Evaluating the influence of a structured nursing protocol on targeted outcomes in rheumatoid arthritis patients

Objective:Rheumatoid arthritis(RA)requires comprehensive *** nursing protocols may enhance outcomes,but evidence is *** study evaluated the effect of a structured nursing protocol on RA *** and Methods:In this one-group pre-post study,30 Egyptian RA patients completed assessments before and after a 12-week nursing protocol comprising education,psychosocial support,and self-management *** included clinical evaluation of joint counts,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)and patient-reported Arthritis Self-Efficacy Scale(ASES),Health Assessment Questionnaire(HAQ),Visual Analog Scale(VAS)for pain,and Hospital Anxiety and Depression Scale(HADS).Results:The study demonstrated significant improvements in both clinical-and patient-reported *** count decreased from 18.4±4.2 to 14.2±3.8(P<0.001),ESR from 30.1±6.8 mm/h to 25.5±6.8 mm/h(P<0.01),and CRP levels from 15.2±3.6 mg/L to 11.8±2.9 mg/L(P<0.01)***-reported outcomes showed a marked increase in ASES score from 140±25 to 170±30(P<0.001)and reductions in HAQ from 1.6±0.4 to 1.3±0.3(P<0.01),VAS pain score from 7.8±1.7 to 6.2±1.2(P<0.001),and HADS anxiety and depression scores from 11±3 to 8±2(P<0.05)and 10±2 to 7±1(P<0.05),***:A structured nursing protocol significantly improved clinical disease activity,physical functioning,pain,self-efficacy,and emotional well-being in RA patients.A multifaceted nursing intervention appears beneficial for optimizing RA outcomes.

A new technique of monsplasty as an adjunctive procedure in cases of abdominoplasty:A prospective clinical trial

Background:No standard monsplasty technique has existed until *** various monsplasty methods have been described,they exhibit high complication rates with sparse postoperative *** that used pubic suspension techniques did not describe exact anatomical landmarks for the mons *** study aimed to improve the aesthetic and functional appearance of the pubic region with long-term results for all grades of pubic ptosis or bulging with abdominoplasty and provide exact anatomical points to anchor the mons to the rectus ***:This non-randomized prospective clinical trial included 30 patients with various degrees of abdominal wall laxity between December 2017 and September *** surgical procedure was performed for female patients with pendulous abdomen,pubic ptosis or bulging,body mass index≤35 kg/m^(2),and age 18–60 *** patients were followed for up to one year;patient satisfaction was assessed before and after surgery regarding aesthetic results and impact on outfits,hygiene,and sexual ***:The marginal homogeneity test revealed a statistically significant patient satisfaction rate regarding outfits,sexual function,hygiene,and *** postoperative complications,seroma(one case)was managed by aspiration,and one case of wound dehiscence was managed conservatively with ***:This study recommends a quick and reproducible monsplasty technique with a low complication rate,a technique that helps determine specific anatomical landmarks for anchoring the mons to the rectus *** this technique with abdominal contouring is advisable for optimal aesthetic results and maintained lymphatic drainage in the pubic region.

Effective properties of a porous inhomogeneously polarized by direction piezoceramic material with full metalized pore boundaries:finite element analysis

This paper concerns the homogenization problems for porous piezocomposites with infinitely thin metalized pore *** determine the effective properties,we used the effective moduli method and the finite element approaches,realized in the ANSYS *** a simple model of the representative volume,we applied a unit cell of porous piezoceramic material in the form of a cube with one spherical *** modeled metallization by introducing an additional layer of material with very large permittivity coefficients along the pore *** we simulated the nonuniform polarization field around the *** taking this effect into account,we previously solved the electrostatic problem for a porous dielectric material with the same geometric *** this problem,we obtained the polarization field in the porous piezomaterial;after that,we modified the material properties of the finite elements from dielectric to piezoelectric with element coordinate systems whose corresponding axes rotated along the polarization *** a result,we obtained the porous unit cell of an inhomogeneously polarized piezoceramic *** the solutions of these homogenization problems,we observed that the examined porous piezoceramics composite with metalized pore boundaries has more extensive effective transverse and shear piezomoduli,and effective dielectric constants compared to the conventional porous *** analysis also showed that the effect of the polarization field inhomogeneity is insignificant on the ordinary porous piezoceramics;however,it is more significant on the porous piezoceramics with metalized pore surfaces.

Identification of potential soil water retention using hydric numerical model at arid regions by land-use changes

Assessment of soil water retention in arid region is an input required parameter in precision water management at large *** were carried out in Tanta catchment in the middle Nile Delta,Egypt(30145 N,30155 E),where collecting soil samples covered different hydrological soil groups and *** on the natural resource conservation service curve number model(NRCS-CN),CN approach was used to investigate the effect of spatio-temporal variations of different land-uses on soil water *** soil water retention from 1990 to 2015 was reduced by 118.1 m^(3) per hectare with decreasing cropland *** encroachment from 1990 to 2015 was increased by 2.13%by decreasing cropland with 15.3%(5300 ha in 2015).This resulted in losing the potential soil water retention by 625,968.42 m^(3) water for the whole catchment *** of land degradation was pronounced,where 2.65%,29.35%,and 1.11%of the initial crop land-use in 1990 were converted to bare soil,fallow,and urban area,respectively in *** of(S)value of the NRCS-CN model with GIS technique provides useful measure to identify land-use changes of potential water storage capacity at catchment scale.

从人们消费的角度来看,目前马铃薯是仅次于小麦和水稻的第三大作物。中国是世界上最大的马铃薯生产国,其产量9000万吨,几乎占全球马铃薯产量的四分之一,并且其种植面积约占总耕地面积的28%。然而,由于马铃薯是营养繁殖作物,病毒可在植株中世代积累并通过引起块茎内部和表面坏死而影响品质。单个病毒就可使马铃薯潜在总产量损失50%以上,而混合病毒则可损失80%以上。因此,病毒是马铃薯生产中最重要的经济性生物限制因素之一。近年来,大量研究表明全世界感染马铃薯的病毒种类约40种,我国仅发现11种。在这11种病毒中,有6种病毒被认为是马铃薯的主要病毒:PLRV、PVA、PVM、PVS、PVX和PVY。然而,病毒已经发展出各种机制来克服植物的抗性,使得对几乎所有病毒都缺少相应的抗性种质。因此,在本研究中,我们利用双链RNA来启动RNA干扰,以靶向马铃薯病毒六大基因组中的不同保守区域,即PLRV、PVA、PVM、PVX、PVY、PVS。我们以微管和节间作为转化外植体来进行比较。与微管相比,十天龄的节间转化效率和转化频率都较高。因此,利用节间可以降低碳源成本和微管培养成本。并且利用节间进行微繁殖,可使转化保持在一个封闭的循环中,减少了时间和精力。与三到四周龄的节间相比,更建议使用十天龄的节间。由于外源基因对农杆菌细胞的影响,不同载体生成的转化芽数量不同。如S17、S51、S34菌株形成多细胞团,最终沉淀在培养管或烧瓶底部,造成细菌过度生长,导致转化效率低下。总共获得了81株AC142-01转化植株。根据内含子“GUS”策略中包含的ami RNA,其中13个用不同的人工s RNAs前体构建,靶向PLRV内的几个保守区域,另外16个靶向PVA,4个靶向PVS,1个靶向PVY,3个靶向PVX,8个靶向PVM。8个GUS和28个ami RNA封闭在外显子PDS中,用不同的人工s RNAs前体构建靶向多种病毒(PLRV、PVA、PVY、PVX、PVS、PVM)中的不同保守区域,再利用PCR技术在基因组DNA水平上对转化体进行筛选。用ami RNA GUS前体修饰的转基因系AC142-01靶向PLRV基因组中的几个保守区域,表现出了对病毒的特异性抗性。最后,通过ELISA检测和表型鉴定,发现转基因系I12和I25中PLRV的积累和疾病症状都有所减少。

Enhancing the explosive characteristics of a Semtex explosive by involving admixtures of BCHMX and HMX

A well-known ternary plastic explosive,Czech Semtex 1H,contains a mixture of PETN and RDX softened by *** this work,BCHMX was used to replace PETN in Semtex 1H to form Sem-BC+*** addition,another mixture based on BCHMX and HMX as energetic fillers bonded by the polymeric matrix of Semtex 1H(Sem-BC+HMX)was *** particle size distribution of each individual explosive was determined to obtain the optimum mixing *** and impact sensitivities were *** velocity of detonation was reported practically and the detonation properties were calculated by EXPLO5 *** explosive strength of each sample was measured by the ballistic mortar *** conclusion confirms that the velocity of detonation of Sem-BC+HMX was the highest in comparison with the prepared ***-BC+RDX has the least impact and frictions ***-BC+RDX has higher detonation velocity,detonation properties and explosive strength than Semtex *** of BCHMX in Semtex 1H as a replacement for PETN is the candidate to produce a high performance advanced Czech plastic explosive.

Integration of Metabolomics and Subcellular Organelle Expression Microarray to Increase Understanding the Organic Acid Changes in Post-harvest Citrus Fruit

Citric acid plays an important role in fresh fruit flavor and its adaptability to post-harvest storage conditions. In order to explore organic acid regulatory mechanisms in post-harvest citrus fruit, systematic biological analyses were conducted on stored Hirado Buntan Pummelo （HBP; Citrus grandis） fruits. High- performance capillary electrophoresis, subcellular organelle expression microarray, real-time quantitative reverse transcription polymerase chain reaction, gas chromatography mass spectrometry （GC-MS）, and conventional physiological and biochemical analyses were undertaken. The results showed that the concentration of organic acids in HBP underwent a regular fluctuation. GC-MS-based metabolic profiling indicated that succinic acid, ~,-aminobutyric acid （GABA）, and glutamine contents increased, but 2- oxoglutaric acid content declined, which further confirmed that the GABA shunt may have some regulatory roles in organic acid catabolism processes. In addition, the concentration of organic acids was significantly correlated with senescence-related physiological processes, such as hydrogen peroxide content as well as superoxide dismutase and peroxidase activities, which showed that organic acids could be regarded as important parameters for measuring citrus fruit post-harvest senescence processes.

Stochastic Analysis of Low-Cost Single-Frequency GPS Receivers

Typically, dual-frequency geodetic grade GNSS receivers are utilized for positioning applications that require high accuracy. Single-frequency high grade receivers can be used to minimize the expenses of such dual-frequency receivers. However, user has to consider the resultant positioning accuracy. Since the evolution of low-cost single-frequency (LCSF) receivers is typically cheaper than single-frequency high grade receivers, it is possible to obtain comparable positioning accuracy if the corresponding observables are accurately modelled. In this paper, two LCSF GPS receivers are used to form short baseline. Raw GPS measurements are recorded for several consecutive days. The collected data are used to develop the stochastic model of GPS observables from such receivers. Different functions are tested to determine the best fitting model which is found to be 3 parameters exponential decay function. The new developed model is used to process different data sets and the results are compared against the traditional model. Both results from the newly developed and the traditional models are compared with the reference solution obtained from dual-frequency receiver. It is shown that the newly developed model improves the root-mean-square of the estimated horizontal coordinates by about 10% and improves the root-mean-square of the up component by about 39%.

Efficient Harmonic Analysis Technique for Prediction of IGS Real-Time Satellite Clock Corrections

Real-time satellite orbit and clock corrections obtained from the broadcast ephemerides can be improved using IGS real-time service (RTS) products. Recent research showed that applying such corrections for broadcast ephemerides can significantly improve the RMS of the estimated coordinates. However, unintentional streaming interruption may happen for many reasons such as software or hardware failure. Streaming interruption, if happened, will cause sudden degradation of the obtained solution if only the broadcast ephemerides are used. A better solution can be obtained in real-time if the predicted part of the ultra-rapid products is used. In this paper, Harmonic analysis technique is used to predict the IGS RTS corrections using historical broadcasted data. It is shown that using the predicted clock corrections improves the RMS of the estimated coordinates by about 72%, 58%, and 72% in latitude, longitude, and height directions, respectively and reduces the 2D and 3D errors by about 80% compared with the predicted part of the IGS ultra-rapid clock corrections.

Comparing the Efficacy of Dexmedetomidine When Used as an Adjuvant to Bupivacaine in Supraclavicular Brachial Plexus Blocks under Ultrasound-Guided

Background: Ultrasound-guided for regional anesthesia offers many potential benefits in the emergency setting. Analgesia can be explicitly targeted to the region of pain and provide relief for many hours and decrease needing to the large volume of local anesthetic. The aim of the work: Comparing the efficacy of dexmedetomidine when used as an adjuvant to bupivacaine in supraclavicular brachial plexus blocks on the onset of sensory, motor blockade and postoperative analgesia. Patients and methods: This prospective, randomized, single-blind clinical study conducted on 60 patients underwent upper limb surgery done by ultrasound-guided supraclavicular brachial plexus block;these patients allocated into two equal groups: Group I (control) received 20 ccs (19 cc bupivacaine 0.5% + 1 cc saline), Group II received 20 cc (19 cc bupivacaine 0.5% + 1 cc volume of Dexmedetomidine 1 ug/kg). Results: Demographic data and surgical characteristics were comparable in both groups. The onset times for sensory and motor blocks were significantly shorter in Group II than Group I (P Conclusion: We recommend adding Dexmedetomidine to local anesthetics in peripheral nerve blocks to take advantage of the prolonged time of both sensory and motor blocks and prolonged postoperative analgesia.
