till it is enlarged 2 to 3 times its own size. The accurate diagnosis of splenic enlargement is a matter of considerable importance as it is a useful guide for arriving at a diagnosis of the disease. it is therefore of utmost importance to resort to a mechanism that will give us an accurate estimation of the size of spleen. Aim of this work was to determine the normal range of spleen dimensions in average adult Saudi Arabian population and compare it with the published data. Methods: CT scans of 34 adult patients (male and female) aged between 20 - 70 years, having no splenic disorders, were collected from department of radio-diagnosis King Khalid Hospital Al-Kharj, KSA. Splenic volume was measured by two methods—by volumetric software and the prolate ellipsoid formula. Results: The average splenic volume of all subjects was 161.42 ± 54.91 cm3 with a range of 106 - 319 cm3. The average splenic volume of males was 196.95 ± 48.70 cm3 and that of female was 196.95 ± 26.97 cm3. Conclusions: These results provide normative data for evaluating patients with splenic enlargement.

Background: Although hyperglycaemia is one of the known side effects of L-asparaginase, its contribution to the development of diabetic ketoacidosis (DKA) is less well known in the literature. Asparaginase is an essential component of treatment protocols for acute lymphoblastic leukaemia (ALL) in combination with other chemotherapeutic drugs. On further evaluation, she was found to have high anion gap metabolic acidosis, hyperglycaemia and ketonuria. in recent decades, the use of these chemotherapeutic agents has led to a decrease in mortality and disease-free survival in ALL. L-asparaginase is one of the chemotherapy protocols used in the treatment of acute lymphoblastic leukaemia (ALL) and can induce hyperglycaemia which is aggravated by the concomitant use of corticosteroids. We report the observation of a 14-year-old girl treated with chemotherapy (GFA LAL protocol) who developed transient diabetes following the use of L-asparaginase. She was treated at the Donka paediatric haemato-oncology unit by a multidisciplinary team including a paediatric oncologist, a psychologist and a diabetologist. The aim of this study was to highlight blood glucose monitoring before and after the use of asparaginase in acute lymphoblastic leukaemia. Conclusion: We conclude that the occurrence of ketoacidosis following the use of asparaginase is a rare event. We recommend close monitoring of blood glucose levels for hyperglycaemia in patients with ALL receiving L-asparaginase.
